[Respiratory symptoms and spirometric tests of quarry workers in Rio de Janeiro].

Respiratory symptoms and past history of chest disease as well as spirometric tests were investigated in 72 of 86 (83.7%) employees of a middle sized quarry in Rio de Janeiro, Brazil. Seventy one were men (98.6%), and the mean age was 36.2 +/- 9.3 years (20-65). Forty two (58.3%) had occupations considered as highly exposed to dust, and 30 (44.7%) were considered as lightly exposed. Forty (55.6%) were smokers, 20 (27.8%) never had been smokers and 12 (16.7%) were former smokers. Symptoms and past history were investigated with a questionnaire based on international models, and adapted for the Brazilian public. A high prevalence of symptoms (except for dyspnea) was noted: 47 (65.3%) had one or more symptoms-there was cough in 31.9%, expectoration in 41.7% dyspnea in 9.7% and wheezing in 33.3%. The symptoms were found to be associated mostly with a past history of chest disease, and also with smoking, factors which explained, in part, the presence of the symptoms. No association with a higher exposure to dust was found. The exposure to dust did not explain the symptoms. The spirometric tests were performed on a 6 liter bell spirometer. The means of the parameters were lower than one would expect in a non selected population sample--89.9 +/- 11.2% of predicted for the Forced Vital Capacity (FVC), 90.1 +/- 12.9% for the Forced Expiratory Volume in 1 second (FEV1) and 92.9 +/- 32.7% for the Forced Expiratory Flow between 25% and 75% of the FVC (FEF25-75). However, the proportions of abnormal results were comparable to the reported ones from unselected samples. There was no statistically significant influence of higher dust exposure, past history or smoking on the means of the results. It is concluded that, in this sample of quarry employees, no association between respiratory symptoms or spirometric results and dust exposure could be demonstrated. There was an association between the symptoms, but not with the spirometric results, and a past history of chest disease, as well as with smoking.

[Relationship between gender and age with quality of life in chronic hemodialysis patients].

BACKGROUND: The stagnant mortality rate of the last decade and the increased prevalence of aged people among hemodialysis patients motivated the present study to determine the relation of gender and age with the quality of life (QL) of patients with chronic kidney failure treated with hemodialysis. METHODS: From the sample of 107 individuals who underwent regular hemodialysis in a Dialysis Unit in the interior of the state of Ceará, Brazil, the Medical Outcomes Study Questionnaire 36-Item Short Form Health Survey (SF-36) was used to assess quality of life. Pearson's correlation coefficient was used to calculate and to test the linear correlation between age and the score generated by the SF-36. The score for gender was compared by the t test and the Mann-Whitney when indicated. RESULTS: There was no difference between the score related to gender. There was no negative and linear correlation between age and the dimensions of QL named: Physical Functioning (r=-0.289, p=0.003), Physical Role (r=-0.224, p=0.020), Body Pain (r=0.252, p=0.008), General Health (r=-0.245, p<0.001), Vitality (r=-0.334, p<0.001) and Social Functioning (r=-0.270, p=0.005). There was no correlation between age and the dimensions: Role of Emotions (r=-0.149, p=0.125) and Mental Health (r=-0.158, p=0.103). CONCLUSION: Gender did not correlate with the level of QL among chronic hemodialysis patients, while there was a linear and negative correlation between age and the dimensions relating to the physical aspect of the QL.

Safety, humoral and cellular immune responses to a pre-pandemic adjuvanted influenza A (H5N8) vaccine.

