[Surgical management of supraglottic laryngeal carcinoma in 182 patients with special emphasis on functional preservation].

OBJECTIVE: To explore the surgical methods and evaluate the long-term result of laryngectomy in patients with supraglottic laryngeal cancer. METHODS: 182 patients with supraglottic laryngeal carcinoma underwent operation from 1979 to 1999, with stage I 11, stage II 45, stage III 49 and stage IV 77 lesions. The choice of surgical procedure was decided with the disease condition of the larynx. The surgical procedures proposed by TD Wang were adhered to as: minor partial laryngectomy 36, major partial laryngectomy 85, subtotal partial laryngectomy with laryngoplasty 22 and total laryngectomy 39. RESULTS: The ultimate rate of larynx preservation was 78.6% (143/182) with 69.8% (88/126) in stage III and IV diseases. The decannulation rate was 81.8% in cases with preservation of laryngeal function. The overall 3- and 5-year survival rates were 82.9% and 67.3%, with 76.88% and 57.4% in the advanced (stage III and IV) cases who survived with preserved laryngeal function. They were 82.5% and 67.0% in similar advanced cases who were treated by total laryngectomy. The difference in the survival rates between these two groups was not statistically significant. CONCLUSION: It is suggested that preservation of the laryngeal function be possible for advanced supraglottic laryngeal carcinoma without compromising the remote survival rate. To improve the rate of larynx preservation, one should follow the surgical methods suggested.

[The detection and clinical significance of telomerase activity in supraglottic carcinomas].

OBJECTIVE: To study the expression and significance of telomerase activity in laryngeal carcinoma. METHOD: We detected telomerase activity of HEP-2 cell line, 26 cases of supraglottic carcinoma tissues and 15 cases of peri-carcinoma tissues with PCR-based assay designated TRAP (for telomeric repeat amplification protocol). RESULT: The telomerase positive rate of supraglottic carcinoma 84.6% (22/26) was obvious higher than that of peri-carcinoma 40% (6/15), P < 0.01. No significant differences in positive rates were found among different groups divided according to clinical stage or metastasis of cervical lymph node, P > 0.05. CONCLUSION: Telomerase activation is related to immortalization process, though not much helpful in diagnosis of supraglottic carcinoma.

[The clinical and pathological research of supraglottic carcinoma with bilateral cervical nodal metastasis].

OBJECTIVE: To evaluate clinical and pathological characteristics in patients with supraglottic carcinoma and bilateral cervical nodal metastasis. METHOD: 60 patients with supraglottic carcinoma treated in this hospital from 1985 to 1994 were retrospectively reviewed, including 30 cases with bilateral metastasis (experiment group) and 30 cases with unilateral metastasis (control group). The two groups were contrasted according to site of tumor, tumor diameter, surface morphology, clinical staging, the degree of invading the contralateral semilarynx, Border's grading, growth mode of tumor edge, the degree of invading preepiglottic space. RESULT: In stage T4 cases with low grading of cell differentiation made up 63.6% (7/11) in the bilateral metastasis group, while 10% in the ipsilateral metastasis group (P < 0.05). Significant differences lied between N0 group and N2 group (P < 0.05). Moderate and severe invasion of contralateral semilarynx in the bilateral group made up 87.5% (14/16), significantly higher than that in ipsilateral group 53.3% (16/30). CONCLUSION: Bilateral neck dissection should be used in T4 cases with low grading of cell differentiation in which the contralateral semilarynx was invaded. Bilateral neck dissection should be taken for the treatment of supraglottic carcinoma with N2.3 diseases. Bilateral neck dissection is recommended for the patients with moderate and severe invasion of the contralateral semilarynx.

[Supraglottic horizontal partial laryngectomy].

OBJECTIVE: To evaluate the supraglottic horizontal partial laryngectomy in the treatment of laryngeal cancers. METHODS: Some researches and modifications are performed on the standard supraglottic horzontal partial laryngectomy: the wounded area in the laryngeal cavity is covered with the lateral membrane of thyroid cartilage; the laryngeal cavity is closed as its anterior wall by folding and sewing up the sternohyoid muscles. From 1983 to 1993, 92 patients (80 men and 12 women) with quamous cell carcinoma of larynx were treated with supraglottic horizontal partial laryngectomy. The patients age ranged from 39 to 72 years; 72% aged from 50 to 59. There were 72 cases (78.3%) in the stages III and IV. RESULTS: The 3 and 5 year survival rates of the patients were 75% and 72% respectively. Most cases have acquired good speech and swallow function with 2-4 weeks; 84.8% (78 patients) were extubated. CONCLUSION: The authors believed that partial laryngectomy was safe and beneficial to some patients with laryngeal cancers.

