[The complementation test with feedback. Preliminary assessment of the test and comparison with a test using a continuous infusion rate].

The complementation test with feedback is described as a method for the assessment of disturbances of volume-pressure compensation. The method is characterized by changing infusion rate and the value of this rate depends on the value of cerebrospinal fluid pressure rise in the test. The infusion is stopped when the relative pressure rise reaches 10 mm Hg. Computer-assisted control of infusion and pressure measurement makes possible achieving of greater accuracy of determination of parameters. The test with feedback was compared with the test with stable infusion rate with regard to the indices characterizing the invasiveness of the method. A statistically significant reduction of this invasiveness was noted.

[Study on antimutagenic effect of lanthanum carbonate in CA test, UDS test and MN test].

Antimutagenic effect of Lanthanum carbonate [La2 (CO3)3] has been evaluated by means of unscheduled DNA synthesis (UDS) test, chromosomal aberration (CA) test in vitro human embryofibroblast (HEF) and micronucleus (MN) test in mouse bonemarrow polychromatic erythrocyte (PCE) induced by four different mutagens, i.e., short wave ultraviolet (UV), hydrochloric acid nitrogen mustard (NH2HCl), arsenic sodium oxide (NaAsO2) and cyclophosphamide (CP). The results of this study indicated that the antimutagenic effect of La2(CO3)3 was obvious in multiple antimutagenic systems in which mutation was induced by UV, NH2HCl and CP, but the effect of La2(CO3)3 on the decrease of CA frequencies induced by NaAsO2 had no statistical significance.

[Intelligence testing in early old age. The correlation between the multiple-choice vocabulary test (MCVT-B) and the progressive matrices test (PMT) in a patient cohort in early old age].

Taking a random sample of 147 patients a substantial correlation of 0.54 between the "Mehrfachwahl-Wortschatz-Test (MWT-B)" by Lehrl and the "Progressiver Matrizen-Test" by Raven could be found. The MWT-B did not show significant differences between the age groups of 34-44 years, 45-54 years and 55-64 years. In the PMT differences between the age groups only showed up when the raw-scores were converted to IQ-scores (reffering to the corresponding age groups). Using the raw-scores no significant differences could be determined. Investigating an average IQ of 109 in the PMT, we found an average IQ of 97 in the MWT-B for the same patients. A main reason for the superelevated IQ-scores in PMT seems to be the reference to different age groups. A new standardization of the test should be undertaken.

[Studies on the immune status of melanoma patients: cell-mediated immune reactions in intracutaneous and epicutaneous tests in the lymphocyte transformation test and the leukocyte-migration inhibition test before and during BCG immunotherapy (author's transl)].

Active immunotherapy with BCG was carried out postoperatively in 22 patients with malignant melanoma. At the beginning and 6 months after treatment cell-mediated immune reactions were checked by both in vivo tests (intracutaneous test with tuberculin and patchtest with DNCB) and in vitro tests (lymphocyte stimulation and leukocyte migration inhibition with tuberculin and DNCB). The results have shown that stimulation of immune response may be obtained by systemic BCG therapy.

Guinea pig maximization test, open epicutaneous test and chamber test in induction of delayed contact hypersensitivity.

A comparison was made between the guinea pig maximization test (GPMT), open epicutaneous test (OET) and chamber test (CT), and the allergens were neomycin sulphate and propylene glycol (PG). The sensitization rate for neomycin in petrolatum was 35% in GPMT, 25% in OET and 25% in CT. The corresponding figures for neomycin in PG were 30% in GPMT, 45% in OET and 32% in CT. PG caused no hypersensitivity in 118 guinea pigs. The results suggested that as a hygroscopic, irritant and penetrating vehicle PG facilitates the induction of delayed contact allergy, especially in OET.

Food-induced contact urticaria syndrome (CUS) in atopic dermatitis: reproducibility of repeated and duplicate testing with a skin provocation test, the skin application food test (SAFT).

