The competence and bacteriologic effect of the telescoped intestinal valve after small bowel resection.

In 20 dogs the distal one-third of the small intestine and the proximal 10 cm of the ascending colon were resected. Bowel continuity was established by the telescoping technique to create an intestinal valve in ten dogs (group A) and by conventional end-to-end anastomosis in ten dogs (group B). Diarrhea and weight loss were similar in both groups. The ileal bacterial growth was significantly higher than the jejunum in group A (P less than 0.001) but not in group B. The jejunal bacterial growth in group B was significantly higher than that in group A (P less than 0.01). Intestinal transit times were 212 +/- 16.0, 219 +/- 152.4, and 163 +/- 85.8 minutes for normal dogs, groups A and B respectively. The competence of the normal ileocecal valve, the telescoped valve, and conventional anastomosis were 56 +/- 1.7, 49.9 +/- 17.4 and 13.2 +/- 7.8 cm of barium, respectively. Artificial intestinal valve formed by the telescoping technique is safe, simple, and effective in reducing bacterial overgrowth in the jejunum and appears to prolong intestinal transit time.

Optimizing peritoneoscopic visualization of the liver utilizing a double telescope technique.

Accurate detection of metastases to the liver is necessary for staging intra-abdominal malignant growths, and precise quantitation of the number and size of metastases is required for monitoring therapy. A great majority of liver metastases occur on the surface of the liver, so that accurate diagnosis may result from more complete visualization of the hepatic surface. A peritoneoscopic technique which optimizes hepatic inspection differs from previous techniques in that two peritoneoscopes are used. The upper abdominal telescope visualizes the superior surface of both lobes of the liver back to the triangular ligament. A telescope inserted at the umbilicus permits visualization of the inferior surface of the liver while the other telescope elevates the edge of the liver.

[Photocystoscopy: experience with a new telescope and polaroid camera].

The ability to photograph the interior of the bladder accurately just by pressing a button has great clinical merit. This approach, which has become a basic method in gastrointestinal endoscopy, is now possible in cystoscopy. We will call this technique which uses a newly designed telescope, Olympus A 3405 and Polaroid Instant Endocamera EC-3 with Polaroid High Speed Color Land Film 600, photocystoscopy . The telescope used in photocystoscopy has more improved resolution and homogeneous brightness through its field of view than the conventional telescope, Olympus O 3405. The photographic image of the bladder interior taken by the Polaroid camera has a good color and fine resolution but its size which is 2.5 cm in diameter is too small for clinical use. Color instantographs by the Polaroid camera are most appropriate for use in photocystoscopy , but many devices and improvements are necessary before it can be used routinely.

New instruments and products. Spectacle-mounted telescopic lenses for children.

Telescopic aids for children have received little attention in the ophthalmic literature. What has been written is anecdotal and stresses the negative aspects of mobility with spectacle mounted aids. The benefits of a telescopic system that also maintains the patient's mobility are presented in this article. Mounted high in a spectacle carrier lens and with a 10 degrees upward inclination the telescope is "out of the way" while the patient is moving about. However, instant availability is obtained by the patient depressing his chin slightly to align his visual axis with the scope. Forty of 167 legally blind students at the Missouri School for the Blind were fitted with these aids. The objective and subjective results were excellent. Students with albinism and aniridia seem well suited for this type of aid.

[Study of the stress distribution of telescope denture restoration of the periodontal tissue].

The stress distribution of telescope and clasp removable partial dentures on the periodontal tissue was studied with strain gauge measuring. The results indicated that the stress distribution of telescope denture was more close to the intact dentitions than the clasp denture did. The telescope denture may produce physiological stimulation to the supporting tissues of edentulous area, hereby it may lengthen the service time of the natural teeth and slow the absorption of alveolar bone.

Reverse telescope anastomotic technique reduces the incidence of bronchial stricture.

BACKGROUND: Bronchial stricture remains a major problem after lung transplantation. We hypothesized that a "reverse" telescope anastomosis, where the donor bronchus is sleeved external to the recipient bronchus, would be associated with a lower incidence of anastomotic stricture. METHODS: Over a 12-month period our Unit performed 35 consecutive single and bilateral sequential lung transplantations. The 56 bronchial anastomoses were constructed as a conventional (n = 27) or as a reverse (n = 29) telescope. RESULTS: Bronchial strictures developed in 48% of the conventional anastomoses but in only 7% of the reverse anastomoses (p < or = 0.05). Furthermore, the reverse telescope anastomosis eliminated the need for stenting. CONCLUSIONS: This technique greatly reduced the need for dilatation, debridement, and stent placement and may reduce the morbidity and mortality associated with anastomotic complications.

An intraocular telescopic lens for macular degeneration.