[Cervical lymph node metastasis and recurrence in supraglottic carcinoma after operation].

OBJECTIVE: To evaluate the regional control efficacy of elective neck dissection (END) in patients with supraglottic carcinoma. METHODS: The incidence of cervical recurrences of 582 patients with supraglottic carcinoma treated in this hospital from 1981 to 1993 were retrospectively reviewed. RESULTS: Three hundred and ninety-two cases (67.35%) of this group had T3 and T4 primary lesions. Among them, 147 cases (37.50%) had positive nodes(N+) and this accounted for 86.47% (147/170) of all N+ patients. Ipsilateral or bilateral cervical recurrence occurred in 33 of 126 (26.19%) node negative (N0) cases who had not had neck dissection and 30 of them (90.90%) were among T3 and T4 groups. However, Contralateral lymph node recurrence occurred only in 40 of 286(13.99%) N0 cases who had had END and 30 of them (75%) were among T3 and T4 groups. There were 163 cases who were readmitted and received surgery again for different purposes. The three and five year survival rates in the second stage END group were 86.67% and 77.78%, in metastasis group 64.81% and 32.50%, in recurrent group 33.33% and 30.77% respectively. There were significant differences in survival rates among these groups. CONCLUSION: Patients underwent elective neck dissection achieved better survival than those on "wait and watch" policy and salvage surgery. Elective neck dissection is recommended for the treatment of supraglottic carcinoma, especially for the T3 and T4 diseases.

[Pathologic feature of occult lymphatic metastasis in supraglottic carcinoma].

OBJECTIVE: To study the pathologic features of occult lymphatic metastasis in patients with supraglottic carcinoma and its implication in treatment. METHODS: Serial sections of 153 neck dissection specimens in 100 patients with supraglottic carcinoma were studied under microscope. RESULTS: Clinically negative nodes had been proved histologically positive in 58 specimens (38%) and 51 occult nodes, size ranged from 0.5 cm to 2.6 cm with average of 1.1 cm. The distribution of metastastic lymph nodes was 1 in level I (2%), 37 in level II (73%), 12 in level III(23%) and 1 in level IV(2%). Among the 51 nodes, 21(41%) were in early stage, 18 (35%) in growth stage, 7(14%) in full stage, 5(10%) were extracapsular. CONCLUSION: Occult metastasis rate of supraglottic carcinoma is as high as 38%, selective neck dissection is recommended for the surgical treatment of this disease.

[Level choice of the neck dissection in clinical N0 patients with supraglottic cancer].

OBJECTIVE: To study the characteristics of the cervical lymph node metastasis in negative node with clinically supraglottic cancer patients and decide which level should be dissected reasonably. METHODS: 57 modified neck dissection (MND) cases in the cN0 with supraglottic cancer were performed. Each lymph node in the 57 (63 sides) MND samples was studied pathologically in order to define the level of the lymph node metastasis. RESULTS: 1,877 lymph nodes were obtained in these MND samples; an average of 29.8 lymph nodes was obtained in one side. Among them 43 lymph nodes were metastasis nodes and 41 lymph nodes were located in the II, III level, 95.4% of metastasis cases. 15 cases (17 sides) in the 57 patients were metastasis (26.3%) and 14 cases of them were in the level of II and III (93.3%). Ipsilateral cervical recurrence occurred in the 3 of 57 cases in the II, III and IV level respectively. The recurrent rate was 5.3%, and 5-year survival rate was 80.7%. CONCLUSION: II and III levels should be dissected in the supraglottic cancer with cN0 and IV level should be included when III level was involved. I and V levels should not be included when the evidence of metastasis is not enough.

[Lateral neck dissection vs radical neck dissection in the management of supraglottic carcinoma with pathologically negative nodes].

