[Causes and incidence of short-life keratitis caused by dacryography. Optimization of protective parameters].

This study was aimed at investigating the changes occurring in superficial corneal epithelium (localized or diffuse, dotted or linear) as observed in 300 eyes after macrodacryography with iodate contrast media (iodized oil and water soluble non-ionic agents). In our opinion, the causes of iatrogenic short-life keratitis are: needle injury, the deposition of iodate contrast medium on the cornea and the reduction of palpebral winking, favoring dry eye, due to superficial anesthesia. The pharmacologic protection of the cornea by means of high-viscosity drugs allows both the number and the degree of keratitis to be markedly reduced. Therefore, contrast media must be chosen on the basis of anamnestic and clinical data, as well as of patient's symptoms, focusing mainly on the characteristics of the various agents--i.e., density, concentration, viscosity.

[Causes and management of gingival coloration caused by metal ceramic crows].

OBJECTIVE: To analyse the marginal gingival coloration caused by metal based ceramic crowns in 28 patients, and to explore the methods to remove or reduce this abnormal coloration. METHODS: 28 patients with metal based ceramic crowns developed gingival coforation. The period of wearing crowns ranged from 2 weeks to 3 years. To remove or reduce gingival coloration, the original metal based ceramic crowns were discarded. Temporary plastic crowns were utilized for 2 weeks. The cervical ceramic approach was used for rehabilitation,or the abutments were re-prepared for metal based ceramic crowns. RESULTS: At the end of follow up,gingival coloration disappeared in patients managed with cervical ceramic approach. While in patients managed with second metal crowns, gingival coloration was improved greatly. CONCLUSION: Gingiral coloration can be removed or reduced by cervical ceramic approach or re-fabrication of metal based ceramic crowns.

[Parvovirus B19 infections. The cause of fifth disease-erythema infectiosum--can also cause aplastic crises, fetal damage and polyarthritis].

Human parvovirus B19 can be an important etiological factor in aplastic crises in patients with chronic hemolytic anemia, in fetal damage, and in acute polyarthritis. B19-virus can only be grown in cell cultures established from human bone marrow, where the virus production occurs in erythroblasts. Parvovirus can cause severe, often fatal, infections in various kinds of animals, also in their fetuses, and many cases of teratogenic changes have been described. The B19-infection is diagnosed by demonstration of either IgM antibodies or B19-DNA in serum samples obtained in the early phases of infection. Patients with EI are infectious before skin eruptions occur, i.e. in the prodromal phase where virus can be detected in respiratory secretion. Transmission of the infection by concentrated, heat-treated factor preparations has been reported. Acute polyarthritis accompanying B19-infections has most often been described to affect the finger, hand and knee joints in adult women. Maternal B19-infections can be complicated by intrauterine infections damaging the fetus and resulting in abortion, hydrops fetalis or stillbirth. The B19-infection has not with certainty been found to be teratogenic. Recent studies seem to show that about 10-20% of primary maternal infections can be complicated by fetal damage, but until further this percentage should be regarded with great reservation.

[Causes and incidence of traffic accidents caused by precipitous disorders of consciousness and illnesses of drivers. A one year study in Upper Austria and Salzburg].

It was attempted to make a survey of all traffic accidents due to sudden disturbances of health on behalf of drivers, being supported by the police of Upper Austria and Salzburg. Only 26 out of a total amount of 53.530 road accidents were made known but the cooperation with the police did not always seem to work too well. Only in the province of Salzburg did the number of these accidents correspond with the expected frequencies of 1.3%. Most of the drivers were internally sick, cardiovascular diseases prevailing. The danger of epilepsy is generally overestimated, that of diabetes and other internal diseases rather underrated. People in general good health were also seen causing accidents due to nausea. In such cases self-endangering is twice as common as the exposure of others to danger. In the majority of the incidents only damage to property resulted. In addition it became apparent, that accidents with damage to people occurred more often in the city than in rural areas, whereas fatalities and severe injuries were more common in the country than in cities.

What causes uptake of technetium-99m methylene diphosphonate by tumors? A case where the tumor appeared to secrete a hypercalcemia-causing substance.

A patient exhibited an unusual constellation of findings: His extraosseous lymphoma sequestered [99mTc]MDP, a bone-seeking agent, while at the same time it appeared to produce a factor that caused hypercalcemia. The dispersed lymphoma cells took up more [99mTc]MDP in vitro than did cultured lymphoblasts suggesting that the in vivo sequestration may have been, at least in part, an active intracellular process.

