Ten-year course of early-onset Weber-Christian syndrome with recurrent pneumonia: a suggestion for pathogenesis.

A surviving 10-year-old boy with infant-onset systemic Weber-Christian syndrome is reported. He has had recurrent episodes of fever, aseptic panniculitis, and pneumonia. Although corticosteroid therapy has succeeded, colchicine and non-steroidal anti-inflammatory drugs have failed to abort or prevent acute episodes. The persistent leukocytosis (even during remission), the recurrent episodes of fever with associated increase in the acute phase reactants, and the failure of the nonsteroidal anti-inflammatory drugs leads us to propose that this form of Weber-Christian syndrome reflects an inborn error in the regulation of the inflammatory response. Systematic investigation of this hypothesis could yield important information on the normal regulation of inflammation and could lead to a rational therapeutic approach to this puzzling and usually devastating illness.

[Hand-Schüller-Christian syndrome arising in the mandible. A case report].

The history of Letterer-Siwe, Hand-Schüller-Christian disease, and eosinophilic granuloma of the bone, and the evolution of the concept of grouping them together under the common term of histiocytosis X are presented. The characteristics of the disease, with particular emphasis on oral manifestations, are reviewed. The therapy for the disease is briefly outlined. One case of Hand-Schüller-Christian syndrome is presented. The discussion is focused on the problem of chemotherapy in disseminated forms.

[Christian Wolff (1679-1754) and the medical concept of his time].

The 300th anniversary of the birthday of Christian Wolff gave the occasion to set his multilateral work in relation to the medical currents of his time and to analyse it from the standpoint of modern medicine. Especially in his early period at Halle University Christian Wolff develops a new research programme with essential accents for the future physician. His philosophical literature with the postulates containing in this are conformable to the aim of the predecessors of the French revolution and on the sector of the art of therapeutic forms the basis decisive improvements in organised public health.

[Weber-Christian disease associated with familial alpha-1-antitrypsin deficiency. Apropos of a case].

The authors report a case of Weber-Christian disease associated with familial alpha-1-antitrypsin deficiency in a 47-year-old woman. The serum alpha-1-antitrypsin level is low and the phenotype is Pi ZZ. The histological picture was consistent with Weber-Christian panniculitis, showing foci of fat cell necrosis with lipophagic granuloma and fibrosis. The same biological trait also exists in the propositus' sister who developed emphysema without cutaneous involvement. The pathogenesis probably rests on a particular reactivity to microtraumata with exacerbation of local inflammatory events due to a partial deficient control of proteolytic activity. The prognosis of the disease varies from a chronic recurrent process to a short fulminant course with possible fatal outcome. The treatment is at the present time ill-codified and any how only symptomatic. In this case, the propositus responded well to a high dose regimen of corticosteroids, 100 mg daily, but relapsed as the treatment was reduced to 50 mg daily.

Alcoholic hyalins (Mallory bodies) in a case of Weber-Christian disease: electron microscopic observations of liver involvement.

A 32-yr-old female who suffered from typical Weber-Christian disease developed hepatosplenomegaly and hepatic dysfunction. A liver biopsy specimen displayed many fat droplets and alcoholic hyalins (Mallory bodies). Electron microscopic hepatic injury was indicated by many fat droplets, alcoholic hyalins, marked deformities of the rough endoplasmic reticulum, and by some deformed nuclei and nuclear bodies. There has been no other report describing alcoholic hyalins in the hepatic changes of Weber-Christian disease. Additionally, based on electron microscopic observation, we assume that fatty changes were due to a relative reduction in lipoprotein synthesis in the deformed rough endoplasmic reticulum.

[The first graduate woman physicians. On the 225th anniversary of the graduation from Halle of Dorothea Christian Erxleben (1715--1762)].

