[Use of multilocus sequence typing and pulsed-field gel electrophoresis for the study of serogroup B Neisseria meningitidis isolates from Casablanca (Morocco)].

A previous study showed that B:4:P1.15 was the most frequent phenotype of Neisseria meningitidis isolated in Casablanca (Morocco). To determine if there was an epidemic clone, MLST and PFGE were used to compare 13 B:4:P1.15 strains isolated from September 1999 to December 2000. MLST showed 4 Sequence Types (ST): ST-33 was the most frequent ST (9/13 strains) and 4 strains belonged to 3 newly described STs. Twelve stains belonged to ST-32 complex, and one strain presenting a new ST (ST-2502) did not belong to any known ST complex. The analysis by PFGE showed that the strains were subdivided into 7 clusters, and that there was no epidemic clone. MLST is useful for long-term epidemiological studies on N. meningitidis strains from varied geographical origins. PFGE seemed to be well adapted to the comparison of a small number of strains isolated during a short period within a defined community.

Epidemiological study of the cystic echinococcosis in Morocco.

The objectives of this epidemiological study on cystic echinococcosis (CE) in Morocco (2001-2004) were to update the prevalence of CE in different animal species living in the most important areas of the country and to collect protoscoleces and germinal layers for genetic research purposes. The post mortem inspection concerned 2948 sheep, 2337 goats, 618 cattle, 482 camels and 455 equines (325 horses, 60 mules and 70 donkeys) in five different regions: the Rif (Mediterranean coast and high mountains of the Rif), the Loukkos (Atlantic northwest plain), the center (Rabat and Casablanca regions), the Middle Atlas mountains and the south (arid and semi desert areas). The global CE infection prevalence rates obtained were 22.98% in cattle, 10.58% in sheep, 12.03% in camels, 17.80% in equines and 1.88% in goats. The infection rates were especially high in the Middle Atlas in cattle (48.72%) and in the Loukkos in cattle and sheep (37.61 and 31.65%, respectively). The majority of infected cattle (49.6%) and sheep (52.1%) had hydatid cysts in both liver and lungs. Except for cattle, the liver was more infected than lungs in all the other animal species. Animals more than 5 years old were the most infected in all species. The mean CE infection rates of these animals were about 56% in cattle, 40% in sheep, 20% in camels, 17.80% in equines and 7% in goats. These rates were much higher in the Loukkos (85% of cattle and 59% of sheep) and in the Middle Atlas (68% of cattle and 45% of sheep) than in the other regions. Results showed that Echinococcus granulosus is in an endemic steady state with no evidence of protective immunity in the intermediate hosts. The mean numbers of infections per year are 0.099 for cattle, 0.063 for sheep, 0.03 for camels and 0.010 for goats.

Evaluation of three methods to control Bulinus truncatus, the intermediate host of Schistosoma haematobium in an irrigation scheme, Tessaout-Amont, Morocco.

In Morocco, the implementation of new irrigation schemes resulted in the introduction of schistosomiasis haematobium to new areas. Canal structures (especially syphon boxes) were found to be favourable habitats for Bulinus truncatus and they constitute important transmission sites. Three methods for snail control in these syphon boxes were assessed in the Tessaout-Amont irrigation scheme. Molluscicide (niclosamide) application resulted in a pronounced reduction in the snail population density, but snails started to recolonise the sites a few months later. Regular brushing of the inner sides of the syphon boxes and covering the syphon boxes, so as to exclude light, resulted in significant reductions in snail numbers. However, the latter methods were not as efficient as the molluscicide.

[High cord blood TSH in Morocco: iatrogenic hypothyroidism?].

A short prospective study was conducted to assess thyroid status in healthy full term newborns (n=90) of a large maternity of Marrakech (Morocco), as part of the validation of a national salt iodisation program. High TSH (>5mU/l) was detected in 89% of infants tested; urinary iodine excretion was measured in 35 of the mothers, and was found to be normal (100-200microg/l) or high (>200microg/l)(n=27) in all of them. Milk iodine concentration was measured in 315 lactating women from the same area. Low values (<41microg/l) were found in 60% of them. The common use of iodinated disinfectants during delivery could be responsible for the high urine iodine values of mothers; however iodine deficiency seems to remain a widespread problem in this population and justifies a large scale survey of iodine status.

