Hemihypoplasia of the mandible combined with lateral open bite corrected by sandwich technique. A case report.

A hemihypoplasia of the body of the mandible and the ascending ramus on the left side, combined with a lateral open bite on the right side is described. Operative reconstruction on the left side was performed by sandwich technique. The correction of the lateral open bite involved the following: vertical ramus osteotomy on the right side, paramedian vertical body osteotomy on the left side, moving the body of the mandible upwards into the predetermined position.

Skeletal and dento-alveolar stability of Le Fort I intrusion osteotomies and bimaxillary osteotomies in anterior open bite deformities. A retrospective three-centre study.

A sample of 267 patients with maxillary hyperplasia, a Class I or Class II/I occlusion and anterior vertical open bites, collected from three different institutions, was analysed regarding stability after surgical corrections. Skeletal and dento-alveolar stability of the maxilla, and positional changes of the mandible and of the incisors were evaluated. All patients underwent Le Fort I intrusion osteotomies and in 92 patients segmentation of the maxillae was performed. An additional bilateral sagittal split advancement osteotomy was performed in 123 patients. Intraosseous wire fixation was used in 153 patients and rigid internal fixation in 114 patients. Cephalometric radiographs were collected before orthodontic treatment, before surgery, immediately after surgery, one year postoperatively and at the latest follow up. The mean follow up was 69 months (range 20-210 months). It can be concluded that patients with anterior open bites, treated with a Le Fort I osteotomy in one-piece or in multi-segments, with or without bilateral sagittal split osteotomy, exhibited good skeletal stability of the maxilla. Rigid internal fixation produced the best maxillary and mandibular stability. The mean overbite at the longest follow up was 1.24 mm and a lack of overlap between opposing incisors was present in 19%. The overbite did not differ significantly between the different treatment procedures, probably due to compensatory movements of the mandibular and maxillary incisors.

Tongue and jaw muscle activity in response to mandibular rotations in a sample of normal and anterior open-bite subjects.

In order to investigate the effect of mandibular rotations in human subjects, the electromyographic activity from the left GG, left masseter, and left orbicularis oris muscles was recorded during voluntary opening movements of the mandible that were monitored by an electronic transducer. For each muscle, a computer-based system was used to calculate a threshold incisor-separation position corresponding to a 20 percent increase in base line muscle activity. The mean threshold level for the GG muscle in a sample of twenty subjects with normal occlusions was 53.0 percent (S.D. +/- 22.8)of maximum jaw opening. By contrast, a sample of nine subjects with anterior open-bite malocclusions had a statistically significant (p less than 0.001) lower mean threshold of 5.9 percent (S.D. +/- 4.5) of maximum jaw opening. No significant differences in masseter or orbicularis oris muscle activity were found between the two samples. Since postural tongue activity could play an important role in the development of the anterior dentition, the lower threshold for the GG muscle activity in response to jaw opening in anterior open-bite subjects may be of significant clinical importance.

Orthopedic approach in the treatment of a skeletal class II division 1 malocclusion with an anterior open bite.

The patient presented with a skeletal class II malocclusion characterized by an anterior open bite and maxillary midline deviation. This mixed dentition case was treated orthopedically with MRI appliance to rotate and impact the maxilla. A Bionator was used advance the mandible. The case was completed using Occlus-O-Guide. The result showed that the facial bones and teeth appear in the correct position.

The stability of the use of bilateral sagittal split osteotomy in the closure of anterior open bite.

This paper is a preliminary report on the stability of the use of bilateral sagittal split osteotomy and rigid internal fixation (2-mm bicortical screws) in the closure of anterior open bite. Ten patients who had finished orthodontic treatment and were at least 1 year out of surgery were evaluated by cephalometric analysis for dental and skeletal changes. One angular and three vertical measurements were evaluated cephalometrically at three times for each patient: immediately preoperatively, a maximum of 1 week postoperatively, and after a minimum of 12 months of follow-up. The results of this small sample seemed to indicate that the use of bilateral sagittal split osteotomy and 2-mm bicortical screws for the closure of the anterior open bite is a relatively stable procedure.

Stomatognathic function in juvenile rheumatoid arthritis and in developmental open-bite subjects.

Patients affected with juvenile rheumatoid arthritis present clear radiological alterations of the condyles of the temporomandibular joint (TMJ) due to the inflammatory process, with a prevalence ranging from 17 percent to 63 percent. This work is a comparison between a group of subjects with juvenile rheumatoid arthritis presenting signs and symptoms of TMT dysfunction and a group of the same age not suffering from any rheumatic morbidity, but presenting an open bite > 3 mm. The aim is to verify whether the open bite can induce an alteration in mandibular function comparable to that observed in juvenile rheumatoid arthritis. Statistically significant difference is found in the juvenile rheumatoid arthritis group only regarding spontaneous and provoked muscle pain and the lateral opening deviations of the mouth, but mostly the two groups seem alike and present the same stomatognathic pattern.

Treatment of anterior skeletal open-bite deformity.

Two patients with severe skeletal anterior open-bite have been presented. Treatment of these patients was accomplished mainly by mandibular repositioning (Case 1 by orthodontic means and Case 2 by a combination of surgery and orthodontics) and by an orthodontic change in the mandibular occlusal plane. A review and a discussion of the cause of this deformity were presented.

