[Omentoplasty in surgical treatment of osteomyelitis of the ribs and sternum].

Seven-year experience in the treatment of 21 patients with chronic osteomyelitis of the ribs and sternum is summarized. Great omentum was used as plastic and disinfecting material in combination with laser radiation. Indications, techniques, short- and long-term results are presented. There were no recurrences of purulent process in the bones. Omentoplasty is the method of choice in surgical treatment of purulent lesions of costal cartilage and sternum.

Force distribution in wire sternum sutures: the consequences for sternal closure rigidity.

BACKGROUND: This article provides a biomechanical analysis of the distribution of force in wire sutures used for sternal closure in cardiothoracic surgery. MATERIAL/METHODS: The parameters we measured included displacement in the elastic region, plastic limit, and rupture load on the closure. Force distribution analysis was carried out for every kind of wire suture. A linear regression line of displacement as a function of increasing load was determined for each closure method. RESULTS: Straight wire had less displacement at each load, figure-8 had medium, and interlocking multitwisted (IM) had the most displacement within the elastic range of the specimen. The differences between straight and interlocking multitwisted sutures were statistically significant. When the plastic limit was compared between straight and interlocking multitwisted closure, the differences were statistically significant. In the straight suture the forces used to approximate the sternal halves are perpendicular to the incision. With the figure-8 and interlocking multitwisted suture, the forces are dispersed and the effective approximation force is lower than that applied to the wires. CONCLUSIONS: Straight wires provide good fixation stability for the durability of the sternum, and the somewhat lower strength could be compensated by increasing the number of sutures. The figure-8 and IM have better strength and are less likely to cut the sternum because of advantageous redistribution of shearing forces of the wire as compared to straight closures.

[Postoperative chondritis and osteomyelitis of the ribs and sternum].

Postoperative chondritis and osteomyelitis of the ribs and sternum were observed in 90 of 4620 operated patients. The main causes contributing to the development of this complication are errors in treatment of cartilages during thoracotomy and suppurative complications in the pleural cavity or in the operative wound. A radical removal of the affected costal and sternal portions as early as possible is considered to be the method of choice in treatment of chondirtis and osteomyelitis of the ribs and sternum.

Injury to the developing sternum in an adolescent football player: a case report and literature review.

When a blow to the sternum causes pain in a young athlete, the differential diagnosis can be complicated by persistence of an unfused growth center that does not normally fuse until age 18. We report a case of physeal injury in the sternum that was originally diagnosed as a stress fracture. The young football player in this case may have been predisposed to such an injury by abnormal synostosis of the sternomanubrial junction.

A variant form of median defect syndrome. Syndrome of combined congenital defects involving the supraumbilical abdominal wall, sternum, diaphragm, pericardium, and heart.

An autopsy case with the syndrome of combined congenital defects involving the supraumbilical abdominal wall, sternum, diaphragm, pericardium, and heart is reported. In this case, abnormal arterial plexus including anastomosis of the left coronary artery and the left internal mammary artery is recognized at the hernial pericardial wall, in addition to the already reported anomaly complex, i.e. diastasis recti abdominis with pericardial hernia, ventral defect of the diaphragm, partial defect of the sternum, and tetralogy of Fallot. It is suggested that this newly revealed vascular anomaly also comes within a specific syndrome of combined congenital defects.

Bilateral sternales in relation to body of sternum.

The sternalis muscle on the manubrium sterni and its communication with sternocleidomastoid has been reported by many workers as unilateral or bilateral in both the sexes. The present report deals with bilateral sternales below the angle of sternum in a male cadaver during routine dissection. The proximal attachment of the muscle was from the manubriosternal joint on either side, right being broader than left and was found to be overlapping the lateral part of sternum and adjoining pectoralis major muscle. The muscle was running downwards and laterally and was attached on the cartilage of 6th and 7th ribs appearing to be continuous with rectus abdominis muscle. The muscle was found to be innervated by branches of the corresponding intercostal nerves of these spaces. The sternalis if present could be used for reconstructive surgical operations on the breast.

Partial resection of the sternum for osseous metastasis of differentiated thyroid cancer: case report.

