Acute mesentero-axial volvulus of the stomach in a child.

A case of acute gastric volvulus associated with eventration of the diaphragm in a previously well 6 year old child is reported. The child presented with acute abdominal pain, abdominal distension and vomiting. At operation, mesentero-axial type gastric volvulus was found, associated with laxity of the gastro-splenic, gastrohepatic and gastrocolic ligaments and eventration of the left hemidiaphragm. Rapid recovery followed surgery that included release of distension and fixation of the stomach to the anterior abdominal wall. There has been no evidence of recurrence on a 2 year follow-up.

Pneumoperitoneum induced by pneumatosis cystoides intestinalis associated with volvulus of the stomach.

A rare association of pneumoperitoneum induced by pneumatosis cystoides intestinalis with chronic mesenteroaxial volvulus of an irreducible stomach in a paraesophageal hiatus hernia is described. The importance of accurate radiologic work-up is emphasized. The possible significance of this association to explain the pathogenesis of pneumatosis cystoides intestinalis is discussed.

Volvulus of the stomach after traumatic hernia diaphragmatica.

A patient with gastric volvulus after traumatic diaphragmatic rupture is reported. The trias of Borchardt (pain in epigastrio with gastric distension, hiccups with the inability to vomit, inability to secure the passage of a nasogastric tube) must raise the suspicion on acute gastric volvulus, especially if these abdominal complaints are associated with blunt abdominal trauma in the past. In these cases, lateral X-ray examinations of the chest and swallowing studies establish the diagnosis.

Acute idiopathic volvulus of the stomach.

A report is made of the occurrence of acute idiopathic volvulus of the stomach in a 65 year old Nigerian male patient. To our knowledge, this is the first reported case of this rare condition in a Nigerian. The classification, diagnosis and surgical management of gastric volvulus are discussed and the world literature on this condition reviewed.

[A case report of Bochdalek's hernia with volvulus of the stomach in an adult].

We report a rare case of Bochdalekhs hernia, congenital posterolateral diaphragmatic hernia with mesenterioaxia volvulus of the stomach, in an adult. A 23-year-old man suddenly felt severe pain in his left chest after drinking beer. Roentgenologic examination of the chest showed air above the left diaphragm, and the mediastinum was displaced to the right. Upper gastrointestinal series revealed mesenterioaxial volvulus of the stomach in which the pylorus was displaced to the left. Thoracotomy through the left seventh intercostal space was performed. The stomach, colon, spleen and greater omentum were displaced into the left thoracic cavity. After reduction of the herniated viscera, a defect measuring 10 X 4 cm, in the posterolateral part of the diaphragm, was closed with double interrupted Tevdek sutures. Three months later, he was asymptomatic and his chest roentgenogram was normal.

Laparoscopic repair of intrathoracic mesenterioaxial volvulus of the stomach in an adult: report of a case.

Intrathoracic gastric volvulus is an uncommon entity, in which the stomach undergoes organoaxial torsion occurring in the chest due to the concomitant presence of an enlargement of the hiatus. We herein report an unusual case of acute intrathoracic mesenterioaxial volvulus of the stomach. Gastric rotation occurred at the transverse axis and the mobile pylorus herniated in the chest through a large hiatal defect. Following a reduction of the volvulus through the use of a nasogastric tube, the patient underwent an elective laparoscopic repair. The mechanisms of volvulus with the relative diagnostic and therapeutic implications are discussed.

Organoaxial volvulus of the stomach following corrective surgery for gastric outlet obstruction.

We report an unusual complication following vagotomy and pyloroplasty for chronic gastric outlet obstruction. Persistence of increased gastric aspirate led to the diagnosis of organoaxial volvulus of the stomach on barium studies. We postulate that laxity of the gastric suspensory ligaments after gastric decompression and postoperative adhesion were responsible for its development.

Traumatic eventration of diaphragm complicated by mesenteroaxial volvulus of the stomach.

Diaphragmatic disruption is an uncommon consequence of blunt trauma and is often overlooked unless there is a high index of clinical suspicion. A case of eventration of left dome of diaphragm following trauma with mesenteroaxial volvulus of the stomach is presented. Anatomical considerations on levels of diaphragm on chest x-ray has been elaborated.

Volvulus of the stomach: an African series and a review.

A radiological diagnosis of gastric volvulus (GV) was made in 11 of 576 consecutive upper gastrointestinal series at the University of Benin Teaching Hospital, Nigeria, over a two-year period. The clinical symptoms were thoracico-abdominal in three and abdominal in eight; these cases were evaluated as acute in three, acute upon chronic in two, and chronic in six. There was a significant delay in the diagnosis in all cases (except a neonate in the series), and no case was diagnosed on clinical grounds alone.THE PREDISPOSING FACTORS (EXCEPT THE CLINICAL MISDIAGNOSIS OF THE NEONATE) IN SIX OF THE SEVEN CASES THAT CAME TO SURGERY WERE: diaphragmatic hernia and perigastritis (left lung abscess, thoracic empyema), arteriomesenteric compression of the duodenum in pregnancy (peptic ulcer), splenomegaly (hepatosplenomegaly, ascites, esophageal varices), previous gastrojejunostomy (stomal ulcer, left subphrenic abscess) and two cases of intestinal malrotation with mesenteric abnormalities (small bowel obstruction in one and duodenal atresia in the other). In one idiopathic case, gastric outlet obstruction was clinically suspected prior to surgery. Thus, the putative rarity of GV in black Africans is not supported by this experience.Gastric volvulus is a clinico-radiologic entity that may present with a confusing thoracico-abdominal symptom complex. A greater awareness of the radiologic features is quintessential to an expeditious and usually successful surgical management that will avoid potentially serious complications. Negative surgical findings do not exclude GV as the underlying cause of acute abdomens necessitating emergency laparotomies.

[Volvulus of the stomach in combination with hiatal hernia (author's transl)].

Cases of acute or chronic abdominal complaints in adults can be accounted for as conditions resulting from volvulus of the stomach with oesophagial hernia. In a total of 73 cases of hiatus hernias the author has observed 6 patients with a simultaneous volvulus of the stomach (8,1%) from 1967 to 1976. Pathogenesis, symptoms, surgical indication and the operative technique are discussed. By means of gastropexy, fundiplication and narrowing of the oesphageal hiatus the patients could be freed of complaints and recurrences.