Age-related macular degeneration is a leading cause of visual loss in adults older than 60 years of age. Once vision has been seriously compromised, the only means of improving visual function are optical devices that produce magnification of images. These devices fall into three categories: (1) high-plus lenses, (2) external telescopes, and (3) a high-minus center intraocular lens combined with external high-plus glasses. The authors designed a new intraocular lens with an entire telescope in its center. In vitro tests were performed to evaluate modulation transfer function, visual field, and optical aberrations. Implantation in cadaver eyes was performed to test the surgical technique and safety. Test results showed that satisfactory modulation transfer function and optic aberration were achieved, and implantation in cadaver eyes was proven to be feasible and safe. It was concluded that a fully implanted intraocular telescopic lens is an effective optical solution for age-related macular degeneration.

Implant-supported telescopic restorations in maxillofacial prosthetics.

Eight partially irradiated oral cancer patients were treated using either mandibular implant and mucosa-supported telescopic coping prostheses or a restoration that was completely implant-supported with telescopic copings. Both types of restorations provided sufficient positional stability and peri-implant hygiene, and functional and esthetic improvement. However, only the completely implant-supported telescopic prosthesis avoided soft tissue ulcers that had the potential for the development of osteoradionecrosis. This type of restoration can especially be recommended for irradiated patients.

Advanced biomaterials used for a new telescopic retainer for removable dentures.

Removable dentures supported by cast-metal telescopic crowns often exhibit an unpredictable increase or decrease in retentive force after being in clinical use for some time. The objective of the present in vitro study was to develop a new retainer for removable dentures and to evaluate its tribological properties. The new retainer is based on a tapered crown design and consists of a conical all-ceramic abutment crown and a coping made of electroplated gold. It was compared with conventional telescopic retainers made of cast metal. There were 30 specimens in groups of equal size by material used (abutment crown/coping): Group 1, gold/gold; Group 2, titanium/titanium; Group 3, ceramic/electroplated gold. Each specimen consisted of 2 conical-shaped abutment crowns (alpha =4 degrees; h = 6 mm; O(base) = 4,5 mm); their copings were rigidly connected at 25 mm intervals. Retentive forces were measured with a universal testing machine following axial loading to 5-400 N. Wear was simulated by 500-100, 000 joining and separating cycles in the presence of artificial saliva. Metallographic cross-sections were made to evaluate the specimens' fit and surfaces with an SEM. Retentive forces in Groups 1 and 2 increased with load, exhibiting nondirectional changes after induced wear. Sometimes the alloys' functional surfaces showed considerable tracks of wear. Neither load nor wear had any effect on Group 3 retentive forces (mean(force) = 5.03 N). The functional ceramic and gold surfaces showed no traces of wear and the best fit (median(gap) = 4.9 microm). Replacing cast metals by ceramics and electroplated gold results in retainers with clinically advantageous tribological effects, implying, in particular, high wear resistance.

New method for autogenous vein grafting with the telescoping anastomotic technique.

An experimental study was performed to examine whether a microsurgical telescoping anastomotic technique could be applied twice in one vessel, for use in autogenous vein grafting. The rat inferior epigastric vein was grafted into a defect created in the femoral artery. The original telescoping method of Lauritzen was used, with two additional suture placements, to allow anastomosis at both proximal and distal sites. By placing the four sutures symmetrically and carefully timing the removal of the proximal and distal clamps, we achieved a patency rate of 77.3%. This is a new method for autogenous vein grafting that may serve as a prototype for an easier and possibly faster vein grafting technique. Our results appear to indicate that complete coaptation of the severed vessel ends is not necessarily required for patency in microvascular repair.

Comparison of the telescoping anastomotic technique with the end-to-end technique utilizing vein grafts for venous defects: short- and long-term results.

The telescoping anastomotic technique was used for both anastomoses of vein grafts placed in the rat left epigastric vein in an effort to make the anastomosis easier and faster. Vein grafting in the right epigastric vein was completed using the conventional technique. The 20 rats with 20 left and 13 right vein grafts, which were patent at 3 weeks, were seen again 3 months postoperatively to compare the two techniques with regard to long-term stenosis and histological changes. Patency of the grafts 2 hr postoperatively was also compared between the two techniques in another 25 rats to eliminate the influence of recanalization on patency. All the vein grafts, which had been patent 3 weeks postoperatively, remained patent after 3 months and no statistically significant difference was found between the telescoping and conventional technique in the degree of stenosis. The vein grafts 2 hr postoperatively were all patent in both groups.

Endoscopy of the eustachian tube: use of the fiberscope and the telescope.

