[Real time diagnostics of instantaneous temperature of combustion and explosion process by modern spectroscopy].

The combustion temperature is one of the important parameters to express flame combustion and explosion characteristics. It will effectively guide the design and manufacture of new model explosives, industrial explosive materials, and weapons. The recent developments and applications of real time diagnostics of instantaneous temperature of combustion and explosion processes by modern spectroscopic methods, such as atomic absorption-emission method, atomic emission two-line spectroscopy, atomic emission multiline spectroscopy, molecular rotation-vibration spectroscopy, coherent anti-stokes Raman scattering (CARS) and plane laser-induced fluorescence (PLIF), were reviewed in this paper. The maximum time resolution of atomic absorption-emission method is 25 microseconds. The time resolution of atomic emission two-line spectroscopy can reach 0.1 microsecond. These two methods can completely suit the need of real time and instantaneous temperature diagnostics of violent explosion and flame combustion. Other methods will also provide new effective research methods for the processes and characteristics of combustion, flame and explosion.

[Modern extracapsular extraction for cataract of uveitis].

Cataract of uveitis is difficult for extracapsular extraction (ECE) because of posterior synechiae. 21 cases (24 eyes) of the cataract were performed modern ECE with satisfactory results. Postoperative visual acuity was 0.1 or better in 20 eyes, and 0.5 or better in 7 eyes. The optimal time for the operation and the procedures were discussed. The authors opined that removal of the cataract could prevent recurrences in some severe refractory cases of uveitis.

Modern surgical treatment of congenital subluxated lenses.

Traditional surgical management of subluxated lenses is usually associated with a high incidence of operative and postoperative complication and a poor visual outcome. We treated a series of 14 patients (26 eyes) whose visual acuity could not be improved with optical correction (phakic or aphakic) with modern microsurgery and automated suction-cutting devices. Preoperative visual acuity ranged from 0.05 to 0.3. Improvement in post-operative visual acuity was documented in all operated eyes, ranging from 0.4 to 1.0 during a follow-up period ranging from 6 to 59 months (medium 43 months). The only complication was a small accidental cut in the pupillary border in three eyes. Other complication did not occur.

Modern management of tuberculosis; the public health implications of recent advances.

Modern treatment for tuberculosis has greatly increased the problem of preventing spread of infection. BCG vaccination, for instance, would cause all persons to react to tuberculin test so that the possibility of tuberculosis could never be ruled out by this means. Streptomycin and more recently developed drugs may sterilize sputum so that diagnosis cannot be confirmed for some time after use of such drugs; when by use of these drugs the disease had been confined to caseous encapsulations, later breakdown of the encapsulations may release strains of bacilli resistant to the drugs both in the patient and in others infected with them. The temporary sterilization of sputum, coupled with the euphoria resulting in part from abrupt remission of the toxic state, may lead to premature discharge of patients from sanatoria and further spread of tuberculosis. Both the public and the profession must be impressed with these facts. For more profitable than minifilm surveys of normal populations are routine x-ray examination of all patients admitted to hospitals (by which two to five times as many cases have been found) and follow-up of persons who have had contact with tuberculosis patients (thirteen times as many cases found).A study being conducted by the California Tuberculosis and Health Association indicates that in many counties neither the number of x-rays made in public surveys nor the number of cases found is known or even to be estimated from existing records. Because of reduction in deaths due to tuberculosis some public officials are reluctant to spend for further treatment facilities. As the actual number of cases is increasing in many areas, however, expenditures will have to be increased.

The effect of modern Balinese Baris dancing exercise (MBBDE) on serum lipid profiles.

