Isolation of an integrated provirus of Moloney murine leukemia virus with long terminal repeats in inverted orientation: integration utilizing two U3 sequences.

We have previously described the construction of a mutant of Moloney murine leukemia virus, in594-2, which carries a 2-base-pair insertion in the U5 region of the genome and is partially defective in forming the integrated proviral DNA. We have now recovered a cloned copy of an unusual provirus from rat cells infected with this mutant. The viral genome is flanked by long terminal repeats in inverted orientation, with U3 sequences joined to cellular DNA at both of the outer edges. In addition, the provirus is a recombinant, containing a segment of a VL30 element in inverted orientation in place of the Moloney murine leukemia virus env region. The recovery of this provirus indicates that two U3 regions can be used for viral integration and suggests that there may be no absolute requirement in the reaction for those U5 sequences outside the 13-base-pair inverted repeats.

Evaluation of mating systems involving five breeds for integrated beef production systems: III. Integrated system.

Angus (A), Charolais (C), Hereford (H), Limousin (L), and Simmental (S) breeds were included in deterministic computer models simulating integrated cow-calf-feedlot production systems. Three mating systems were used: pure-breeding and two-and three-breed rotational crossbreeding. Breed information was taken from the literature. Herd sizes were unrestricted; however, 100 heifers were saved as replacements. Cows were removed for reproductive failure, age (greater than 10.5 yr), or death. Calves produced in the cow-calf segment were fed in a custom feedlot to four slaughter end points: 440 d, 457 d, 288-kg carcass weight, and low Choice. All animals were fed to requirements. Cull cows were slaughtered after weaning. Biological and economic efficiencies improved with crossbreeding; however, rankings of breed combinations depended on how efficiencies were measured (weight, lean, or value basis). Among purebreds, reproductive performance had a large influence on breed rankings at age and weight end points, whereas feedlot performance was important at the low Choice end point. Crossbred combinations involving British (A or H) and Continental (C or S) breeds were more efficient than other crossbred combinations at all end points. However, choosing specific breed combinations for integrated systems depends on slaughter end points, market end points (weight vs lean), and measures of efficiency (weight, lean, or value basis).

Neuropsychological performance and integrated evaluation for disabled people using Virtual Reality: integrated VR profile.

This chapter describes a Virtual Reality (VR) based innovative model of evaluation of the performance and potentiality of young mentally/psychically disabled subjects with learning difficulties. Using an immersive PC-based VR system, the study investigated the characteristics of 150 disabled subjects in the EU funded project "Horizon O.D.A.--Catania-1998--2000". The result is the definition of an individual neuropsychological "Integrated Profile", based on VR performance, that allows an objective functional benchmark between different subjects. This model can be used to investigate the possibility of job integration for mentally/psychically disabled subjects.

[Multi-origin usage for chromosome replication of suppressive integration strain of dnaA46 mutant of Escherichia coli integrated with R6K].

The chromosome of temperature sensitive initiation mutant dnaA46 of Escherichia coli K-12 fails to replicate at 42 degrees C. Suppressive integration (Sin) strain integrated with the R6K plasmid was screened at 42 degrees C. Marker frequency determination of the Sin strain reveals that replication was initiated at the normal site of initiation at 30 degrees C, while at three different sites at 42 degrees C. Two of the sites have been reported in the stable DNA replication mutant, one of them is a novel site. Inhibition of chromosome replication was not observed for the recA derivative of Sin initiated at two of the initiation sites close to the normal initiation site. Inhibition was observed for chromosome replication initiated near the site where chromosome replication normally terminates. It indicates that chromosome replication initiated at sites close to the terminus is recA gene dependent.

[Public vocational integration: a new method for integrating psychiatrically handicapped patients into occupational life?].

