In this study, the relationship between psychopathy and the prepetration of sexual homicide was investigated. The official file descriptions of sexual homicides committed by 18 psychopathic and 20 nonpsychopathic Canadian offenders were coded (by coders unaware of Psychopathy Checklist--Revised [PCL--R] scores) for characteristics of the victim, victim/perpetrator relationship, and evidence of gratuitous and sadistic violent behavior. Results indicated that most (84.7%) of the sexual murderers scored in the moderate to high range on the PCL--R. The majority of victims (66.67%) were female strangers, with no apparent influence of psychopathy on victim choice. Homicides committed by psychopathic offenders (using a PCL--R cut-off of 30) contained a significantly higher level of both gratuitous and sadistic violence than nonpsychopathic offenders. Most (82.4%) of the psychopaths exhibited some degree of sadistic behavior in their homicides compared to 52.6% of the nonpsychopaths. Implications for homicide investigations are discussed.
One hundred and eighty-four subjects (22 women and 162 men) who were members of two sadomasochistically oriented clubs answered a semistructured questionnaire containing items relating to a variety of sexual behaviors. Using a multivariate statistical analysis that geometrically represents the co-occurrence of individual actions as a visual array (Guttman (1954). In Lazarfeld, P. E. (ed.), Mathematical Thinking in the Social Sciences, Free Press, Glencoe, IL.) four qualitatively different sexual scripts emerged: hypermasculinity; administration and receiving of pain; physical restriction; and psychological humiliation. Although similar themes have been suggested before, this study demonstrated their empirical base. Humiliation was significantly associated more with females and with heterosexual orientation in men, while hypermasculinity was associated with males and with homosexual orientation in men.
The present study investigated the reliability of Sexually Violent Predator (SVP) civil commitment criteria under Florida's Jimmy Ryce Act. The purpose of the study was to determine if, independently, 2 evaluators would reach the same conclusions about the same client (n = 295). According to civil commitment criteria outlined by the United States Supreme Court (Kansas v. Hendricks, 1997), SVPs must display a mental abnormality predisposing them to sexual violence and a likelihood of future sexual violence. The interrater reliability of 8 DSM-IV diagnoses applied by evaluators to determine whether a client has a "mental abnormality that predisposes him to sexual violence" was found to be poor to fair (kappa = .23 to . 70). The interrater reliability of risk assessment instruments used to determine "likelihood of reoffense" was good (ICC = .77 to .85). The recommendations made by evaluators regarding whether or not to refer a client for civil commitment demonstrated poor reliability (kappa = .54). Implications for practice and policy are explored.
In this study I investigated the relation between normal heterosexual attraction and autogynephilia (a man's propensity to be sexually aroused by the thought or image of himself as a woman). The subjects were 427 adult male outpatients who reported histories of dressing in women's garments, of feeling like women, or both. The data were questionnaire measures of autogynephilia, heterosexual interest, and other psychosexual variables. As predicted, the highest levels of autogynephilia were observed at intermediate rather than high levels of heterosexual interest; that is, the function relating these variables took the form of an inverted U. This finding supports the hypothesis that autogynephilia is a misdirected type of heterosexual impulse, which arises in association with normal heterosexuality but also competes with it.

The case of recidivous sexual offender with genetically caused mental retardation and primary hypogonadism (Klinefelter's syndrome with karyotype 48, XXXY) is described. He was examined after sadistic abuse of a boy aged 13 that he had committed 19 years after performed testicular pulpectomy. Plasmatic level of testosterone was found 4x higher than mean level in men after orchidectomy. Histological examination of residual scrotal tissues proved that the source of androgens were hyperplastic nodules of extratesticular Leydig cells.
Little is known about the aetiology, course and therapy of obscene telephone calls as a paraphilic behaviour. Overlaps to other paraphilias like voyeurism, paraphilia related disorders and the concept of stalking are reported in the literature. In case reports we describe the paraphilic development and course with a focus on the relationship to the victim. We discuss this on the background of different concepts regarding risk assessment and therapy.
A 37-year old female with borderline personality disorder, who had experienced the cumulative trauma of a history of sadistic child abuse by her mother, developed the symptomatology of chronic factitious illness after an operation necessitated by an accident injury. Relationships between chronic factitious disease and obsession, addiction, perversion and suicidal behaviour are discussed against the background of a literature review.

In this study, we investigated the role of age in three categories of 168 sex offenders, including 53 rapists, 62 pedophiles, and 53 sadists. On the basis of the match between requisites of completing each type of sexual offense, we hypothesized that rapists would be less represented in the older age group, whereas pedophilic and sexually sadistic offenders would show a more muted decline with age. This hypothesis was supported by the data, clearly a showing smaller number of rapists in the over 40 group relative to the number of pedophiles and sadists (p = 0.0009.) Further examination of the files eliminated the alternate explanation ascribing this pattern to lack of opportunity in older rapists because of incarceration. These results suggest that age over 40 may mitigate incidence of recidivism in rapists, but not in pedophiles or sexual sadists. Approximately one third of the sexual sadists had committed either first- or second-degree murder, whereas none of the nonsadistic rapists or pedophiles had these crimes on their records. The data further justify categorization of sex offenders into nonsadistic rapist, sadistic rapist, and pedophilic offenders in future research to enhance precision in delineating demographic characteristics and putative causal explanations for sexual offending.
A paraphiliac with syndrome of exhibitionism has received treatment with the anti-androgenic hormone, Depo-Provera, for three years and is maintained on 350 mg, intramuscularly each week. He and his wife had been in a pathological oppressor/martyr relationship, he as a paraphiliac-oppressor and she as an abuse-martyr. They both receive reciprocal counseling therapy in individual and couple sessions. Antiandrogen diminishes paraphilic fantasy and enhances self-governance of its behavioral concomitants, but does not totally suppress erotosexual expression. Reciprocal counseling therapy enhances erotosexuality and pair-bondedness between the two partners as they both change, not singly, but mutually, toward a more healthy life style.
Four phases of normal human male courtship behavior may be differentiated: location and initial appraisal of a potential partner; pretactile interaction (e.g., smiling at, posturing for, or talking to a prospective partner); tactile interaction (e.g., embracing, petting); and effecting genital union. A particular class of erotic anomalies may be seen as distorted counterparts of the four normal phases: voyeurism; exhibitionism and obscene telephoning; toucheurism and frotteurism; and the preferential rape pattern. Data from male samples are presented that show that two or more of these anomalies are often found in the same individual. These data suggest the existence of a discrete syndrome which may be described as a courtship disorder.
