[Suprascapular nerve entrapment at the spinoglenoid notch secondary to a ganglion cyst].

Suprascapular neuropathy may occur as a result of traction, direct trauma, repetitive microtraumas, and extrinsic compression. A thirty-one-year-old man presented with non-traumatic complaints of right shoulder pain and stiffness. After exclusion of other causes associated with shoulder pain, a diagnosis of suprascapular nerve entrapment was considered. Electromyographic activity of the right shoulder was compared with that of the left side and involvement of the infraspinatus muscle was detected, with the supraspinatus muscle appearing normal. Magnetic resonance imaging revealed a ganglion cyst around the spinoglenoid notch. The patient underwent arthroscopic examination and open cyst excision. Arthroscopy did not show any labral tears. Pathological diagnosis was made as a cystic ganglion. On follow-up examinations, the patient's complaints disappeared and only minimal infraspinatus muscle atrophy was observed. Suprascapular nerve entrapment should be considered in the differential diagnosis of posterior shoulder pain.

Mutation of the Notch 3 gene in a Thai cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy family.

The authors report the first Thai family with cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL) in which the family members had a classical history of progressive vascular dementia. The proband was a 31-year old Thai male who presented with an acute stroke in the subcortical region. His past history revealed mental disturbance, including poor judgement and regressive behavior as well as mood changes for 1 year. He did not have a history of migraine or any other vascular risk factors except for a strong family history of ischemic stroke and progressive dementia. Magnetic resonance imaging demonstrated multiple small infarctions in the subcortical white matter of the bilateral frontal, parietal and occipital lobes with another small lesion in the pons. Genetic study demonstrated a Notch 3 mutation consisting of the substitution of a nucleotide at position 406 in exon 3 leading to the replacement of an Arginine by Cysteine at position 110 in the 2nd EGF motif, which is compatable with CADASIL.

[Specific notch in early expiration region of a flow-volume curve in a case of Mounier-Kuhn syndrome].

A 75-year-old man presented at our hospital for evaluation of a chronic cough and sputa. Radiographic examination showed enlargement of the trachea and main bronchi. On chest radiography, the transverse diameter of the trachea was 39 mm at the level of the third thoracic spine. On bronchoscopy, the trachea and main bronchi were dilated on inspiration and were completely collapsed on expiration. A 3-D CT examination showed the trachea and main bronchi dilated, and the cartilage of the trachea and bronchi distorted. In pulmonary function testing, this disorder is characterized by the appearance of a specific notch in the early phase of expiration on the flow-volume curve.

SUPRASTERNAL NOTCH ECHOCARDIOGRAPHY AND ATRIAL ARRHYTHMIAS.

Anterior-posterior single-crystal echocardiography has been used in atrial flutter and in sinus rhythm after cardioversion to demonstrate anterior movement of the posterior left atrial wall, representing the effects of atrial flutter and sinus contractions.(1) This is thought to be the first report of characteristic superior movement of the inferior left atrial wall during atrial flutter, atrial tachycardia, and sinus rhythm by single-crystal suprasternal notch echocardiography.

Lateral inhibition and cell fate during neurogenesis in Drosophila: the interactions between scute, Notch and Delta.

A comparison of the patterns of expression of AS-C (T3) RNA and protein suggests that an important level of regulation occurs post-transcriptionally. First, when the RNA is abundant in the early embryo the protein is barely detectable. Later, the protein starts to accumulate in only a subset of the nuclei of those cells expressing the RNA. Only the cells in the subsets become the neuroblasts. This post-transcriptional regulation is suppressed in embryos mutant for the genes Notch and Delta; where all cells expressing RNA accumulate protein. These findings suggest that deployment of T3 protein expression is one of the causal factors that assigns specific fates to the neuroblasts and, in consequence, a basis for the mechanism of lateral inhibition is proposed.

[Electron microscopy of the spiral limbus and notch in the normal state and in chronic keltane poisoning].

Results of electron microscopic examinations of the membraneous cochlea of 6 untreated rats and 36 rats fed with keltane (chloroorganic pesticide) at a 1/20 LD50 dose are presented. Biosamples were withdrawn 30, 60 and 120 days after onset of treatment. In the spiral limb (SL) area, light or dark cells, fibroblasts, fibrillar structures, and blood capillaries were discerned. Light cells included few organelles. Dark cells contained secreting cells located near blood capillaries. Internal spiral notch (ISN) cells had osmiophobic cytoplasm, single organelles, and many microvilli. At an early experimental stage (day 30) light cells, fibroblasts, and blood capillaries proved most sensitive to keltane. They displayed mitochondrial swelling, Golgi organ hyperplasia, intercellular space enlargement, perivascular edema, thickening of the capillary endothelium, and basal membrane swelling. At day 60 edema and ultrastructural changes diminished and dark cells showed signs of enhanced activity. At a late stage (day 120) ultrastructural changes occurred in all SL and ISN organelles. The cytoplasm of some cells showed destructive changes while that of others displayed reparative changes. Ultrastructural changes in the SL and ISN produced a direct effect on toxic lesions of receptor elements of the spiral organ.

Level dependence of critical bandwidth: notched-noise masking paradigm.