[Profile of prescription of psychotropic drugs in a National Fund of Social Foresight Organisation in Morocco].

INTRODUCTION: Psychotropic drugs have undergone significant development in the last few years and their prescription is no longer limited to psychiatrists alone but also to other specialists and general practitioners. In Morocco, many topical questions arise regarding the profile of the prescription of psychotropic drugs. DESIGN: of the study. The present work is a transverse epidemiological study carried out by the CNOPS (National Fund of Social Foresight Organisations) consisting on the random selection of prescriptions drawn-up during the year 1997. The research work consisted at first in making the whole prescriptions including at least one psychotropic drug. Subsequently, we considered various parameters (age, gender, number and family of psychotropic drugs, prescriber, associations between psychotropic drugs...). RESULTS: Four percent of prescriptions included at least one psychotropic drug, 64.1% included one psychotropic drug and more than 33% included 2 to 3 psychotropic drugs. We have noticed a slight female predominance and half of the prescribed psychotropics was prescribed to patients aged between 40 and 60 years. General practitioners ranked first among doctors who prescribed psychotropic drugs (27.7%) followed by psychiatric-s (19%). Among psychotic-type drugs, anxiolytics dominated in the prescriptions (44.9%) followed by neuroleptics (25.2%), antidepressants (18.7%) and finally hypnotics (3%). Women consumed more anxiolytics and antidepressants than men. Among prescriptions drawn-up by general practitioners 52.4% were composed of anxiolytics. Psychiatrists prescribed neuroleptics in 35.3% of cases, then anxiolytics in 35.2%, and antidepressants in 28.7% of cases. Hypnotics concerned only 2.2% of their prescriptions. For the other specialists, the majority of prescriptions were for anxiolytics. Monotherapy was respected in 64.1% of cases. Antidepressants were combined associated with anxiolytics in more than 55% of cases. CONCLUSION: Psychotropic drugs are prescribed more by general practitioners than by psychiatric-s, and anxiolytics constitute the main component of the prescriptions, bearing in mind that women consume more anxiolytics and antidepressants than men.

Study of paralytic shellfish poisoning toxin profile in shellfish from the Mediterranean shore of Morocco.

Since 1992, a monitoring program for bivalve molluscs contaminated by algal toxins was established at different stations along the Mediterranean Moroccan shores. The monitored stations were tested every 2 weeks. The presence of toxicity was determined using the mouse bioassay method. Toxin profile was carried out by HPLC/FD in selected contaminated tissues. According to the outcomes of this surveillance from 1994 to 1999, reliable information on toxicity of shellfish was obtained. They indicate that PSP is a recurrent toxicity in molluscs along the Mediterranean shore of Morocco. It has been noted a difference of PSP accumulation among individual shellfish. The cockle (Achanthocardia tuberculatum) presents toxicity throughout the year, while other specimens from the same area such as clam (Callista chione), warty venus (Venus gallina) and marine beans (Donax trunculus) accumulate it seasonally from January to April, after which they depurate the toxin. Moreover, the study of toxin profiles among individual shellfish was undertaken. It was found that shellfish presented a complex profile pointing to contamination by Gymnodinium catenatum.

Spatial and temporal evolution of PSP toxins along the Atlantic shore of Morocco.

A monitoring program for bivalve molluscs contaminated by algal toxins was established in 1992 at different stations along the Atlantic Moroccan shore. The presence of toxicity in bivalve molluscs commercially exploited was tested fortnightly using the mouse bioassay method. Results obtained from this surveillance indicate paralytic shellfish poisoning is responsible for bivalve molluscs contamination along the Atlantic coastline of Morocco. Toxin profile was established by automated pre-column HPLC/FLD in selected contaminated tissues. The study of individual toxins in mussel during a bloom in the northern Atlantic coastline in 1994 showed a very complex profile, typical to that obtained with cultures of the toxigenic dinoflagellate Gymnodinium catenatum isolated from the Iberian region. However, toxin composition of mussels and marine beans from later blooms in the southern Atlantic coastline in 1999 showed a strong resemblance with that of Alexandrium minutum, due to dominance of gonyautoxins 1/4. A minor contamination by G. catenatum due to the presence of decarbamoyl-saxitoxin is hypothesized.