There is still a lack of information on the effect of regular dancing exercise on lipid profiles. On the other hand, many studies have been carried out on the effect of aerobic exercise on lipid profiles. This study tried to find out the effects of Modern Balinese Baris Dancing Exercise (MBBDE) on serum lipid profiles. Subjects of the study were 30 healthy young male Balinese as an experimental group, and another 30 healthy young Balinese as control group. The MBBDE involved exercise intensity at 70-80% of targeted heart rate, for 50 min period, 3 times per week for 8 weeks. Pre- and post-control group design was applied. Total cholesterol and triglyceride were measured enzymatically. Following MBBDE 3 x 50 min/week for 8 weeks duration, serum level of high density lipoprotein cholesterol (HDL-C) concentration increased significantly from 55.3 +/- 2.32 mg/dl to 63.2 +/- 2.82 mg/dl (p < 0.001). It was also associated with the decrease of total cholesterol concentration from 195.5 +/- 21.10 mg/dl to 161.8 +/- 21.29 mg/dl (p < 0.001); triglyceride concentration from 132.2 +/- 9.65 mg/dl to 110.6 +/- 9.08 mg/dl (p < 0.001); and low density lipoprotein cholesterol (LDL-C) concentration from 113.8 +/- 21.68 mg/dl to 76.9 +/- 20.76 mg/dl (p < 0.001). No significant differences were found in the above parameters in the control group. It is concluded that MBBDE is an aerobic, endurance exercise, and therefore produces beneficial effect on the serum lipid profiles.

[Use of modern methods in prenatal diagnosis of congenital adrenal hyperplasia--examination protocol].

The prenatal diagnosis of congenital adrenal hyperplasia (CAH) in the second trimester of gestation (as done so far in this country) is late and unsuitable with regard to possible interruption of pregnancy. Modern methods of early prenatal diagnosis are based on examination of material from chorionic villi (6th-10th week of gestation) or early amniocentesis (10th-12th week of gestation). The authors present their decision taking scheme, examination protocol and their own experience with the use of molecular genetic analysis and assessment of 17-hydroxyprogesterone from early amniocentesis in the prenatal diagnosis of CAH. This procedure is a priority in this country.

[Modern problems of drug allergy to antibiotics in the clinical aspects of skin and venereal diseases].

Allergic dermatoses induced by antibiotics amounted to 45.2 per cent of all determined drug reactions according to the data of the Kharkov Research Institute of Dermatology and Venerology. The data on the factors favouring development of the drug dermatoses are presented. Difficulties in differential diagnosis by means of clinical and laboratory methods are indicated. The modern immunological tests of etiological diagnosis of drug resistance and the results of studying non-specific reactivity in patients with medicamentous dermatoses were analyzed. The principles of therapy and prophylaxis are summarized.

[The modern concepts of hereditary nephritis].

The authors describe the results of modern studies into the problems of genetics, clinical picture, prognosis and prospects of the treatment of inherited nephritis. It is assumed that at the basis of inherited nephritis there lies generalized impairment of the basal membranes, which is determined by mutation of X chromosome that codes the structure of the chains of the fourth fraction of collagen. The phenotypic heterogeneity of the disease is accounted for by mutation of different alleles in a solitary locus. The clinical characteristics of inherited nephritis without hypoacusis and Alport's syndrome in inbred and outbred families is provided as are specific features of the disease evolution. The results and efficacy of kidney transplantation in patients with inherited nephritis in the phase of chronic renal failure are discussed.

[The autopsy: its importance for modern clinical medicine].

Autopsies are absolutely necessary for the progress of modern clinical medicine and for solving many problems: diagnostic, control of treatment quality, detection of infectious, hereditary and systemic diseases, establishing causes and mechanisms of sudden death, qualification of unclear diseases and syndromes, classification and nomenclature of nosological entities, detection and study of spontaneous and induced pathomorphosis. The pathologist performing autopsies should except necropsy also make macrochemical examination and send the material for microbiological and biochemical examination. Special attention should be paid to establishing the diagnosis and clinicoanatomical epicrisis. Undoubtful is the role of autopsy in teaching and training, postgraduate education, accumulation of archives and their retrospective analysis, creation of macro- and micromuseums of human diseases. Physician's errors, general requirements and cadaveric tissue observation are also discussed.

[Role of DNA diagnosis in modern oncology].

DNA-diagnosis became to be a mandatory element of the modern oncological care. The molecular-genetic approaches aimed at identifying the high-risk categories in oncology are mostly-renowned. They comprise the diagnosis of the so-called familial cancerous syndromes, the identification of unfavorable normal genome variations (gene polymorphisms) and the detection of a number of oncoviral infections. Laboratory procedures facilitating the individualization of treatment schemes and the monitoring of oncology patients have been widely introduced into clinical practice. The turn between the 20th and 21st centuries was marked by the emergence of principally new DNA-analysis methods designed for a comprehensive and integral genome evaluation. The application of such radically novel techniques in the molecular diagnostics essentially promoted the detection and treatment of neoplasms.

