[Epidemiological diagnosis of suppurative-septic hospital infections in surgery].

The authors have analyzed the results of epidemiologic diagnosis of suppurative-septic hospital infections after surgery, that helped them develop a preventive system permitting a decrease of the incidence rate of suppurative-septic infections in surgery 1.8-fold within a year. The main features of the epidemic process in suppurative-septic infections is described. A total systems approach to epidemiological diagnosis is necessitated.

[Changes in the ruminal contents in suppurative surgical infection in cattle].

Rumen content (pH, number of infusoria, quantity of volatile fatty acids and ammonia) was studied in 151 cattle (50 healthy animals, 15 with experimental suppurative infection and 86 with spontaneous suppurative infection). In cases of cattle suppurative infection a statistically significant reduction was observed in the four rumen content indices: pH from 7.61 +/- 0.02 fell to 6.90 +/- 0.06; number of infusoria--from 295 820 +/- 2890 per cm3 fell to 75 867 +/- 1260 per cm3; volatile fatty acids from 919.7 +/- 10.08 mg% diminished to 489.48 +/- 18.91, and ammonia from 27.69 +/- 0.51 mg% was reduced to 12.23 +/- 0.73 mg%. In case of recovery a tendency toward normalizing rumen content was observed.

[Experience with the treatment of suppuration surrounding synthetic vascular prostheses].

Experience with the conservative and operative treatment of suppurations around vascular prostheses in 16 patients is presented. Spread of the purulent inflammation around prostheses from the place of primary suppuration on the femur up to the portions proximal to the inguinal ligament was taking place within the first 1-2 weeks after the appearance of the first symptoms of suppuration. Partial resection of infected portions of the prostheses and substitution of them by autovenous grafts are thought to be more expedient within these terms. Such operations are less traumatic and technically more simple than interventions associated with a complete ablation of aorto-femoral prostheses and a simultaneous substitution of them by new vascular prostheses or transplants.

The early signs and symptoms of neurotologic complications of chronic suppurative otitis media.

The delay in the diagnosis of neurotologic complications of chronic suppurative otitis media has been explained in a number of ways, including antibiotic suppression, surgical drainage, and a lowered index of clinical suspicion because the problem is relatively rare today. Twelve cases of neurotologic complications of chronic suppurative otitis media were reviewed to determine the early and late findings that should raise the physician's index of suspicion. Purulent, malodorous drainage, headache, and fever were the significant early findings. Altered mental status was a late finding, and usually indicated established intracranial infection. By drawing attention to the early findings, we hope to lower the morbidity and mortality associated with these complications.

[Effects of Burow's solution as an ear drop on Intractable chronic suppurative diseases of the external ear canal and middle ear].

OBJECTIVE: Burow's solution was developed by Karl August von Burow in the 19th century as an ear drop. Thorp et al. reported its excellent effect on chronic suppurative otitis media in 1998. We applied Burow's solution to intractable chronic purulent diseases of the external and middle ear in the 12 months from February 2001. SUBJECTS: Subjects were 25 ears of 21 patients--35-79 years old, 10 men's ears and 15 women's ears--whose disease has continued for an average of 3.78 years. Diseases and patients are as follows: 1) 11 ears with postoperative mastoid cavity problems, 2) 7 ears with chronic external otitis and chronic eczema of the external ear canal, 3) 7 ears with fungal otitis externa, 4) 6 ears with chronic otitis media with perforation, and 5) 2 ears of chronic granulated myringitis. METHOD: The solution was dropped into the ear canal once a day for 10 min or cotton balls soaked with the solution were applied to the canal wall. Criteria of efficacy were divided into cured, effective and unchanged. RESULTS: 8/11 ears with a cavity problem were cured (72.7%) and 3 ears showed treatment to be effective. All 7 ears with chronic external otitis and chronic eczema were cured. All 7 ears with fungal otitis externa were cured. 4/6 ears (66.6%) with chronic perforated otitis media were cured and 1 ear each showed treatment to be effective and 1 unchanged. Two chronic granurated myringitis were cured. Twenty of 25 ears (80%) were cured. Pre- and posttreatment audiometries showed almost the same. The cure appeared within 3 days to 3 weeks. The solution was not effective against mucoid secretion, cholesteatoma, or residual mastoid cells. The day after treatment, serous secretion appears temporarily. CONCLUSION: Burows's solution is seemed to be very effective and nonototoxic as an otic drop for treating suppurative ear diseases.

[KANAMYCIN AEROSOL THERAPY IN 200 CASES OF BRONCHOPULMONARY SUPPURATIONS].

This paper describes the results obtained in 200 patients with bronchopulmonary suppurative diseases who received kanamycin aerosol therapy under intermittent positive pressure and in whom bacterial sensitivity study (antibiogram) was favourable. This method of treatment was well tolerated and was practically free of side effects. In most cases the improvement was rapid, was maintained and was often life-saving.The first series contained nine patients with pulmonary tuberculosis and a constant finding of sputum positive for Koch bacillus; four of these became negative after eight weeks of treatment. This suggested that kanamycin aerosol therapy had some effect on the Koch bacillus and that the study should be continued with a much larger group of patients with tuberculous suppurative pulmonary diseases to determine the efficacy of this antibiotic.

