Softisan--a new vehicle for patch testing.

Petrolatum is a good vehicle for many patch test allergens, because it is stable, unreactive, and allergy to it is rare. Some difficulties exist, however, when water-soluble allergens are tested, because emulsifiers are often needed; these can change the properties of both the test material and the vehicle itself. In the present study, we compared petrolatum with a new lanolin substitute based on vegetable fatty acids as a patch test vehicle for nickel sulphate.

The usefulness of detailed information to patients with contact allergy.

To find out if patients with contact allergy are helped by computerized information lists, a retrospective study was carried out on 58 patients with contact allergy to lanolin, traced through our local database DALUK. All were sent a questionnaire about their usage of the information list, clearance of their eczema, their education and other details. Clearance of the patient's eczema was found to correlate with use of the information list. It was also found that the effectiveness of the information depended on factors such as education, family circumstances, ethnic background and, most of all, how and where the information list was used.

Maintenance of silastic-teflon shunts for intermitten haemodialysis.

The occurrence of infection in the tissues surrounding external arteriovenous shunts was studied and die important relationship of pyogenic infection to clotting was confirmed. The local application of fusidic add tulle and lanolin greatly reduced the occurrence of both infection and clotting and the need for cannula replacement.Urokinase used for declotting shunts when standard procedures had failed, restored blood flow whether dotting was related to infection or to local vascular factors. This treatment is not advised when clotting is associated with a local abscess, as it may make cannula replacement necessary. Severe local vascular factors, such as metastatic calcification, Raynaud's phenomenon, and venous stenosis, may lead to poor blood flow, so that despite clot lysis elective cannula replacement or the creation of a subcutaneous arteriovenous fistula is required.

Prevention of and therapies for nipple pain: a systematic review.

OBJECTIVE: To review the literature on nipple pain and to delineate effective strategies for the prevention and treatment of nipple pain in breastfeeding mothers. DATA SOURCES: Computerized searches on MEDLINE, Pre-MEDLINE, CINAHL, and the Cochrane Library. STUDY SELECTION: Articles from indexed journals relevant to the objective were reviewed from January 1983 to April 2004. Preference was given to research-based studies in English. DATA EXTRACTION: Data were extracted and organized under two headings: prevention of nipple pain or trauma and treatment of nipple pain or trauma. The Critical Appraisal Form by J. Briggs was used to extract the data from research-based articles. DATA SYNTHESIS: The health benefits of breastfeeding for mother and infant are well documented; however, nipple pain is a common reason reported by women for the early termination of breastfeeding. Several studies have compared various treatments for either the prevention of or treatment for nipple pain. These treatments include warm water compresses, tea bag compresses, heat, application of expressed mother's milk, lanolin, vitamin A, collagenase, dexpanthenol, hydrogel therapy, glycerin gel therapy, moist occlusive dressing, education regarding proper latch-on and positioning, and no treatment. CONCLUSIONS: No one topical agent showed superior results in the relief of nipple discomfort. The most important factor in decreasing the incidence of nipple pain is the provision of education in relation to proper breastfeeding technique and latch-on as well as anticipatory guidance regarding the high incidence of early postpartum nipple pain.

[Preparation of a gel-forming ointment base applicable to the recovery stage of bedsore and clinical evaluation of a treatment method with different ointment bases suitable to each stage of bedsore].

An ointment base for better treatment of bedsores was developed to improve the release rate of minocycline hydrochloride (MH) and the water absorption capacity using various types of hydrophobic to hydrophilic ointment bases. The effect of purified lanolin (PL) on the release behavior of MH from a hydrophilic ointment (HO) base was the primary focus. It was found that the drug release rate from the ointment base was modified according to the method of preparation of the ointment base and the type of cyclodextrins admixed. The physicochemical properties, such as viscosity, elution volume, and water absorption, of the ointment base were also modified by those factors. To develop an ointment formulation suitable for the recovery stages of bedsores, including the proliferation period of granulation and the formative period of epidermis, the physicochemical properties of Macrogol ointment containing various hydrophilic polymers, which have gel-forming ability, were tested. A novel ointment base suitable for the treatment in the recovery stage of bedsores was developed using hydrophobically modified hydroxypropyl methylcellulose (HM-HPMC). Considerably sustained release of MH (T50 of 170 at a min) was attained with a macrogol ointment mixed with HM-HPMC and Carbopol formulation ratio of 3:7. We clinically evaluated the effectiveness of bedsore treatment by applying different ointment bases to patients with different stages of bedsores.

