Randomized, double-blind study on effectiveness of intra-alveolar chlorhexidine gel in reducing the incidence of alveolar osteitis in mandibular third molar surgery.

Chlorhexidine (CHX) mouthwash is a good prophylactic agent for postextraction alveolar osteitis (AO), a very common complication. Recently, a bio-adhesive gel to deliver the active substance, CHX, has been introduced. The intra-alveolar positioning of the bio-adhesive gel allows more direct and prolonged action of CHX, which could be useful in the prevention of AO following extraction of impacted third molars. Presented here is a double-blind, randomised and parallel-group study evaluating the efficacy of the bio-adhesive 0.2% CHX gel (n=53) applied once within the alveolus during third molar surgery, compared to placebo gel only (n=50), in reducing the incidence of impacted third-molar postextraction AO. A 63.33% reduction in the occurrence of AO (P=0.019) was observed in the experimental group. In the control group, the occurrence of AO was 30% compared to 11% in the experimental group. Bio-adhesive 0.2% CHX gel, applied only once within the alveolus site at the time of surgery, may reduce the incidence of AO following removal of impacted third molars.

Osteitis in chronic rhinosinusitis.

The presence of inflammation and remodeling within the bone of the paranasal sinuses has been demonstrated in animal and human models of chronic rhinosinusitis. This form of osteitis is present in the underlying bone of affected mucosa and can spread to involve distant sites within the paranasal sinuses. This potential for distant involvement has implications for the medical and surgical management of chronic rhinosinusitis and may contribute to chronic rhinosinusitis refractory to management.

Osteitis deformans (Paget's disease) and calcific disease of the heart valves.

The prevalence of calcific aortic valve stenosis in Paget's disease (osteitis deformans) was investigated by reviewing autopsy data of severe cases (> or = 75% involvement of > or = 3 major bones, the femur, tibia, skull, and pelvis) and moderate cases (> or = 75% involvement of only 1 or 2 major bones) of Paget's disease. Comparisons were made with normal age-matched controls. Aortic stenosis (AS) was present in 24% of 27 autopsies of severe Paget's disease compared with 3.5% in 201 controls (p <0.01). Clinical signs of AS were present in 39% of 102 patients with severe Paget's disease compared with 4% in 417 controls (p <0.101). The prevalence of AS in 18 cases of moderate Paget's disease was similar to that of controls. Electrocardiograms were reviewed in 45 cases of Paget's disease and compared with 80 controls of similar age. Complete atrioventricular (AV) block, incomplete AV block, bundle branch block, and left ventricular hypertrophy were present in 11%, 11%, 20%, and 13% of the Paget's cases and in only 2.5%, 1.3%, 2.5%, and 3.8% in the control cases (p <0.05, <0.05, <0.01, and <0.05, respectively). It is concluded that in severe Paget's disease there is a high prevalence of AS, heart block, and bundle branch block, but these are not present in moderate degrees of bone involvement.

Hemifacial spasm and osteitis deformans.

PURPOSE/METHODS: A patient with osteitis deformans (Paget's disease) and hemifacial spasm underwent magnetic resonance tomographic angiography. Bone-modulating bisphosphonates and botulinum injection were administered to treat the hemifacial spasm. RESULTS/CONCLUSIONS: Computed tomography showed marked temporal bone overgrowth. Magnetic resonance tomographic angiography showed no vascular compression of the facial nerve root. The hemifacial spasm failed to resolve with intravenous pamidronate. Subsequent botulinum injection rendered the patient spasm free for 22 weeks. Further research on the use of bisphosphonates in the treatment of pagetoid hemifacial spasm is required.

Paget's osteitis deformans: a carbonate-rich calcium alkali phosphate in the pre-malignant bone mineral and implications for lymphoma production by CO2.

Comparison of powder diffraction patterns of unheated normal bone mineral with the damaged by Paget's Osteitis Deformans showed that due to a difference in the spacing of one weak inner reflection the possibility exists that carbonate-containing alpha-calcium sodium orthophosphate may be a minor constituent of the osteitic bone mineral. The chemical and physical properties of this substance make it a likely minor constituent of bone salt. The presence of this calcium alkali phosphate in the osteitic mineral was tentatively proposed after demonstrating the similarity of the diffraction patterns of synthetic mixtures of this substance with tricalcium phosphate hydrate to those of the osteitic bone salt.

Diffuse sclerosing osteomyelitis of the mandible: its characteristics and possible relationship to synovitis, acne, pustulosis, hyperostosis, osteitis (SAPHO) syndrome.