[Recurrent laterocervical suppurations. Role of fistulae and cysts of the 4th branchial pouch].

Four cases of fistula of fourth branchial pouch are used as a basis for a description of the two possible clinical pictures: left cervical suppuration in a neonate with respiratory distress or recurrent cervical cellulitis in an older child or young adult usually diagnosed as a suppurative thyroiditis. Diagnostic features are outlined and emphasis placed on the need for an adapted surgical excision originating at the pharyngeal orifice of the fistula. In two of the four cases reported its trajectory was between muscle and mucosa planes and then in contact with the cricothyroid joint as a small cord, otherwise unidentifiable.

Treatment of chronic suppurative otitis media with topical tobramycin and dexamethasone.

OBJECTIVES: To investigate the safety and efficacy of a topical combination of tobramycin and dexamethasone in a primate model of chronic suppurative otitis media (CSOM) and to explore the contribution of the added topical steroid for the treatment of CSOM. DESIGN: Blinded, randomized, placebo-controlled trial. SUBJECTS: Sixty juvenile cynomolgus monkeys randomized into the following 6 treatment groups of 10 monkeys each: 0.3% tobramycin (group 1), combined 0.3% tobramycin-0.1% dexamethasone (group 2), combined 1.0% tobramycin-0.33% dexamethasone (group 3), 0.1% dexamethasone (group 4), vehicle (group 5), and phosphate-buffered saline solution (group 6). INTERVENTIONS: Chronic suppurative otitis media was established by inoculating the right ear with Pseudomonas aeruginosa. After 4 weeks of drainage, animals were treated according to the group assignment with 3 drops twice daily for 7 weeks. Hearing thresholds were monitored with repeated auditory brainstem response testing (ABR), and clinical response was monitored with repeated otoscopic examinations and cultures throughout the study. Cytocochleograms were evaluated for quantification of outer hair cell loss. RESULTS: Rapid resolution of otorrhea and eradication of P aeruginosa occurred in all groups receiving tobramycin. The inclusion of dexamethasone accelerated the resolution of otorrhea and negative yields of cultures compared with tobramycin alone. Otorrhea and positive culture findings persisted in the groups not treated with topical antibiotic. Results of ABRs at 4 and 8 weeks and cytocochleograms for outer cell hair loss were not affected by drug administration. Perilymph samples collected at the end of the study showed no detectable tobramycin. CONCLUSIONS: Combined tobramycin-dexamethasone ear drops were safe and effective in the monkey CSOM model. Dexamethasone enhanced the efficacy of tobramycin.

Moxalactam therapy. Its use in chronic suppurative otitis media and malignant external otitis.

Moxalactam disodium is a new broad-spectrum antibiotic structurally related to the cephalosporins. Eleven patients with chronic suppurative otitis media with mastoiditis and four patients with malignant external otitis were treated with moxalactam. Pseudomonas aeruginosa was isolated from external ear or mastoid in all patients. Twelve of 15 patients had undergone prior surgical procedures and 11 of 15 had failed recent antibiotic therapy. Three patients with malignant external otitis did not respond to parenteral carboxypenicillin and aminoglycoside therapy. Success rate for chronic suppurative otitis media was 73% as judged by eradication of P aeruginosa and resolution of symptoms 30 days after moxalactam therapy. Two patients with malignant external otitis were cured, and two were unevaluable since they may have died of noninfectious causes. Moxalactam disodium, at a dosage of 9 g/day, may be useful for serious Pseudomonas ear infections, including those refractory to other antibiotic therapy.

Impaired vocal cord mobility in the setting of acute suppurative thyroiditis.

BACKGROUND: Acute suppurative thyroiditis is an increasingly rare entity, accounting for less than 0.1% of thyroid surgery. This is the first report in the literature of acute thyroiditis causing a unilateral reversible vocal cord paralysis. METHODS: A 41-year-old woman was initially seen with a unilateral vocal cord paralysis resulting from acute suppurative thyroiditis. RESULTS: The vocal cord paralysis resolved following surgical drainage of the thyroid abscess and treatment with intravenous antibiotics. CONCLUSIONS: We present this case to emphasize the need for a complete and thorough head and neck exam including preoperative documentation of vocal cord mobility.

Susceptibility of microorganisms isolated from chronic suppurative otitis media to ciprofloxacin.

In order to evaluate the susceptibility of the microorganisms isolated from chronic suppurative otitis media to ciprofloxacin, cultures of specimens from 127 patients with chronic suppurative otitis media and their antibiotic sensitivity results were examined. The most common aerobic isolates were Pseudomonas sp., Proteus sp., and Staphylococcus aureus with recovery rates of 40.7%, 21.6% and 19.1% respectively. Sensitivity results showed that 6.2% of Pseudomonas isolates, 2.9% of Proteus isolates, 10% of Staphylococcus aureus isolates and 8.3% of Escherichia coli isolates were resistant to ciprofloxacin.