Behavior of methyl isothiocyanate in soils under field conditions in Morocco.

The behavior of methyl isothiocyanate (MITC), active metabolite of metam-sodium (MS), was studied under field conditions in Morocco. MS was applied through drip irrigation in: (i) uncovered soil, (ii) soil covered with transparent polyethylene, and (iii) soil covered with virtual impermeable film. Concentrations of MITC were determined at different soil depths to determine the distribution of MITC and the concentration-time product (CTP). Six hours after MS application, in a sandy soil, MITC reaches the 20-30 cm soil layer, but remains highly concentrated in the upper 10-20 cm soil layer. In a silty clay soil, MITC was concentrated in the upper 0-10 cm soil layer. The dissipation of MITC under different conditions of application was fast and complete after seven days. However, MITC dissipation time (DT(50)) was <24 h in sandy soil treated, but 63 h in silty clay soil. Under these application conditions of MS, the plastic film reduced MITC loss to the atmosphere but the plastic film quality did not affect the behavior of MITC. The use of plastic film maintained high MITC concentrations and appropriate CTP at different soil depths.

Toxic and essential trace metals in muscle, liver and kidney of bovines from a polluted area of Morocco.

Toxic and essential trace metals were measured in muscle, bone, liver and kidney of bovine grazing on the municipal wastewater spreading field of Marrakech City (Morocco). Bovines were found to be seriously contaminated by toxic metals, for metal bioaccumulation. The high cadmium content seemed to contribute to a reduction in zinc and copper levels. The arithmetic mean concentrations of zinc, especially cadmium, and levels were higher in liver and kidney, specific target organs copper and cadmium in liver and kidney, were respectively: 126, 112 and 5.1 microg/g in liver; 89, 33 and 10.3 microg/g in kidney.

Trace elements in children's hair, as related exposure in wastewater spreading field of Marrakesh (Morocco).

Lead and cadmium concentration was determined in the hair of 327 school children living in a wastewater spreading field of Marrakesh (Morocco). The influence of age, sex, food habits and family occupation on the children's hair Pb and Cd concentration was also evaluated. Girls had more metal in their hair than boys (16.5 +/- 5.4 micrograms/g and 12.5 +/- 3.5 micrograms/g, respectively). However, for Cd the boys had more metal (2.9 +/- 0.6 and 2.2 +/- 0.4, respectively) but the difference was not statistically significant and metal levels decreased with age. Family occupation, direct contact with wastewater, customs and food habits were the most significant factors influencing the metal content of children's hair. The average Pb and Cd content were higher in the exposed children (14.8 +/- 4.5 micrograms/g and 2.5 +/- 0.5 micrograms/g, respectively) than in the non-exposed children (4.6 +/- 2.2 micrograms/g and 0.6 +/- 0.2, respectively), but the difference was not statistically significant. This study shows that in this area all the population (especially children) was extremely exposed to the danger caused by potentially toxic metals.

Determination and comparison of uranium and radium isotopes activities and activity ratios in samples from some natural water sources in Morocco.

Radiochemical results (238U, 226Ra and 228Ra activities; 234U/238U, 228Ra/226Ra and 226Ra/238U activity ratios) are reported for 42 natural water samples collected from wells, hot mineral springs, rivers, tap water, lakes and irrigation water in 15 Moroccan locations. Results show that 238U activity varies between 4.5 and about 309 mBq l(-1) in wells, 0.6 and 8.5 mBq l(-1) in hot springs, 9.7 and 28 mBq l(-1) in rivers, 2.5 and 16 mBq l(-1) in tap waters and between 6 and 24 mBq l(-1) in lakes. The 234U/238U activity ratio varies in the range 0.87-3.35 in all analyzed water samples except for hot springs where it reaches values higher than 7. Unlike well water, mineral water samples present low 238U activities and high 234U/238U activity ratios and 226Ra activities. The highest activity of radium in mineral water is 150 times higher than the highest activity of 226 Ra found in well water. 226Ra/238U activity ratios are in the ranges 0.07-1.14 in wells, 0.04-0.38 in rivers, 0.04-2.48 in lakes, and 1.79-2115 in springs. The calculated equivalent doses to all the measured activities are inferior to the maximum contaminant levels recommended by the International Commission of Radioprotection and they do not present any risk for public health in Morocco.