IgE-mediated contact urticaria syndrome (CUS) is one of the manifestations of allergy in childhood atopic dermatitis (AD). Allergens such as foods and animal products penetrate the skin easily. They can then cause urticarial reactions in sensitized individuals. A provocation test system for foods, called the skin application food test (SAFT), has been developed. Over more than 5 years, a group of 175 patients with AD was built-up and investigated in a prospective follow-up study with SAFT. SAFT was more frequently positive in AD children aged 0-2 years than in older children. In several children of this population (Group 1), we repeated SAFT within a period of 1 year. In another unrelated group of children (Group 2-1), we compared the results of 'original' SAFT and SAFT using square chambers (Van der Bend) or Silver patches. In the 3rd group (Group 2-2) we compared 'original' SAFT with SAFT using big Finn Chambers. The agreement between the tests was high: in Group 1, we observed 88 to 93% concordant scores, and in Group 2, the scores were 96% to 100%. Statistically, the kappa coefficient ranged from 0.71-0.87 in Group 1, and from 0.83-1.00 in Group 2. SAFT is therefore highly reproducible. Agreement was at least > or = 88% between the scores (the lowest kappa value observed was at least 0.71).

[Irritation test of dexamethasone valerate (DV-17) and other steroid ointments in rabbits. Skin and eye primary irritation tests, and skin cumulative irritation test].

The skin and eye primary irritation tests, and skin cumulative irritation test were performed on male rabbits, which were topically treated with a volume of 1 g/body of 0.06% and 0.12% dexamethasone valerate (DV-17) ointments as a test drug, and 0.1% hydrocortisone butyrate (HB) and 0.12% betamethasone valerate (BMV) ointments as an active control drug in order to evaluate the topical and systemic effects. No skin and eye primary irritations except minimal skin erythema and slight conjunctival redness were seen in above four corticosteroids. Judging from the results such as skin atrophy, inhibition of body weight gains, and decreases of thymus, spleen and adrenal weights, the potency of toxicities among the four corticosteroid ointments appeared to be as follows: 0.12% DV-17, 0.12% BMV greater than 0.1% HB greater than 0.06% DV-17.

Comparative studies of two potency tests for antirabies serum: neutralization test in mice (MNT) and rapid fluorescent focus inhibition test (RFFIT).

At our institute the mouse neutralization test (MNT) was replaced by the rapid fluorescent focus inhibition test (RFFIT). Therefore, RFFIT results were statistically analyzed for their percentage limits at the 95% confidence level and were compared with those of the MNT. An anti-rabies immunoglobulin of human origin and the WHO standard serum were assayed on 20 consecutive days to determine the reproducibility of the RFFIT. Sera of 60 vaccinees were assayed for rabies antibodies by the RFFIT and MNT. The titers as well as the international units per ml showed a correlation of 0.95 and 0.87 respectively. In accordance with previous publications by other authors it was found that the RFFIT is a significantly better reproducticible test system than the MNT and excellently correlates with the latter method.

[Evaluation of the agglutination test (AT), complement fixation test (CFT) and the antiglobulin test (AGT) in the diagnosis of swine brucellosis. III. Basic studies].

In the examinations of the swine sera obtained from swines immunized s.c. with adjuvant Br.abortus S19 vaccine or Br.suis 1417 vaccine, it was found that agglutinins were present after 3 weeks, and C.F. antibodies or incomplete agglutinins normally after injections. Probably, in the first period of Brucella infection negative results of C.F.T. or AGT or both will be obtained. In the swine sera from Brucella free herds, agglutinins reacting with the Brucellognost antigen were present. The performance of mercaptoethanol test or C.F.T. lead in most cases to suitable diagnosis. In our conditions we have not obtained results which permit to classify AGT as a supplement test in serodiagnosis of swine brucellosis.

Sib-pair linkage tests for disease susceptibility loci: common tests vs. the asymptotically most powerful test.