Highly pathogenic avian influenza (HPAI) A(H5) viruses can be transmitted from infected birds to various mammalian species, including humans. Avian influenza viruses (AIVs), members of the Orthomyxoviridae family, possess segmented RNA genomes prone to reassortment, favoring the emergence of novel genetic traits that may alter transmissibility, pathogenicity, and antigenicity. Although no sustained human-to-human transmission has been reported, the potential adaptation of these viruses poses a significant pandemic threat. This study aimed to evaluate the non-clinical safety, toxicity, and humoral immune responses induced by an adjuvanted H5 influenza vaccine in rats and rabbits, to support future clinical safety trials in humans. Male and female Wistar rats and New Zealand rabbits were observed for 14, 28, and 90&#xa0;days after receiving two intramuscular doses of the H5N8 vaccine (15&#xa0;&#x3bc;g HA/dose) formulated with the IB160 oil-in-water emulsion adjuvant. No systemic comorbidities, central nervous system alterations, or relevant clinical signs were observed. Hematological parameters remained within normal ranges, with total and differential leukocyte counts showing only minor fluctuations (<1% of total leukocytes). Mild biochemical variations in urea and hepatic transaminase levels were not correlated with histopathological alterations. The vaccine elicited a robust humoral response soon after immunization, with all groups reaching protective HAI-antibody titers. Although antibody levels declined over time, particularly in males, they remained significantly above baseline, indicating durable immunological memory. Furthermore, the vaccine induced a specific cellular immune response, confirmed by IL-2 and TNF production by antigen-specific T lymphocytes in splenic cell cultures after the booster dose. In conclusion, the H5N8 vaccine with the IB160 adjuvant was well tolerated locally and systemically, without compromising vital organ function. The safety and immunogenicity findings are consistent with expectations for adjuvanted influenza vaccines, demonstrating strong and durable humoral and cellular immune responses.

[Probability of dying in the first year of life in an urban area of southern Brazil].

A birth-cohort of 4,876 children born alive in hospital were selected and followed through up to the age of one year with a view to estimating the risk of dying in the first year of life. All of them were born in 1989, in one of the seven hospitals of an urban area of Southern Brazil and the only requirement for belonging to the cohort was that of residence on the area. The selected variables were: sex, birthweight, age at moment of death, underlying cause of death, and maternal age. The estimated probability of dying in the first year was of 19.9 per 1,000 (77.3% of the deaths occurred during the neonatal period). Perinatal causes and congenital malformations contributed to 80% of the deaths, and infectious diseases were the underlying cause of death in only 1.1% of the losses. The risk of dying in the first year of life due to afections arising during the perinatal period was higher among vaginally delivered babies (20.3 per 1,000) than it was for those born by cesarean section (9 per 1,000). A higher probability of death was present among infants born to adolescent mothers, and those with low birthweight (less than 2,500g). The results brought out the need for improving the quality of prenatal and infant care. They also suggested the hypothesis of a possible association between higher infant mortality and lower socio-economic level.

Cohort study of endodontic therapy success.

A retrospective cohort study was carried out aiming to assess the success rate of endodontic treatment of patients from the Faculty of Dentistry, University of Pernambuco, Brazil. The dental records of the sampling comprised all patients treated in 1998 and 1999. Trials were standardized and a pilot study was carried out to determine the feasibility of the proposed study. Chi-square and Fisher's exact tests were used to calculate the level of significance that was set at 5%. Logistic regression models were used to confirm significant effect of some variables on the endodontic outcome. The final sample size comprised 311 (75.9%) women and 99 (21.1%) men. The majority of cases (82.9%) were considered successful. Success was less frequent for patients with up to primary school education (55.6%) than for those with a higher degree of education (89.7%). The vital condition of the pulp showed a statistically significant relationship with the success of endodontic therapy (p < 0.05). These results suggest that it is possible to attain very high success rates for endodontic treatment when both the intraradicular andthe extraradicular causes of failure of endodontic treatment are well managed.

[Quality of life in chronic renal disease patients submitted to hemodialysis evaluated with SF-36 instrument].

UNLABELLED: Quality of life is a new concept in the health area and controversies exist about its application to clinical practice. Studies carried out in Brazil in the nephrology area have indicated the need for further investigations. OBJECTIVE: To analyze the quality of life of patients with different times of hemodialysis. METHODS: The study consisted of application of the questionnare SF-36, collection of demographic, socioeconomic and biochemical data, as well as of the clinical characteristics. RESULTS: The study was conducted on 184 patients, 63% of them males, aged 46 15 years (X SD), with a hemodialysis time of 30 36 months; 48% had up to 4 years of schooling and 53% belonged to the economic-social classes D and E. Impairment of the different dimensions analyzed was observed, with the poorest results being obtained for Physical Aspects and Vitality. A negative correlation was detected between Age and Functional Capacity, Physical Aspects, Pain, and Vitality, and between Hemodialysis Time and Emotional Aspects. A positive correlation was observed between Schooling and Emotional Aspects and between Hemoglobin and Vitality. We observed that diabetic patients were older than non-diabetic patients and presented lower values for the Functional Capacity and General Health condition dimensions. CONCLUSION: SF-36 proved to be a good instrument for the assessment of the quality of life of hemodialysis patients. The presence of a chronic disease, the need for continuous treatment over a long period of time, advanced age, and the presence of co-morbidities were factors that might interfere with the quality of life of this population.

