Paddy field dermatitis in Assam: a cercarial dermatitis.

Rice field dermatitis is an occupational health problem in Assam. The afflicted paddy field workers develop exanthema on the exposed parts of their skin mainly legs and arms when they come in contact with water in rice fields. Itching sensations start after 30 minutes to 1 1/2 hours after initial exposure. After 6-12 hours there is intensely pruritic macular eruption and in a few cases secondary skin infection may also develop. The clinical course of the disease is suggestive of cercarial dermatitis. Numerous gastropod snails belonging to families Lymnaeidae, Bulinidae and Planorbidae were collected from paddy fields and were investigated for cercarial infection. It was found that planorbid snails were infected with animal schistosome cercariae. These cercariae were able to invade human skin and caused dermatitis in human volunteers after repeated exposure.

Paramedical programs in rural Bangladesh.

Two programs using locally trained paramedical personnel provide health care to villagers in Bangladesh. The general level of care rendered appears to be above average for the country, but varies with the individual paramedical worker. Field supervision is crucial to quality of care. The structure of the two programs places physicians in direct contact with the paramedical workers for supervision and consultation. Unlike most programs in less developed countries, these emphasize preventive care, prepaid insurance at minimal premiums (in one plan, premiums are payable by barter), and apparently high motivation of the staff, many showing personal concern as well as evidence of sound training.

Reproductive hazards in the workplace. Development of epidemiologic research.

Application of the techniques of epidemiology and clinical toxicology has accelerated study of the reproductive effects of toxic chemical and physical exposures in the workplace. Three examples of work in progress are included in the present communication. The first concerns 1,2-dibromo-3-chloropropane, a known cause of male sterility, which continues to be used as a nematocide in Hawaii. Occupational exposures of Hawaiian agricultural workers to airborne 1,2-dibromo-3-chloropropane are mainly in the range of parts per billion. A prospective study of pineapple field workers has been undertaken to evaluate sperm counts and morphology before, during, and after 1,2-dibromo-3-chloropropane application. To date, no sperm count depression is evident at this level of exposure. The second example involves a cluster of seven spontaneous abortions in female office workers exposed to video display terminals. The cluster has been analyzed with the use of fetal life tables. Excess incidence was confirmed (p = 0.0045), but no etiology was determined. The findings may have been due to chance. The third example pertains to male chemical workers manufacturing diaminostilbene, an optical brightener, and the workers' reported sexual impotence. Impotence was confirmed in 7 of 29 workers by questionnaire and suggested for another 7. Serum testosterone analyses found depressed values (less than 300 mg/ml) in 8 of 28 exposed workers. The luteinizing hormone and follicular stimulating hormone levels were generally normal.

Ventilatory function of factory workers exposed to tea dust.

A cross-sectional study was conducted in order to determine the prevalence of respiratory symptoms and the effect on ventilatory capacity in workers exposed to tea dust for at least five years during the sifting process of tea manufacture compared to a control group of field workers who were not exposed to tea dust previously. Fifty-three subjects each in the study and control groups were matched for age, sex, ethnic group and height. Prevalence of chronic respiratory symptoms was obtained by questionnaire. Spirometric measurements included forced vital capacity (FVC), forced expiratory volume in the first second (FEV1.0) and forced mid-expiratory flow rate (FEF 25-75%). The study group had a chest radiograph. The odds ratio for any chronic respiratory symptom was 11.6 (95% confidence interval [CI] = 3.7-39.4) in the study group. Mean values for the spirometric tests were lower in the study group; the differences in FEV 1.0 and FEF 25-75% were significant. Tuberculosis was not found in the study group, while one subject (2.4%) had radiological evidence of bronchiectasis. It may therefore be concluded that chronic tea dust exposure causes increased prevalence of respiratory symptoms and a significant degree of small airways obstruction.

Summary of illnesses and injuries reported in California by physicians in 1986 as potentially related to pesticides.

As a result of legislation in the early 1970's, physicians are required to report all cases of illness or injury which may have been a result of exposure to pesticides. The California Department of Food and Agriculture receives these reports through a variety of reporting mechanisms and compiles them into an annual data base. In 1986, 2099 illness/injury reports were received by the Worker Health and Safety Branch of the California Department of Food and Agriculture. After investigation by the County Agricultural Commissioners' staff, 1065 (51%) were determined to be confirmed cases of occupational illness/injury related to pesticide exposure. In addition, there were also 146 (7%) cases of non-occupational pesticide-related illness/injury, 424 (20%) cases determined to be unrelated to pesticide exposure, and 464 (22%) cases for which there was insufficient information to determine a cause and effect relationship. The number of confirmed cases of pesticide-related illness/injury decreased by nearly 30% from the corresponding 1985 figures. This apparent decrease is an artifact of the classification scheme employed for 1986. The change is primarily a result of a reduction in the number of field worker cases determined to be confirmed pesticide-related exposures. In previous years most investigations of pesticide application history for grape vineyard worker dermatitis cases were confined to a history from the last field worked prior to visiting a physician. This method assumes no latency period between exposure and onset of symptoms and/or that the worker immediately visited a physician at the onset of symptoms.(ABSTRACT TRUNCATED AT 250 WORDS)

Airborne rice pollen and pollen allergen in an agricultural field: aerobiological and immunochemical evidence.

