[Suitability of different procedures for the sterilization of microsurgical instruments. A review of current and future procedures].

A review of common methods of sterilization is presented. The principles of operation of steam, hot-air, and gas sterilizers, as well as that of a completely new method which uses H2O2 plasma, are described. The suitability of these methods for sterilizing microsurgical instruments was studied. This paper deals in particular with hot-air sterilization, which is preferred because of the low humidity involved. This method was investigated by recording temperature curves during sterilization. Sterilization with hot steam (autoclaving) has been shown to damage instruments by causing corrosion, to which the types of steel used to make these instruments are highly susceptible. A H2O2 plasma sterilization method is described which operates at low temperatures and with very low humidity. Its suitability for microsurgical instruments has been investigated.

Valve reconstruction procedures for nonobstructive venous insufficiency: rationale, techniques, and results in 107 procedures with two- to eight-year follow-up.

Among 211 limbs with nonobstructive chronic venous insufficiency, valve reflux of the deep system was the predominant (more than 70%) pathologic condition. Superficial venous or perforator incompetence when present invariably occurred in combination with valve reflux of the deep veins, suggesting that the latter is a common denominator for symptom production. Single level-single system reflux was only occasionally symptomatic (10%), whereas the incidence of single level-multisystem reflux (25%) and multilevel-multisystem reflux (65%) in symptomatic limbs was much higher. Our experience with 107 venous valve reconstructions with a 2- to 8-year follow-up is described. Different techniques of valve reconstruction employed are detailed. The most common pathologic feature is a redundant valve with malcoaptation probably of nonthrombotic origin. Valsalva foot venous pressure elevation is a useful hemodynamic technique for assessing surgical results. Valvuloplasty may be superior to other reconstruction techniques in relieving symptoms of stasis, including stasis ulceration.

Prosthodontic procedures for implant reconstruction. 2. Post-surgical procedures.

There is essentially no flexibility in an osseo-integrated implant system, so that the final fit of the prosthesis must be absolutely precise. A simple approach to this problem is presented with details as to impression taking, confirmation of accuracy of the master cast, fitting of the cast metal framework and construction of the superstructure. The information presented describes a full arch fixed restoration attached to mandibular implants, but may easily be followed for the maxilla or modified for quadrant restorations.

The use of metoclopramide for the small bowel meal examination: pre-procedural versus peri-procedural oral administration.

Administration of oral metoclopramide reduces the transit time for barium to reach the caecum during a small bowel meal (SBM). Current practice is to administer the metoclopramide 5-10 min prior to the barium. We devised a prospective randomized, controlled trial to assess whether administration of metoclopramide 90 min prior to the barium would further reduce the transit time without adversely affecting the quality of the study. Although administration of oral metoclopramide significantly reduced the transit time when compared with a control population, there was no significant difference in transit time between the groups which received metoclopramide.