The necessity for direct observation of the pharyngeal outlet of the normal or pathological eustachian tube has always stimulated research for suitable diagnostic instruments. As the result of various investigation procedures currently available, we performed a series of endoscopic screenings using both the Hopkins telescope and the fiberoptic rhinopharyngolaryngoscope. The present study aimed at evaluating and demonstrating the morphological and dynamic functional condition of the pharyngeal outlet of the tube. The present investigation was carried out on 60 males and females, ranging in age from 4 to 70 years. Inspection of the pharyngeal outlet of the tube and its adjoining region enabled us to document some of the various causes for tubal dysfunction. By visualizing tubal movements, endoscopy also made it possible to perform a dynamic study of tubal functions. However, the investigation procedure utilized did not allow us to record any significant differences between the two diagnostic instruments used, except that the fiberscope can be more easily handled, whereas the photographs taken with the telescope are of better quality.

Diagnosis of nosocomial pneumonia in mechanically ventilated patients by the blind protected telescoping catheter.

OBJECTIVES: To demonstrate that blind insertion of the protected telescoping catheter (PTC-NB) through the orotracheal tube can provide reliable pulmonary samples for the diagnosis of nosocomial pneumonia (NP) in ventilated patients. DESIGN: We performed a random comparison between the protected telescoping catheter introduced through a bronchofiberscope (PTC-B) and the PTC-NB to diagnose NP. SETTING: A general intensive care unit of a University Hospital. PATIENTS: 40 consecutive patients on mechanical ventilation and with suspicion of NP. The diagnosis of NP was suspected by clinical and chest X-ray findings. MEASUREMENTS AND RESULTS: NP was confirmed microbiologically in 26 (65%) patients and maintained in 8 patients by clinical and radiological criteria. PTC-NB confirmed the microbiological diagnosis of PN in 21 (80%) patients. The use of antibiotics prior taking respiratory samples reduced the sensitivity of PTC-NB and PTC-B from 100-74% and from 94-70% (p = 0.001). Both techniques agreed in 24 of 33 (73%) patients but such agreement was better when PN was on the right lung. Two patients developed a self-limiting hemoptysis after the PTC-B procedure. CONCLUSIONS: PTC-NB is as sensitive as specific as PTC-B for diagnosing PN in mechanically ventilated patients, being a much easier technique to use.

The role of 70-degree telescopic examination during direct laryngoscopic evaluation of laryngeal cancers.

Endoscopy has currently been an indispensable method in many areas of otorhinolaryngology, especially in paranasal sinus surgery. Telescopic evaluation of the larynx has not drawn much attention. In this study, a 70-degree angled rigid telescope was used along with computerized tomography (CT), indirect and direct laryngoscopy in the evaluation of laryngeal cancer. The critical areas such as the anterior commissure, subglottic region and ventricles were investigated specifically. Nineteen patients were collected in the study. We found the sensitivity for indirect laryngoscopy to be 20-25% and for direct laryngoscopy to be 50-70%. The scores for CT and 70-degree telescopy were 70-100%. In the subglottic region telescopy was superior to CT. We believe that, when it is used in association with CT, telescopy provides better insight when deciding between partial or total laryngectomy and a healthier evaluation of the resection margin.

A variable focus telescopic instrument for indirect ophthalmoscopy with increased magnification and stereopsis.

We devised a new binocular indirect ophthalmoscope and a complimentary examination technique that provide enhanced fundus magnification and optimum stereopsis through a wide range of patient pupillary apertures. The lightweight, mechanically simple instrument uses a variable focus Galilean telescopic observation system to enlarge the condensing lens image of the fundus seen by the examiner. The present device is capable of about X 1.5 fundus magnification with negligible stereopsis through a 20-mm patient pupil, continuously increasing to X 8.5 with greatly enhanced stereopsis through an 8-mm pupil. A zoom focusing capability considerably extends the magnification, stereopsis, and small pupip penetrating characteristics of a given condensing lens by allowing the examiner to binocularly observe the telescopically magnified fundus image throughout the 30-cm to 60-cm focusing range of the ophthalmoscope.

Telescope retainers and their clinical application.

This article reviews the history and types of double crown retainers, known in clinical terms as telescope crowns. It compares rigidly interlocked and resilient retainers, where the design allows for movement between the primary and secondary components of the unit. The nature of the relationship between the components determines the relationship between the removable denture, the remaining dentition, and the edentulous ridge. The resiliency or rigidity of the telescope retainers depends on the configuration of the primary and secondary components and is applied accordingly to prosthodontic requirements and clinical assessment.

Telescopic copings in restorative dentistry.

The complete mouth reconstruction of a periodontally compromised dentition is a tremendous challenge. Although the treatment is complex, the rewards can be satisfying. In some cases, the use of telescopic copings overcomes several of the problems associated with periodontal prosthetic treatment. This article reviews the advantages and disadvantages in the use of telescopic copings in restorative dentistry.