OBJECTIVE: To compare the effectiveness of lateral neck dissection (LND) with radical or modified radical neck dissection (RND) for the management of supraglottic carcinoma with pathologically negative nodes (pNO). METHODS: Two treatment groups with pNO supraglottic carcinoma were retrospectively compared: 39 patients who were treated with RND between March of 1980 and December of 1996, and 54 patients who were treated with LND between January 1997 and December 2001. RESULTS: The neck recurrent for LND population was 2.3%, which was not statistically different from the neck recurrent in the RND population (0.0%). Also, the 5-year survival rates were no statistic difference between LND and RND groups (97% vs 94%). Complications and period of hospitalization were both decreased in LND group comparison of RND group (6.7% vs 38.5%; 28 days vs 39 days). CONCLUSION: Comparing to the radical or modified neck dissection, the LND offers less invasive surgery and less morbidity without oncologic compromise for patients with pNO supraglottic carcinoma.

[Clinical analysis of 57 patients with poorly differentiated carcinomas of the supraglottic larynx].

OBJECTIVE: To investigate the clinical characteristics, treatment and prognosis for poorly differentiated supraglottic carcinomas. METHODS: A retrospective study was conducted in 57 cases of poorly differentiated supraglottic carcinomas treated in our hospital from 1980 to 1998. The distribution of the patients according to UICC in 1997 was as follows: stage I 4, stage II 15, stage III 18, stage IV 30. Of the 57 patients, 25 were treated with surgery alone, 9 with irradiation alone, 14 with surgery following preoperative radiation, 7 with postoperative radiation following surgery and 2 with surgery following preoperative chemotherapy. Total laryngectomy was performed on 23 patients and partial laryngectomy on 25 patients. The concurrent neck dissections were undergone for 31 cases (17 unilateral side, 14 bilateral side) and the upper neck dissections for 12 cases. RESULTS: The overall 5-year survival rate, accumulated cervical metastasis rate, metastasis rate of bilateral side of neck, distant metastasis rate, cervical recurrent rate and locally recurrent rate were 47.4% (27/57), 63.2% (36/57), 24.6% (14/57), 21.1% (12/57), 28.1% (16/57) and 10.5% (6/57), respectively. In addition, the local recurrent rate for partial laryngectomy was 12% (3/25). 5-year survival rate for each TNM staging decreases gradually. The difference in 5-year survival rate between T1 + T2 and T3 + T4 and the difference between N0 + N1 and N2 + N3 were statistically significant (chi2 = 4.942, P = 0.026; chi2 = 4.306, P = 0.038). No evidence in our analysis was found about the difference in 5-year survival rate between surgery alone and surgery combined with radiotherapy. The effect of surgery combined with radiotherapy on patients at N2 and N3 was relatively superior to that of surgery alone. CONCLUSIONS: Poorly differentiated carcinomas of the supraglottic larynx had characteristics of the advanced stage in terms of earlier lymph node metastasis and a relatively high rate of cervical and distant metastasis. Surgery was still the primary treatment for this disease and it was feasible to perform partial laryngectomy on certain patients. For patients with T3 who need partial laryngectomy and patients with advanced N stage, the combination of surgery with radiotherapy was supposed to be a priority.

[Features of regional spread of supraglottic larynx cancer].

Development of adequate methods for surgical regional control in supraglottic cancer is one of the most important issues in the management of this disease. The retrospective analysis of 863 patients with supraglottic cancer was conducted. The main clinical and pathological factors influencing the metastatic process were studied. According to the study results the supraglottic cancer most frequently had metastasis in the group I-III lymph nodes. The frequency of regional involvement was high in the period of 6 months after the tumor occurrence and especially increased after 12 months, which indicates to tumor aggressiveness. The possibility of regional metastases increases with the raising of T criteria. The endophytic and mixed growth pattern tumors had metastases with higher frequency compared to egsophytic tumors. The tumor ulceration and low differentiation correlated with high frequency of regional metastases.

Post-transplant lymphoproliferative disorder with supraglottic involvement.

