A long-term prospective study of the Hauser and Roux-Goldthwait procedures for recurrent patellar dislocation.

In a prospective study extending from 1966 to 1974, the results of the Hauser procedure were compared with the Roux-Goldthwait procedure for the correction of recurrent dislocations of the patella. In 87 knees in 75 patients, repaired consecutively and in a random manner with one procedure or the other, 100% retrieval was achieved. The average follow-up period was 7.7 years. Significant complications, though not necessarily of long term, occurred in 24 of 47 knees receiving Hauser repairs as compared with 6 of 40 knees in the Roux-Goldthwait group. Satisfactory long-term results were obtained in 72% with the Hauser technique and 93% with the Roux-Goldthwait technique.

[Clinical and radiologic results of surgically treated patellar dislocation].

In this retrospective study 72 patients were followed up clinically and radiologically after surgical treatment for dislocation of the patella. The period of observation varied between 13 and 76 months, with an average of 32 months. Acute traumatic dislocation of the patella was only rarely encountered (n = 6 [8%]). Primary dislocation occurred most frequently during the second and third decades of life. Chondral and osteochondral fractures were fairly frequent (n = 24 [33%]). Additional injuries involved the anterior cruciate ligament, the medial collateral ligament and the menisci. At clinical examination 32 patients (44%) could be assessed as "very good" and "good" (classification of Larsen and Lauridsen). Redislocation occurred in 12 patients (16.7%). The radiological condition of the patella and that of the trochlea femoris were assessed in terms of the classification of Wiberg and Baumgartl and of Hepp, respectively. The condylar-joint angle (Brattström) and the condylar depth index (Ficat and Bizou) were also evaluated.

[Medial patella dislocation].

A patellar dislocation is usually understood to mean a lateral dislocation of the patella. Medial patellar dislocations have mainly been described after surgical treatment, such as release of the lateral retinaculum. Here a case of medial patellar dislocation is reported which did not occur subsequent to surgery. Wilberg-III patella, genu recurvatum with patella alta and a m. vastus medialis insertion widely distal to the patella were predisposing factors.