BACKGROUND: Due to their resistance to radio-iodine therapy, skeletal metastases from differentiated thyroid cancer (DTC) are difficult to treat. Surgical resection of solitary skeletal metastasis may offer cure with prolonged survival. CASE REPORT: A patient with the simultaneous appearance of local recurrence of DTC and skeletal metastasis of the cranial sternum is reported. After radical excision of the tumour recurrence and the lymph nodes, a partial sternal resection was carried out, and the defect in the chest wall was reconstructed with polypropylene mesh. Radio-iodine ablation therapy was carried out 4 times after the operation for locally recurrent disease. At the follow-up, 4 1/2 years after the resection of the sternal metastasis, the patient is free of disease. CONCLUSION: Sternal resection for solitary osseous metastasis of DTC should be performed on selected, individual patients. Reconstruction of the sternum with polypropylene mesh, as a simple and effective method, produces satisfactory functional and cosmetic results and allows a more effective radio-iodine treatment.

Multislice computed tomography of the sternum: which image reconstruction do we need?

OBJECTIVE: To assess the diagnostic value and to optimize image reconstruction for multislice spiral computed tomography (MSCT) of the sternum. MATERIALS AND METHODS: Forty patients underwent MSCT of the sternum using a standardized examination protocol (4x1 mm or 4x2.5 mm collimation; 120 kV, 120 mAseff). Axial images with an effective slice thickness of 1.25 mm or 3 mm and an increment of 0.8 mm or 1 mm were reconstructed. Coronal and sagittal reformations with a slice thickness between 2 mm or 3 mm and 5 mm and a slice distance ranging from 1 mm to 4 mm (overlap 1 mm) were calculated. In all patients, additional plain film radiographs (n=9) or clinical follow-up data were available. Image quality was assessed visually using a five-point grading scale. RESULTS: In all patients, the correct diagnosis was established using multiplanar reformats (MPR) exclusively. Best image quality for MPR was achieved using a slice thickness of 3 mm and a slice distance of 2 mm. Images acquired with a 4x1 mm collimation proved superior to images reconstructed from data obtained with a 4x2.5 mm collimation. CONCLUSION: MPR from MSCT data are a valuable tool for the diagnosis of sternal pathologies. For best image quality the thinnest possible collimation should be chosen.

[The reconstruction of funnel chest deformity with the reversed transplantation of sternum-costicartilage flap carried by the abdominal rectus pedicle].

OBJECTIVE: To investigate the reconstructive operative procedures of funnel chest with "sternum-costicartilage" flap carried by the abdominal rectus pedicle. METHODS: (1) In accordance with the lesioned area of funnel-like depressed deformity of anterior thoracic wall, a perpendicular median incision was designed and made; (2) The "sternum-costicartilage" flap carrying the abdominal rectus pedicle was used and reversed and transplanted to reconstruct severe funnel chest deformity. RESULTS: The procedure was used in 7 cases from 1999 to 2005. The results of surgery were satisfactory. There were no recurrence after operation. CONCLUSIONS: The procedure reported here is rather safe, solid and sound with good therapeutic results, and is of great value in clinical practice.

[Resection of metastatic nondifferentiated carcinoma of sternum with thoracic wall reconstruction and simultaneous myocardial revascularization for ischemic heart disease--case report].

Authors offer the case report of a patient with metastatic non differentiated carcinoma of sternum simultaneously suffering from ischemic heart disease. The patient underwent actinotherapy & hyperthermia followed by resection of sternum and coronary artery bypass grafting in one session. Chest wall defect was closed by means of latissimus dorsi muscle rotation. Postoperative palliative chemotherapy started 16 weeks postoperatively. Primary tumor was not found, neither preoperatively nor during the 36 months' postoperative remission.

[Endoscopic surgery with the sternum-lifting method].

Fourteen years have passed since video-assisted thoracic thymectomy (VATS-T) began to be used to treat diseases of the thymus. Indications for VATS-T have been expanded by ensuring sufficient space for easy endoscope manipulation using the sternum-lifting method. A variety of VATS-T techniques are available, including the cervical approach, bilateral VATS, infrasternal approach, and others. Although these techniques have the common feature of not involving sternotomy, they differ from each other in terms of the extent of thymectomy. In Japan, bilateral VATS (involving neck incision combined with sternum lifting) or the infrasternal approach (based on the full-directional technique) is often used for extended or maximal thymectomy in myasthenia gravis patients. In a short-term prospective study, VATS-T was shown to be more beneficial than sternotomy in myasthenia gravis patients and those with noninvasive thymoma. With the overall trend toward increased performance of minimally invasive surgery, VATS-T has been more widely adopted as a surgical approach for patients with diseases of the thymus. However, as the number of patients treated with VATS-T has increased, problems specific to endoscopic surgery have become an increasing concern. Whether VATS-T can be considered truly beneficial for patients will depend on the success or failure in solving these problems and the results of long-term prospective studies on the prognosis of patients after treatment with this procedure.

