[Pain in the side].

In a 54-year-old patient who entered the hospital because of flank pain of the left side, a prevesicular urethral calculus was found. On further investigation hypercalcaemia and hypophosphataemia were detected, possibly indicating hyperparathyroidism. When an ultrasound of the abdomen was made to exclude hydronephrosis, a tumor in the pancreas was found. The result of the fine needle biopsy of this tumor showed a neuroendocrine tumor. The examination of neuroendocrine parameters was without any reference to an active secreting process. On suspicion of a multiple endocrine neoplasia, a MRI of the hypophysis was made which showed no adenomatous alterations. The examination of the neuroendocrine parameters were without any reference to an active secreting process of the hypophysis, too.

[Image-guided thin needle puncture of tumors of the gastrointestinal system. Puncture technique and handling of materials].

The image-guided fine-needle aspiration biopsy in the diagnosis of gastro-intestinal tumors has reached a very high value out of other diagnostic tools. The results of this method depend directly on the technical performance (aspirator, technicians, cytologist) of the punction and processing of the obtained cytological material. The following paper is a brief communication on technical methods and experience in performing the image-guided fine-needle aspiration biopsy at the university hospital of Zurich.

[Intraoperative sonography].

Intraoperative sonography was introduced at our institution in November 1987. We have since examined 55 patients, of which 27 suffered from liver disease, 16 from pancreatic problems and 11 from hyperparathyroidism. A short description of the technique applied is given for each organ. The intraoperative sonography of the liver showed additional information in one third of the cases. The sensitivity of intraoperative sonography in hyperparathyroidism was 78%, the specificity 72%.

Adenoma↗

Hypercalcaemia. The case of the missing adenoma.

A case of severe hypercalcaemia is described. The biochemical picture strongly suggested primary hyperparathyroidism, yet no adenoma could be found in the neck or upper mediastinum. The tumour was eventually demonstrated by computerized tomographic scan and was removed at surgery from the posterior mediastinum beneath the arch of the aorta. Preoperative and postoperative problems of the case are discussed.
