Clinical evaluation of a new method for determining tooth length without radiographs.

A new instrument (the Apexfinder) for measuring tooth length was recently introduced for use in endodontics. The procedure of measuring tooth length with this instrument was found to be simple and easy. It also eliminated the need for radiographs, with their problems. One hundred forty-two teeth were used to compare the new method with two different methods, widely used in endodontics, that employ radiographs. The investigation showed that the new technique offers a direct method for measuring the distance between the coronal and apical ends of the tooth. Statistical analysis of the data indicated that the new method was superior to the others. The Apexfinder was accurate in 90 percent of the cases. When radiographs were used, the results were accurate in 75 percent of the cases, while the use of the radiograph and the tooth-length formula gave 60.4 percent accuracy.

A new method for cell immobilization.

A new method for producing particles and membranes containing immobilized bacteria is presented. These immobilized bacteria display good stability over time making them well suited for use in a packed-bed reactor. Such a reactor is tested as a function of the different parameters of the system. The results are qualitatively similar to those obtained with purified enzyme reactors, but some discrepancies with the plug-flow model are noted. It is necessary to use a more sophisticated model in order to fit the experimental data.

Reconstruction of the middle-aged torn earlobe: a new method.

This paper describes a new method of reconstructing acquired complete and incomplete cleft earlobes which are elongated and have lost their normal bulk due to the wearing of heavy, dangling earrings. Results have been good and problems of breakdown of the repair have not been reported over a period of 2 years.

Do thin filaments of smooth muscle contain calponin? A new method for the preparation.

A new method for the preparation of smooth muscle thin filaments which include calponin was established. We found that calponin readily separated from thin filaments in the presence of 10 mM ATP. By preventing thin filament extract from exposing to ATP, we obtained thin filaments which contained actin, tropomyosin, caldesmon and calponin in molar ratios of 7:0.9:0.6:0.7. We studied myosin Mg-ATPase activity by using the thin filaments in comparison with classical thin filaments prepared by the method of Marston and Smith, which contained the same amounts of caldesmon and tropomyosin as our thin filaments but lost almost all calponin. The presence of calponin reduced the Vmax value for thin filament-activated myosin Mg-ATPase activity by 33% without a significant change in Km value. These findings suggest that calponin inhibits myosin Mg-ATPase activity by modulation of a kinetic step as an integral component of smooth muscle thin filaments.

Determination of isocitrate lyase and malate synthase activities in a marine bivalve mollusk by a new method of assay.

1. A new method for the assay of isocitrate lyase (EC 4.1.3.1) was developed, based on the isolation of 14C-glyoxylate semicarbazone by co-crystallization with authentic carrier. The method was easily adapted to measure malate synthase (EC 4.1.3.2). 2. Interfering reactions were avoided with this method, and isocitrate dehydrogenase (ID) was easily distinguished from isocitrate lyase (IL). Assay of IL in germinating pumpkin seeds gave rates proportional to the amount of extract, with greater sensitivity and less variability than the spectrophotometric method. 3. Six species of marine bivalve mollusks were tested for IL activity, and two species produced glyoxylate: Crassostrea virginica at 0.10 mumol/hr/g tissue, and Petricola pholadiformis at 0.85 mumol/hr/g. The other four species, and four other marine invertebrates from other phyla, lacked detectable IL activity. 4. The rate of disappearance of glyoxylate in the malate synthase (MS) reaction indicated that Petricola had an activity of 0.60 mumol/hr/g: this is the first demonstration of activities of both IL and MS in a marine invertebrate.

A new method of sternal reclosure.

A new method is described for stabilizing dehiscence or instability of median sternotomy incisions using stainless steel retention sutures. The method has been used successfully in patients who had disruption of the sternum following open-heart surgery and resulted in no complications. It is recommended also as a preventive measure for closure of the sternum in elderly, debilitated patients with a fragile sternum and with prolonged low cardiac output after operation.

Physiologic motion after laryngeal nerve reinnervation: a new method.

