Periappendicitis and chlamydial salpingitis.

Periappendicitis seems to be a novel manifestation of infections with Chlamydia trachomatis. In seven of 112 women with laparoscopically verified acute salpingitis, secondary appendicitis was diagnosed and histologically confirmed. The genital tract in all seven patients harbored Chlamydia trachomatis but not Neisseria gonorrhoeae. The fallopian tubes should be scrutinized when an inflamed appendix is removed from a sexually active woman. Signs of salpingitis should then lead to appropriate microbiologic, therapeutic and epidemiologic measures, including contact tracing.

Nodular salpingitis and tubal endometriosis. II. Diagnosis and differential diagnosis.

Nodular salpingitis and tubal endometriosis are dystrophic disorders (salpingioses) the clinical importance of which is mainly due to their interference with female fertility. We have studied 42 cases of women operated for tubal sterility, viz. 23 cases of nodular salpingitis (NS), 17 of endometriosis (EM) and two cases in which both lesions were present. Pre-operative x-ray and celioscopic exploration was performed in order to assess the severity of the lesions and the outlook after tubal plastic surgery. X-ray examination reveals the diverticular nature of these lesions. Both diseases may be localized in any of the tubal segments. The postoperative prognosis depends on early surgery, site of the lesion and its nature.

Second-look laparoscopy after acute salpingitis.

Patients with a laparoscopically verified first episode of acute salpingitis were offered a second-look laparoscopy to determine the effectiveness of the treatment regimen in terms of anatomic end result as well as to correlate the anatomic end result with future fertility. Until now, 13 women have been subjected to such relaparoscopy 16 to 33 weeks after the acute illness. At the acute stage of the disease, 11 of the women had serologic and/or cultural evidence of genital infection with Chlamydia trachomatis, three of whom also had Neisseria gonorrhoeae. All were culture-negative after treatment. In eight patients relaparoscopy showed that previously adherent adnexa were apparently normal, whereas in two patients a deterioration of the laparoscopic findings was found. Two patients had bilateral occluded tubes, for a preliminary infertility rate of 15%. These preliminary observations suggest that second-look laparoscopy after acute salpingitis might be a useful method for early objective evaluation of treatment results.

Contraceptives and acute salpingitis.

In 546 women with a first episode of salpingitis, the degree of inflammation of the fallopian tubes, as seen during laparoscopy, was graded into mild, moderate, and severe. The patients were subgrouped according to contraceptive use into (1) reference group (women not using intrauterine [contraceptive] devices [IUDs] or oral contraceptives [OCs]) (2) IUD group, and (3) OC group. Mild, moderate, and severe salpingitis occurred in 51.3%, 34.6%, and 24.1% of the women in the reference group. The corresponding percentages for the women in the IUD group were 54.6%, 29.7%, and 15.7%, and for the patients in the OC group, 73.5%, 17.1%, and 9.4%, respectively. Women using OCs had significantly milder degrees of inflammation of the fallopian tubes than women with other contraceptive methods. Age could not explain the differences.

Salpingitis isthmica nodosa in female infertility and ectopic tubal pregnancy.

Salpingitis isthmica nodosa has been studied in relation to female infertility and tubal ectopic pregnancy. The incidence of this lesion in a control Caucasian population was 0.6%, as compared with incidences of 2.86% in an ectopic group and 50% in a small infertility group undergoing tuboplasty. It is suggested that salpingitis isthmica nodosa should be considered as an etiologic factor in these reproductive disorders. Chronic tubal spasm is suggested as the underlying process.

[A microbiological study of swabs taken laparoscopically in cases of salpingitis and tubal sterility. Research for Chlamydia trachomatis and for mycoplasmas (author's transl)].

Research was made for chlamydia trachomatis and ureaplasma urealyticum in the peritoneum and the tubes of 99 women divided into 4 groups: 17 of them were being investigated because of acute salpingitis (Group A), 17 were being investigated for tubal sterility with chronic inflammation diagnosed laparoscopically (Group B), 29 were being investigated for tubal sterility without any laparoscopic evidence of inflammation (Group C) and 36 women had absolutely normal pelves and were being investigated for sterility. These were the control group (D). Swabs were also taken from the lower genital tracts as well as serological tests for chlamydia trachomatis and cytological samplings of the fluid from the Pouch of Douglas and the histology of the tubes. In the 17 women who had acute salpingitis the swabs 4 cases of C.T. and 4 of U.U. In the 46 women who had tubal sterility the laparoscopic swabs showed cases of C.T. and 7 of U.U. The swabs were most often positive in Group B. This group is characterised by a special appearence of the inflammation, with fluid present and viscous adhesions as well as peritoneal inflammatory cysts. These altogether help to make a presumptive diagnosis of C.T. infection on laparoscopy. In the control group of 36 cases there was no sign of C.T. in any case, although 2 swabs from the peritoneum showed U.U. So there is a statistically significant difference between the groups that were suspicious and the control group whether the results were obtained by cultures or by serological diagnosis. On the other hand there is no definitive difference as far as U.U. is concerned. These observations, which are similar to those published by other authors, lead us to think that micro-organisms and especially chlamydia trachomatis could be the bacteriological agent responsible for chronic inflammatory states found so frequently in women with tubal sterility.