Dose evaluation in paediatric radiology and adult bone densitometry examinations.

Dose measurements are acknowledged to be a vital part of the quality assurance process in diagnostic radiology, and the use of thermoluminescence dosemeters (TLDs) is a recommended method of entrance dose evaluation. Measurement of doses in radiographic examinations is widely adopted in clinical practice for adults as well as for children. Phantoms can be used to simulate different parts of the body, depending on the materials used to build them. In this work two different sets of phantoms have been prepared with acrylic blocks. The first set was used to simulate children of different ages. The second set was used to simulate the adult spine. The dosimetric measurements have been carried out using TLD and an ionising chamber. Measurements were performed in three X-ray equipments in Aracaju, Brazil. The entrance, half thickness and exit surface doses were analysed.

Early childhood mortality in a Brazilian cohort: the roles of birthweight and socioeconomic status.

The deaths of children aged 1-4 years were studied in a cohort of 5914 Brazilian liveborns. A total of 29 early childhood deaths were recorded (cumulative mortality risk of approximately 6 per 1000), 17 of which (59%) were due to infectious diseases. The death rate was highest in the second year. Deaths were highly concentrated in children from low income (< US $50/month) families, where the cumulative risk of early childhood death was about 10 per 1000; on the other hand, there were no deaths among the 616 children from families with a monthly income of US $300 or more. Birthweight was also associated with mortality: the cumulative risk of children weighing less than 2000 g at birth was 21 per 1000, compared to 4 per 1000 among those with birthweights of 3500 g or more. Simultaneous adjustment for income and birthweight did not substantially change these differentials. These findings confirm the strong association between early childhood mortality and socioeconomic conditions, but also make evident the long-term effects of low birthweight.

[Serving the fatherland: the mobilization of Brazilian nurses during World War II].

In 1944, 73 women nurses were sent to Italy to serve in World War II as part of the Forca Expedicionaria Brasileira (FEB) and the Brazilian Air Force (FAB). This event is studied in the context of the Vargas policies to mobilize the civilian population and to construct an internal front. The Estado Novo and the war constituted an important period in the affirmation of 'modern' nursing, as it was likewise vital in affirming nursing as a professional model for middle-class women. Nursing allowed the State to shape one of its most persuasive images: that of motherland. This image brought maternal care right to the battle front, helping make the war a collective experience that should bring together all men and women--all Brazilians--regardless of social stratum. The drive to forge a domestic front combined war mobilization efforts with efforts to win the middle classes over to the Estado Novo.

[Clinical diagnosis versus autopsy].

Records from 910 autopsies performed at a university hospital in Salvador, Bahia, Brazil were examined in order to assess the accuracy of clinical diagnoses of the patients' underlying causes of death. This study found inaccurate clinical diagnoses in 31% of the cases. The overall rate of diagnostic error appeared to remain fairly stable from 1970 to 1982, being highest for older patients. Thirty-six percent of the 263 cancer deaths were incorrectly diagnosed, and a number of pathologies considered relatively easy to diagnose were not always correctly identified--the underlying cause of death being incorrectly diagnosed in many of the fatalities caused by such ailments as arterial hypertension, chronic obstructive lung disease, pneumonia/bronchopneumonia, and schistosomiasis. Quite aside from their direct medical implications, diagnostic errors of the magnitude observed in this and other studies seriously jeopardize the quality of vital statistics and such statistics' usefulness for improving public health.

Establishment of a computerized biodiversity/biotechnology network: the Brazilian effort.

Information technology plays an increasingly vital role in bioindustrial development and in the establishment of biodiversity conservation and sustainable management of the environment. Recognizing the need and importance of local and regional database developments, the Tropical Data Base is establishing an interactive biodiversity/biotechnology information resource. The system is linked to major international networks and information centres, and is based on a workstation system and personal computers. All software was developed to attend specific needs and to cope with hardware limitations, making the system adequate to other developing countries.

Impaired quality of life of healthy young smokers.