Grass pollens are well known among the health hazardous bioaerosols causing respiratory allergy. Being an important member of the grass family, the rice plants contribute a huge pollen load in agricultural fields during flowering. This results in a seasonal trigger of hay fever and respiratory allergy in the field workers and people living in the vicinity. Studies on the monitoring of airborne rice pollen and the intensity of the released allergen in agricultural fields are largely lacking. The aims of the present study were: (1) daily and hourly monitoring of airborne rice pollen in an agricultural field during the flowering period of plants in a winter crop season by using the Burkard 7-Day Volumetric Sampler and (2) the measurement of hourly airborne allergen intensity in the field in a peak rice pollen period by the double-antibody and chemiluminescence techniques to find out its relationship with the airborne rice pollen concentration. The monthly average concentration of rice pollen was 95 pollen m(-3) and the range of daily average pollen concentration was 0 to 386 pollen m(-3). A bimodal diurnal periodicity showed that the airborne rice pollen concentrations remained high at two different times of the day: between 08:00 h to 12:00 h and 14:00 h to 16:00 h. Deposition of airborne rice pollen allergen showed darker intensities on the immunostained tapes from the Burkard Sampler at the same two positions corresponding to higher pollen counts. These observations provided direct evidence of the allergenicity of airborne rice pollen in field conditions.

Dermatitis in Ecuadorean farm workers.

Using a cross-sectional survey of potato farm workers in northern Ecuador, we examined the relationship between pesticide exposure and skin disorders. From a farm population census, all pesticide applicators and 1/2 of exposed field workers were selected. Controls were age-matched from urban occupations. Individual exposure measures included overall years and current hours working with pesticides. Farm-level measures included numbers of applications over the last 6 months and an application practices score (range 0-4). Each participant underwent a clinical skin examination, with patch tests to maneb on a sample of dermatitis cases. Exposure-related increases in conjunctivitis (7% of applicators, 0% in other groups), dermatitis (68% of exposed and 55% of applicators versus 31% of controls, p < 0.001) and pigmentation disorders (25% of exposed and applicators vs 10% controls, p = 0.06) were found. Among dermatitis cases (n = 117), 5% were positive to maneb on patch testing. In logistic regression analysis, significant predictors (p < 0.1) of dermatitis included years using fungicides (OR = 1.12 per year) and poor application practices (OR = 1.42 per score unit). Agricultural fungicide application in high-exposure situations can contribute to dermatitis prevalence among farm populations.

[Risk factors related with occupational and environmental exposure to organochlorine insecticides in the state of Bahia, Brazil, 1985].

In the cacao-growing region in the southern part of the state of Bahia, the organochlorine insecticides, mainly gamma-benzene hexachloride (BHC) and dichlorodiphenyltrichloroethane (DDT), have been used for about 40 years on cacao crops and in public health programs for control of the insect vectors of different diseases, especially malaria. This paper presents the results of tests performed on 127 persons, all males, between the ages of 15 and 52 years, divided into eight groups as follows: three groups consisted of persons occupationally exposed to 1.5% BHC, that is, technical hexachlorocyclohexane (HCH); two groups consisted of individuals who had had occasional contact with the products or worked in areas near those in which they were used; two groups were appliers of DDT, and the last group--the control group--consisted of 50 individuals who had had no history of occupational exposure to insecticides. All the participants underwent testing to determine the parameters of biochemistry, hematology, and organochlorine insecticide residues in the blood. It was found that improper handling of the products and failure to use individual protective equipment, together with longer time of exposure, significantly increased the rates of GOT and GPT in the appliers of DDT and technical HCH, and in the latter the rates of alkaline phosphatase, albumin, and cholesterol were also found to be higher. In view of the high morbidity among pesticide appliers in agriculture and public health campaigns, it is important to institute programs to teach these workers to avoid contamination of their persons and of the environment by developing good hygiene habits, using individual protective equipment, and correctly handling the products. Rural workers and public health authorities must become aware of the importance of protective equipment, periodic health examinations, and reduced environmental pollution in order to lessen occupational risks of field workers and promote improved conditions of life for the rural population at large.

THE ASSESSMENT OF INSECTICIDAL IMPACT ON THE MALARIA MOSQUITO'S VECTORIAL CAPACITY, FROM DATA ON THE PROPORTION OF PAROUS FEMALES.

In malaria eradication the residual insecticide exerts upon the mosquito's vectorial capacity a direct insecticidal impact, the order of which may be measured by observing the decrease in the proportion of parous females. The impact is expressed as the product of the degree of reduction of the expectation of infective life (termed the longevity factor of impact) and that of the expectation of life (the density factor). To compute the factors from the proportion parous it is necessary to know also the mean difference in age between the nulliparous and the youngest parous females in the sample, and the sporogonic period of the parasite.Graphs are presented to enable the field worker, who has observed these parameters, to read off from his data the proportion surviving one day, the expectation of infective life and the expectation of life. Examples from the field are used to illustrate the manner of computing the direct insecticidal impact with the aid of the graphs. It is emphasized that this method can only measure the relative impact on vectorial capacity, and will not show whether the actual level of vectorial capacity is such that a malaria reproduction rate of <1 is indicated.