OBJECTIVE: Transplant patients with primary Epstein-Barr virus (EBV) infection may develop post-transplant lymphoproliferative disorder (PTLD). Since many infants are seronegative at the time of transplantation, PTLD is a major concern for paediatric transplant centres. First manifestations of PTLD are frequently observed in the ENT area with adenoidal and/or tonsillar involvement. DESIGN: Retrospective study of two cases of PTLD with confirmed supraglottic involvement, their management and outcome. Only patients with pathologically and immunologically demonstrated B-cell proliferation were diagnosed as PTLD. RESULT: Two infants developed an acute stridor during PTLD respectively 8 and 10 months after orthotopic liver transplantation (OLT). These infants were seronegative for EBV at the time of transplantation. IgM anti-EBV and/or detection of EBV genome by polymerase chain reaction were positive. Laryngeal examination revealed hypopharyngeal and/or supraglottic mucosal hyperplasia. Immunostaining of laryngeal biopsy was positive for latent membrane protein-1 (LMP1). Patients were treated by a reduction in immunosuppression as far as tolerated with the intent to recover natural immune response by the patient over the proliferation of EBV-infected cells. Complete remission of PTLD was observed in these two cases. CONCLUSION: Tonsillar hypertrophy and adenoid enlargement are the most encountered features of PTLD in OLT occurring in the ENT area. Acute stridor with supraglottic involvement may also be observed in PTLD and must be promptly diagnosed as the prognosis of this disorder is related to rapid reduction in immunosuppression and consequently to the recovering of a natural immune response against the EBV infection.

[Self-assessment of the effect of dysphagia on the quality of life in patients after partial laryngectomy for cancer initially located in the supraglottic area].

Dysphagia is frequent in patients after partial laryngectomy for cancer initially located in the supraglottic area. To ensure the best quality of life, establishing how the patient feels after treatment is necessary. Therefore, a self-administered questionnaire was designed to evaluate the dysphagia. 95 patients (75 male and 20 women) who were operated in ENT Department CMUJ between 1998-2004 participated in this study. The MDADI questionnaire in the Polish version was used. The subjective evaluation by the patient of dysphagia is directly related to the size of the tumor and the extent of the partial laryngectomy treatment. Patients, whose tumor was located in supraglottic-glottic area, had subtotal laryngectomy m. Miodoński. Their quality of life was significantly worse due to dysphagia than those with tumors located only in the supraglottic area. Swallowing dysfunctionality was significantly greater after the resection of the hyoid bone. Reconstruction of the base of a tongue by a graft of angiopedunculated submandibular gland diminished the difficulties in swallowing. Moreover, the longer the period after treatment, the higher the quality of life becomes because of the improvement in the swallowing functionality.

[Distribution of cervical lymph node metastasis in patients with cN1 supraglottic cancer].

OBJECTIVE: To study the characteristics of the cervical lymph node metastasis of patients with cN1 supraglottic cancer and to elucidate which levels should be involved during neck dissection. METHODS: Modified neck dissections or radical neck dissections were performed for 108 cases (147 sides) of cN1 supraglottic cancer. All the lymph nodes of the samples of 147 operation sides were examined pathologically to identify the metastatic levels. RESULTS: Among samples of 108 cases (147 sides), 126 lymph nodes were metastasis, among which 113 lymph nodes were located in levels II and III (89.7%, 113/126), and 123 lymph nodes in levels II, III and IV (97.6%, 123/126). The 126 metastatic nodes were distributed in the 109 levels and 96 of them were in the levels II and III (88.1%, 96/109) and 106 were in the levels II, III and IV (97.2%, 106/109). Additionally, 45 cases (63 sides) of the 108 patients were pathologically metastasis (41.7%) and bilateral cervical metastasis occurred in 20 cases (18.5%). The cervical recurrent rate was 7.4% (8/108) and occurred in the levels II, III and IV respectively, during five to fourteen years follow-up. CONCLUSION: Levels II, III and IV dissection may be reasonable for patients with cN1 supraglottic carcinomas, levels I and V dissection may be avoidable when pathological evidence of metastasis in levels I and V has not been obtained. Contralateral lymph node dissection at the levels II, III and IV should be performed in the case of ipsilateral pN + or contralateral metastasis cN1.

[Pathological feature and management of occult lymphatic metastasis in supraglottic carcinoma].

