[Disseminated actinic keratoses. Comparison of topical photodynamic therapy with 5-aminolevulinic acid and topical 5% imiquimod cream].

We report on a 75-year-old woman with skin type I suffering for 20 years from multiple actinic keratoses of areas exposed to the sun. The skin of her forearms was treated on one side with imiquimod 5% cream over 4 weeks and on the contralateral side with photodynamic therapy (PDT) using topical 5-aminolevulinic acid (ALA). Both sides showed good clinical response after 3-months follow-up, suggesting that these modalities can be effectively used for treating widespread multiple actinic keratoses.

The causes of skin diseases associated with photosensitivity are presented. The combined effect of light and exogenous light sensitizing substances leads to the occurrence of phototoxic and photoallergic reactions, and what is more, UVA spectrum is most active in their pathogenesis. The light sensitizing compounds are contained in cosmetic preparations, drugs and certain plants. They are characterized by a tricyclic structure and the ability to absorb ultraviolet radiation. The light sensitizing properties are exhibited primarily by non-steroid anti-inflammatory drugs, preservatives, sun filters, phenothiazine derivatives and odorants. Photoallergic reactions may be of occupational nature. The majority of reports found in the literature concern farmers, gardeners, fruit-growers and breeders, as well as representatives of other occupations. They usually emerge after the contact with pesticides of the thiocarbamide group, tetrachloroisophthalonitryl and veterinary drugs. The cases of occupational eczema in farmers and gardeners induced by the effect of light sensitizing plants have also been reported. In addition, the photoallergy diagnostic methods are discussed.

Phototoxic activity and the possibilities of its testing.

The photodynamically active compounds have been attracting an attention of specialists for relatively long time. The most fruitful period of their research has been probably the last decade. The structures of the photosensitizers are very different. The biological activity is based on a formation of free reactive radicals after an excitation of the molecules of the phototoxins by e.g. UV radiation. Some photosensitizers are used for treatment of various diseases in dermatology and oncology. Since the substances can occur in plants, food or cosmetics, the photodynamic activity of these compounds is necessary to be studied under various conditions. Now e.g. some photochemical reactions, cell cultures, cultures of microorganisms, suspensions of erythrocytes, and different species of animals are used to the research of the photodynamic activity of the substances.

[Photosensitization due to drugs: adverse reaction and beneficial effect].

In all areas of everyday life men get in touch with photosensitizers which increase the susceptibility to solar light. Dermal reactions arise after exposure to a photosensitizer and light. Quality and intensity depend on the compound and the radiation dose. Many drugs provoke these responses appearing as an adverse reaction during conventional treatment or as beneficial effect in the context of photodynamic therapy (PDT) or photochemotherapy (PUVA, ECP). Basic mechanisms and clinical aspects are reviewed.