Short communication: disinfectant containing a complex of skin conditioners.

The efficacies of 2 new teat dip formulations were tested against experimental challenge by contagious mastitis pathogens Staphylococcus aureus and Streptococcus agalactiae over a 12-wk period. Formulations contained an iodine complex (0.5 or 1.0% iodine) and skin conditioning agents (propylene glycol, polyvinylpyrridone, glycerine, lanolin, allantoin, and aloe). Percentage reduction (dipped vs. control mammary quarters) in new contagious mastitis pathogen intramammary infections for the 0.5 and 1.0% iodine dips was 65.4 and 84.5, respectively. Both dips were significantly effective in reducing new contagious intramammary infections. Teat skin scores and teat end scores varied over time but were virtually identical for both treated and control teats, for both treatments. Thus, both dips were effective in reducing new contagious mastitis infections without untoward effects on teat skin condition.

A comparison of the cost effectiveness of pressure-indicating materials and their ability to detect pressure areas in complete dentures.

Pressure areas on the fitting surfaces of dentures can cause great discomfort and pain to denture wearers if not detected and removed. Pressure-indicating materials are commonly used to detect these areas, and several commercial varieties are available, but these tend to be expensive. The cost effectiveness of these materials has not been investigated, nor has this been linked to their efficacy and ease of use. The aim of this study therefore, was to compare the different pressure-indicating materials available commercially with that of a home-made paste. An assumption of efficacy was made by the number and size of pressure areas revealed, by taking standardised photographs and analysing the images. User friendliness was determined by the time taken to mix, apply and remove the material. Cost was determined by a cost per unit calculation based on the average or minimum quantity required for each material. The results indicated that a home-made paste made of equal quantities of hand lanolin (BP) and zinc oxide powder was not only the most effective, but was also the cheapest, being only 3% of the cost of the most expensive of the materials.

Non-comedogenic cosmetics.

The need for a "comedogenic-free" cosmetic for acne patients led to this study using the only available mode, the rabbit. To facilitate the formulation of this benign cover-up, a range of cosmetics and ingredients was tested. Many cosmetics, fractions, and modifications of lanolins were comedogenic, as were emulsifiers such as butyl sterate, isopropyl myristate and sodium lauryl sulfate. From this data we incorporated non-comedogenic, into an acceptable cosmetic formulation for patients. The use of this formulation in susceptible acne-prone women reduced the rate of "acne cosmetica" in our clinic patients from 25% to less than 5%. We were unable to confirm a recent study suggesting that sulfur was a potent acnegen.

[Allergic phenomena in the treatment of leg ulcer].

First mentioning the immunological processes of contact dermatoses, the authors study the six contact allergies encountered most frequently by them during the course of 133 cases of leg ulcers which they treated. They consider in detail the allergenic action of the following six substances: lanolin, neomycin, balsam of Peru, paraben, phenylenediamine and benzocaine. They pioneer therapies intended to prevent the risk of sensitization.

Studies on the release of solubilized drugs from ointment bases. Part 8: The effect of emulsifiers on pharmaceutical availability of ethanolamine, diethanolamine and morpholine from emulsive ointments.

The effect of addition of lanolin and Span 60 to white vaseline and of their concentration on the pharmaceutical availability of monoethanolamine , diethanolamine and morpholine was investigated. The emulsifier type and its concentration exercise an influence on constant release rates. The liberation of the investigated amines proceeds at almost the same speed as therapeutic substances within the limits from 10 to 30%. The release half life periods showed the same dependence in relation to the content of investigated substances in the ointment.