Relapse following surgical treatment of anterior open bite.

From a series of 40 cases of anterior open bite treated surgically by Le Fort I osteotomy, four cases relapsed. The nature of the relapse was investigated by computer aided analysis of lateral cephalometric radiographs. The location, aetiology and possible prevention of the relapse is discussed for each case.

Open bite deformity in amelogenesis imperfecta. Part 2: Le Fort I osteotomies and treatment results.

Functional conditions, skeletal and dento-alveolar stability and condylar changes in 15 patients with mandibular hypoplasia, anterior open bite (AOB) and amelogenesis imperfecta (AI), who had undergone a Le Fort I osteotomy, were analysed after a mean follow-up of 5 years. Two patients underwent a one-piece Le Fort I intrusion osteotomy and 13 patients a multi-segment Le Fort I osteotomy. In three of these patients, an additional bilateral sagittal split osteotomy was performed. Thirteen patients underwent a genioplasty. Surgery was followed by prosthetic rehabilitation in 10 patients. Skeletal and dento-alveolar stability were analysed on lateral cephalometric radiographs and condylar changes on orthopantomographic radiographs. Transverse stability of the dental arches was analysed on dental casts. The treatment results in this group were compared with patients with similar skeletal features but without amelogenesis imperfecta. The harmony of the long faces was restored and a reasonable vertical stability of the maxilla was achieved, however, a slight open bite and tongue interposition was still present. The transverse stability of dental arches (60%) was disappointing. Rigid internal fixation produced better transverse stability. Progressive condylar resorption was seen in two patients (13%). Less occlusal stability could be achieved in patients with AI, but resulted neither in less skeletal stability nor in more susceptibility to morphological condylar changes.

Elastic activator for treatment of open bite.

This article presents a modified activator for treatment of open bite cases. The intermaxillary acrylic of the lateral occlusal zones is replaced by elastic rubber tubes. By stimulating orthopaedic gymnastics (chewing gum effect), the elastic activator intrudes upper and lower posterior teeth. A noticeable counterclockwise rotation of the mandible was accomplished by a decrease of the gonial angle. Besides the simple fabrication of the device and uncomplicated replacement of the elastic rubber tubes, treatment can be started even in mixed dentition when affixing plates may be difficult.

The functional treatment of anterior-open bite: three case reports.

Functional treatment of three skeletal open-bite patients with Bionator, Frankel IV and Posterior bite-block appliances is presented. Pretreatment and post treatment records were evaluated. Correction of malocclusion by changing direction of the mandibular development, adaptive bony development of the condyle, vertical development in posterior region, an increase in ramus length, a decrease in the lower facial height were observed in this report. The results of functional treatment of three appliances had different effect mechanisms.

[The changing of orofacial structure by bite opening with Begg technique].

Cephalometric study of 30 deep-bite cases treated with extraction of four first premolars and Begg stage I appliance shows that the bite opening consists of not only intrusion of the upper and lower incisors but also extrusion of the upper and lower molars, furthermore, the lower incisors depressed more than the upper ones and the lower molars are elevated more than the upper ones. The moving type of teeth, the changing of base bone and soft tissue profile are also discussed in this article.

Long-term stability of treatment results after upper jaw segmented osteotomy according to Schuchardt for correction of anterior open bite.

Upper jaw osteotomy according to Schuchardt is a relatively rarely performed surgical procedure due to its narrow range of indications. Within the framework of a clinical follow-up examination using this method, we studied the long-term results in 26 patients, employing cephalometric analysis and analyzing the clinical findings. The results of this study show that the outcome of the operative correction of anterior open bite remained largely stable. Although partial relapse occurred in 3 cases (= 11.5%), there was a significant functional and esthetic improvement in comparison with the preoperative initial findings. When indicated, upper jaw osteotomy according to Schuchardt is thus a suitable surgical technique for correction of anterior open bite.

Cephalometrics of anterior open bite: a receiver operating characteristic (ROC) analysis.

A new approach is presented for the evaluation of cephalometric measurements in which a measurement is considered to be a diagnostic test for the presence or the absence of some component of malocclusion. This approach allows cephalometric measurements to be judged by the criteria that are generally used for clinical diagnostic tests, including determination of sensitivities, specificities, positive and negative predictive values, and, most important, receiver operating characteristic (ROC) curves. In this study, ROC curves are generated for the relationship between several skeletal cephalometric measurements and anterior dental open bite in a sample of 1541 orthodontic patients. The overbite depth indicator is found to be a better diagnostic criterion for the presence of dental open bite than any other commonly used skeletal cephalometric measurement or ratio. ROC curves are of substantial value for evaluating the diagnostic information of cephalometric measurements.

Lateral open bite resulting from acute temporomandibular joint effusion.

Two cases of acute open bite, one arising from trauma to the mandible without actual fracture, and one from an acute episode of rheumatoid arthritis, are presented. The resultant malocclusions were due to a sudden increase of volume in the temporomandibular joint space with subsequent displacement of the mandibular condyle, following acute effusion.