Several statistical tests for linkage between a disease susceptibility locus and a marker locus for sib-pair data are examined analytically. Two common statistics, a test based on the mean number of marker alleles shared identical by descent by sib-pairs, and a test based on the proportion of sib-pairs sharing exactly two marker alleles, are shown to be special cases of a more general statistic. We use this more general statistic to derive the asymptotically most powerful statistic for a given genetic alternative hypothesis, and then compare this statistic with the "mean" statistic and the "proportion" statistic. Results indicate that the "mean" statistic generally compares well with the most powerful statistic. However, in some instances the "mean" statistic may lose power, relative to the most powerful. To guard against this, a new statistic (the maximum of the "mean" and "proportions" statistics) is considered and its asymptotic distribution is derived. Results indicate that this new statistic performs well.

Free amino acids of testes. Concentrations of free amino acids in the testes of several species and the precursors of glutamate and glutamine in rat testes in vivo.

Determination of the free amino acid and lactate content of testicular tissue in rat, guinea pig, rabbit, cat, gerbil, hamster, chicken and bullfrog indicates a substantial species variation. Insulin hypoglycaemia and ammonium acetate toxicity changes the concentration of several free amino acids of rat testes. (14)C radioactivity from labelled acetate and ethanol is rapidly incorporated into some of the free amino acids of rat testes in vivo, whereas incorporation from [(14)C]glucose is relatively slow. These results have been compared with those obtained from similar studies with rat brain. In contrast to brain, there is no evidence for glutamate compartmentation in testes.

Antibody binding patterns in infertile males and females as detected by immunobead test, gel-agglutination test, and sperm immobilization test.

Sera from 214 infertile patients were assayed for antisperm antibodies using the IBT, GAT, and SIT. The new IBT methodology was compared with the more classical tests. Although SIT and GAT did not correlate to any particular antibody class, both were negative if IgA was present alone. The immunoglobulin class presented a preferential sperm region binding site: IgG to the head and tail, IgM to tail tip only, IgA to head and tail. Furthermore, these immunoglobulins from the serum of male patients bound differently to sperm than immunoglobulins from female serum. Finally, we were able to calculate the relative sensitivity, specificity, and predictive values for these tests.

[A new test (C-test) for quantitative examination of crowding with test results in amblyopic and ametropic patients (author's transl)].

A new acuity test was developed with a defined space between symbols to determine the effect of crowding on visual line acuity. Landolt-C was used as a symbol and the space between symbols (either 2 to 3 min. of arc or 17 to 18 min. of arc) was kept constant for all acuity values. The test permits a quantitative examination of crowding phenomena. The experimental results obtained in measurements of visual line acuity in normal-sighted, amblyopic and ametropic subjects are presented. The results of treatment for amblyopia had evidently been interpreted too optimistically in many patients hitherto.

A new, simple, inexpensive means of testing functional vitamin A status: the night vision threshold test (NVTT). A preliminary field-test report.

Vitamin A deficiency is the leading cause of preventable childhood blindness in developing countries. Each year, an estimated 13.5 million children world-wide are unable to adapt to the dark and half a million children progress to complete blindness annually from lack of vitamin A. Most of the currently available methods for assessing vitamin A status are expensive, require sophisticated instrumentation and are not efficacious in field conditions. A simple, inexpensive method was developed to identify children with defective dark-adaptability, thereby providing a reflection of marginal vitamin A stores. The purpose of this preliminary study was to test the field-efficacy of the Night Vision Threshold Tester (NVTT). Analysis of discordant test results among five second-generation assays for anti-hepatitis C virus antibodies also tested by polymerase chain reaction-RNA assay and other laboratory and clinical tests for hepatitis.