Salpingitis in poultry. I. Prevalence, bacteriology and possible pathogenesis in broilers.

The prevalence of salpingitis in broilers at slaughter seems to be rather constant, constituting 0.02--0.03% of the broilers slaughtered (Table I & Figure 1). Profuse growth of Escherichia coli in pure culture was obtained from the salpinx of all 123 investigated carcasses with chronic salpingitis. Primary blood agar plates examined showed a pure culture as to O-group as well. Of 22 different O-groups demonstrated, 01, 02, 07 and 053 were most prevalent, constituting 47% of the strains (Table III). Salmonella spp. were not demonstrated. Etiology, pathogenesis and possible food hygienic consequences are discussed in the light of the present findings.

Experimental salpingitis in rabbits provoked by Chlamydia trachomatis.

Experimental salpingitis was studied by the introduction of Chlamydia trachomatis, immunotype D, directly into the oviducts of New Zealand White rabbits. Some rabbits were rechallenged with immunotype F to elicit a second infection. Both experiments produced self-limited acute salpingitis. Light microscopy (LM) and transmission electron microscopy (TEM) showed an extensive polymorphonuclear leukocytic infiltration in the submucosa and mucosa. Scanning electron microscopy (SEM) revealed various alterations of the endosalpingeal surface, including deciliation and flattening of the epithelial cells. These morphologic changes were more pronounced following rechallenge. High and long-lasting serum antibody titers were observed in those rabbits receiving intratubal inoculation of live infectious organisms; no response was evident when inactivated organisms were used. These results indicate that the rabbit is useful in the study of the effects of infection on oviductal function.

[Descending salpingitis following ruptured appendicitis (author's transl)].

The retrospective study of ovarian abscesses alerted us to the possibility of a descending salpingitis. The salpingitis arises from the pelvic periteneum following perforation of abscesses. Usually the fimbriated end remains open despite the inflammation. The pelvic peritonitis starts with ovarian abscesses, periappendiceal abscesses or complications of inflammatory intestinal diseases.

[Salpingitis and tubal pregnancy in patients treated at the first Clinic of Surgical Gynecology in Lublin between the years 1978-1988].

Retrospective study was performed upon incidence of tubal pregnancy and histopathologically proved post-infection changes in oviducts. The incidence of tubal pregnancy was significantly decreasing in studied period in our institution despite the fact that salpingitis was found more often. Authors conclude that recently reported increase of tubal pregnancy is valid only in the populations studied and not in general. Relation between salpingitis and tubal pregnancy is not always evident.

Salpingitis isthmica nodosa: review of four cases from the General Hospital, Kota Bharu.

Four cases of salpingitis isthmica nodosa diagnosed from a total of 82 hysterosalpingograms, over a period of three years at the General Hospital, Kota Bharu, were reviewed. The diagnosis was made based on the radiographic appearance of globules of contrast medium in the periluminal tissues which were in continuity with the lumen of the fallopian tubes. Three of these patients had ectopic pregnancy. Diagnosis of salpingitis isthmica nodosa which is readily demonstrated by hysterosalpingogram, is extremely important because of its impact on ectopic pregnancy rates. Hysterosalpingography is indicated following ectopic pregnancy to show whether there is diverticulosis of the contralateral tube. By such knowledge, optimal treatment can be offered to patients wishing to attempt further conception. As the disease is relatively localised it is amenable to resection with reimplantation of the distal portion of the tube.

The role of spermatozoa in the pathogenesis of Chlamydia trachomatis salpingitis in a primate model.

BACKGROUND AND OBJECTIVES: The role of spermatozoa in carrying microorganisms from the cervix to the fallopian tubes and promoting infection was examined in eight pig-tailed macaques. GOAL OF THIS STUDY: The aim of this study was to determine whether spermatozoa play a role in the transport of Chlamydia trachomatis into the upper reproductive tract and in the pathogenesis of chlamydial salpingitis. STUDY DESIGN: Five monkeys were inoculated with Chlamydia trachomatis in the cervix and mated, and three other monkeys were infected in the cervix but not mated. RESULTS: Culture positive results were obtained for cervical samples from all animals. After cervical inoculation, no mated animals and one unmated animal had positive culture results for samples from the fallopian tubes. After mating, no spermatozoa examined in samples taken from the upper tract or cul de sac had elementary bodies (EBs) on their surfaces, and only one sample obtained from the cervix showed C. trachomatis attached to the spermatozoa, as determined by fluorescent antibody (FA) staining. Two of the mated monkeys became pregnant; one spontaneously aborted late in gestation, and the other delivered a normal female infant. The fallopian tubes of all eight monkeys were challenged directly with serovar D and examined for evidence of salpingitis. Repeat laparoscopy was performed to monitor the inflammatory response. Tubal specimens for isolation and polymerase chain reaction (PCR) were obtained. Hysterectomies were performed between 3 and 4 weeks after tubal inoculation. Histopathologic examination of the fallopian tubes revealed widespread inflammation consisting of focal collections of lymphocytes and plasma cells in the submucosa. Peritubal adhesions were observed in four of five mated monkeys and in two of three unmated monkeys. CONCLUSION: This study failed to confirm that spermatozoa play a role in the pathogenesis of C. trachomatis infection.