The effect of increased stimulus level on critical bandwidth was investigated. Noise-masked, pulsed-tone thresholds were obtained by a Bekesy tracking procedure from 4 practiced normal-hearing adults at .5, 1, 2, and 4 kc/s. Tones were placed symmetrically within a band-reject region. Critical bandwidth was taken as the frequency separation between noise bands at which masked tonal threshold began to change and was found to increase fairly regularly as the spectrum level of the noise increased from 30-60 db SPL. However, the data at 60 db SPL may have been influenced a 2 and 4 kc/s by the detection of aural distortion products. The suggestion was made that when signal frequency is outside the spectral limits of the masker, critical bandwidth widens as masker level is raised; however, when signal frequency is within the spectral limits of the masker (as with a continuous noise wit no notches), critical bandwidth remains unchanged up to 80 bd SPL.

A composite nature for the photopic b-wave of the human electroretinogram as evidenced by the use of the 60 Hz notch filter.

The impact of the 60 Hz notch (rejection) filter was examined in typical photopic electroretinograms recorded with two different bandwidths: 1-1000 Hz and 100-1000 Hz. The peak times of the major ERG components (a- and b-waves) are not modified by the line filter. The amplitude of the b-wave decreases by 30%, and the morphology of the ERG wave changes slightly. Segmental analysis of the ascending limb of the b-wave reveals that the sole reduction in the amplitude of its first step (from the a-wave trough to the peak of the first oscillatory potential) accounts for more than 70% of the overall reduction measured for the b-wave. Similar results are also found in ERGs recorded with the 100-1000 Hz bandwidth. The data suggests that the b-wave of the ERG should be considered as a composite of potentials rather than as a single potential.

[Rupture of the posterior cruciate ligament. Palliative treatment by transposition of the internal gastrocnemius mucle into the intercondyloid notch (author's transl)].

In old ruptures of the posterior cruciate ligament, dynamic knee stabilization can be obtained by transposition of the internal gastrocnemius muscle into the intercondyloid notch. The muscle contracts when the patient walks, thus preventing posterior sliding. The technique, derived from Hugston's technique, is simple, and the intial results are very encouraging, both objectively and subjectively.

ABG, dips and notches evaluation of cochlear reserve in otosclerosis.

This study of a possible relationship between the ABG and the remodeling of the niche in otosclerosis in a significant number of patients operated on by stapedectomy allows the authors to define six types of ABG shape, corresponding to various types of stapedial fixation and remodeling of the niche. But the difficulties in finding the real bone conduction threshold levels, which ABG shapes depend on, lead us first to explain the mechanism of the various dips and notches altering bone conduction curves; and second, to try to find a safe and sure method for precise evaluation of the real cochlear reserve in otospongiosis/otosclerosis. We conclude that the best method is the bone conduction speech audiometry, mainly the bone conduction speech Weber tes, which automatically shows the true cochlear reserve of the ear to be operated on.

Infraspinatus paralysis due to spinoglenoid notch ganglion.

We describe five patients, seen since 1984, with posterior shoulder pain and isolated wasting and weakness of the infraspinatus. In four of these a ganglion in the spinoglenoid notch was demonstrated by MRI and in one recent case ultrasound scans were positive. Three patients have been treated by operation, but there was recurrence in one after five years. In each confirmed case, the ganglion straddled the base of the spine of the scapula, extending into both supraspinatus and infraspinatus fossae. The nerve was either compressed against the spine or stretched over the posterior aspect of the ganglion. Adequate surgical exposure is essential to preserve the nerve to the infraspinatus and to allow complete removal of the ganglion. This is difficult because of the location and thin-walled nature of the cysts.

[The intercellular interactions in the ontogeny of Notch-84k35 mutant specimens of Drosophila melanogaster].

The Notch-84k35 mutants of Drosophila melanogaster with hypertrophy of the nervous system and changed pattern of the embryo organization were used for studying cell interactions between different embryonic layers. The purposeful migration of mesodermal cells toward ectoderm on the dorsal side of the embryo is described. It is shown that differentiation of the visceral muscles does not depend on the development of endodermal layer.

The acetabular notch in hip dysplasia.

In the first few months of life, a notch may be seen on radiographs of the superolateral margin of the infant hip. It may be associated with a steeply inclined acetabular roof and may be an indicator of persistent neonatal instability or displacement of the femoral head.

[Predicting the difficulty of laryngoscopy using the distance between the lower border of the mandible and the thyroid notch].

We examined the relationship between the degree of difficulty in visualization of the larynx and the distance from the lower border of the mandible to the thyroid notch (M-T distance). Patients were examined and the M-T distance was measured with their neck fully extended during preoperative period. Difficulty of laryngoscopy was graded as reported previously. Five hundred sixty one patients were assessed, and those with obvious neck and head pathology were excluded. There were 440 cases (78.4%) in grade 1, 116 cases (20.7%) in grade 2 and only 5 cases (0.9%) in grade 3. There were no grade 4 cases. The M-T distance was 5.4 +/- 0.6 cm in grade 1, 4.6 +/- 0.7 cm in grade 2 and 3.6 +/- 0.7 cm in grade 3. These were significantly different with each other (P < 0.05). If the M-T distance is 4.5 cm or less, the difficulty in visualization of laryngoscope increases. Therefore, in the case in which the M-T distance is 4.5 cm or less, we recommend further examination and preparation for difficult laryngoscopy.