[Eczema of the lower leg--clinical, allergological and differential diagnostic aspects].

More than 50% of patients with dermatitis of the lower legs have a contact allergy. In 60 of our out-patients, the most frequent allergens were found to be balsam of Peru, lanolin and terpentine, followed by benzocaine, p-phenylenediamine, neomycin, oxychinoline, and parabens. Contact sensitization should be chiefly suspected in patients showing resistance to treatment despite apparently adequate therapy, and in those complaining of intolerance to definite topical treatment. Further indices are vesiculation and oozing in the environment of the ulceration, or scattered foci of dermatitis in the face (blepharoedema) or on the hands. The importance of patch testing is emphasized. Suggestions for differential diagnosis and treatment are given.

A new technique for evaluating bath oil in the treatment of dry skin.

Patients with moderate-to severe dry skin of the lower legs bathed daily for 17 days using either an emulsified lanolin and mineral oil bath additive (Treatment Group) or their usual bath soap (Control Group). The affected sites were evaluated and photographed before and at selected times following treatment, using a standardized technique. The severity of the dry skin as it appeared in projected color transparencies was evaluated on a 10-point grading scale by a second investigator under blinded conditions. The differential response between treatment and control groups increased with time and was found statistically significant (p less than 0.05), demonstrating the sensitivity of the method as well as the apparent therapeutic effects of the bath additive.

Bioavailability of hydrocortisone acetate from ophthalmic ointments.

The biopharmaceutical studies of 1% hydrocortisone acetate ophthalmic ointments were carried out with isolated pig's eyes and demonstrated that the drug concentration in intraocular fluids depended on: a) ointment base used - the best bioavailability was obtained using white petrolatum with addition of 10% anhydrous lanolin, b) eye fluid examined - concentration of hydrocortisone acetate in aqueous humor was several times higher than in vitreous humor, c) presence of corneal epithelium - in all studied cases abrasion of corneal epithelium was accompanied by well-marked increase of drug concentration in intraocular fluids.

Myospherulosis: a preventable iatrogenic nasal and paranasal entity.

We discuss a disorder that, although at first considered of possible fungal origin, now is known to be due to an alteration of erythrocytes induced by petrolatum-based antibiotic ointment. In this country, the structures of myospherulosis have been seen most often in tissues from the nose and paranasal sinuses following surgery in this region. Our studies show that both components of the vehicle of a commonly used tetracycline antibiotic ointment, lanolin and petrolatum, either separately or together, produce the structures of myospherulosis in vitro. It is recommended that a nonpetrolatum-based substance be used in nasal packing for purposes of hemostasis.

Kinetics of indomethacin degradation II: Presence of alkali plus surfactant.

The kinetics of indomethacin were studied in the presence of the surfactants ethoxylated lanolin, polysorbate 80, and cetrimonium bromide under alkaline conditions at 30.3 degrees. The degradation followed apparent first-order kinetics. Plots of kobs versus surfactants; concentrations were curved with negative slopes for nonionic surfactants; but with the ionic surfactant, the plots showed a marked positive change in kobs as the surfactant concentration passed through the critical micelle concentration. Literature model systems adequately explained the data for nonionic surfactants but not for the ionic surfactant. A new set of equations was derived for each case using electrochemical potentials. The experimental data for all three surfactants fit the derived equations quite well.

Prevention of adriamycin-induced skin necrosis with various free radical scavengers.

