Changes in the rheologic properties of blood after a high altitude expedition.

The changes in blood rheologic properties induced by exposure to high altitude plus heavy and prolonged physical strain were investigated in 14 climbers of an Italian expedition to Himalaya (maximum altitude reached m 7350). In blood samples collected immediately after return, alterations of some haemorheologic parameters were observed in comparison with baseline values, and namely an increase in relative blood viscosity, fibrinogen and erythrocyte filtration time along with a fall in platelet count. Treatment with a drug acting on the red cell deformability (Pentoxifylline) appeared to prevent the changes in blood viscosity and erythrocyte filterability. In the 9 climbers whom we were able to re-examine after three months, all altered parameters were found to be restored to baseline values. These results contribute to the knowledge of the pathogenesis of high altitude disease and stress the significance of altered haemorheology as one of the mechanisms possibly involved in the vascular events observed in this condition.

Enteral methadone to expedite fentanyl discontinuation and prevent opioid abstinence syndrome in the PICU.

STUDY OBJECTIVE: To determine if enterally administered methadone can facilitate fentanyl discontinuation and prevent withdrawal in children at high risk for opioid abstinence syndrome. DESIGN: Retrospective analysis. SETTING: Pediatric intensive care unit (PICU) in a tertiary care children's hospital. PATIENTS: Twenty-two children (aged 6.1 +/- 5.4 yrs) who received continuous fentanyl infusion for 9 days or longer. INTERVENTION: Guidelines for initiating enteral methadone, rapidly tapering and discontinuing fentanyl infusions, and tapering methadone were implemented in the PICU. Development of opioid abstinence syndrome was evaluated during fentanyl and methadone dosage reductions and for 72 hours thereafter. MEASUREMENTS AND MAIN RESULTS: Children received fentanyl by continuous infusion for 17.8 +/- 8.4 days. Peak fentanyl infusion rate was 5.9 +/- 3.8 microg/kg/hour, and the median cumulative dose was 1302 microg/kg (range 354-7535 microg/kg). Methadone 0.50 +/- 0.22 mg/kg/day was begun 1.6 +/- 1.9 days before tapering fentanyl. The fentanyl infusion rate on starting the taper was 5.0 +/- 3.6 microg/kg/hour. Fentanyl was tapered and discontinued in a median of 2.6 days (range 0-11.9 days). Twenty-one patients had no opioid abstinence syndrome during or after fentanyl taper. One patient experienced significant opioid withdrawal after fentanyl discontinuation, which resolved after reinstitution of fentanyl and increasing the dosage of methadone to 0.3 mg/kg every 6 hours. Overall, methadone was tapered and discontinued in 18.2 +/- 11.9 days without precipitating opioid abstinence syndrome. CONCLUSION: Enteral administration of methadone may expedite fentanyl discontinuation and reduce the risk of withdrawal in critically ill children at high risk for opioid abstinence syndrome.

Comparison of the RT3 Research Tracker and Tritrac R3D accelerometers during a backpacking expedition by a single subject.

This study compared the RT3 Research Tracker accelerometer and the Tritrac R3D accelerometer in a field setting. A six-day backpacking expedition (122.4 km in length) was completed by a single subject in the Grand Canyon National Park, Arizona. The overall correlation between the counts of vector magnitude activity for the RT3 and R3D was moderate (r =.75, p<.001), with the overall calculated bias [mean difference (RT3 minus R3D) and standard deviation of the differences] across all six days estimated at 235+/-436 vector magnitude activity counts. However, agreement between the instruments is problematic; the RT3 might be 201 activity counts below or 671 activity counts above the R3D in assessing physical activity during backpacking.

Morbidity and health survey of wintering members in Japanese Antarctic research expedition.

STUDY DESIGN: The Japanese Antarctic Research Expedition (JARE) started in 1956. Syowa Station is the mother station of JARE at 69 degrees 00'S and 39 degrees 35'E in East Antarctica. An epidemiological survey of the wintering team of JARE was carried out based on the annual reports of JARE over the period 1956-2001. METHODS: The total number of personnel was 1,236 including 3 females. The age of personnel was from 21 to 56 (average 33.1). We examined the proportion of personnel who had contracted disease with healthy personnel, as reported by the medical department at Syowa as well as a health survey of the latest party by biochemical analyses. RESULTS: The total number of diseases was 4,760. The ratios of surgical-orthopedic, internal medical, and dental cases were 45%, 23%, and 13%, respectively. Only one death from a blizzard was recorded. In the latest party, serum levels for triglyceride, calcium, and gamma-glutamyl transpeptidase increased during the wintering period. CONCLUSION: The very low mortality of JARE may be due to the effective personnel selection and that there have been no severe accidents. There has been no deterioration in nutritional parameters in JARE recently.

Medicare program; establishment of an expedited review process for Medicare beneficiaries enrolled in health maintenance organizations, competitive medical plans, and health care prepayment plans--HCFA. Final rule with comment period.

This final rule with comment period establishes a new administrative review requirement for Medicare beneficiaries enrolled in health maintenance organizations (HMOs), competitive medical plans (CMPs), and health care prepayment plans (HCPPs). This rule implements section 1876(c)(5) of the Social Security Act, which specifies the appeal and grievance rights for Medicare enrollees in HMOs and CMPs. This rule requires that an HMO, CMP, or HCPP establish and maintain, as part of the health plan's appeals procedures, an expedited process for making organization determinations and reconsidered determinations when an adverse determination could seriously jeopardize the life or health of the enrollee or the enrollee's ability to regain maximum function. This rule also revises the definition of appealable determinations to clarify that it includes a decision to discontinue services.