Suitable conditions of employment represent a central factor in the rehabilitation of mentally ill people. In this case both the normality of the job--as felt by the individual--and adequate instrumental and social working conditions are determining factors. According to the ideas of the Workshop for the Handicapped (Werkstatt für Behinderte) and the movement for the Self-help Firms, the integration of the mentally ill into normal jobs is an important step in the right direction. Appropriate preparations (i.e. assessment of patients' working capabilities and the drawing up of expectation profiles, work-preparation measures) and well-organized follow-up care are required to achieve this. Statutes which make access easier must be created in the fields of both labour and social law in order to make integration into the labour market possible. In this respect the non profit-making provision of employees (Gemeinnützige Arbeitnehmerüberlassung) represents a model. The procedure has been described and first experiences with its realization have been reported upon.

Human herpesvirus 6 (HHV-6) is transmitted from parent to child in an integrated form and characterization of cases with chromosomally integrated HHV-6 DNA.

We obtained 7,566 peripheral blood mononuclear cell (PBMC) samples from 2,332 individuals and screened them for human herpesvirus infection. We identified five individuals who persistently harbored high copy numbers of human herpesvirus 6 (HHV-6) DNA in their PBMCs. HHV-6 DNA was also detected in other somatic tissues of these individuals. Five additional cases were identified among their family members. For two of these families, chromosomally integrated HHV-6 DNA (CIHHV-6) was detected in the PBMCs by fluorescence in situ hybridization. The prevalence of CIHHV-6 among all the subjects was 0.21%. The HHV-6 DNA was variant B in four families and variant A in one family. Antibodies to immediate early antigen and glycoprotein B were detected in 57 and 14% of individuals with CIHHV-6 and in 0 and 60% of healthy volunteers without CIHHV-6, respectively. HHV-6 could not be isolated from PBMCs with CIHHV-6. These cases shared no clinical features, and included three healthy individuals. Our data suggest that CIHHV-6 is rare but detectable in the general population and that hereditary transmission is one of the routes of HHV-6 transmission.

The newly discovered Ini1 cellular protein binds HIV-1 integrase and is part of a protein complex thought to alter nucleosomal structure; such alterations may influence the selection of sites for HIV-1 DNA integration.

Virtual integrated practice: integrating teams and technology to manage chronic disease in primary care.

The use of teams in health care has generally relied on the notion that teams must physically meet and function in person in the same location, on a regular, scheduled basis, in order to maximize the value of the interdisciplinary process. This article examines the concept of creating a different kind of team in primary care, out-patient settings--one which relies upon communications technology to link together clinicians from different locations to coordinate and manage the care of patients, particularly those with chronic disease. This approach--referred to as Virtual Integrated Practice--is designed to overcome the barriers of traditional in-person teams by creating a "virtual team" with the potential to function more efficiently, productively, and satisfactorily for clinicians and patients alike.

The integration host factor (IHF) integrates stationary-phase and virulence gene expression in Salmonella enterica serovar Typhimurium.

The integration host factor (IHF) is a DNA-binding and -bending protein with roles in local DNA structural organization and transcriptional regulation in Gram-negative bacteria. This heterodimeric protein is composed of the two highly homologous subunits IHFalpha and IHFbeta. DNA microarray analysis was used to define the regulon of genes subject to IHF control in Salmonella enterica serovar Typhimurium (S. Typhimurium). The transcription profile of the wild type was compared with those of mutants deficient in IHFalpha, IHFbeta, or both IHFalpha and IHFbeta. Our data reveal a new connection between IHF and the expression of genes required by the bacterium to undergo the physiological changes associated with the transition from exponential growth to stationary phase. When a mutant lacking IHF entered stationary phase, it displayed downregulated expression of classic stationary-phase genes in the absence of any concomitant change in expression of the RpoS sigma factor. Purified IHF was found to bind to the regulatory regions of stationary-phase genes indicating an auxiliary and direct role for IHF in RpoS-dependent gene activation. Loss of IHF also had a profound influence on expression of the major virulence genes and epithelial cell invasion, indicating a role in co-ordinating regulation of the pathogenic traits with adaptation to stationary phase. Although the three mutants showed considerable overlaps in the genes affected by the ihf lesions, the observed patterns were not identical, showing that S. Typhimurium has not one but three overlapping IHF regulons.

Integrated self-inactivating lentiviral vectors produce full-length genomic transcripts competent for encapsidation and integration.

