Finally, let me emphasize that the National Medical Association stands ready to assist the Congress in structuring and implementing any new programs that may result from this gathering. We do have a concern regarding the lack of basic race-specific data, including the extent of long-term care problems in the black communities. It is widely believed that an improved data base to assess the needs and to deal with the issues confronting the black and other minority or economically disadvantaged patient would greatly help to improve existing modes of treatment and outcome.

The influence of a Federal comprehensive health care system on the detection and outcome of primary small bowel malignancies.

Twenty-one cases of primary small bowel malignant tumors treated at our institution from 1983 to 1993 were reviewed. The mean age at diagnosis was 51.6 +/- 16.8 years. Twelve patients (57%) reported symptoms of less than 1 month duration. Diagnosis was made at laparotomy in 13 patients (62%), and nine patients (43%) had three or more preoperative studies. Five patients (24%) presented with abdominal emergencies. The 5-year survival rate for the series was 19 per cent. This study was performed at a tertiary care military hospital where patients and physicians are not subjected to the financial constraints of civilian health care. This system should eliminate delays in seeking medical care and expedite diagnosis. Despite almost immediate medical attention for a majority of the patients, overall survival is not significantly different from that in previous reviews. This study emphasizes that the presentation of small bowel malignancies is indolent and difficult to diagnose. Prognosis remains poor despite the patient cost-free system and almost immediate medical attention. This study suggests that a high index of suspicion and a thorough evaluation, including laparotomy, are required to improve outcome.

Child health care centres: an academic model for comprehensive child health care in the community.

The challenge of the discipline of paediatrics in the 21st century is to promote health and development of children in a way that will enable them to maximize their biological and social potential. The community child health centre (CHC) in Israel is a model of community health care service built to provide comprehensive health care to children and adolescents, as well as an academic setting for under- and postgraduate paediatric training. Today there are 34 CHCs in Israel, serving a population of 220 000 children from birth to 18 years of age. The CHC combines the advantages of group practice with those of an academic medical centre and enables flexibility and mutual learning. Further expansion and development are required to realize the CHC's mission of a true comprehensive academic centre for paediatric community health.