Granulomatous salpingitis secondary to Crohn's disease.

A 30-year-old woman with longstanding Crohn's disease underwent resection of bowel and adherent adnexa. Granulomatous enterocolitis and granulomatous salpingitis and oophoritis were present histologically. Unusual features accompanying this tubal process included formation of an adnexal mass and florid atypical proliferation of the tubal epithelium. This proliferation is similar to that epithelial change described in tuberculous salpingitis and should not be confused with carcinoma in situ.

[Use of an experimental Chlamydia trachomatis salpingitis model for evaluating the effectiveness of antibiotics and anti-inflammatory agents on fertility].

OBJECTIVES: An experimental model of Chlamydia trachomatis salpingitis was developed in mice to study the effect of combination antibiotic and antiinflammatory therapy on restoring fertility after tubal infection. METHODS: The mice were infected by injection via the ovarian bursa with a suspension containing a human strain of Chlamydia trachomatis sevar F. After treatment with antibiotics alone or in combination with antiinflammatory agents, fertility was studied at 4 months. RESULTS: With doxycycline and ofloxacine, there was not a significant difference in fertility between animals treated with antibiotics alone and those treated with antibiotics and antiinflammatory agents. However, a piroxicam and azythromycin combination improved fertility with a longer delay than with azythromycin alone. The most important factor appeared to be the antibiotic treatment. CONCLUSION: Associating an antiinflammatory agent was not shown to have a beneficial effect in reducing infertility after Chlamydia trachomatis salpingitis.

Hyperamylasemia associated with gonococcal salpingitis and perihepatitis.

A case of hyperamylasemia associated with gonococcal salpingitis and perihepatitis (Fitzhugh-Curtis syndrome) is reported. Other potential causes of hyperamylasemia such as pancreatic, parotic, biliary, or intestinal disease were carefully excluded. In addition, the amylase to creatinine clearance ratio was within normal limits, suggesting tubal rather than pancreatic source of hyperamylasemia. This finding is of clinical importance since acute salpingitis is a prevalent disease and its initial presentation may mimic acute pancreatitis or other causes of surgical abdomen. The amylase to creatinine clearance ratio is of value in differentiating such cases from those with hyperamylasemia due to pancreatitis.

Perihepatitis associated with salpingitis in adolescents.

Perihepatitis, one manifestation of complicated gonorrhea, is infrequently diagnosed and reported. This study was undertaken to examine the incidence of gonococcal perihepatitis in adolescents as well as to study the effect, if any, of this condition on tests of liver function. Accordingly, the records of 137 adolescents with salpingitis were evaluated. Right upper quadrant tenderness or hepatic enlargement was noted in 27, while amino transferase level elevations were observed in 19 of the 59 in whom these tests were performed. All findings resolved promptly following initiation of intravenous penicillin therapy. The observation of hepatic dysfunction in 27% of these adolescents suggests that perihepatitis occurs more frequently than reported in adults and should be considered in the evaluation of the conditions of adolescents with salpingitis or upper abdominal tenderness.

Antibiotic prophylaxis in gynecology: cefoxitin concentrations in serum, myometrium, endometrium and salpinges.

Thirty-six patients received an intravenous bolus injection of 2 g cefoxitin over 5 min at various times before abdominal or vaginal hysterectomy. Cefoxitin levels in myometrium and salpinges were substantially higher than those in endometrium and almost as high as those in serum during the first two hours following the i. v. administration of the drug. Tissue concentrations of 8 micrograms/g could be maintained for two hours. The cefoxitin tissue concentrations attained in myometrium, endometrium and salpinges justify controlled prospective clinical studies on the prevention of postoperative wound infections with this antibiotic.

Interposition of fallopian salpinges in the treatment of sigmoidovaginal fistula, secondary to vaginal hysterectomy with failure of previous repair.

A 50-year-old woman, para 4, suffering from uterine fibromatosis and recurrent menometrorrhagia, underwent vaginal hysterectomy with preservation of salpinges. About 15 days after surgery, hydrosoluble contrast enema showed sigmoidovaginal fistula; after about two months there was failure of surgery repair by the rectal endoscopic technique. A month later, we performed repair surgery by the abdominal approach interposing fallopian salpinges between the sigmoid and the vagina. About two months later, a enema showed absence of fistula and today the women is free from disease.