The diagnostic performances of five commercially available second-generation assays for anti-hepatitis C virus antibody, two enzyme-linked immunosorbent assays, one enzyme immunoassay, and two particle agglutination assays (passive hemagglutination assay and particle agglutination assay), were evaluated. Among 104 samples from healthy subjects and 300 consecutive samples from patient ordered for routine determinations of anti-hepatitis C virus antibody in serum, assay results showed variable degrees of discordance for 17 samples (4.2%). These 17 samples were further tested by an immunoblot assay, the polymerase chain reaction-RNA assay, and the hemagglutination inhibition assay. Four of the 17 samples were regarded as true positive, since all supplementary assays and clinical data indicated active hepatitis C virus infection. Another five samples were considered false positive because no confirmatory evidence was obtained from the laboratory analysis or clinical data. The remaining eight samples were negative for hepatitis C virus RNA, but the results of the other supplementary tests were indeterminate. Some of these samples with indeterminate results may have been from patients with subclinical cases of disease who spontaneously recovered from hepatitis with persistent anti-hepatitis C virus antibody in their sera.

The relationship between antidiuretic hormone and plasma or urine osmolalities during water restriction test and hypertonic saline loading test in normal children--a change in the apparent tubular response to AVP during these two tests.

We present here the results of water restriction test (WRT) and hypertonic saline loading test (HSLT) in normal children. Maximal urine osmolality during WRT (W-Umax; 1040 +/- 154 mOsm/kg) may be age-dependent (W-Umax = 812 + 23*age, r = 0.52, p < 0.05), although maximal arginine vasopressin (AVP) levels during WRT did not show any correlation with age. The relationship between plasma osmolality (Posm) and AVP during HSLT in children (AVP = 0.31* (Posm-277)) was similar to that in normal adults. A plateau urine osmolality during HSLT (H-Umax) was 713 +/- 109 mOsm/kg. It did not increase with age. AVP levels 3 h after the infusion did not correlate with age. Minimal AVP and Posm values (about 6 pg/ml, 295 mOsm/kg, respectively) for creating H-Umax apparently existed during HSLT. The minimal AVP value (about 6 pg/ml) for H-Umax (during HSLT) was higher than the AVP levels (2.41 +/- 1.37 pg/ml) at W-Umax (during WRT). W-Umax (1040 +/- 154 mOsm/kg) was significantly higher than H-Umax (713 +/- 109 mOsm/kg). Judging from the above comparison of AVP and Uosm (W, H-Umax) at the plateau state of WRT and HSLT in normal children, a change in the apparent tubular response to AVP may be one of the important factors to maintain circulatory volume (CV).

Comparative evaluation of the Candida agglutinin test, precipitin test, and germ tube dispersion test in the diagnosis of candidiasis.

Normal sera and sera from burned patients were examined for Candida agglutinin titers, precipitin titers, and the ability to disperse germ tubes of Candida albicans in an attempt to determine whether germ tube dispersion is correlated with Candida infection as animal models have indicated. Other investigators have reported that immunoglobulin G antibody to Candida interferes with a serum clumping factor resulting in germ tube dispersion. Germ tube dispersion in sera from burned patients with varying degrees of Candida infection is significantly greater than that found in uninfected controls. In addition, the germ tube dispersion test indicated the presence of Candida infection in several patients who had clinical evidence of infection but no detectable agglutinins or precipitins.

A quantitative test for xerostomia. The Saxon test, an oral equivalent of the Schirmer test.

We describe a simple, reproducible, and low-cost test for xerostomia, which involves chewing on a folded sterile sponge for 2 minutes. Saliva production is quantitated by weighing the sponge before and after chewing. Normal control subjects produced greater than or equal to 2.75 gm of saliva in 2 minutes. Three of 32 consecutive, unselected outpatients in allergy-immunology clinics and 9 of 38 patients in rheumatology clinics had decreased saliva production, which was significantly different compared with controls (P less than 0.01). The presence of sicca symptoms was highly correlated with quantitatively abnormal tear and saliva production, according to the results of the Saxon and Schirmer's tests.