Thyrotoxicosis caused by functioning metastatic thyroid carcinoma. A rare and elusive cause of hyperthyroidism with low radioactive iodine uptake.

A patient with progressively worsening thyrotoxicosis, refractory to medical therapy, is described. Repeated measurements of thyroidal RAI uptake over a 13 month period were low consistently and could not be explained by iodine ingestion, thyroiditis, or administration of exogenous thyroid hormone. An I-131 scan ultimately revealed striking activity at the base of the skull, reflecting ectopic excessive production of thyroid hormone by a solitary functioning metastatic thyroid carcinoma. The thyrotoxic state resolved after large doses of therapeutic I-131. Typical features of this rare cause of hyperthyroidism are discussed.

[Preliminary results of studies of the incidence, causes and effects of occupational accidents in the Bełchatow Industrial District. I. Direct causes of occupational accidents].

The work has been aimed at an analysis of accidents at work occurring throughout 1981-1982 at the Bełchatów Industrial District. The most common causes of accidents at work have been direct factors resulting mainly from workers' inadvertence or recklessness, improper performance of occupational tasks, lack of appropriate tools, and settlement of private affairs at work. The incidence of accidents at work might be reduced by compliance with safety-and-hygiene-of-work rules, better supervision, work organization and efficient working tools.

In vivo activity of interleukin-2: conversion of a stimulus causing unresponsiveness to a stimulus causing contact hypersensitivity by the injection of interleukin-2.

The intravenous injection of hapten-modified (picrylated) cells causes unresponsiveness. When conventional or recombinant interleukin-2 (IL-2) is also injected, strong contact sensitivity occurs. This IL-2 is effective when given 7 hr after the injection of the picrylated cells or 2 days later, but has no effect when given beforehand. It is suggested that picrylated cells given intravenously fail to induce contact sensitivity secondary to a failure of IL-2 production, and that IL-2 may be one of the second signals which converts a 'tolerogenic' stimulus into an immunogenic stimulus.

[Digital tenosynovitis caused by non-tuberculous mycobacteria. Apropos of the 4th case caused by Mycobacterium terrae].

A 75 year old man developed chronic flexor tenosynovitis in a finger, caused by Mycobacterium terrae. He had a history of diabetes mellitus and arteritis, but not of local trauma. However, triamcinolone had been injected into the tendon sheath on 3 occasions. Chemotherapy was instituted, but after 3 months amputation became mandatory. This is the fourth case reported in which M. terrae was involved, and it supports the responsibility of this organism as a human pathogen. Attention is drawn to the hazards of corticosteroid injections into tendon sheaths.

Alteration in osmolarity of inhaled aerosols cause bronchoconstriction and cough, but absence of a permeant anion causes cough alone.

To determine whether cough and bronchoconstriction result from alterations in the osmolarity or alterations in the ion concentration of inhaled aerosols and to determine if the specific ions in the aerosol are important, we had 9 subjects with mild asthma inhale various solutions while we recorded cough and measured specific airway resistance. To evaluate the effects of altering osmolarity and ion concentration separately, we administered aerosols of hypo-osmolar distilled water (0 mosm), iso-osmolar sodium chloride (308 mosm), iso-osmolar dextrose in water (308 mosm), hyperosmolar sodium chloride (1,232 mosm), and a hyperosmolar solution of dextrose and sodium chloride (1,232 mosm). To evaluate cough without bronchoconstriction, we had the subjects inhale metaproterenol before inhaling the same aerosols. To determine whether the absence of a specific ion was important in causing cough or bronchoconstriction, we had the subjects inhale iso-osmolar solutions of sodium bromide, sodium gluconate, and lysine monohydrochloride. We found that alteration in osmolarity away from iso-osmolarity of inhaled aerosols is a stimulus for bronchoconstriction in subjects with mild asthma. Absence of ions in the presence of iso-osmolarity is not a stimulus for bronchoconstriction, but the absence of a permeant anion is a stimulus for cough. Thus, we found that the responses of cough and bronchoconstriction to inhaled aerosols can be separated.

[Fulminant or sub-fulminant hepatocellular failure caused by metastatic invasiveness of the liver: an other cause of hypoxic hepatitis?].