To make human immunodeficiency virus type 1 (HIV-1)-based vectors safer for use in the research and clinical setting, a significant modification to the HIV-1 genome has been the deletion of promoter and enhancer elements from the U3 region of the long terminal repeat (LTR). Vectors containing this deletion are thought to have no LTR-directed transcription and are called self-inactivating (SIN) lentivectors. Using four distinct approaches, we show that SIN lentivectors continue to have promoter activity near the 5' LTR, which is responsible for the production of full-length vector transcripts. To verify that transcripts derived from the LTR in SIN lentivectors are competent for encapsidation and integration, we transduced a lentiviral packaging cell line with a SIN lentivector and then observed the production of viable vector particles containing full-length SIN lentivector genomes. We have also attempted to identify sequences in the SIN lentivector which are responsible for transcriptional activation at the 5' LTR. Using different segments of the vector LTR and leader region in a promoter assay, we have determined that the residual promoter activity is contained entirely within the leader region and that, although this element is downstream of the transcription initiation site, it is capable of initiating transcription from the 5' end of R in the LTR. Mutation of leader region binding sites for the transcriptional activators downstream binding factor 1 (DBF1) and SP1 reduces transcription from the SIN LTR by up to 80%. Knowledge of the potential for mobilization of HIV-1-derived SIN lentivectors will be important for the design of future gene therapy trials with such vectors.

Integrating quality management into clinical systems: tools for achieving clinical integration.

This article describes the use of quality systems as a tool for managers of health-care clinical services to establish a common framework across the different disciplines in the continuum of care. Quality systems used internationally in the manufacturing and service sectors are applicable to multidisciplinary clinical systems management because the essential elements of quality are universal. These elements have been redefined into health-care terminology and can be used by clinical systems managers as a basis for implementing a quality system. The structure for documents in a quality system also has been adapted to health care and allows for a common set of quality policies and processes to unify the different clinical services. By using a consistent quality management approach and documentation, managers can transcend technical operational differences and ensure effective clinical integration.

IHE: integrating the healthcare enterprise, towards complete integration of healthcare information systems.

Information systems of a modern hospital govern extremely important functions as patient management, control of work flows, administration etc. However a great variety of recommended standards are used while in most cases no effective coordination and intercommunication is possible. Some years ago to simplify and resolve this problem IHE was created; it is a technical framework which identifies a number of components of the healthcare enterprise (the "actors") whose interactions are defined in terms of "transactions", that should be implemented according to HL7 and DICOM standards. Attempting an in-depth understanding of IHE structure and its principles of function, those transactions that can occur among the various actors, which of these are supported and which require specific actors, are analyzed. IHE is continuously evolving, therefore open to the factual contribution of all health professionals who can point out the main instances that emerge daily from their activity in the field. While its origin was specific for the field of radiology, the true soul and driving force of the project lay on the type of approach used: to get together medical specialists and information technology professionals in order to identify and solve the difficulties which hindered an effective and functional integration of healthcare information systems.

Integration of behavioral and relaxation approaches into the treatment of chronic pain and insomnia. NIH Technology Assessment Panel on Integration of Behavioral and Relaxation Approaches into the Treatment of Chronic Pain and Insomnia.

