Development of a California Q-set indicator of primary psychopathy.

Seven judges described the personality characteristics of the primary psychopath by sorting the 100 items of the California Q-set in a forced-normal distribution. Item scores resulting from these sorts were aggregated across judges to form a Psychopathy Prototype. The reliability of the seven-judge aggregate prototype was .90. To examine the reliability of scores derived from the prototype, the personalities of 65 target subjects were described by two peer judges using the California Q-set (Block, 1961). For each subject, Psychopathy Prototype scores were derived by correlating each judge's Q-sort profile with the Psychopathy Prototype. Findings indicated that the peer-generated psychopathy scores correlated r = .61 between judges. Using the Spearman-Brown formula, Psychopathy Prototype scores have reliabilities of .75, .82, and .86 when aggregated over two, three, and four peer judges, respectively. To further explore properties of the measure, the Psychopathy Prototype was compared with independently developed California Q-set prototypes describing the narcissist and the female hysteric. The results revealed some interesting contrasts among these concepts and serve to support our contention that the Psychopathy Prototype has utility in regard to distinguishing between pathologies with overlapping features.

[The dysorexia-sialomegaly-amenorrhea syndrome (author's transl)].

The enlarged parotid glands seen in both poor nutritional states and in women with amenorrhea have always been considered to be separate entities. Thirteen observations are reported of a syndrome consisting of dysorexia (or anorexia), swelling of the salivary glands (sialomegaly), and menstrual disturbances (hypomenorrhea or amenorrhea). This syndrome can be defined as a clinical entity combining the symptomatology of parotid enlargements due to poor nutrition and those caused by sex-hormone disorders. In the Dysorexia-Sialomegaly-Amenorrhea syndrome, the salivary gland swellings are due to poor nutrition and the amenorrhea, which is of the hypothalamic type, is also secondary to poor nutrition.

Hysteria, depression and the nosological problem of chronic pain.

In this article the nosological relevance of hysteria and depression for somatically unexplained chronic pain is critically discussed. Despite many clinical controversies and methodological problems, it is concluded from the available literature that both concepts merit a place in the psychiatric nosology of chronic pain. Nevertheless, more research is needed to further clarify the complex relationships between hysteria, depression and pain, and to more exactly determine the relative importance of hysterical and depressive features in chronic pain patients.

Factitious lymphedema of the hand: a diagnostic challenge.

A case of factitious lymphedema of the hand is presented. Attention is drawn to this entity and its differential diagnosis. Delayed detection in our case resulted in prolonged and costly hospitalization with some loss of hand function. Treatment should be left to those skilled in psychotherapy.

[Hysteria and psychosis].

The relationship between endogenous-psychotic and hysterical behavior is presented in this paper on the basis of typical patients' accounts. Various patterns of behavior are revealed: chronic hysterical psychotic syndromes, acute hysterical psychotic syndromes, hysterical exalted psychotic episodes, simulated hysterical psychotic syndromes, hysterical behavior in the attempt to cope with psychosis, and pseudohysterical behavior in endogenous psychoses. The specific aspects of the doctor-patient relationship in the case of these patients are described.

[Activity of the sympatho-adrenal system in patients with hysterical psychopathy and psychasthenia].

The paper is concerned with studies of the sympathoadrenal system activity by the indices of urine excretion of catecholamine and dofa in patients with hysterical and psychasthenic psychopathy. The disorders inherent in each of the groups are demonstrated. The patients with hysterical psychopathy show an exhaustion of all links in the catecholamine metabolism, while the patients with psychasthenical psychopathy an exhaustion of the noradrenaline link. In attempting to explain the mechanisms of disturbed activity in the sympathoadrenal system in both groups the role of the functional state of nonspecific activizing brain systems was taken into consideration.