PURPOSE: This article reports on the possible relationship of diffuse sclerosing osteomyelitis (DSO) of the mandible to synovitis, acne, pustulosis, hyperostosis, osteitis (SAPHO) syndrome. PATIENTS AND METHODS: The pathologic features in 12 new DSO patients and those in previously reported cases were reviewed and compared with those of SAPHO syndrome. RESULTS: Many similarities were noted between the two entities in terms of the clinical, radiographic, and histologic features. Furthermore, multiple bone lesions and skin lesions (palmoplantar pustulosis and psoriasis) were observed not only in SAPHO syndrome but also in DSO patients. CONCLUSION: DSO is concluded to be one manifestation of SAPHO syndrome.

[Recurrent osteitis and Coxiella burnetii: the relation to chronic recurrent multifocal osteomyelitis].

UNLABELLED: Chronic recurrent multifocal osteomyelitis (CRMO) is a disorder of suspected--but unproved-infectious etiology. OBSERVATION: A girl presented with a typical CRMO involving successively the left fibula, radius, humerus and the right carpus. A Coxiella burnetii infection was indicated during the first attack. Two recurrences occurred in spite of suitable antibiotic treatment and with negative infectious investigation. Two months after stopping antibiotic treatment, a new recurrence associated with antibodies increase and positive bone culture occurred. CONCLUSION: Coxiella burnetii can initiate a CRMO. The mechanism involved is probably a delayed hypersensitivity. CRMO would therefore be the first type of reactive osteitis.

Benign necrotizing osteitis of the external auditory meatus.

Benign necrotizing osteitis of the external auditory meatus is a rare condition which occurs in non-diabetic healthy people. The aetiology of the necrotic process with the formation of sequestrum in the bony external meatus is unknown. It is important to differentiate this benign process from malignant otitis externa and radionecrosis of the temporal bone as the management of these conditions differs. We present two cases, one managed surgically and the other medically, and review the literature. Immediate surgical excision of the sequestrum is advocated instead of long-term medical therapy and waiting for it to separate spontaneously.

Skull base osteitis following fungal sinusitis.

Aspergillus sp. sinusitis is not uncommon in immunocompromised patients but is unusual in patients who are not immunocompromised. The disease may occur as a saprophytic condition, as an allergic sinusitis or as a potentially lethal invasive disease. The differentiation between non-invasive and invasive Aspergillus sp. sinusitis is crucial and this distinction is fully discussed. The treatment options are also considered. Invasive disease requires aggressive treatment with long-term antifungal agents in sufficient doses combined with wide surgical excision. We present a patient who presented with invasive Aspergillus fumigatus sinusitis and subsequently developed cranial neuropathies and skull base osteitis. She was initially treated with oral itraconazole (400 mg daily) for 18 months but due to lack of response this was changed to a new experimental oral azole (voriconazole) which was continued for a further 14 months. She has since remained well for the last five years.

A longitudinal study of idiopathic osteosclerosis and condensing osteitis.

OBJECTIVE: To document the prevalence of idiopathic osteosclerosis (IO) and condensing osteitis (CO) in a middle-age-to-older adult population and determine their long term behavior. STUDY DESIGN: Full-mouth radiographs of 1585 adults, with a mean age 44.0 years, were evaluated for the presence of radiopaque masses diagnosed as IO or CO. All lesions were followed for 2 to 28 years, mean 10.4, to determine changes in size and shape. RESULTS: There were 187 lesions detected, 100 IO in 90 subjects (5.7%) and 87 CO in 71 subjects (4.5%). At follow-up, 180 lesions (96%) were still present, of which 155 were unchanged in size, 18 were smaller, and 7 were larger. CONCLUSIONS: Idiopathic osteosclerosis in middle age to older adults is stable and requires no further action after documentation in the patient's clinical records.

The effect of periapical rarefying osteitis on cortical and cancellous bone. A study comparing conventional radiographs with computed tomography.

OBJECTIVE: To determine whether erosion of the cortical plate is necessary for successful radiographic identification of periapical rarefying osteitis. METHODS: Forty-two periapical osteitic lesions were diagnosed from periapical radiographs of 26 patients who were subsequently examined by cross-sectional computed tomography (CT). The relation of the lesions to the buccal and lingual cortical plates was determined and their size measured. RESULTS: The 42 lesions varied in size from 2 - 10 mm. Thirty one (74%) lesions diagnosed on the periapical radiographs were limited on CT to the cancellous bone with no erosion of the cortical plates. The 11 lesions with cortical erosion were randomly located throughout the jaws. Erosion was due to either the eccentric buccal or lingual location of the tooth apex, large size (>6 mm) of the lesion or narrowness of the jaw. CONCLUSIONS: Periapical lesions can be diagnosed from periapical radiographs while they are limited to the cancellous bone and before they have eroded the cortices.

