Recurrence of IgA nephropathy after renal transplantation.

With time recurrence of IgA nephropathy in renal allografts may be found in most, if not all patients. However, at the present graft loss due to recurrent IgA nephropathy appears to be of limited importance as compared to other causes. No definite risk factors for recurrent disease and/or graft failure have been identified yet. Although recurrence rates appear to be higher in allografts from living related donors, graft loss rates due to IgA nephropathy are not significantly different. More important, sporadic cases of IgA nephropathy should be excluded in all living related donors.

Treatment results of stereotactic interstitial brachytherapy for primary and metastatic brain tumors.

A total of 41 stereotactic interstitial brain implants in 39 patients were performed for recurrence after teletherapy (recurrence implant), or as part of initial treatment in conjunction with teletherapy (primary implant). Implanted tumors consisted of malignant gliomas (33), other primary brain tumors (3), and single metastatic lesions (3). All patients were temporarily implanted with Ir-192 using a coaxial catheter afterloading system; two patients were implanted twice. Survival post-implant for glioblastoma multiforme (GBM), 13 patients, was 10 months whether implanted primarily or for recurrence. Mean time to recurrence, measured from initiation of teletherapy to implantation, was 10 months. Twenty patients with anaplastic astrocytoma (AA) had a median survival post-implant of 23 months for primary implants (7 patients) and 11 months for recurrence implants (13 patients). Mean time to recurrence, measured from initiation of teletherapy to implantation, was 19 months. Three patients (9%) of the evaluable group required reoperation for symptomatic mass effect, all with initial diagnosis of AA. Survival for this subgroup was 14, 22, and 32 months post-implantation. Using stereotactic techniques, interstitial brachytherapy of brain tumors was technically feasible with negligible acute morbidity and mortality, and appeared to offer limited prolongation of control for a subset of patients with recurrent malignant gliomas. The role of this modality in primary treatment for malignant gliomas needs to be further defined by prospectively randomized trials.

[Conservative treatment of hyperthyroidism (author's transl)].

Antithyroid medication was given to 158 patients with hyperthyroidism over a period of 3 to 60 months. After cessation of therapy patients were followed up for 18 to 90 months. Permanent euthyroidism was seen in 70 patients (44.3%) after stopping treatment, however, 88 patients (55.7%) showed recurrence of hyperthyroidism occurring 1 to 56 months after ceasing treatment. In more than 50% recurrence of hyperthyroidism was within the first 3 months and in almost 80% within the first year after end of treatment. There was no connection either between the length of thyrostatic treatment and the recurrence rate or between the length of treatment and recurrence time. Comparison of patients with and without recurrence according to various parameters prior, during and after thyrostatic treatment indicates that there is a high risk of recurrence in patients with 1) nodular and (or) large goitres, 2) marked clinical symptomatology and delayed attainment of a euthyroid state after starting conservative treatment, and 3) the symptom of sweating remaining uninfluenced by antithyroid treatment.

Thymic carcinoma: involvement of great vessels indicates poor prognosis.

BACKGROUND: Thymic carcinoma is a rare, indolent, and invasive cancer. This study investigated the treatment results of thymic carcinoma and clinical prognostic factors. METHODS: From June 1988 to January 2002, 38 patients were enrolled in this study with the diagnosis of thymic carcinoma in the Cheng-Kung University Hospital based on Rosai's and Muller-Hermelink's classification. Clinical and pathologic data were retrospectively reviewed. Survival analysis was performed using the Kaplan-Meier, log rank, and Wilcoxon tests. Statistical significance was defined as p < 0.05. RESULTS: Pathology revealed 14 poorly differentiated, 6 moderately differentiated, and 8 well-differentiated squamous cell carcinomas; 8 lymphoepithelioma-like carcinomas; and 2 other carcinomas. Pathologic staging using the Masaoka system included 6 stage II, 23 stage III, and 9 stage IV patients. Six biopsies, five debulkings, and 27 complete resections were performed. All patients were followed from 15 months to 10 years 9 months, with an average of 53.8 months. Median survival time was 81 months, and median recurrence time was 52 months. Eighteen patients are still alive, and 7 are alive with disease. Well-differentiated squamous cell carcinoma had better prognosis than other carcinomas (p = 0.022). Complete resection significantly increased survival rate (p < 0.001). Tumor invasion of the superior vena cava, pulmonary vessels, or aorta were significant predictors for poor prognosis (p = 0.016, 0.002, and 0.002, respectively). CONCLUSIONS: Only patients with thymic carcinoma who underwent complete resection had long-term survival. Prognosis of thymic carcinoma seemed mainly dependent on tumor invasion of the great vessels.