[Use of modern technologies in the diagnosis and combined treatment of brain stem tumors].

The latest technologies, introduced into neurosurgery during the last 2 decades of the 20th century, ensured progress in the diagnostics and treatment of brainstem tumors--pathologies that remain until now a most complicated problem in neurooncology. The modern diagnostic tools comprising profound dynamic neurological and neurophysiological examinations (AVP, EEG) visualization techniques (CT, MRT with contrasting, MRT, spectroscopy, PET) provide for detecting not only the initial signs of stem tumors and their sizes but also their topographic interaction with the stem structures; it, in its turn, opens up a possibility to choose between the combined surgical treatment and the radiotherapy or chemotherapy alone. The clinical course, diagnosis and combined treatment of 333 children and adults with tumors bordering on the brainstem are analyzed within the case study.

[Modern technologies of diagnosis and combined surgical treatment in liver tumors].

Modern technologies used at St. Petersburg Central Research Institute of Roentgenology and Radiotherapy of Russia's Ministry of Health in the diagnosis and treatment of malignant and benign liver cancers are surveyed in the paper. Apart from routine examinations, like ultrasound scanning, computer tomography, MRT, angiography, the below new technologies were widely used: multi-stratum spiral computer tomography, special MRT techniques and positron-emission tomography. The new methods enabled us to diagnose the tumor and its extension, to choose an optimal intervention technique, to make an objective assessment of the results and to correct the treatment strategy. Preoperative embolization of the portal-vein branches in the damaged hepatic lobe and postoperative adjuvant local chemotherapy of the hepatic artery and portal vein belong to the new combined surgical treatment scheme. The method of combined arterial-portal chemoembolization, used in unresectable malignant hepatic pathologies, extends the survival of patients by 3-5 times. Ferromagnetic embolization presupposes the administration of ferromagnetic, through the arterial bed or direct puncture, into the tumor with a subsequent SHF-current treatment which brings about a selective tumor hyperthermia without heating the normal hepatic parenchyma. Arterial embolization combined with local injection sclerotherapy in hemangiomas of the liver provides for a complete regeneration of benign tumors and can be regarded as a low-trauma alternative to liver resection. The use of new methods of diagnosis and treatment expands the possibilities of surgical care for patients with hepatic neoplasms and essentially improves the treatment results.

[Early detection of the toxic effects of benzene on the hematopoietic system--the imperative of modern occupational medicine].

Surveillance of workers exposed to benzene calls for a complex approach. This includes the determination of benzene concentration in the working atmosphere, which, according to standards applied in Croatia, should not exceed 15 ppm. In the developed countries the allowable workplace level has been reduced to 1 ppm. The monitoring of benzene metabolites in the organism of exposed workers is also necessary. As an indicator of benzene exposure, the urinary phenol concentration determined before and after work in the middle of the week has been generally accepted in Croatia. An essential laboratory test is a complete blood count including red cell indices. Other haematological tests that might be valuable early indicators of benzene haematotoxicity need to be more fully evaluated. Alterations in any of these indicators are not pathognomonic and all findings should therefore be interpreted at the same time and with caution. Early detection of benzene haematotoxicity can be accomplished by continuous health monitoring of exposed workers through preplacement and periodic health examinations. This is in accordance with the principles of modern occupational medicine which requires identification of early changes and prevention of irreversible benzene induced toxic changes in the haematopoietic system.

[Modern conceptions about regulation of cell cycle in plants].

The most modern conceptions about regulation of cell cycle in plants are considered in the article. According to literary data, the main regulators of cell cycle are cyclines and cycline-dependent kinases. Four basic types of plant cyclines and series of cycline-depending kinases, that control different phases of cell cycle, are describe. As for the influence of physical factors (temperature, radiation, light, vibration, magnetic field and microgravity) on the parameters of cell cycle, then in most cases they arise similar reactions--increasing of the duration of cell cycle, that may be explained by the extension of presinthetic phase of the cycle. The study of influence of altered gravity on a cell proliferation is very important for understanding and further, investigations of the nature of peculiarities of a plant development and the study of molecular mechanisms of the cell cycle regulation under altered gravity.