Recently the question of the first practising and graduated at a university lady-physicians has repeatedly been put and has differently been answered in world literature. In these cases English author referred to Elizabeth Blackwell graduated in Geneva in 1849, who as the first woman received the title of M. D. Including the continental Europe at least three lady-physicians rank before Elizabeth Backwell: Dorothea Christiane Erxleben, Regina Josepha Henning-von Siebold and Charlotte Heiland-von Siebold. Recently the 225th day of promotion and examination of Dorothea Christiane Erxleben, the first German lady-physician graduated in Halle in 1754, came round. This was the occasion to describe the stage of the development of this lady-physician on the basis of the remaining documents.

Multifocal eosinophilic granuloma with skin ulceration. Histiocytosis X of the Hand-Schüller-Christian type.

A patient with multifocal eosinophilic granuloma or histiocytosis X of the Hand-Schüller-Christian type was diagnosed as having hidradenitis suppurativa on the basis of ulceration and draining sinuses of the axillary, perianal, and inguinal regions. He also had diabetes insipidus and thyroid and pulmonary disease. Microscopic examination of the thyroid, lungs, and involved skin indicated massive infiltration of these organs by histiocytes and eosinophils. Ulcerations of this type are a rare manifestation of Hand-Schüller-Christian disease. Surgical excision of the involved areas with healing by secondary closure produced good results.

[Fatal course of a fungal pneumonia in glucocorticoid-treated recurrent febrile nodular panniculitis; Pfeifer-Weber-Christian syndrome].

The history of a 63 yr. old woman with a serious form of Weber-Christian's disease is reported. During treatment with glucocorticosteriods and antibiotics bilateral condida pneumonitis developed, which ended fatally. A short review of Weber-Christian's disease is given, and the problems concerning its treatment are outlined. The systemic application of glucocorticosteroids in this disease should be reserved for the serious forms.

[Christian Wisbech--his hospital and surgery in the 1820s].

Christian Wisbech (1801-1869) was medical superintendent of "Bergen civile Sygehus" during the period 1825-1848. Some of his annual reports have been published. The hospital had 56 beds. The permanent staff included one "spisemester" (caterer) and two "sygeopvartersker" (nurses). Christian Wisbech's medicine was based on strict scientific principles and pathological anatomy. A post-mortem was performed on the deceased. Other current trends in medical practice at that time were foreign to him. It is assumed that he was inspired by Giovanni Morgagni (1682-1771) and John Hunter (1728-1793). His treatment was partly medical and partly surgical. Wisbech treated surprisingly few injuries. A possible explanation is the large number of "barbers and surgeons" in Bergen at that time. It was probably a centuries-long tradition among the population to go to such persons to be treated for broken bones, wounds and other complaints.

Paleoserology of the Christian population at Sayala (Lower Nubia): an evaluation of the reliability of the results.

A paleoserological study of human remains from the Christian cemeteries of Sayala (Lower Nubia, 6th-11th centuries A.D.) was carried out by applying two techniques (absorption-elution and haemagglutination-inhibition), on two types of substratum (bones and hair), in separate laboratories (Pisa and Prague). The aim of research was to evaluate the degree of repeatability of the results and the reliability of the final paleoserological diagnoses. The results indicated different degrees of repeatability depending on the differential sensitivity of the two techniques and on the nature of the substratum. The frequencies of concordant and discordant final diagnoses within and between the two laboratories and by the two methods were significantly different from those expected under the conditions of random distribution, indicating a consistency in results from different laboratories and methods. The reliability of the resulting gene and phenotype distributions is also supported by their consistency both with the theoretical Hardy-Weinberg equilibrium and with the present distribution of the ABO gene frequencies in Egyptian, Sudanese, and Nubian populations.

Cultural differences in the perception of magazine alcohol advertisements by Israeli Jewish, Moslem, Druze and Christian high school students.