STUDY OBJECTIVE: To investigate the health-related quality of life (HRQL) profile of healthy young subjects with a short smoking history. DESIGN: Observational data at a single point in time. SETTING: Survey in two public universities. PARTICIPANTS: Seventy-seven smoker students without any comorbidities (39 men; mean +/- SD age, 20.5 +/- 2.0 years). A control group for HRQL measurements was composed of 97 healthy, never-smoker students from the same universities (55 men; mean +/- SD age, 20.6 +/- 2.0 years). INTERVENTIONS: All subjects were blinded to the study proposal, and answered autoapplicable forms dealing with healthy habits, smoking, and the 36-item short form questionnaire. RESULTS: Never-smokers showed higher mean quality-of-life scores than smokers in all domains. Statistically significant differences were observed for the domains physical functioning (86.5 +/- 12.9 vs 93.4 +/- 9.6), general health perceptions (64.3 +/- 19.8 vs 79.2 +/- 13.4), vitality (58.4 +/- 20.0 vs 64.6 +/- 16.5), social functioning (59.3 +/- 19.7 vs 76.3 +/- 19.6), and mental health index (66.4 +/- 21.1 vs 71.9 +/- 15.5). CONCLUSIONS: Healthy, light-to-moderate smokers with a short smoking history show significant impairment in physical and mental domains of HRQL in comparison to never-smokers. A better elucidation of these aspects may provide useful information for planning smoking-cessation interventions.

A double-blind clinical trial comparing a fixed daily dose of 150 and 250 IU of recombinant follicle-stimulating hormone in women undergoing in vitro fertilization.

OBJECTIVE: To determine the efficacy and efficiency of two fixed doses of recombinant follicle-stimulating hormone (FSH) in controlled ovarian hyperstimulation. DESIGN: Randomized, double-blind clinical trial. SETTING: Fifteen IVF clinics in Argentina, Brazil, Chile, Colombia, Mexico, and Venezuela. PATIENT(S): Women between 30 and 39 years of age undergoing IVF or intracytoplasmic sperm injection (ICSI). INTERVENTION(S): Daily doses of either 150 IU or 250 IU of recombinant FSH (Puregon) until at least two follicles >or=20 mm were seen on ultrasound. MAIN OUTCOME MEASURE(S): Number of cumulus-oocyte complexes retrieved and total dose of recombinant FSH used. RESULT(S): Two hundred one women received 150 IU and 203 used 250 IU. In the low-dose group 8.9 oocytes were retrieved compared to 10.2 in the high-dose group (not significant). The 150 IU-treated women received a total of 1,589 IU and the total dose used in 250 IU treated women was 2,492 IU. Implantation rates were 10.0% in the 150 IU group and 10.9% in the 250 IU group. The vital pregnancy rates per started cycle in the low-dose and high-dose groups were 17.1% and 16.7%, respectively. Two women, both in the 250 IU group, were hospitalized because of the ovarian hyperstimulation syndrome (OHSS). CONCLUSION(S): An increase from 150 IU to 250 IU daily dose of recombinant FSH in women between 30 and 39 years of age has only limited value in augmenting ovarian response.

A Bayesian space varying parameter model applied to estimating fertility schedules.

We propose a spatial generalized linear model (GLM) to analyse the vital rates for small areas. In each small area, we have a response vector and covariates to explain its variability. The statistical methodology is based on a spatial Bayesian approach and it allows the covariates' parameters of the generalized linear model to vary smoothly on space. Hence, the effect of a covariate on the response varies depending on the random variables measurement location. Our model is an extension of disease mapping models allowing the space-covariate interaction to be modelled in a natural way and giving space a position of intrinsic interest. We introduce the model in the context of fertility curve estimation. In each small area, we have a curve describing the variation of fertility rates by age modelled by Coale's fertility model, which implies a GLM in each area. A simulation shows the advantages of our approach. In addition, the paper applies the procedure to census data used to study the diffusion of low fertility behaviour in Brazil.

[Hospital nursing service--introducing this silent giant].

This article shows the importance of the nursing service in the health organizations. The metaphor "giant" will be used to describe the extent of this service, and the term "silent" will be used to refer to the little acknowledgment the nursing service receives from the community. The extent and complexity of the nursing assistance is vital for the effectiveness of the processes of health care, however its structure and organization is unknown by the community. The nursing service congregates a great number of personnel with different levels of education. The planning, organization, structure and operation of this service follow the principles of the General Theory of Administration. Knowledge from this area has also been used to help health institutions to achieve its goals and objectives. This study also approaches the client and the quality of health assistance, pointing out the need of reexamining the administrative model adopted and looking forward to systemic alternatives.
