[The causes of death of patients with rheumatic diseases in Moscow].

The study was held in order to analyze the main causes of death in cases of rheumatic diseases (RD) in Moscow. The authors studied the pathology records of autopsies performed in 1999-2002 in two pathology departments of Moscow clinics. Cases with RD were selected. The study found 165 cases of RD, which constituted 2% of all autopsies performed in these departments. There were 99 cases (60%) of rheumatic heart disease (RHD), 4 cases (2.4%) of rheumatic fever (RF) relapse, 28 cases (17%) of rheumatoid arthritis (RA), 8 cases (4.8%) of systemic lupus erythematosus (SLE), 3 cases (1.8%) of scleroderma systematica (SS), 2 cases (1.2%) of ankylosing spondylitis (AS), 2 cases (1.2%) of systemic vasculitis (SPV), 11 cases (7.3%) of osteoarthrosis, 3 cases (1.8%) of gout, 1 case (0.6%) of polymyositis. The death of patients with RHD had been caused by hemodynamic decompensation (HD) in 54% of the cases, acute cardiovascular collapse (ACC) in 14% of the cases, 6% of the patients had died from thromboembolism (TE) and 26%--from other conditions (intoxication, uremia, brain and lung edema etc). The death of patients with RF was caused by TE in 2 cases, by HD in 1 case and by ACC in 1 case. Secondary amyloidosis resulting in chronic renal failure and uremia occurred in 5 out of 28 cases of RA, HD--in 3, ACC--in 7, TE--in 1, infectious complications--in 5, other complications--in 7 cases. Patients with SLE died from various conditions: uremia in 2 cases, acute adrenal failure in 1 case, infectious complications in 2, ACC--in 2, brain edema--in 1 case. The complications of SS were uremia and intoxication. ACC was the cause of death in cases of gout and SS. The majority of RD cases were patients with RHD. The main cause of death in RD was cardiovascular disorders.

[Pulse-electrotypes of Listeria monocytogenes strains, isolated in Moscow].

The characterization of the pulse-electrotypes of L. monocytogenes, isolated in 2003-2004 in Moscow from different sources, is presented. Among the cultures, isolated from humans, one outbreak pulse electrotype was detected and from different objects in buildings where a wide variety of food products was produced several probably related and unrelated pulse-electrotypes were obtained. The conclusion was made that several independent L. monocytogenes clones existed on the territory of Moscow, and many products supplied to retail trade and public catering enterprises were contaminated with these clones. Pulse electrophoresis was shown to be the most effective method for intraspecific typing and the study of the molecular epidemiology of listeriosis. Grounds for the necessity to improve the microbiological diagnostics of L. monocytogenes infection are given.

[Distribution and control of acari as vectors of human disease in urban population of Moscow].

Human ectoparasites, such as itch mites and the follicular mites Demodex brevis and D. folliculorum are notified on the territory of Moscow. The bird and rodent mites Ornythonyssus bacoti and the larvae of harvest mites may attack human beings indoors. The ticks Ixodes ricinus and I. persulcatus begin to inhabit the tree belts and environs of Moscow and other cities and towns. In the cities and towns of more southern regions of the Russian Federation, the red hen-mites Dermanyssus gallinae are also encountered and the ticks Rhipicephalus sanguineus and Ornithodoros papillipes begin to inhabit urban structures. A differential system of their controlling measures is proposed in relation to the habitats of mites and ticks on the urban territories and the results of use of a number of currently available disinfectants are presented.

[Malaria in Moscow: evaluation of the vulnerability and susceptibility of an area].

Before eradication of malaria in the USSR, there was a steady transmission of vivax malaria in the Moscow Region. In the posteradication period, there were two insignificant cases of local transmission: in 1972 and 1982. However, since 1999, there has been a local transmission of malaria every year. The possible causes of the transmission are analyzed. This includes a change in favor of better climatic conditions for malaria transmission in 1948 to 2004. It is shown that a weather quality jump in about 1984, which appeared as the area's higher susceptibility. A great deal of anophelogenic water reservoirs open the way to a local transmission in summer months. The vulnerability of the area has increased with a larger number of migrants from the endemic areas of the former USSR. Thus, more cases of malaria import (increased vulnerability), possibilities of the carrier to multiply, better climatic conditions for transmitting malaria (increased susceptibility) have created conditions for worsening its situation in the Moscow Region.

[Human rhinovíruses circulating in Moscow (author's transl)].

In the period of 1971--1973 in Moscow, 85 strains of rhinoviruses (types IB, 14, 20, 27, 33, 48, 56, 60 and 69) were isolated from patients with respiratory diseases. In March-May 1971 in one administrative district of Moscow, from ambulatory patients with ARD rhinoviruses of types 48 (24 strains) and 27 (8 strains) were predominantly isolated. An outbreak of diseases due to these two types appeared to have occurred. From children rhinoviruses were isolated not only from nasopharyngeal washings. From 3 children they were isolated from the lower respiratory tract (from the blood collected before his death due to staphylococcal sepsis. Surveys for virus-neutralizing antibody to 17 rhinovirus serotypes in 10 lots of gamma globulin prepared in different towns of the Soviet Union revealed wide spread of rhinovirus types IB, 13, 18, 31 and 32 limited spread of types 17 and 42 and moderate spread of types 2, 3, 7, 10, 14, 21, 22, 24, 48 and 53.

[Antigenic characteristics of influenza B virus strains isolated in an orphanage during an influenza outbreak in Moscow in the winter of 1988].