In this study a new method of reinnervation for unilateral recurrent laryngeal nerve paralysis was performed in canines, producing physiologic vocal fold motion in each of a small series of animals. During the procedure the left anterior division of the recurrent laryngeal nerve was reinnervated with axons from the thyroarytenoid branch of the contralateral recurrent laryngeal nerve. The posterior branch of the left recurrent laryngeal nerve was divided and sutured to the ansa cervicalis to maintain tone in the posterior cricoarytenoid muscle. In all four animals, the right distal vocalis stump was reinnervated with an ansa cervicalis nerve branch. After 3 months physiologic vocal fold motion and electromyographic activity could be demonstrated during mechanical stimulation of the supraglottis (adduction) and during tracheostomy obstruction (abduction). Acoustic data revealed improvement of jitter, shimmer, signal-to-noise ratio, and vocal efficiency in reinnervated animals compared with paralyzed canines before treatment, although the results lacked statistical significance. This approach to the rehabilitation of unilateral vocal fold paralysis is discussed.

[Retrograde renal phlebography with balloon occlusion of the renal artery. Technique and scope of a new method (author's transl)].

A new method of retrograde phlebography of the kidney is described; it is carried out by using a balloon catheter in the renal artery for occluding the circulation. The procedure has been carried out in 13 patients without complication. Early experience has provided very good results in the diagnosis of malignant renal tumours; invasion into the venous system can be demonstrated radiologically. In particular, in cases of carcinoma of the renal pelvis, this method represents a significant improvement of the hitherto unsatisfactory angiographic techniques.

Candidacidal activity of macrophages from three mouse strains as demonstrated by a new method: neutral red staining.

A new method for assessing the candidacidal activity of macrophages utilizing neutral red stain gave results comparable to well established vital staining methods, methylene blue and acridine orange, and to the absence of germ tube formation of Candida albicans. Dead yeast cells are uniformly stained red while viable yeast cells are unstained except for a red stained vacuole. This assay was used to demonstrate the difference in candidacidal activity between resident and in vivo stimulated peritoneal macrophages of CBA/J, BALB/c and CFW mice. Stimulation of peritoneal macrophages with Concanavalin A in vivo resulted in a significant increase in candidacidal activity in the macrophages from male and female mice. In addition, Concanavalin A-elicited macrophages from female BALB/c and CFW mice exhibited greater candidacidal activity than macrophages from male BALB/c and CFW mice whereas Concanavalin A-elicited macrophages from CBA/J male and female mice exhibited a similar candidacidal ability. Resident peritoneal macrophages from male mice were significantly less candidacidal than were macrophages from female mice with the exception of the macrophages from male CBA/J mice which were as active as the macrophages from female mice.

Hydration status measurement by radio frequency absorptiometry in young athletes--a new method and preliminary results.

A new method for non-invasive measurement of the human state of hydration is presented. This method is based on frequency-dependent absorptiometry of radio-waves passing through tissues. A device utilizing this method was constructed and applied to 12 young (24 +/- 1) male volunteers, who were dehydrated for 1-2.5% of their weight by performance of a physical effort (two 30 min bouts of treadmill walking/running at 2, 3, 4, 5, 6 and 7 mph, 5 min at each speed, separated by 10 min rest), under moderate heat stress (40 degrees C, 40% RH). Hypohydration level was determined by body weight measurements taken before each session, after 30 min and at the end of each session. Concomitantly, measurements of radio frequency (RF) absorption were taken. Each volunteer underwent the heat stress exercise twice: one in which no drinking was permitted, and another with free drinking. A correlation (R2 = 0.734) between weight loss and a change in the radio-waves absorption pattern was observed in most of the volunteers, in both hypo and euhydration sessions. Further work to establish the reproducibility and validity of the RF methodology in larger and different populations, i.e., females, other age groups and different health conditions, is already being researched.

Face imaging reconstructive morphography. A new method for physiognomic reconstruction.

A new method of facial identification--Face Imaging Reconstructive Morphography (FIRM)--is herein described. This method permits the construction of objective composites of facial features based on precise cephalometric measurements. The anthropometric measurements are derived from radiographic films taken by the Analytic Morphograph (KLS). FIRM is a rapid, objective, and reproducible method that is distinctly better than the current methods of forensic-anthropological identification.

Sensitive detection of viral antigens with a new method, "laser magnet immunoassay".

A new method, "laser magnet immunoassay" (LMIA), has been developed for sensitive detection of viral antigens. Target viruses captured on microbeads were made to react with antibodies labeled with magnetite particles. In a magnetic field, magnetically labeled antigens dispersed in water were attracted to and concentrated at one point on the surface, resulting in the lifting up of a small surface area. A laser beam which was incident on the point reflected, making an interference fringe. The intensity of the fringe indicates the amount of the magnetite conjugated with antigen. A very low concentration of antigens, such as 5 particles of influenza virus and 0.1 pg/ml of human immunodeficiency virus (HIV) p24 antigen in human serum, could be detected by this method. Application of this method to diagnoses of viral diseases in early stages is discussed.