The study of human behavior and schistosomiasis transmission in an irrigated area in Morocco.

This paper presents a research strategy for studying water contact, water use and schistosomiasis transmission in an irrigated area of Morocco. This setting, with many scattered water contact sites, many activities carried out at these sites, and the small number of people involved, was not appropriate for a conventional water contact study based on the observation of water contact sites, such as had been carried out in the Nile delta. The Moroccan study utilizes three related concepts: the household, time geography, and the gendered use of space. It seeks to understand processes and interrelationships underlying the daily mobility pattern of individual households, and seen as part of a larger system of organization and structure in time and space. The preliminary results of the study indicated the complexity and dynamism of water use and water contact, which need to be considered in planning disease control strategies especially in changing settings, such as those associated with environmental interventions in the study area.

Camel trypanosomosis in Morocco. 2. A pilot disease control trial.

A first epidemiological study on camel trypanosomosis was conducted in 1997 and 1998 in five Moroccan Saharan provinces. It revealed two major foci at Merzouga (Tafilalet province) and Zagora (Ouarzazate province) where tour camel seroprevalences were 35.4 and 43.3%, respectively, for each region. The first area was chosen in the present study as a pilot region for a control program based on epidemiological data with the aim of evaluating the efficiency of the trial in terms of seroconversion, incidence and health condition. After finding the endemic focus at Merzouga, four other field studies were organised in this area, at 4-month intervals, in order, firstly, to control the camel reservoir by treating all the camels found in the region during the visit using melarsomine (Cymelarsan, Rhône-Mérieux, the only trypanocide registered in Morocco), and, secondly, to collect blood samples for studying temporal changes in seroprevalence as well as in anaemia rate. All the 291 tour camels of the area, that were present in at least one of the field visits were examined and included in the trial. Among them, 162 were treated once, 84 twice and 41 three times. A total of 676 serum samples were collected from these camels. At the end of the trial--1 year later--seroprevalence had dropped from 58% (May 1998) to 19% and the mean Ab-ELISA value had decreased from 0.52 (before treatment) to 0.12 (which is below the threshold level), after the third campaign. The haematocrit of the animals was also slightly improved, indicating that trypanosomosis was a major cause, but it was not the only incriminated factor. A seasonal effect was observed. The overall seroprevalence was reduced between spring 1999 and summer 1999, without treatment, whereas a negative effect was recorded on packed cell volume, more camels being found anaemic in summer than in spring. The application of these strategic treatments was associated with a large reduction in the seroprevalence of trypanosomosis, but there was no complete eradication because of the continual exchange of animals from other foci such that of Zagora. The overall seroconversion after 1 year reached 55% and no post-treatment complication was recorded, whereas the incidence due to new infections approached 4.3%. Moreover, several factors like the unusual way of life of the dromedaries, including stress, excessive work, overcrowding and continual contact with other animals coming from other infected regions contribute to the persistence of the disease.

Serological evidence of FMD subclinical infection in sheep population during the 1999 epidemic in Morocco.

During 1999, 11 outbreaks of foot and mouth disease (FMD) were declared in the east and central part of Morocco. All the FMD clinical cases reported were cattle. In order to analyse the serological status of sheep from the FMD outbreak areas, 598 sheep sera were tested using a liquid-phase blocking ELISA (LPBE) to detect antibodies against FMDV structural proteins. The study confirmed the presence of FMDV specific antibodies in 77 clinically normal sheep, indicating that unrecognised FMDV-infected sheep could represent a potential risk of FMD dissemination in Morocco.Subsequently, sera from flocks of sheep that had been exposed to FMD outbreaks were assayed by an indirect ELISA using the recombinant FMDV non-structural protein 3ABC expressed in E. coli to evaluate the potential use of this serological test in future epidemiological studies and the development of FMD control strategies. The results indicated that the 3ABC-ELISA was able to detect antibodies indicative of infection with FMDV in asymptomatic sheep in field conditions.