OBJECTIVE: To provide physicians with a responsible assessment of the integration of behavioral and relaxation approaches into the treatment of chronic pain and insomnia. PARTICIPANTS: A nonfederal, nonadvocate, 12-member panel representing the fields of family medicine, social medicine, psychiatry, psychology, public health, nursing, and epidemiology. In addition, 23 experts in behavioral medicine, pain medicine, sleep medicine, psychiatry, nursing, psychology, neurology, and behavioral and neurosciences presented data to the panel and a conference audience of 528 during a 1 1/2-day public session. Questions and statements from conference attendees were considered during the open session. Closed deliberations by the panel occurred during the remainder of the second day and the morning of the third day. EVIDENCE: The literature was searched through MEDLINE, and an extensive bibliography of references was provided to the panel and the conference audience. Experts prepared abstracts with relevant citations from the literature. Scientific evidence was given precedence over clinical anecdotal experience. ASSESSMENT PROCESS: The panel, answering predefined questions, developed their conclusions based on the scientific evidence presented in open forum and the scientific literature. The panel composed a draft statement that was read in its entirety and circulated to the experts and the audience for comment. Thereafter, the panel resolved conflicting recommendations and released a revised statement at the end of the conference. The panel finalized the revisions within a few weeks after the conference. CONCLUSIONS: A number of well-defined behavioral and relaxation interventions now exist and are effective in the treatment of chronic pain and insomnia. The panel found strong evidence for the use of relaxation techniques in reducing chronic pain in a variety of medical conditions as well as strong evidence for the use of hypnosis in alleviating pain associated with cancer. The evidence was moderate for the effectiveness of cognitive-behavioral techniques and biofeedback in relieving chronic pain. Regarding insomnia, behavioral techniques, particularly relaxation and biofeedback, produce improvements in some aspects of sleep, but it is questionable whether the magnitude of the improvement in sleep onset and total sleep time are clinically significant.

Integrated Academic Information Management Systems (IAIMS). Part II. Planning and implementing integrated information services. The management of change: lessons learned from the IAIMS experience.

The critical elements of the change process were designed into the strategic planning process and the pilot project for the Integrated Academic Information Management System (IAIMS) at the University of Maryland. These elements were: Support by the institutional leadership; a critical mass of interested participants from diverse groups across the organization, committed to the project and with ownership of the plan; a motivating level of dissatisfaction with the status quo; the construction of a scenario describing the desired future and an assessment of needs to achieve it; technical and consulting help; a pilot project with replicable features to demonstrate the concept and feasibility of the approach; and participation of opinion leaders initially with later identification of additional opinion leaders who would become part of the pattern of acceptance of the innovation and diffusion of the technology across the campus.

Integrated Academic Information Management Systems (IAIMS). Part II. Planning and implementing integrated information services. IAIMS infrastructure: technological base.

The IAIMS concept calls for the logical integration of information derived from databases in the functional areas of administration, clinical care, education, libraries, and research. To accomplish this task a technological base must be established. It is suggested that such an infrastructure will consist of a systems component, the linked databases component, and a management component. An IAIMS should rapidly evolve into an information utility and, within a decade, provide a knowledge management system.

The Consensus Conference and Combined-Integrated model of doctoral training in professional psychology--overview of part 1: nature and scope of the Combined-Integrated model.

This special series of articles on the Consensus Conference and Combined-Integrated (C-I) model of doctoral training in professional psychology consists of 13 articles in two successive volumes of the Journal of Clinical Psychology. Six articles are presented in Part 1 (Vol. 60, Issue 9), which collectively describe the "nature and scope" of the C-I model (e.g., historic and definitional issues; the potential advantages of this model; implications for the profession). In Part 2 of this special series (Vol. 60, Issue 10), articles 7 through 12 address the broader implications and potential applications of the C-I model within a range of professional and societal contexts (e.g., for interprofessional collaboration; the health care field; development of a global curriculum; the unified psychology movement; issues of assessment and professional identity; and higher education); article 13 provides a summary of the series as well as a discussion of future directions. As an overview, this paper provides the abstract for each of the articles in Part 1, and describes the various topics of the articles in Part 2. Taken together, the articles in this special series are designed to provide a coherent account of how and why the C-I model is timely and relevant, and therefore warrants serious consideration by the larger education and training community in professional psychology.

Directed integration of an F' plasmid by integrative suppression: isolation of plaque forming lambda transducing phage for the dnaC gene.

A new approach for isolation of a plaque forming lambda specialized transducing phage is described. It consists of directed transposition of an F' plasmid into the gal region of a dnaAts galE- Escherichia coli strain by integrative suppression and deletion of the chlD region in order to shorten the distance between the marker of interest on the F' and the prophage serving to prepare an LFT1 lysate. An F' danC+thr+ plasmid was used here and lambdadthr and lambdaddnaC phages were isolated. In addition, lambdapdnaC was obtained from a double lysogen for lambdaddnaC and lambda b2.