Effect of propranolol on the QT intervals of normal individuals during exercise: a new method for studying interventions.

A new method was used to study the effect of a single dose of propranolol on the QT intervals during exercise in 11 normal volunteers. They exercised maximally on a bicycle ergometer and repeated the test after taking propranolol (40 mg) by mouth two hours before. Electrocardiograms were continuously recorded on magnetic tape and the cardiac cycle length (RR interval) and the QT interval were measured every five seconds by a computer aided method. The RR-QT data from each test during the exercise phase were analysed by an exponential formula, QT = A - B x exp (-k x RR) and by Bazett's formula, QT = K x square root of (RR). Three reference QT intervals, QTc1, QTc2, and QTc3, estimated at RR = 400, 700, and 1000 ms respectively from the regression curves of both formulas were compared. The exponential formula, which consistently gave a better fit with the data, showed that propranolol had a biphasic action on the QT intervals during exercise. It significantly prolonged the mean (SD) interval at longer cycle lengths (from 287 (27) to 305 (18) ms at RR = 1000 ms and shortened it at shorter cycle lengths (from 198 (14) to 179 (16) ms at RR = 400 ms). In contrast, Bazett's formula did not show any significant effect when the same raw data were used. The exponential formula can be adapted to study other interventions or conditions that affect QT intervals.

A new method for percutaneous nephrostomy.

A new method of percutaneous nephrostomy using techniques to place a No. 12 French balloon catheter in the renal collecting system is described. Case reports demonstrate tumor palliation and temporary drainage to improve the very ill prior to definitive surgery. A translumbar aortogram catheter is placed lateral to paraspinous muscles and Dotter vascular dilators are used to allow insertion of the large balloon catheter.

Application of longitudinal magnification effect to magnification stereoscopic angiography: a new method of cerebral angiography.

A new method of stereoscopic cerebral angiography has been developed which employs 2X radiographic magnification. In order to obtain the same depth perception in the object as with conventional contact stereoscopic angiography, one can made the x-ray exposures at two focal spot positions which are separated by only 1 inch (2.5 cm), whereas the contact technique requires a separation of 4 inches (10 cm). The smaller distance is possible because, with 2X magnification, the transverse detail in the object is magnified by a factor of two, but the longitudinal detail, which is related to the stereo effect, is magnified by a factor of four, due to the longitudinal magnification effect. The small focal spot separation results in advantages such as improved stereoscopic image detail, better image quality, and low radiation exposure to the patient.

Fitting of an analogue cochlear prosthesis--introduction of a new method and preliminary findings.

A new method is suggested for the fitting of a broad-band analogue cochlear prosthesis in terms of frequency response adjustment. This 'frequency-stepped sweep' (FSS) method employs a feedback loop including the patient's loudness judgement to determine a frequency response for the patient's sound processor with which a continuous sinusoidal stimulus with constant amplitude and small increments in frequency can be heard equally loud at the most comfortable loudness level. The FSS-method has been successfully used for the fitting of the sound processors of 40 cochlear implant users. An evaluation of the FSS-method through speech tests employing five different frequency response adjustments with two implant patients confirms its appropriateness. The importance of proper fitting in terms of the frequency response adjustment for a cochlear prosthesis based on broad-band analogue stimulation is demonstrated. The necessary accuracy for the frequency response adjustment is approximately +/- 2 to +/- 5 dB (depending on the dynamic range of the patient) over the frequency range between 100 and 3000 Hz. For the proper adjustments no significant difference has been found between the results for a male and a female speaker.

Open reduction of central compression fractures of the tibial plateau. Preliminary report of a new method and device arrangement.

A new method of reduction and fixation of a uniformly depressed fracture of the tibial condyle is described. A curved steel probe, introduced through a cortical window in the uninjured condyle, is used to reduce the fracture fragments as well as to compact the fracture region (TV-monitored x-ray projectioning is desired). The instruments needed for the operative procedure are as follows: a probe for reduction and compaction, a tube and a plunger for deposition of bone transplants, and a forked plate. Five cases in which the curved probe reduction method has been used are reported.