Osteitis fibrosa cystica generalizata with adult T-cell leukaemia: a case report.

We report on a 62-year-old female initially suffering from extreme pain in both her lower extremities. Plain radiographs revealed multiple osteolytic lesions. Laboratory analyses indicated high levels of serum calcium and parathyroid hormone related protein (PTHrP) and detected HTLV-1 antibody. Histological examination of biopsied specimens which were obtained from osteolytic lesions indicated osteitis fibrosa cystica (OFC) without tumour cells. After the initial biopsy, a subcutaneous mass developed in the left thigh. Microsections of the biopsied mass disclosed adult T-cell leukaemia (ATL) neoplastic cells. Immunohistochemical findings revealed that PTHrP secreted from ATL neoplastic cells induced the OFC.

Hypoplasia of primary and permanent teeth following osteitis and the implications of delayed diagnosis of a neonatal maxillary primary molar.

A 2-year-and-6-month-old Japanese girl with delayed eruption of a maxillary right primary canine and disturbance in the development of maxillary right primary molars was examined. The crown of the maxillary right first primary molar was severely decayed and hypoplastic. The periodontal tissues on the maxillary right posterior region were swollen and slight erythrogenic change was observed. Radiographic examination revealed a disturbance in the development of the permanent successor and the primary teeth in the maxillary right quadrant. According to the history, the maxillary right first primary molar had erupted on day seven after birth. It is suggested that the disturbance in development of the permanent successor and the primary teeth was secondary to osteitis caused by infection of the neonatal tooth. Appropriate dental treatment had not been administered for more than two years because the neonatal molar had not been diagnosed.

[An analysis of the radiological appearance of osteoplastic metastases and osteitis deformans (Paget's disease) (author's transl)].

The validity of individual radiological signs for the differentiation of osteoplastic bone metastases from osteitis deformans has been investigated quantitatively (103 skeletal metastases from carcinoma of the prostate, 45 cases of Paget's disease, two osteoplastic bone metastases from a carcinoma of rectum and bronchus). The similarities were demonstrated by three cases observed by us. Problems in the quantitative evaluation of the radiographs are discussed.

Osteitis fibrosa cystica simulating metastatic tumor. An almost-forgotten relationship.

The diagnosis of primary hyperparathyroidism most often results from the incidental finding of hypercalcemia. In two recent cases of osteitis fibrosa cystica (OFC), however, patients without adequate access to health care served as graphic reminders that the clinical spectrum of the disease includes bone disease, and that OFC can be the presenting manifestation of long-standing primary hyperparathyroidism. Both patients complained of bone pain and had widespread osteolytic bone lesions in addition to hypercalcemia on a multitest biochemical panel. The presumptive diagnosis of malignancy with bone involvement (metastatic cancer or multiple myeloma) led to random bone marrow trephine biopsies. Examination of the bone marrow biopsy material revealed the characteristic pathology of OFC, leading to appropriate diagnosis and surgical management of large parathyroid adenomas in both patients.

Granulomatous synovitis and osteitis caused by Sporothrix schenckii.

Sporotrichosis must be considered in the differential diagnosis of granulomatous inflammation involving bones and joints. The organisms are difficult to demonstrate in direct smears and in histiologic sections, but they grow readily on routine fungal culture media. The cases of two patients, one with sporothrix arthritis and one with sporothrix arthritis and osteitis, are presented. The latter patient underwent ten surgical procedures over a period of 6 1/2 years and was treated for tuberculous arthritis without a definite diagnosis before fungal cultures were obtained and Sporothrix schenckii isolated.

Sarcomas of bone complicating osteitis deformans (Paget's disease): fifty years' experience.

Osteitis deformans (Paget's disease of bone) may be complicated by osseous cancer. Of 3964 patients between 1927 and 1977, 38 had primary malignant bone tumors. This represents an incidence of sarcomatous change of 0.95%. The bones of the pelvis were most often involved; the humerus and the femur were next in frequency. No region of the skeleton was spared, with the exception of the forearms and hands. Of the 38 tumors, six were fibrosarcomas and 32 were osteosarcomas. Irrespective of the site of involvement, the histopathologic extremely poor. The 5-year survival rate was 7.9%, with a mortality rate of 86.8% 36 months after diagnosis. The main cause of death was pulmonary metastasis or local extension of tumor growth. Three patients were long-term survivors, but no clinical or pathologic elements could be identified which differentiated these survivors from the other patients in the study.