OBJECTIVE: To study the pathologic feature and management methods of occult lymphatic metastasis in patients with supraglottic carcinoma. METHODS: The following are criteria of patients selection: (1) Supraglottic squamous cell carcinoma; (2) Clinical N0M0; (3) no preoperative radiotherapy and (or) chemotherapy. Thirty patients were recruited, male 19 cases, female 11; age: 40 - 72yr, average 54.8yr; T2 8 cases, T3 18, T4 4 (by UICC 1997). Ipsilateral supraomohyoid neck dissections were performed in all cases. Lymph nodes were studied histologically according to the levels of dissection. RESULTS: Six of 30 cases were positive nodes histologically in first operation, 3 were occurrence neck metastasis in opposite side during follow ups. Occult metastasis rate was 20% (6/30) ipsilaterdly and 10% contralaterally. 527 lymph nodes were collected in all of 30 patients, average 17.6 nodes in every side neck. Ten positive lymph nodes histologically were harvested. The distribution of metastatic lymph nodes was 9 in level II, 1 in level III, no in level I. N0 recurrence in larynx and (or) at the neck after dissection. Two years survival rates was 86.7% (26/30) without tumor. CONCLUSION: Occult metastasis rate of supraglottic carcinoma is as high as 30%. The selective lateral neck dissection of level II, III and occasionally, level IV was recommended.

Airway management of a rare huge-size supraglottic mass.

Laser excision of a huge-sized supraglottic mass nearly obstructing the airway passage is a real challenge to anesthesiologists. Upper airway obstruction due to neoplasm in supraglottic region, is traditionally managed by preoperative tracheostomy, however, such a common procedure can potentially have an impact on long-term outcome. A 26-year-old patient presented with dysphagia caused by left cystic vallecular synovial sarcoma. The airway was successfully secured via fiberoptic bronchoscopy, followed by excision of the supraglottic tumor with CO(2) laser surgery. Tracheostomy was not required. The patient was discharged from the hospital on the 4th day of surgery. This case, highlights the possibility to secure the airway passage without performing preoperative tracheostomy resulting in good outcome and short hospital stay.

[Evaluation of effectiveness of radiation therapy for regional control in supraglottic larynx cancer].

The supraglottic cancer is characterised by high incidence of regional metastasis significantly worsening the prognosis. Working out of an adequate regional control represents the one of the important problems. The aim of the study was the evaluation of pre-operative radiation therapy in 331 patients with N-negative supraglottic cancer. Three groups of patients were analyzed: I-radiation therapy/laryngectomy, II-laryngectomy/radiation therapy, III-only laryngectomy. It was found that the main prognostic factors of regional metastasis are: extension, differentiation, ulceration and growth pattern of tumor. The pre-operative radiation therapy is reasonable for effective regional control. The fact that the regional recurrence is developing faster in patients not conducted the radiation therapy at all is also indicating on the above-mentioned. Despite the fact, that the regional control is improving with pre-operative radiation, the overall survival of supraglottic cancer could not be regarded adequate, which requires the further study in terms of evaluation of preventive neck dissections with laryngectomy is this group of patients.

[Preservation of laryngeal function in surgical treatment of T3 supraglottic laryngeal carcinoma].

OBJECTIVE: To explore the feasibility, surgical techniques and the clinical curative effect of preservation of laryngeal function in surgical treatment of T3 supraglottic laryngeal carcinoma (SLC). METHOD: A retrospective review of 70 cases with T3 SLC that we treated from 1994 to 2003 were accomplished. All of the cases were treated with partial laryngectomy. Among them, 42 cases underwent supraglottic horizontal partial laryngectomy, 19 supraglottic horizontovertical laryngectomy, 8 supracricoid partial laryngectomy and 1 near total laryngectomy. All patients were received postoperation radiotherapy with the dosage for 50-60 Gy. RESULT: The 3 year and 5 year-survival rates were 79.2% and 68.4%, respectively. Out of the 70 cases, 60 cases were decannulated with a decannulation rate of 85.7%. The normal diet was resumed in all patients, no buckling and dysphagia obviously. All patients had their successful phonation, no one failure for laryngotracheal atresia. CONCLUSION: It is feasibility to preservative laryngeal function in surgical treatment of T3 SLC, if surgeons master indications correctly, use many kinds restorative procedure expertly and improve surgical skill.
