Informed consent for case reports: the ethical dilemma of right to privacy versus pedagogical freedom.

A new international standard of editorial policy calls for written informed consent by the subject of every case report. Although this appears to be ethically appealing, the authors posit that in some situations, requesting informed consent may be unethical, can harm patients, and may erode the use of case reports as a valuable teaching method in psychiatry and psychotherapy. The authors discuss concerns regarding this new policy for mental health publication based on issues of transference, countertransference, best interest of the patient, and practicality.

[Simulator training in medicine and health care. A new pedagogic model for good patient safety].

Advanced simulation within medicine and health care is a rapidly growing field. Simulator based training can be applied in minimal invasive surgery, in endoscopic procedures as well as in anaesthesia and critical care management. At Huddinge University Hospital a center for advanced simulation of both endoscopic surgery and anaesthesia/critical care management is currently being set up. The objective is to focus on improved medical and health care training and thus improving patient safety by reducing medical errors.

[The use of telephone for counseling diabetic patients: a descriptive and pedagogic approach].

This descriptive study of telephone interviews, conducted between the staff of a diabetes unit and patients, is intended to clarify a professional manner in which the telephone should be used as part of patient care. Having examined simulated calls from diabetic patients, analysis of the interviews conducted by the doctors, nurses and members of the office staff in a hospital unit demonstrates wide differences in the content, the quality of the content, and the interview strategy. An interpretation of the results signals the importance of the manner in which the staff member answers, a knowledge of patients' genuine needs, and an ability to adequately acknowledge the emotional state of the patient. Structuring telephone conversations, allowing sufficient time for patient responses, avoids over-long interviews. The decisions taken by the staff member answering the call, in terms of advice and suggestions of actions to take, must be systematically backed-up by ensuring that the patient has properly understood what has been said. The study poses a number of questions, notably concerning the specific requirements demanded by these new remote-care services in the organization of care within a hospital unit, and the professional approach to the use of the telephone as part of the relationship with patients. Recommendations are given concerning the general principles governing a telephone interview and the definition of training objectives for healthcare professionals.

The pedagogical aspects in the physical measurements provided in medicine.

Nowadays, the graduates of medical faculties should be able to use up to date complicated technical devices as well as various diagnostic and therapeutic methods based on physical laws and processes. Therefore good knowledge in physics (including some fields of modern physics) is expected to be mastered by students during their studies of physics at the faculty. The shortness of time that is available for the teaching of physics has forced us to the laboratory exercises to the maximum extent. In this contribution, some special measurements will be described. The authors also discuss the possibilities of applying new practical and theoretical approaches, and stress the significance of the teaching of applied physics at medical faculties for the quality of medical care in the future. [General practice of pedagogic management by teachers of hyperkinetic attention deficit disordered children in the classroom].

Attention Deficit/Hyperactivity Disorder is one of the most common behavior disorders in child and adolescent psychiatry. The problems resulting from the core symptoms of the disorder often endure into adolescence and adulthood placing these children at significant long-term risk for academic psychological and social morbidity. Despite the importance of the school in this process relatively few teachers of regular schools have sufficient knowledge about the foundations and principles of treatment concerning ADHD nor do they receive adequate training how to deal with ADHD related problems in the classroom. Moreover there is a significant lack of cooperation between schools, parents and therapeutic institutions inhibiting a multimodal treatment. This article resumes the experiences of a 3 months ADHD intervention program for teachers in a Cologne elementary school. It gives informations and advices for appropriate measurements in the classroom setting that include: 1. intensive information of teachers about the disorder, 2. intensified involvement of teachers in the treatment process and 3. the implementation of distinct didactic elements and well structured principles of behavior therapy in the school lessons.
