[Characteristics of favorable and poor development of health services and social conditions for the aged].

Two stage probability samples were selected to represent the populations resident in one metropolitan (Prague-1 n = 484) and one country (Opava n = 538) geographical and administrative district. The index cases were contacted and surveyed in a field study. Data were collected by means of standardized interviews by nurses and social workers and complemented from GP's case notes, if available. A significant proportion of cases had no contact with health services at all. Both samples were followed-up for three years. The description of cases was condensed into 27 meaningful variables assessing social, health and living characteristics at the time of the beginning of the study. 11 follow-up course and outcome criteria were derived from the follow-up data in a similar way. Results of the metropolitan and country sample comparison of social and health care needs of five age groups, and of multivariate analysis of predictive relation between the initial characteristics and development and outcome criteria dependent variables are presented and discussed.

[Computer survey of the rehabilitation data and social conditions of children with motor disabilities in Györ-Sopron and Baranya Counties].

To investigate the occurrence of motor disability a pilot study was carried out in the total children's population of Györ-Sopron county and in 57% of the children of Baranya county by computer-based questionnaires. The data of the 492 motor handicapped children were analysed by 55 computer programs. These programs are constructed for the analysis of the somatic and mental health, schooling, rehabilitation possibilities, social background, care of these children and their further needs for specialists and institutions. This procedure is suitable for national and longitudinal data collection and for the complex registration of the status of these children.

Social conditions and menarcheal age: the importance of early years of life.

Data on the historical trends for menarcheal age and socio-economic conditions in Norway are used to investigate whether the reproductive system in humans is especially sensitive to environmental stimuli during any particular short age periods. The method which has been used is based on the irregularity in the trend for socio-economic development. These irregularities are reflected in the Gross Domestic Product (GDP), a quantity for which official estimates are available from 1865 onwards. Estimates of menarcheal age for women born in each year from about 1840 have been calculated from information obtained from maternity clinic records. For the period between 1900 and World War II, a close correspondence is found between the GDP in a year, and the menarcheal age of women born in the same or the preceding year. For the years before 1900, the data on menarcheal age is less satisfactory, but the correspondence may be fairly good also for this period. When values of GDP corresponding to higher ages is used, the correlation is less satisfactory. Other information on the susceptibility of the reproductive system towards environmental influences is reviewed. The following conclusion is reached: during the period around or after birth, the processes leading to menarche are clearly more sensitive than at later ages. During adolescence, the maturation process may be influenced somewhat, but probably not much as long as the conditions are not adverse. These observations may be seen as illustrating that phenomena corresponding to the critical or sensitive periods described for animal species, are also observable during the longer-lasting process of human development.

Associations between symptoms of irritable colon and psychological and social conditions and lifestyle.

In a survey of risk factors for coronary heart disease 14 102 middle aged men and women answered a questionnaire on lifestyle, diet, and health, including symptoms of functional abdominal disorders. The overall prevalence of reports of one or both of the abdominal symptoms of "bloating and rumbling" or "cramping abdominal pain" was 28% in men and 35% in women. Only a weak negative association between age and prevalence of reported pain was found in both sexes. Women reported abdominal symptoms, especially cramping abdominal pain, significantly more commonly than men. In a multiple regression analysis abdominal symptoms were much more strongly associated with symptoms of mental stress such as depression, sleeping difficulties, problems of coping, and the use of analgesics than with lifestyle, dietary, and social variables together. The association was stronger in subjects reporting both symptoms. This strong and consistent association between functional abdominal disorders and psychological and social problems suggests that action other than prescribing drugs, diets, or radiography is required.