Infiltration of antitumor agents into subcutaneous tissues may either result in a local area of self-resolving inflammation or progress to full-thickness loss of skin and underlying vital structures. Inadvertent extravasation of adriamycin can result in severe tissue necrosis. The mechanism of this tissue damage is believed to be release of oxygen free radicals into the tissue. After adriamycin extravasation, the treatment groups were made up according to drugs used, EGb 761, pentoxifylline, alpha-tocopherol acetate, and alpha-tocopherol succinate in rats. To prevent the necrosis and to decrease the tissue malondialdehyde levels, the most effective agent was found to be EGb 761, and pentoxifylline was also effective (P < 0.001). No difference was found between topical lanoline and saline (P > 0.05). The maximum ulcer diameter was obtained in 2 weeks. The maximum tissue malondialdehyde levels were obtained in 24 h, and in comparison to the control group the treatment groups showed lower levels. Our aim is to show the role of free radicals in the formation of skin necrosis as a cause of adriamycin extravasation and to prevent or decrease the skin necrosis using various free radical scavengers.

The adjuvancy of silicones: dependency on compartmentalization.

Studies have been conducted in mice (B6C3F1) and rats (Sprague Dawley, Fischer 344) to investigate the adjuvancy potential of silicone mammary gel and the low molecular weight silicone fluid, octamethylcyclotetrasiloxane (D4). Dependent on the experimental conditions employed, a divergent data profile emerges. If the antigen (bovine serum albumin, BSA) is emulsified with either the gel or the D4 prior to intramuscular immunization, an amplified anti-BSA IgG antibody response, as measured by multipoint ELISA methodology, is noted over the 8 week measurement period. In parallel studies, a variety of non-silicone personal care ingredients (lanolin, white mineral oil, isopropyl palmitate) were also capable of amplifying this humoral response relative to the non-adjuvant phosphate buffered saline control. These observations are consistent with the empirical knowledge that hydrophobic substances tend to augment immune responses. However, under conditions in which the antigen is not blended with the silicone prior to immunization, normal immune responses are noted. In short (10 day) and long (180 day) term gel implant studies, the optimal IgM and IgG antibody responses, as determined in the antibody forming cell assay, were equivalent between the gel implanted and control animals. Moreover, under similar exposure conditions, no adjuvancy was noted in the three Host Resistance models (B16F10 Melanoma, Listeria monocytogenes, and Streptococcus pneumoniae) tested. Antibody forming cell studies conducted after 28 days of oral or inhalation exposure to D4 have also yielded responses similar to the non-silicone exposed vehicle controls. Collectively, these data suggest that in the absence of premixing the antigen with the silicone test material, there does not appear to be any silicone induced adjuvant response.

Allergic contact sensitivity in elderly patients.

BACKGROUND AND AIMS: Aging has been shown to be correlated with the rate and type of contact sensitization, but only a few studies have evaluated patch test reactivity in elderly subjects with an adequately large population. METHODS: The response patterns to patch testing in 1444 elderly subjects (>65 years) with suspected allergic contact dermatitis were studied, and the results compared with a control group of individuals with suspected allergic contact dermatitis, aged between 20 and 40 years. RESULTS: The prevalence of the positive patch test to at least one hapten was significantly lower in the group of elderly patients compared with adult patients (40.7 vs 47.8%, p<0.0001). However, some allergens, i.e., primin, diaminodiphenylmethane, neomycin, lanolin alcohols, paraben mix, Euxyl K400 and quinoline mix, showed an increased sensitization rate in elderly patients compared with adult patients. These allergens are now less frequently employed in the workplace, or are substances particularly used in the formulation of topical treatment of age-related diseases, i.e., leg ulcer and xerosis. It was also found that the intensity of positive patch test reactions was significantly lower in elderly patients compared with younger subjects, with higher proportions of weak (+) positive reactions. Moreover, elderly patients showed a dynamic pattern of increasing intensity of patch test reactions at the second reading after 3 days compared with the first reading after 2 days more frequently than younger patients (60 vs 53%, p<0.0001). CONCLUSIONS: These findings suggest an age-dependent decline of overall positive patch test reactions, but a higher sensitization rate to some allergens frequently used in the composition of topical treatments. The development of an allergic response in elderly patients was found to be delayed, and this may require an additional reading after 7 days and the interpretation of even weak reactions as valid positive patch test reactions.