[Causes, surgical treatment and prevention of chondritis and osteomyelitis of the ribs and sternum].

The causes of chondritis and osteomyelitis of the ribs and sternum in 33 patients were analysed. It was found that in 70% of them purulent inflammation developed in the first 2-12 weeks after injury to and operations on the thoracic and abdominal organs. The average duration of the disease prior to hospitalization was about 16 months. Pathogenic microflora was isolated from the secretions of the affected tissues in 70.96% of patients. Different forms of the clinical course of the disease were distinguished according to the manifestation of the principal symptom: infiltrative-inflammatory, algesic, fistulous, empyema-like, mediastinitis-like, and a form poor in symptoms. The results of nonoperative treatment were studied. The use of X-ray therapy in a complex with nonoperative measures in 9 (25.71%) patients proved to be ineffective. Surgery was the main method for the management of chondritis and osteomyelitis of the ribs and sternum. It led to recovery of 31 (89.57%) patients without a second surgical intervention.

[Postponed closure of the sternum after operations on the heart with extracorporeal circulation].

Data on postponed closure of the sternum in 3 patients who were operated on with extracorporeal circulation are discussed. Various clinical and technical aspects of the applied method are considered. On the basis of their personal experience and the literature data, the authors discuss the causes of the development of acute heart dilatation in operations with extracorporeal circulation and, with due regard for this, substantiate the indications for the use of the method. Postponed closure of the sternum is a promising method of treatment of critical hemodynamic disorders which occur at the stage of closure of the sternotomy and are characterized by the picture of cardiac tamponade.

[Pathologic fracture of the sternum in a patient with anorexia nervosa].

It has been observed that anorexia nervosa (AN) patients present osteoporosis (OP) although the mechanism by which it occurs is not well known. The reduction in calcium intake as well as a decrease in estrogen production which is associated with involutive OP, could be involved. We have found in the literature 16 cases of AN and OP, one of which presented a fracture of the sternum. We now present another case of fracture of the sternum. This implies a high incidence of this type of fracture in OP due to AN which is extremely rare in involutive OP, which suggests that the ethiopathogenic factors of the former are different from those of the latter.

[Treatment of sternum fractures].

The frequency and the treatment of the sternal fractures is discussed by the authors. 5 patients with sternal fractures have been treated by the authors, and 3 of them were operated on. In one patient early operation has been performed, and in another the osteosynthesis of the sternum has been carried out 3 years after the lesion. In the 3d patient the bone piece broken down from the sternum damaged the arteria mammaria int. sin., necessitating thus surgical intervention.

[Surgical treatment of chronic osteomyelitis of the sternum].

The efficacy of surgical rehabilitation of patients with chronic osteomyelitis of the sternum is shown. Surgical treatment was divided in most cases into two stages: removal of the involved areas of the sternum and adjoining ribs and fixed muscular or musculocutaneous plastics. The obtained results are reassuring and promote further development of surgery of this region.

[Pseudomonas aeruginosa infection of the sternum and costal cartilages in cardiosurgery. Therapeutic experience using aztreonam].

Clinical effectiveness of Aztreonam was studied. This new monobactam antimicrobial agent was tasted in the treatment of post-operative infectious complications of cardiac surgical sternal wounds. Ten patients (4 men and 6 women, age range 20 to 68 years) were entered into the study. All had a Pseudomonas aeruginosa infection of sternum or sternum and ribs and all underwent an extensive regional surgery of the infection wound and received a topical and/or IV treatment with Aztreonam. In all cases we obtained a satisfactory result, with complete eradication of the infection. Both local and systemic tolerance to the drug were excellent and no side-effect was registered. Therefore Aztreonam can be considered an active and safe antibiotic for the treatment of sternal and/or costal postsurgical infections by Pseudomonas aeruginosa.
