The importance of applying MRI (CT)/SPECT fusion in the abdominal and thoracic areas has been recognized in recent studies aiming at radionuclide therapy of cancer. According to our earlier results spleen and liver volume determination with different segmentation methods is inaccurate with SPECT alone. We therefore applied a SPECT/MRI registration procedure to the estimation of spleen and liver volumes and spleen/liver activity ratios in three male volunteers administered 111In-labeled thrombocytes and 99mTc-labeled colloids. The objectives of the study were to investigate if the uptake of thrombocytes in the spleen and liver can be measured more accurately when the anatomical borders of these organs are transferred from MRI to SPECT, and to test a SPECT/MRI registration method for improving three-dimensional dosimetry for radiotherapy treatment planning. A good correlation was found between spleen/liver activity ratios calculated from volumetric average activity per pixel values and from total volumetric counts derived from registered data but not from projection data. The average registration residual with this SPECT/MRI fusion method is approximately 1-2 cm in the abdominal area. Combining anatomical images with SPECT is therefore important for improving quantitative SPECT also in the abdomen.
Investigations on the biodistribution of 99mTc-BPHA in normal and probenecid treated mice and a comparison with 99mTc-DTPA injected mice suggested that 99mTc-BPHA is also a glomerular filtration agent. Whole body radioactivity measurements in mice and rabbits after administration of 99mTc-BPHA showed negligible whole body radioactivity at 24 h. Excellent kidney images both in rabbits and a human volunteer were obtained. The significance of higher GFR estimates using 99mTc-BPHA compared to 99mTc-DTPA is discussed.

Ninety-six patients with various diffuse hepatic diseases were observed using both 99mTc colloid and 99mTc iminodiacetic acid (IDA), focusing on the potential etiologies of the disease process as well as its chronicity and severity. The presence of acute hepatic disease was more sensitively depicted with 99mTc IDA than with 99mTc colloid. In chronic hepatic disease, on the other hand, 99mTc colloid and 99mTc IDA demonstrated a similar sensitivity. The potential etiology of the disease process (differential diagnosis) and the chronicity of the disease could be evaluated better with 99mTc colloid. Among the patients with different stages of liver cirrhosis, however, 99mTc IDA significantly discriminated the severity of the hepatic disease. These results suggest that 99mTc IDA may be used to determine the degree of functional disorder in acute hepatic disease and evaluate the severity of diffuse hepatic disease, whereas 99mTc colloid can effectively evaluate the potential etiology and chronicity of the disease.

With a current annual mortality rate of around 35% worldwide, infection remains a significant concern, and the diagnosis and localization of infectious foci is an important health issue. As an established infection-imaging modality, nuclear medicine plays a vital health-care role in the diagnosis and subsequent effective treatment of this condition. Despite the development of several newer radiopharmaceuticals, (67)Ga and leukocyte imaging procedures have maintained their established place for infection. Several techniques in nuclear medicine significantly aid infection diagnosis, including imaging with (111)In-oxine-, (99m)Tc-hexamethylpropyleneamine oxime-, and (99m)Tc-stannous fluoride colloid-labeled leukocytes and with (67)Ga-citrate. Each radiopharmaceutical has specific advantages and disadvantages that make it suitable to diagnose different infectious processes (e.g., soft-tissue sepsis, inflammatory bowel disease, osteomyelitis, occult fever, fever of unknown origin, and infections commonly found in immunocompromised patients). After finishing this article, the reader should be able to identify the properties of an ideal radiopharmaceutical for infection imaging, list a range of available infection-imaging radiopharmaceuticals, compare the relative results of a range of radiopharmaceuticals used internationally to detect infection in the body, understand several common infectious processes that can be diagnosed using nuclear medicine techniques, and select an appropriate radiopharmaceutical to image a range of infectious processes.