Detecting unstable periodic orbits from transient chaotic time series

We address the detection of unstable periodic orbits from experimentally measured transient chaotic time series. In particular, we examine recurrence times of trajectories in the vector space reconstructed from an ensemble of such time series. Numerical experiments demonstrate that this strategy can yield periodic orbits of low periods even when noise is present. We analyze the probability of finding periodic orbits from transient chaotic time series and derive a scaling law for this probability. The scaling law implies that unstable periodic orbits of high periods are practically undetectable from transient chaos.

Decision feedback recurrent neural equalization with fast convergence rate.

Real-time recurrent learning (RTRL), commonly employed for training a fully connected recurrent neural network (RNN), has a drawback of slow convergence rate. In the light of this deficiency, a decision feedback recurrent neural equalizer (DFRNE) using the RTRL requires long training sequences to achieve good performance. In this paper, extended Kalman filter (EKF) algorithms based on the RTRL for the DFRNE are presented in state-space formulation of the system, in particular for complex-valued signal processing. The main features of global EKF and decoupled EKF algorithms are fast convergence and good tracking performance. Through nonlinear channel equalization, performance of the DFRNE with the EKF algorithms is evaluated and compared with that of the DFRNE with the RTRL.

A complex-valued RTRL algorithm for recurrent neural networks.

A complex-valued real-time recurrent learning (CRTRL) algorithm for the class of nonlinear adaptive filters realized as fully connected recurrent neural networks is introduced. The proposed CRTRL is derived for a general complex activation function of a neuron, which makes it suitable for nonlinear adaptive filtering of complex-valued nonlinear and nonstationary signals and complex signals with strong component correlations. In addition, this algorithm is generic and represents a natural extension of the real-valued RTRL. Simulations on benchmark and real-world complex-valued signals support the approach.

Local excision carcinoma in early stage.

AIM: To assess the validity of local excision for the early stage low rectal cancer as an effective treatment alternative to radical resection. METHODS: A retrospective medical chart review was done in 47 patients with early stage low rectal carcinoma who underwent local excision from November 1980 through November 1999 at Cancer Hospital of Chinese Academy of Medical Sciences (CAMS). The patients were treated by either transanal (40 cases), trans-sacral (5 cases), or trans-vaginal (2 cases) excision of tumors and no death was related to surgery. Sixteen patients received postoperative radiotherapy. RESULTS: T1 and T2 lesion was found in 36 (76.6 %) and 11 patients (23.4 %) respectively. The overall local tumor recurrence rate was 14.9 % (7/47), with an average recurrence time of 21 months. Among these 7 recurrent patients, there were 4 T1 and 3 T2 lesions. Microscopically, the surgical incisal margin was negative in 45 (95.7 %) and positive in 2 patients (4.3 %); Both of the later had developed local recurrence. The overall 5-year survival rate was 91.7 %, in which there were 94.4 % for T1 and 83.3 % for T2 tumors. T stage, intravessel tumor thrombosis, lymphocytic infiltration and histological grade were not found to be significant by related to the local recurrence and survival (P>0.05). CONCLUSION: Local tumor excision was a safe procedure for the treatment of early stage low rectal carcinoma with minimal morbidity and mortality, which might serves as one of the primary surgical treatment methods for the disease of this kind.

Outcomes of the Rives-Stoppa technique in incisional hernia repair: ten years of experience.

BACKGROUND: Use of the Rives-Stoppa (R-S) technique has markedly diminished recurrence rate. Our aim was to assess the outcomes of Iranian patients undergoing the R-S technique and to determine factors affecting recurrence rate and time. METHODS: Over a 10-year period, 86 open abdominal wall incisional hernia repairs were performed using the R-S technique. Prolene and Mersilene meshes were used. Main outcome measures were the rate and time of recurrence and mortality. RESULTS: Sixty-six women and 20 men underwent the R-S technique (mean age: 51.21 +/- 13.01 years; mean follow up period: 67.60 +/- 32.55 months). The total recurrence rate was 5.8%. Most of these occurred in the first 12 months of follow up. Mersilene was used in 61%. There was no correlation between hernia recurrence and age, gender, defect size, mesh size, mesh type, number of meshes used, type of operation, position of incision, seroma, respiratory complications, ICU admission, or wound complications (P > 0.05). Overall postoperative complication rate was 18.2%. Except for type of operation, there was no correlation between the factors mentioned and recurrence time. Cumulative 5 and 10-year recurrence rate were 4.2 and 12.5%, respectively. CONCLUSION: The R-S technique is the most promising open technique, with low recurrence rates, excellent long-time results, and minimal serious morbidity. Age might be an important factor in recurrence rate.

