Service line assessment and performance management through information integration: the case for cardiovascular services.

Understanding the performance of clinically sophisticated services has become increasingly vital to ensuring the success of a health care organization. Health care delivery systems and providers will require well-designed performance management systems, which is underscored by an operating environment of declining margins, increased capacity issues, and labor shortages in key areas. This article reviews the case for cardiovascular service line assessment and performance management, specifically as a case study example of strategic, focused, and actionable measurement and assessment of clinical and financial performance for both clinical and administrative leaders. The integration of administrative data for comprehensive and strategic assessment of clinically sophisticated services is explored in depth, along with product line definition and positioning.

Management of prenatal care information: integration of the problem list and clinical comments.

To improve the functionality of our MAternal Record System (MARS), we reviewed the clinical problems entered into the MARS database from approximately 2800 patient records and interviewed representative clinicians about their use of the Clinical Comments and Problem List sections of MARS. Problems were assigned a unique term that corrected for spelling, spacing, synonyms, and abbreviation variations. Analysis of these terms suggested design changes that would increase the number of unique entries into the Problem List, including (1) modifications in the automated problem entry functions within MARS and (2) integration of the Problem List and Clinical Comments sections.

CRC Clinical Trials Management System (CTMS): an integrated information management solution for collaborative clinical research.

The Chronic Lymphocytic Leukemia (CLL) Research Consortium (CRC) consists of 9 geographically distributed sites conducting a program of research including both basic science and clinical components. To enable the CRC's clinical research efforts, a system providing for real-time collaboration was required. CTMS provides such functionality, and demonstrates that the use of novel data modeling, web-application platforms, and management strategies provides for the deployment of an extensible, cost effective solution in such an environment.

An intrapreneurial approach to integrating information production services into an academic medical center library.

The many challenges faced by health sciences libraries of all types and sizes often require innovative solutions. When an innovative solution involves calculated risk taking, the approach is called intrapreneurial. At the University of Miami School of Medicine, an intrapreneurial approach solved the fiscal problems of the biomedical communications unit. The Louis Calder Memorial Library inherited these problems when the Department of the Library and Biomedical Communications was created in the early 1980s. In this paper, two intrapreneurial programs are described, and the benefit and suitability of this management style to information services are demonstrated.

Providing integrated information resources to physicians' offices.

Pathologists have been and remain the first major providers of clinical data processed by information tools. The need simply to "get out the numbers" has been superseded by the need to transfer information efficiently. Computerized tools extract trends and convey image information.

Information integration by children: the identification of rules by an alternative method.

Two empirical studies were conducted with 136 preschool children, aged 5 to 6, and 89 adults. Using rectangles of varying sizes, a paired-comparisons procedure revealed two integration rules that determined the children's responses: a multiplying rule and a centration rule characterized by a preference for the maximum of height and width. The adults applied the multiplying rule, but if this rule failed, they used rules similar to those applied by the children. In none of the age groups was there evidence for an adding rule. In a second study, these results were replicated for two other types of response scales, rank ordering and rating, indicating that the results of the first study were not due to the use of the paired-comparison procedure. The present results differed from results of previous studies that used different techniques for the analysis of integration rules.

Computer security: a necessary element of integrated information systems.

The Matheson Report sees the medical library as playing a key role in a network of interlocking information bases that will extend from central repositories of medical information to each physician's personal records. It appears, however, that the role of security in this vision has not been fully delineated. This paper discusses problems in maintaining the security of confidential medical information, the state of the applicable law, and techniques for security (with special emphasis on the UNIX operating system). It is argued that the absence of security threatens any plan to build an information network, as there will be resistance to any system that may give intruders access to confidential data.