Medicare program; Provider Reimbursement Review Board: expedited administrative review--Health Care Financing Administration. Final rule with comment period.

Current Medicare regulations permit providers of services to appeal reimbursement decisions of Medicare fiscal intermediaries to the Provider Reimbursement Review Board. Following the Board's decision, the provider may also request judicial review of that decision (and of any action by the Secretary with respect to that decision). These amendments to our regulations permit the provider to obtain expedited administrative review when the Board determines that it lacks authority to decide a question of law, regulation, or HCFA ruling relevant to the case. The question of whether authority is lacking may be raised by the provider or on the Board's own motion. The regulations eliminate the current requirement for a Board hearing before judicial review when the Board lacks authority to decide an issue, and they establish time limits within which the Board must act. These rules implement section 955 of the Omnibus Reconciliation Act of 1980, Pub. L. 96-499.

[Recovery and use of deuterium-free water in extended space expeditions].

The methods of deuterium-free water, as applied to the conditions of long-term space expeditions have been discussed. It is shown that with the use of the method of electrolysis of distilled water or urine condensate followed by the catalytic or high-temperature synthesis one can obtained water with four-fold decrease in deuterium concentration. In order to reduce the energy expenditure it is necessary to use the existing routine technologies of water and oxygen regeneration or the versions with applying the fuel cells in which hydrogen and oxygen resulting from electrolysis would generate the electrical power with simultaneous isolation of deuterium-free water.

Expediting the transfer of evidence into practice: building clinical partnerships.

A librarian/clinician partnership was fostered in one hospital through the formation of the Evidence-based Practice Committee, with an ulterior goal of facilitating the transfer of evidence into practice. The paper will describe barriers to evidence-based practice and outline the committee's strategies for overcoming these barriers, including the development and promotion of a Web-based guide to evidence-based practice specifically designed for clinicians (health professionals). Educational strategies for use of the Web-based guide will also be addressed. Advantages of this partnership are that the skills of librarians in meeting the needs of clinicians are maximized. The evidence-based practice skills of clinicians are honed and librarians make a valuable contribution to the knowledge-base of the clinical staff. The knowledge acquired through the partnership by both clinicians and librarians will increase the sophistication of the dialogue between the two groups and in turn will expedite the transfer of evidence into practice.

[Specific aspects of hemostasis and lipid peroxidation in thrombocyte membranes during expedition and shift organization of labor].

The aim of the present study is to investigate dynamics of the hemostasis system and structural and functional state of thrombocyte membranes by the LPO and AOD parameters under the conditions of expedition and shift organization of labour of worker tripping from the West Siberia middle latitudes to the Far North. A trend to hypocoagulemia and increases in LPO and AOD reactions were characteristic of the type-1 (adaptive) reaction. The type-2 (hypotensive) reaction included a trend to hypercoagulemia, an increase in LPO and a decrease in AOD reactions in thrombocyte membranes as well as a hypertensive reaction during the whole shift period. The type-3 (asthenic) reaction was described by hypocoagulemia, an increase in the amount of LPO products with a simultaneous decrease in the AOD activity and asthenic sleep disturbance.

[The herbariums from Spanish scientific expeditions in the New World].

The herbaria compounded by the botanical Spanish expeditions to the New world during the XVIIIth century constitutes one of the most important treasures of the Royal Botanical Garden from Madrid. These collections of dry plants document the existence of the species in time and in space, and they are essential resources for the botanical investigation. The specimens type and of reference of these collections are fundamental for the correct identification of each species and their varieties. They are used to give validity to the botanical investigation securing that one could repeat or compare with future investigations.

Expedited permanency planning: evaluation of the Kentucky Adoptions Opportunities Project.

This article presents the evaluation findings of a Kentucky Adoptions Opportunities Project (KAOP), a three-year project funded by the U.S. Department of Health and Human Services, Administration on Children, Youth and Families, Children's Bureau. The primary goal of the KAOP was implementation of three permanency planning activities: (1) risk assessment/concurrent planning, (2) one child/one legal voice, and (3) early placement in kinship or foster/adoptive homes. These activities were designed to expedite a permanency placement decision within 12 months for high-risk children. The evaluation of 124 high-risk children in the KAOP revealed that the majority of children had one or both parents coping with multiple risk factors, including mental illness, substance abuse, mental retardation, or family violence. The major barriers to permanency are discussed, as well as the policy and practice implications in the context of Adoption and Safe Families Act of 1997.

["P" as in Program Package. About torturing data and significant "fishing expeditions"].

For the study of prognostic factors, the medical researchers have access to a number of advanced statistical techniques available in standard program packages. A tradition has developed where survival or time to relapse is analysed on the basis of statistical materials with few patients but a large number of possible explanatory variables. In statistical "fishing expeditions" the p-values are used to sort out potentially useful prognostic variables. Since the number of observations is small, all relevant prognostic factors do not give statistical significance. Since a large number of variables are tested there is a considerable risk for spurious significances. It is not enough to show that a prognostic factor seems to be efficient in the patient group where it was first found. The result must be verified in further studies of independent groups of similar patients.