The effect of local recurrence on survival time in adult high-grade soft tissue sarcomas.

Data gathered on 262 adults with high-grade soft tissue sarcoma, operated on at the same institution for curative intent, were used to study formally, and to quantitate for the first time, the relationship between local recurrence of the tumor and survival time. Using Cox's proportional hazards model with a time-dependent covariate representing the local recurrence of the tumor, it was found that local recurrence is significantly associated with a shorter survival time (estimated relative risk (relative hazard) = 2.5, p less than 0.0001). The estimated 5 yr survival rate and median survival time for patients without a local recurrence were 44% and 42 months, respectively, while the corresponding figures for patients with a local recurrence were only 26% and 28 months. Hence, a local recurrence resulted in a relative decline in the estimated 5 yr survival rate of 41%. This strong relationship continued to hold even after adjusting for several other important, time-independent prognostic variables (stage, type of surgery, type of resection, signs of sarcomatous skin invasion, and presence of postoperative fever) in a multivariable analysis. Local control of high-grade soft tissue sarcomas is vitally important for successful management of these patients.

CA 15-3 and carcinoembryonic antigen in the clinical evaluation of breast cancer.

The individual and combined value of CA 15-3 and carcinoembryonic antigen (CEA) as breast cancer tumor markers was investigated in longitudinal studies. Patients included women at high risk for recurrence after primary therapy or undergoing treatment for metastatic disease. During follow-up, recurrent disease was documented in 33 of 39 (85%) patients including 11 with local recurrence and 22 with distant metastases. At the time recurrence was first documented by objective criteria 23 of 33 (70%) of the patients presented with abnormal CA 15-3 levels (greater than 36.7 U/ml) compared with 19/33 (58%) with abnormal CEA levels (5 ng/ml). Tumor marker elevations predominated in patients with advanced disease indicating that CA 15-3 and CEA are not reliable for the detection of early breast cancer. Both markers were helpful in monitoring therapeutic response since antigen levels correlated closely with disease status.

Ultrafast electron diffraction: oriented molecular structures in space and time.

The technique of ultrafast electron diffraction allows direct measurement of changes which occur in the molecular structures of isolated molecules upon excitation by femtosecond laser pulses. The vectorial nature of the molecule-radiation interaction also ensures that the orientation of the transient populations created by the laser excitation is not isotropic. Here, we examine the influence on electron diffraction measurements--on the femtosecond and picosecond timescales--of this induced initial anisotropy and subsequent inertial (collision-free) molecular reorientation, accounting for the geometry and dynamics of a laser-induced reaction (dissociation). The orientations of both the residual ground-state population and the excited- or product-state populations evolve in time, with different characteristic rotational dephasing and recurrence times due to differing moments of inertia. This purely orientational evolution imposes a corresponding evolution on the electron scattering pattern, which we show may be similar to evolution due to intrinsic structural changes in the molecule, and thus potentially subject to misinterpretation. The contribution of each internuclear separation is shown to depend on its orientation in the molecular frame relative to the transition dipole for the photoexcitation; thus not only bond lengths, but also bond angles leave a characteristic imprint on the diffraction. Of particular note is the fact that the influence of anisotropy persists at all times, producing distinct differences between the asymptotic "static" diffraction image and the predictions of isotropic diffraction theory.

Tumor stenoses of the upper gastrointestinal tract--therapeutic alternatives to laser therapy.

Alternatives to laser treatment of tumor stenoses of upper gastrointestinal tract include dilatation, implantation of bridging tubes, radiation therapy and chemotherapy. Based on the experience of 170 bridging tubes implanted and laser therapy in 70 patients with tumor stenoses (presented elsewhere) "tubes and laser" are compared. Complication rate, applicability in very proximal and distal stenoses are in favour of laser therapy, costs, hospitalisation time, possibility to occlude fistulae and short recurrence time after laser therapy are facts speaking for oesophageal bridging tubes. The question "tubes or laser" has to be answered by "tubes and laser" already.

Delayed self-synchronization in homoclinic chaos.

The chaotic spike train of a homoclinic dynamical system is self-synchronized by applying a time-delayed correction proportional to the laser output intensity. Due to the sensitive nature of the homoclinic chaos to external perturbations, stabilization of very long-periodic orbits is possible. On these orbits, the dynamics appears chaotic over a finite time, but then it repeats with a recurrence time that is slightly longer than the delay time. The effect, called delayed self-synchronization, displays analogies with neurodynamic events that occur in the buildup of long-term memories.