Menopause in Morocco: symptomatology and medical management.

OBJECTIVES: To assess the frequency of menopausal symptoms and patterns of recourse to medical care in Rabat, Morocco. METHODS: Face to face interviews with a representative sample of 300 women aged 45-55 years; information was collected on socio-demographic variables, reproductive history, use of health care, symptom checklist, and medical management of menopause. RESULTS: The most frequent complaints are fatigue and hot flashes, each reported by 61% of women, headaches (57%), joint pain (54%), anxiety (44%) and irritability (42%). Hot flashes, but not cardiovascular symptoms, are statistically associated with menopausal status. Only 5% of women in the sample take hormones, and 4% calcium; 13% continue to take contraceptives. The frequency of some symptoms and the use of health care for menopause are influenced by socio-economic factors. CONCLUSIONS: Reports of hot flashes and joint pains are relatively high, but the frequency of use of medical services for menopause is low.

Ethnobotanical survey of medicinal plants used for the treatment of diabetes, cardiac and renal diseases in the North centre region of Morocco (Fez-Boulemane).

In order to make an inventory of herbal remedies commonly used in the treatment of diabetes, hypertension and renal diseases in the North centre region of Morocco, 1527 patients (1095 diabetic patients, 274 with renal disorders and 158 with cardiac disorders) and 25 traditional herbal healers were interviewed in four different areas of Fez-Boulemane region. More than 1153 of the total patients interviewed (76%) used regularly medicinal plants to treat diabetes, cardiac and renal diseases. These data showed that phytotherapy has always be practiced in this region. All the persons interviewed have indicated that the reasons of using phytotherapy is that the plant medicines are cheapest (54%) and more efficient (38%) than modern medicine. They also reported that the result of phytotherapy is better (72%). Our survey started at May 1997. About 90 plants were cited (54 plants for diabetes, 11 for cardiac diseases, 19 for hypertension and 33 for renal diseases). The plants reported have been identified and are presented in a table with the vernacular name, useful parts, ecological distribution and medicinal uses. Only 12% of the total patients have a relative knowledge of the toxic plants. The result indicated that nine plants are extremely toxic at high doses and chronic treatment. Fifty nine percent of the interviewers have indicated that they used medicinal plants from the experience of the other.

Ethnobotanical studies and economic evaluation of medicinal plants in Taounate province (Northern Morocco).

An ethnobotanical survey was carried out among the Taounate population in Northern Morocco to identify plants used in folk-medicine. Two distinct physiographic regions of the province, populated by two ethnic groups, were surveyed. Extensive investigation undertaken during the past 5 years has brought to light 102 medicinal plants belonging to 48 families. The scientific and vernacular names of plants, their ecological distribution, and the popular uses of the plant, the part of the plant used, the preparation and mode of administration are presented. Plants are widely used in indigenous pharmacopoeia to alleviate the common symptoms of cardiovascular (5.8%), gastrointestinal (24.9%), bronchopulmonary systems (9.8%), urogenital (12.2%) and skin (9.2%) diseases, and other disorders which are often associated with magic. The majority of medicinal plants grow in the wild (61%), while others are cultivated (37%) and some (1.9%) are domesticated. These plants are more abundant in the northern part (62%) of the province where they grow in forested areas. Among the 102 species inventoried, 13(12.7%) medicinal plants are widely commercialized in the region and exploited outside of the province. The data collected from 17 wholesalers, show the income derived from medicinal plants to be about USD 1,826,900 per year. This survey demonstrates that the medicinal plant sector in the province is a promising economic resource for developing this region, but it needs planned exploitation, and that the tribes should continue to master the folk-medicine.