OBJECTIVES: The purpose of this retrospective study was to report a series of 9 new cases of fulminant hepatic failure due to metastatic liver disease and to identify signs to support a theory of hypoxia. METHODS: In these 9 cases and in 38 previously published cases of fulminant or subfulminant liver failure, we looked for clinical (shock, sepsis, and cutaneous signs of circulatory failure), laboratory (marked increase in serum aminotransferases levels) and histological (cell necrosis) parameters compatible with liver hypoxia. RESULTS: Cutaneous signs of circulatory failure or shock were observed in 3 of the 9 cases in this study, and were not due to cardiogenic or septic shock. A marked increase in serum aminotransferases levels, 10 times above the upper limit of normal, was observed in 8 of the 9 cases in the present study and in 23 of 37 cases of the literature. Liver cell necrosis was observed in 6 of the 7 autopsied patients in this series and in 20 of 34 cases in the literature. Generally, liver cell necrosis was more severe than tumor cell necrosis and was related to the degree of sinusoidal involvement by tumoral cells. In one case, hepatic blood flow was decreased despite a high level of cardiac blood flow suggesting impairment of circulation in the liver. CONCLUSIONS: These 9 cases and a review of the literature support the hypothesis of hypoxic liver cell necrosis leading to acute liver failure in cases of metastatic liver disease. However, liver hypoxia cannot explain all the reported cases and was never due to circulatory failure from cardiac or septic causes, but to the sinusoidal involvement by tumor cells.

A novel mutation in intron K of the PROS1 gene causes aberrant RNA splicing and is a common cause of protein S deficiency in a UK thrombophilia cohort.

In the course of investigating the molecular basis of protein S deficiency in 31 index cases with thrombophilia, we identified seven kindred where the underlying defect was a novel A to G transition 9 bp upstream of exon 12 in intron K of the PROS1 gene. In all but one case, the mutation caused type I deficiency. One individual had type III deficiency. While ectopic transcript analysis using the BstXI dimorphism in exon 15 failed to detect a transcript from the mutated allele, analysis of transcripts spanning exons 11 and 12 revealed a minor mRNA species. Sequencing confirmed the mutation created a new RNA acceptor site introducing 8 nucleotides of intronic sequence into the mature mRNA. Haplotype analysis of the defective PROS1 alleles in six families revealed the same haplotype in all affected individuals suggesting the presence of a common ancestor. Six of the fourteen relatives with the mutation experienced at least one venous thrombotic event strongly supporting the association of the mutation with venous thrombosis.

[Lethal paraquat poisoning caused by spraying in a vinyl greenhouse of causing pulmonary fibrosis with a hepatorenal dysfunction].

A 46-year-old man was hospitalized because of general fatigue. He died of respiratory failure caused by hepatorenal dysfunction. Histologic findings in the lungs were consistent with paraquat poisoning. However, he did not have a history of drinking paraquat, but did have an occupational history of spraying paraquat in a vinyl greenhouse. Retrospectively, chest X-ray films and CT scans showed findings typical of paraquat poisoning. We should inform farmers that paraquat can be toxic when used in vinyl greenhouses.

All-cause and cause specific mortality in a cohort of 20 000 construction workers; results from a 10 year follow up.

BACKGROUND: Construction workers are potentially exposed to many health hazards, including human carcinogens such as asbestos, silica, and other so-called "bystander" exposures from shared work places. The construction industry is also a high risk trade with respect to accidents. METHODS: A total of 19 943 male employees from the German construction industry who underwent occupational health examinations between 1986 and 1992 were followed up until 1999/2000. RESULTS: A total of 818 deaths occurred during the 10 year follow up (SMR 0.71; 95% CI 0.66 to 0.76). Among those were 299 deaths due to cancer (SMR 0.89; 95% CI 0.79 to 1.00) and 312 deaths due to cardiovascular diseases (SMR 0.59; 95% CI 0.51 to 0.68). Increased risk of mortality was found for non-transport accidents (SMR 1.61; 95% CI 1.15 to 2.27), especially due to falls (SMR 1.87; 95% CI 1.18 to 2.92) and being struck by falling objects (SMR 1.90; 95% CI 0.88 to 3.64). Excess mortality due to non-transport accidents was highest among labourers and young and middle-aged workers. Risk of getting killed by falling objects was especially high for foreign workers (SMR 4.28; 95% CI 1.17 to 11.01) and labourers (SMR 6.01; 95% CI 1.63 to 15.29). CONCLUSION: Fatal injuries due to falls and being struck by falling objects pose particular health hazards among construction workers. Further efforts are necessary to reduce the number of fatal accidents and should address young and middle-aged, semi-skilled and foreign workers, in particular. The lower than expected cancer mortality deserves careful interpretation and further follow up of the cohort.