Clinico-pathological study of 12 cases of patients with leprosy admitted to Sina Hospital, Hamadan, Iran, from 1991 to 2000.

BACKGROUND: Leprosy is considered a chronic disabling condition. Many clinical and immunological aspects of the disease remain ill defined. AIM: The study of clinico-pathological and laboratory findings of patients with leprosy admitted to Sina Hospital, Hamadan, Iran, from 1991 to 2000. METHODS AND PATIENTS: This is a descriptive retrospective cross-sectional study. The statistical community comprised all patients diagnosed leprosy. This diagnosis was clinical and confirmed through pathology (skin-biopsy) and laboratory (peripheral smear) measures. RESULTS: In this study, the disease was more common in males than females with a mean age of 48.5 +/- 16.2 years. Most of the patients were more than 40 years old. Among 12 patients in this study, six cases were urban and six cases were rural. Six cases were living in Hamadan province and two cases migrated to Hamadan province (one of them from Afghanistan and the other from Kurdestan). Clinical diagnosis was confirmed by pathology in 11 cases, but in one case the clinical diagnosis did not match the pathology. In four cases the clinical diagnosis did not match the peripheral smear. Eight cases were admitted just once. Four cases had a history of recurrence and readmission (two patients had one time recurrence and the other two patients had two recurrences). There was no difference in the clinical findings between first presentation and recurrence. From the point of complication and disability, extremity disability was more common than eye disability. Increased severity of complications was found in patients with a delayed diagnosis and incomplete treatment. CONCLUSION: This study showed that a rapid and correct diagnosis and complete treatment was necessary for prevention of complication and disability in patients with leprosy. Also the accuracy of pathology (skin biopsy) in the diagnosis exceeded the peripheral smear. Skin biopsy is recommended to confirm the diagnosis in all cases of leprosy. In the absence of pathology, patients must be considered as multibacillary patients and treated as such.

[Clinical characteristics of different cordectomy types with potassium titanium phosphate laser].

OBJECTIVE: To study the clinical characteristics of different potassium titanium phosphate(KTP) laser types and surgical the recuperating course of cordectomy under suspension laryngoscope. METHOD: Follow-up observation were applied on 76 patients with cordectomy. Restoration of laryngeal structure and change of the quality of voice were observed and recorded. RESULTS: Type 1, normal laryngeal structure and function were restored postoperatively after 3 months; Type 2, a fold like vocal cord (new vocal cord) appeared 3 months later. Laryngeal structure was basically restored; Type 3, a mucosal fold appeared postoperatively after 6 months in most patients, but it lost motion function. Recurrence time was mainly found 2-4 months. Recurrence rate was 6.7%. Two and 5-year survival rate were 100%. CONCLUSIONS: Cordectomy with KTP laser can result in good effect and less injury. Type 2, 3 can retain thyroarytenoid muscle as much as possible on the basis of radical cure.

Phyllodes tumor of the breast: case series of 40 patients.

Cystosarcoma phyllodes is a rare, uncommon fibroepithelial tumor of the breast. We analyzed the clinical situation in relation to the histopathologic findings. Forty types of surgery, recurrences, histopathologic diagnosis and follow-up of patients were studied retrospectively. Histopathologic examination results were evaluated by logistic regression analysis. Surgery was performed on all patients as the initial treatment. At the first examination 38 cases were evaluated as benign and the remaining two as malignant. Recurrent tumors were seen in nine (22.5%) cases during follow-up with a mean recurrence time of 30.1 months. Five of the recurrent cases were evaluated as malign. The total number of malignant cases reached seven (17.5%). In statistical analysis evidence of tumor necrosis, stromal atypism, stromal cellularity, number of mitoses and stromal overgrowth were found to be significantly correlated with malignancy (p < 0.05). Recurrences were also significantly correlated with stromal cellularity, stromal overgrowth, necrosis and malignancy (p < 0.05). Cystosarcoma phyllodes recur with a high incidence and may transform to malignant disease. The patients should be followed strictly in order to detect recurrence earlier.

[The role of radiation therapy in the treatment of recurrent carcinoma of the vulva after surgery].

This is a retrospective analysis of 25 patients with recurrent carcinoma of the vulva treated by radiation therapy at Bydgoszcz Regional Cancer Center between 1980-95 years. Tumour complete regression were observed in 16 cases. Nine patients were alive without evidence of the disease with a follow-up from 20 to 76 months; median 35.2 months. The results showed that important prognostic significance had: the pattern of recurrence, time interval to relapse and size of recurrent tumour. Also, it seems that results of treatment strongly depends on the total dose of radiation used in the treatment.
