The method of primarily postponed radical surgical treatment of acute purulent paraproctitis consists in puncture of the abscess, introduction of a double-channel drain, and continuous drop irrigation of the cavity for 1-2 days with antiseptic solutions and active aspiration for preoperative management. When the inflammatory process abates, fistulochromography is conducted and a radical operation is carried out on the same day with excision of the inner opening of rectal fistula. The method makes it possible to combine two operations in one stage which are undertaken in one third of patients in routine surgical treatment of acute paraproctitis. The described method shortens the period of treatment to 14-15 days and improves the immediate and long-term results. Eight patients were operated on with a good outcome.

Gastrointestinal bleeding remains one of the most challenging areas in medicine today, especially in the patient with cancer. New techniques and technology have added much to the diagnosis and management of gastrointestinal bleeding. Although gastroenterologists remain at the center of the evaluation of these patients, the incorporation of new techniques has meant that interventional radiologists and nuclear medicine specialists often have important roles in assessing and treating these patients as well. Because patients with cancer are members of a unique group, gastrointestinal bleeding may also require intervention by surgical oncologists, medical oncologists, and radiation oncologists. This review will address the multidisciplinary approach required for the proper diagnosis and management of gastrointestinal bleeding in patients with cancer.
In a prospective study the results of local surgery in perianal infections in patients with and without Crohn's disease were compared. In patients with Crohn's disease (CD) the abscesses and fistulas were more extensive and complicated than in patients without Crohn's disease (NCD). The source of infection was cryptoglandular in 81% (CD) resp. 100% (NCD). The wound healing in CD-patients was four weeks longer than in NCD patients. Abscesses or fistulas recurred in 6/18 (CD) resp. 1/25 (NCD). Partial incontinence occurred in 6/18 CD and 5/25 NCD. It is concluded that perianal infections can be treated successful in most of patients with Crohn's disease.
Under observation there were 174 patients with purulent wounds, 54 of them were treated with application of 0.06% solution of sodium hypochlorite, and 20 patients made up a control group and were treated by traditional methods. It was found that the application of the solution of sodium hypochlorite results in sharp elevation of susceptibility of wound microflora to antibiotics and facilitates more rapid debridement of the wounds and makes the treatment at the hospital 3.7 and 2.4 times shorter correspondingly (as compared with the control group).
Ulcerative colitis (UC) is a chronic inflammatory disease usually localized to the mucosa of the rectum and colon. Its cause remains unknown. A few fundamental notions underlie the therapeutic approach: (1) UC generally evolves by acute episodes separated by remissions, and the therapeutic indications depend on the severity and periodicity of these acute episodes; (2) although UC is a usually benign disease it may kill in two circumstances: when a severe (or complicated) acute episode is operated belatedly, or when cancer of the colon develops; (3) the present medical treatment only has a suspensive effect on the disease; the maintenance treatment must be taken for life; (4) only total proctocolectomy can provide complete cure, and until recently this procedure required permanent ileostomy. The recently introduced ileo-anal anastomosis (after total colectomy and rectal mucosectomy) gives the same results and enable the natural anus to be spared.
Low amounts of immunoglobulins of different classes may be measured by the red cell agglutination inhibition test with the red cells conjugated with the respective blood serum IgG and IgM and colostrum IgA isotypes. A feedback between IgM and IgG levels was detected in extracts of the tissues adjacent to the focus of inflammation during the necrotic phase of the process, whereas during the exudative phase the IgG/IgM ratio grows, probably at the expense of the IgM-IgG change over during lymphocyte maturation.

An experience with treatment of 3160 patients with acute paraproctitis under conditions of a specialized department is presented. Rational curative methods allow to achieve healing of 2280 patients (72.2%) with small amount of complications (22 cases). The duration of treatment was shortened to 11.2 days. No recurrences were noted.