This article describes the Israeli part of an international comparative research project conducted simultaneously in 1988 also in Australia and in the U.S.A., in order to determine the attitudes of high school students from different nations and cultures toward alcohol advertising. The Israeli study used a set of 64 magazine alcohol advertisements from 10 countries and with the help of an open-ended questionnaire examined differences and similarities in the perception of Israeli and foreign advertisements by Jewish, Moslem, Druze and Christian teenagers. Each response was coded twice. The first code indicated the general tone of the response and the second code indicated its theme. The article presents selected general results and conclusions. The results have implications for designing alcohol abuse education messages and can aid alcohol advertisers interested in responsible advertising to find advertising standards suitable to each culture.

Equity and resource allocation in health care: dialogue between Islam and Christianity.

Inequities in health and health care are one of the greatest challenges facing the international community today. This problem raises serious questions for health care planners, politicians and ethicists alike. The major world religions can play an important role in this discussion. Therefore, interreligious dialogue on this topic between ethicists and health care professionals is of increasing relevance and urgency. This article gives an overview on the positions of Islam and Christianity on equity and the distribution of resources in health care. It has been written in close collaboration and constant dialogue between the two authors coming from the two religions. Although there is no specific concept for the modern term equity in either of the two religions, several areas of agreement have been identified: All human beings share the same values and status, which constitutes the basis for an equitable distribution of rights and benefits. Special provisions need to be made for the most needy and disadvantaged. The obligation to provide equitable health services extends beyond national and religious boundaries. Several areas require intensified research and further dialogue: the relationship between the individual and the community in terms of rights and responsibilities, how to operationalize the moral duty to decrease global inequalities in health, and the understanding and interpretation of human rights in regard to social services.

The last chapter of the book: who is the author? Christian reflections on assisted suicide.

In this paper the author argues that a narrative approach to understanding assisted suicide has been compromised by the notion that all narratives must be both coherent and unified. He asks what we are to do with those narratives that cannot seem to cohere or be other than full of disunity? Is suicide the only way to make meaning out of suffering? He then proposes that the narrative found in the Gospel of Mark leads Christians to a life in hope and compassion in spite of apparent incoherence and disunity and threats of abandonment and suffering.

The stage-specific role of spirituality among African American Christian women throughout the breast cancer experience.

This study presents the results of semistructured interviews conducted with 18 African American Christian women regarding the role of spirituality throughout their breast cancer experiences. The spiritual themes relevant for phases of the breast cancer experience are identified. Analysis resulted in the identification of 11 codes and 5 subcodes that corresponded to the diagnosis, treatment, and posttreatment phases of the breast cancer experience. Most of the survivors indicated that their spirituality and faith assisted them throughout the breast cancer experience. Discussion focuses on the spiritual resources used by the participants at the different stages in the breast cancer experience. Attention is given to implications for how professionals can use these resources to assist African American women coping with breast cancer.

Cloning humans? Current science, current views, and a perspective from Christianity.

Therapeutic cloning is urgent and should be vigorously supported. To successfully argue for this position, the distinction between a human embryo and a human nuclear transplant may be helpful. Even if current technical difficulties should be solved, global legislation should prohibit cloning for the purpose of fabricating babies. This position originates from a view on human nature in general and from a Christian perspective in particular.

Christianity↗

A Christian approach to bioethics.

Christian moral insight into complex medical issues depends mostly on beliefs about what it is to be human, what makes for human well-being, and how social attitudes that reinforce such human values can best be strengthened. Issues concerning sex selection are used to illustrate how such beliefs work in practice. To dismiss therapeutic selection as 'playing God' is not helpful. Human beings are meant to use their creative powers, but there is a line to be drawn between therapy and eugenics. There is a good moral case for trying to eliminate serious sex-linked diseases. Although there may be strong personal reasons for wanting a child of a particular sex, deliberate selection for non-medical reasons, if it became widespread, would subtly change social attitudes, reinforcing consumerist values at one of the most awesome and personally significant moments in anyone's experience. There is a moral distinction between influencing a child's development during its upbringing, and making choices about its genes. Even the most manipulative parents treat their child as a person. To manipulate its genes is to treat it as an object.