The results of analysis of 99mTc-Pyrophosphate (99mPyP), taken as a representative of the group of compounds having an organic P-O-P bond, and of the three diphosphonate compounds: methylene diphosphonate (MDP), 2,3-dicarboxy propane diphosphonate (DPD) and ethane-1-hydroxy-1, 1-diphosphonate (EHDP), which differ in their chemical structure, are shown. Also, some physicochemical parameters such as chloroform-water apparent partition coefficient, the osmotic pressure and pH values in final preparations were studied. The radiochemical purity of these radiopharmaceuticals was determined by the two methods: Sephadex chromatography for separation of 99mTc-hydrolysate and TLC on silica gel with 85% methanol for the determination of free 99mTcO-4. The yield of labelling for both methods was over 90%. Also, pharmacokinetic parameters such as binding to the plasma proteins and to erythrocytes were determined. 99mTc-PyP binding to plasma proteins was higher than the binding of diphosphonate compounds. The quantitative distribution of preparations was determined in experimental animals.
Chemical or electrochemical reduction of the 1,4,7-trithiacyclononane (9S3) complexes [MII(9S3)2][BF4]2 (M = Re (3a) or Tc (3b)) results in instantaneous C-S bond cleavage to yield ethene and the stable MIII thiolate complexes [MIII(9S3)L][BF4] (M = Re (4a) or Tc (4b), L = SCH2CH2SCH2CH2S). Compounds 4 have been characterized by 1H NMR spectroscopy, and the pseudo-octahedral geometry of 4b has been confirmed by X-ray crystallography. Upon electrochemical reduction 4a loses ethene, while 4b can be reversibly reduced to [TcII(9S3)L], which is then further reduced to Tc(I) with loss of ethene. Successive ethene loss is observed in the mass spectra of compounds 3 and 4. The radiosynthesis of 4a with 188Re can be comfortably completed within 10 min starting with 188ReO4- from a 188W/188Re generator, with a radiochemical yield in excess of 90%, and thus represents a practical approach to the preparation of stable 188Re (and 99mTc) thioether complex derivatives/conjugates for clinical use. Crystal data: 4b, C10H20S6Tc, orthorhombic Pbca, a = 12.233(2) A, b = 14.341(2) A, c = 20.726(3) A, Z = 8.
Distribution of osteotropic radiopharmaceuticals was studied in bone, different organs and tissues of rats. 99mTc-pyrophosphate (USSR and France) and 99mTc-diphosphonate (GDR and Hungary) were investigated. Differences in their behaviour were demonstrated. According to a degree of accumulation in the bone tissue, the preparations may be arranged as follows: 99mTc-diphosphonate (GDR), 99mTc-diphosphonate (Hungary), 99mTc-pyrophosphate (USSR and France).
Several cationic or neutral technetium-nitrido complexes of Schiff bases [for which 5-methyl 3-(2-hydroxyphenyl methylene) dithiocarbazate (L1) was a prototype], bis-aminoethanethiol (BAT-TM) and macrocyclic amines were prepared. We report here, the synthesis and isolation of these TcNLn complexes from reaction mixtures by high pressure liquid chromatography or sephadex G25 column. In vivo tissue distribution studies were performed on tumor bearing mice (B16, EMT6, 3LL) after i.v. injection of 10-20 microCi of TcNLn. Although pharmacokinetic differences appeared between the three studied ranges, we did not obtain proof of any particular specificity to tumor tissues. Nevertheless, the complexes were very stable and some of the ligands could be used as chelators after linking side chains or groups inducing specific tumor localization.
Recent studies suggest that 99mTc-labeled radiocolloid (SC) compounded with hydrogen sulfide can be used to visualize lymph channels and nodes. Our study prospectively compared SC with 99mTc antimony sulfide (SbS) colloid, in 28 patients undergoing internal mammary lymphoscintigraphy. Images were recorded on a scintillation camera and computer at 0.5, 1.0, and 3.0 hr. Quantitative analysis included assessment of percent (%) injected dose in nodes, the percent remaining at the injection site, and the relative intensity of the most cephalad node compared to a 57Co standard. The mean (means) % injected dose of both radiocolloids within visualized nodes was less than 1% at each time interval, with no significant differences between means's. The means % injected dose remaining at the injection site at 3.0 hr was 83 for SbS and 76 for SC not statistically significant (N.S.). The means of the ratio of counts within the most cephalad node at 3.0 hr to counts within a 57Co standard was 0.98 for SbS and 1.03 for SC (N.S.). Clinical assessment of number of nodes visualized and extent of radiocolloid migration showed no difference between the two agents. The biological and clinical parameters for the two colloids appear similar when used for internal mammary lymphoscintigraphy.
In the initial stages of tumor formation, overexpression of integrins identifying the RGD sequence (Arg-Gly-Asp) is observed. The aim of the present study was the synthesis and labeling of two novel RGD derivatives, via the precursor [99mTc(H2O)3(CO)3]+, as well as the radiochemical and radiopharmacological evaluation of the labeled products. The labeling led to the formation of a single product in each case (>98%), with noteworthy in vitro stability, fast blood clearance and elimination by the hepatobiliary and the urinary systems.