Examinations of ARD patients in an orphanage for defective children in Moscow during an influenza outbreak in the winter of 1988 yielded 12 influenza virus strains, including 6 influenza B strains and 6 influenza A (H3N2) strains. The antigenic analysis of hemagglutinin of influenza B virus isolates showed that with respect to the B/Leningrad/179/86 strain (an antigenic analogue of B/Ann Arbor/1/86 strain recommended for inclusion into the influenza vaccine for 1987-1988) they could be divided into 2 groups: antigenically close to the B/Leningrad/86 strain (isolate B/712) and markedly differing from it (the remaining isolates). As compared with reference strains of the previous years, all the new virus isolates fell into 4 groups: isolate B/712 antigenically related to B/Hong Kong/73 and B/Leningrad/86 strains; B/722 antigenically close to B/Singapore/222/79 and B/USSR/100/83 strains; B/724, an antigenic analogue to B/USSR/100/83; and the remaining isolates, to some or other extent, differing from various reference strains. This attests to simultaneous circulation of various antigenic variants of influenza B virus during the 1988 winter outbreak of influenza in Moscow. An interesting feature of the B/712/88 isolate consists in its antigenic specificity of hemagglutinin being indistinguishable from that of B/Hong Kong/8/73 strains.

[The manifestations of an epidemic process of typhoid infection in Moscow].

The work deals with the results of typhoid morbidity in Moscow for the period of 1980-1988. The morbidity rate varied from 0.98 to 0.17 cases per 100,000 of population with a tendency towards decrease. In the structure of morbidity local cases accounted for 46.8% and cases of infection contacted outside Moscow accounted for 53.2%. The morbidity level was low throughout the whole territory, only in two districts morbidity rate was significantly higher (P = 95%) than the average morbidity rate in the whole of the city. In the group of local cases morbidity had no pronounced seasonal character, in contrast to cases due to infection contacted outside the city which were mostly registered in summer and autumn. Persons aged 21-30 years were found to be the most affected age group. The general factors, and not the contact route, are supposed to play the main role in the realization of the mechanism of infection transfer. Under modern conditions, some manifestations of the epidemic process of typhoid infection can be detected only at the level of the whole city, or even the whole country.

[Use of the indicators of morbidity with temporary disability during conducting of the sanitary control at the Moscow subway].

Current sanitary surveillance at the Moscow Metro was analyzed under the conditions of scientific and technical revolution. A new form of current sanitary surveillance based on the data derived from medical certificates processed at the Moscow Metro Computer Centre was proposed. Excess coefficient of disease rates was calculated. If it exceeded 2, working conditions at the bay were considered unfavourable. A special impetus was given to the analysis of catarrhal diseases. Highlighted was the importance of economic interest in disease control, involving both individual members of work collectives and sanitary workers.

[Various trends in the somatic development of city children and adolescents during the past 20 years (the study based on the examination of schoolchildren in Moscow)].

Comprehensive studies of Moscow children are summarized. In 1961-1962, 1968-1972, 1981-1984 7000 of Moscow boys and girls from 8 to 17 years of age were investigated using cross-sectional and mixed-longitudinal methods (1968-1972). Trends in body dimensions, maturation levels distribution of somatotypes are analyzed. It is shown that, the process of acceleration of somatic development intensively continued up to mid-seventies then began to stabilize and in the early 80-s gave place to some retardation of body development in spite of the high standards of living. The possible causes of such trends are discussed.

[Comparative characteristics of the plasma lipid composition of 10-11-year-old schoolchildren in Moscow and Novosibirsk].

The role of lipoproteins in the etiology of atherosclerosis has been proved by the present time and dyslipoproteinemia is considered one of the main risk factors like smoking and arterial hypertension. In 1983-1984 an epidemiological survey of a representative sample of schoolchildren was conducted in one of the Moscow districts and according to a unified program among standardized participants in Novosibirsk. As a result, regional normal limits and mean values of blood plasma lipids were established in the populations of schoolchildren aged 10-11 in Moscow and Novosibirsk, their comparative characteristics and interrelationships between physical development and a degree of puberty were studied. Considerable differences in the TG level of plasma were found. The fact might be due to regional nutrition habits.

[Lung cancer in the elderly and middle-aged in Moscow and its surgical treatment].

Lung cancer morbidity in Moscow in recent years is discussed. Two-thirds of the patients were 60 years of age or older. Lung cancer operability in Moscow has remained relatively stable accounting for 10-15%. The age distribution of the said parameter was as follows: patients under 60 years--30-32%; 60-69 years--12-14% and those over 70--4-5%. Results can be better if more middle-aged and elderly patients are operated on. The authors' clinical data on 842 surgical cases of lung cancer (males and females of varying age) are analysed.

[Comparative characteristics of the lipoprotein spectrum in hypercholesterolemia in 40- to 59-year-old men living in Moscow and Kharkov].

Total cholesterol, triglycerides and high- and low-density lipoprotein cholesterol were measured in a study of 40-59-years-old male populations in Moscow and Kharkov. Hypercholesterolemic individuals were identified, using standard criteria, in both cities. Although mean lipid parameters were similar in individuals with second-type hyperlipoproteinemia from Moscow and Kharkov, the pattern of hypercholesterolemia was different in terms of the lipoprotein spectrum. It is concluded that detailed differentiation of second-type hyperlipoproteinemia is essential for the identification of major mechanisms of the atherogenic potential of hypercholesterolemia in different populations.
