Efficacy and safety of sildenafil in the treatment of severe pulmonary hypertension in patients with hemoglobinopathies.

BACKGROUND AND OBJECTIVES: During the last decade new approaches to the treatment of pulmonary arterial hypertension (PH) have increased symptomatic relief and prolonged survival. PH is a common sequel of the hemoglobinopathies, thalassemia and sickle cell anemia, but the use of standard oral treatment options, such as calcium channel blockers, endothelin receptor antagonists, and long-term anticoagulation therapy, is limited because of toxicity and poor effectiveness. Sildenafil citrate is a selective and potent inhibitor of cGMP-specific phosphodiesterase-5 (PDE5) which promotes selective smooth muscle relaxation in lung vasculature and has been utilized successfully in the treatment of PH. The primary objective of this study was to evaluate the efficacy of sildenafil treatment in the control of PH in patients with hemoglobinopathies. DESIGN AND METHODS: In this study patients with hemoglobinopathies (thalassemia intermedia n=4; thalassemia major n=2; sickle thalassemia n=1) suffering from severe PH were treated with sildenafil citrate (50 mg b.i.d.) for periods ranging from 4 to 48 months. RESULTS: A significant decrease in pulmonary pressure and improvement in exercise capacity and functional class were observed in all patients. No significant adverse events were reported. INTERPRETATION AND CONCLUSIONS: These data, in a small group of patients, indicate that sildenafil citrate is effective in the treatment of PH in hemoglobinopathies that cannot be treated with alternative oral drugs and is well tolerated long-term at a daily dose of 100 mg, though studies including more patients may uncover toxicities and limitations of efficacy.

[The treatment of prolonged erections using phenylephrine].

Phenylephrine was used intracavernously to treat prolonged erections in 38 cases. A rapid detumescence was obtained in every case. Phenylephrine seems to be superior to all other adrenergic drugs in reason of its rapid action and its lack of side effects.

[Prolonged erections after diagnostic injection of papaverine chlorhydrate].

We analyzed the incidence of prolonged erection (PE) in 495 impotent subjects following intracavernous papaverine chlorhydrate injection to evaluate sexual dysfunction. Patient course was individually assessed. The overall the incidence of PE was 5.05%. The etiology of sexual dysfunction was psychogenic in all cases. No evidence of dose- or age-related PE was observed. Fibrosis of the corpus cavernosum, although more prevalent in PEs greater than 6 hours, was also observed in PEs of a shorter duration. This suggests that a probable predisposing factor might previously be present in some patients. None of the different methods utilized to achieve penile detumescence appears to afford any significant advantage.

[Neurotransmitters of penile erection other than vasoactive intestinal polypeptide. Physiologic data and therapeutic consequences].

It has been suggested that vasoactive intestinal polypeptide (VIP) may be involved in the nervous control of penile tumescence and erection. VIP ergic nerves are present in the human penis, especially around the prudendal arteries and the corpus cavernosum. However, we have shown that VIP is unlikely to be non-adrenergic, non-cholinergic inhibitory transmitter for penile erection. The aim of this presentation is also to analyse other neurotransmitters of penile erection. This included discussion on: 1. motor neurotransmission; 2. non-adrenergic, non-cholinergic inhibitory transmission; 3. endogenously formed relaxants in the corpus cavernosum; 4. involvement of endothelium-derived relaxing factor(s) and; 5. role of prostanoids in penile erection and others.

[Cavernosography using digital luminescence radiography (DLR) in the diagnosis of venous induced erectile function and of diseases of the corpus cavernosum].

Dynamic cavernosography is the most important technique for identifying and localizing venous leakage in erectile dysfunction. The use of digital radiography with a greater degree of tolerance in the exposure, a more dynamic signal, and a shorter exposure time can enhance the diagnostic value of conventional cavernosography. A further advantage of the method is the lower dosage required, which means that the gonads are exposed to less radiation. For 30 patients with erectile dysfunction and 7 other cases, conventional cavernosography images were compared with those obtained by this new method. The advantages of digital radiography are discussed.

[Hormones and the nasal mucosa. A bibliographic review].

The development and activity of the nasal mucosa are influenced by many hormonal substances. In this work we realize a bibliographical review about the effect of adrenaline, thyroid hormones, corticosteroids and sex hormones on the nasal respiratory mucous membrane. We emphasize the clinic consequences.

[Metaraminol--an antidote in corpus cavernosum autoinjection therapy-induced prolonged erection].

Up to now 65 patients with erectile dysfunction were treated by means of corpus cavernosum-autoinjection-therapy (CAT). The only side effect observed were 6 prolonged erections, which could successfully be treated in 3 cases by puncture and aspiration of the corpora cavernosa. In another 3 cases prolonged erection subsided after an intracavernous injection of 2 mg metaraminol. Erection induced by CAT was immediately interrupted by metaraminol in 10 other patients.

[The intracavernosal administration of papaverine in the diagnosis and treatment of sexual disorders in men].

The authors examined 68 patients with different forms of copulative disorders. They studied the efficacy of intracavernous administration of papaverine in the diagnosis and treatment of sexual disorders in males. A new method of diagnosis of sexual disorders of organic (vascular) nature in males was developed and practically employed. The efficacy of intracavernous administration of papaverine in the treatment of different forms of copulative disorders in males is analyzed.

New therapy for impotence.

In the not too distant past, the physician caring for the impotent patient had few therapeutic options. With the recent advances in penile pharmacotherapy, vascular surgery, and prosthetics, several possibilities for treatment are now available. Before a decision is made, however, numerous factors must be taken into consideration: age, manual dexterity, cause of erectile dysfunction, psychological attitude, and social considerations. Clearly, the choice of therapy for impotence must now be made on an individual basis after a careful diagnostic evaluation and in-depth consultation with the patient. Only then can the treatment that offers the highest likelihood of success, both functional and emotional, be chosen.