Suppurative keratitis in rural Bangladesh: the value of gram stain in planning management.

External eye disease which result in corneal scarring are an important cause of blindness in Bangladesh and at the Chittagong Eye Infirmary and Training Complex (EITC) over 200 cases of suppurative keratitis are managed each year. We reviewed the records of 127 cases of microbial keratitis to determine the relative contributions of Gram stain and culture to diagnosis of the causative organism. There were 107 culture-proven cases of microbial keratitis amongst the 127 patients in this study. Gram stain was positive in 89 cases which represents 70% of the total and 83% of all culture-proven cases. Streptococcus pneumoniae and Pseudomonas sp were the commonest bacteria isolated and Aspergillus sp and Fusarium sp the commonest fungi. In 20 cases (16%) no organism was isolated on Gram stain or culture. Our results support the use of both Gram stain and culture in isolation of the causative organism in cases of suppurative keratitis in Bangladesh. However the low cost of Gram stain and its useful recovery rates for both bacteria and fungi support its use as an initial investigation for microbial keratitis at the secondary level of eye care in rural Bangladesh.

Cefdinir versus amoxicillin/clavulanic acid in the treatment of suppurative acute otitis media in children.

An investigator-blinded, randomized, multicenter study was conducted to compare the efficacy and safety of cefdinir and amoxicillin/clavulanate (amoxicillin/CA) in the treatment of pediatric patients with acute suppurative otitis media. Patients 6 months to 12 years of age were randomized in a 1:1:1 ratio to receive cefdinir 14 mg/kg once-daily, cefdinir 7 mg/kg b.i.d., or amoxicillin/CA 13.3 mg/kg t.i.d. Test-of-cure was determined 11 to 16 days post therapy. Of the 752 patients who entered the study, 665 (88%) completed treatment and 595 (79%) were evaluable. Response rates in the three treatment groups were similar. Overall rates of adverse events were statistically lower in the cefdinir once-daily group than in the amoxicillin/CA group. Diarrhea was the most common adverse event in all treatment groups. Cefdinir given either once-daily or twice-daily is a safe and effective treatment for pediatric patients with acute suppurative otitis media.

Recurrent suppurative thyroiditis due to pyriform sinus fistula: a case report.

Acute suppurative thyroiditis is a rare disease, particularly in childhood. We present a case with recurrent acute suppurative thyroiditis due to a pyriform sinus fistula originating from the fourth branchial pouch. The typical symptoms of a pyriform sinus fistula are recurrent left-sided pain and swelling of the neck with signs of acute bacterial inflammation. Diagnosis should be made by high resolution ultrasound, barium meal studies and endoscopic examination. During acute exacerbations treatment with antibiotics is indicated, but permanent cure can only be attained by complete fistulectomy.

Acute suppurative renal inflammation.

Diagnostic and therapeutic aspects of acute suppurative renal inflammation are discussed in connection with 374 cases. Despite negative X-ray and laboratory findings, patients were submitted to operative exploration and 27% of them revealed subcapsular abscesses. Anamnestic facts (fever, shiver, etc.) and closely observed progression in the patient's morbid condition are factors to indicate surgery. Better results from the second part of the observation period (1981-1989) are due partly to better timing of the surgical intervention, partly to modern, highly effective chemotherapy. The mortality rate of acute suppurative renal inflammation dropped from 11 to 3.5%.

Pathophysiological aspect of the liver in acute obstructive suppurative cholangitis.

This study is prompted in order to clarify the pathophysiological aspect of the liver in acute obstructive suppurative cholangitis (AOSC). An experimental model of AOSC, was prepared by intracholedochal infusion of endotoxin in dogs with and without obstructive jaundice. In the patients with AOSC, liver function was aggravated remarkably compared with that in the stage of non suppurative cholangitis. A rapid fall of platelet count occurred. Experimentally, after intracholedochal infusion of endotoxin, liver function revealed significant hepatic cellular damage. A considerable increase in the S-OCT level was accompanied by a marked rise in blood ammonia concentration. Liver damage due to obstructive jaundice was further aggravated by endotoxin infusion. The level of serotonin in the liver tissue increased markedly after endotoxin infusion. This was accompanied by a rapid fall of platelet counts. Serotonin is considered to be a factor which may cause an impairment of hepatic microcirculation and then hepatic cellular damages. It may be concluded that AOSC is induced by cholangio-venous reflux of endotoxin. Liver function is impaired remarkably due to increased bile canalicular pressure, to the direct affect of endotoxin on liver cells during the process of cholangio-venous reflux, and to impairment of hepatic microcirculation in endotoxin shock. Liver dysfunction contributes to develop this clinical entity and play an important role to make its outcome fatal.

Hyperthyroidism associated with acute suppurative thyroiditis.

Acute suppurative thyroiditis is a rare condition with potentially serious complications. It has been suggested that hyperthyroidism may be a feature in occasional cases but this has been poorly documented. We describe a patient in whom acute suppurative thyroiditis caused transient hyperthyroidism.