Epidemiological aspects of snake bites on a Liberian rubber plantation.

During a one-year period 95 patients with a history of snake bite were admitted to the hospital of a Liberian rubber plantation. The population at risk included the field workers (tappers and slashers) with an incidence of 4.2 symptomatic snake bites per thousand per year. The incidence of symptomatic bites was 1.7 per thousand in the group of non-field employees and 0.4 per thousand per year in the group of non-employees. The temporary disability was between 3 and 5 days, and the loss of workings days due to snake bites was one day per 10,000 working days on the plantation. Among the 95 patients 27 did not show any symptoms of envenoming except occasional fang marks. 64 patients developed cytotoxic symptoms alone. In this group, the night adder (Causus maculatus) was the main responsible snake. 4 patients showed signs of systemic envenoming. Two were haematological and two were neurological in nature and caused by Bitis species and Naja species, respectively. No fatalities were noted. A definite maximum of snake bites was observed during October and November which corresponds to the transition from rainy to dry season.

Integrating a theoretical framework with street outreach services: issues for successful training.

HIV/AIDS prevention projects utilizing indigenous outreach workers often rely on the life experiences and skills of the staff to structure the intervention, without grounding in theory. However, to be most effective, community outreach projects which target harder-to-reach high-risk populations should both utilize and enhance the natural strengths of indigenous field workers' experience and style of interaction, while guiding intervention content with theoretical rigor. In this paper we demonstrate that the challenge of successfully integrating a theoretically guided program design with field staff's credibility with, and sensitivity toward, drug-using clients can be practically and satisfactorily met through appropriate training. This training is an important investment for better utilizing valued and scarce prevention resources. The Philadelphia site of the AIDS Evaluation of Street Outreach Project (AESOP), a cooperative agreement of the Centers for Disease Control and Prevention, designed and implemented staff trainings to reflect the enhancement of the outreach program by the Stages of Change model. Through these trainings, the outreach workers have learned to integrate their natural street and intervention skills into the structure of a theoretical framework. This paper presents specific training components, relevant issues within these components, and areas for evaluation and feedback.

The persistence of ethion and zolone residues on grape folliage in the central valley of California.

The hazard of serious intoxication of agricultural field workers by organophosphate insecticide residues has led to the establishment of reentry intervals in California. In the attempt to extend this concept nationally, it has been found that there is a lack of relevant field data for setting of such standards. In an effort to expand knowledge of the relationship between foliar residue levels and occupational hazard, an examination of the persistence and transport of ethion, Zolone, and their oxygen analogs on grape foliage was conducted. Both dislodgeable and penetrated residues were followed for 28 days post-application for both insecticides while soil surface residues were followed for ethion only. A marked difference is seen in the decay rates of the dislodgeable vs. the penetrated residues of ethion, the dislodgeable residues decaying more quickly. This difference is not apparent for Zolone. Of particular significance to worker hazard is the finding that the oxones of both ethion and Zolone in the dislodgeable residues reach a plateau after approximately seven days and do not degrade further by day 28.

Validation of verbal autopsy to determine the cause of 137 neonatal deaths in Karachi, Pakistan.

Verbal autopsy (VA) aims to estimate a community's mortality experience in the absence of contact with formal registration or health care systems. Application of VA to neonatal deaths is problematic as the agonal phase of a neonatal death tends to be indistinct. This is the first attempt to validate the technique exclusively on newborns who died. Seriously ill neonates (n = 137) were enrolled from the Civil Hospital, Karachi, Pakistan, between 31 October 1993 and 31 July 1994. All died as newborns, and caregivers were interviewed at home 3-230 days later. Surveillance physicians completed case questionnaires in the hospital, and investigator physicians assigned the main and associated causes of death using clinical criteria. Field questionnaires including a verbatim open-ended history, and syndrome modules were completed by a field worker, and investigator physicians again assigned the main and associated causes of death based on three diagnostic methods: verbatim alone, modules alone and verbatim and modules combined. We assessed the validity of VA by comparing field against hospital diagnoses by diagnostic (verbatim vs. modules vs. both) and analytic method (main vs. any diagnosis). VA identified at least one diagnosis accurately in 71% of the newborns. VA underdiagnosed low birthweight and prematurity in the field. Verbatim and modules diagnostic method comparing any field against main hospital diagnoses revealed high sensitivities for too early/too small syndrome (90%) and neonatal tetanus (84%). VA correctly identified some important causes of neonatal death in the field. Assigning multiple diagnoses using both open- and closed-ended questions increases the likelihood of correct ascertainment.

Weighing Vietnamese children: how accurate are child weights adjusted for estimates of clothing weight?

Children who are weighed for growth monitoring are frequently clothed, especially in the cold weather. Health workers commonly estimate and subtract the weight of these clothes, but the accuracy of these estimates is unknown. We assessed the accuracy of child weights adjusted for estimated clothing typical of hot, cold, and extremely cold ambient temperatures. Trained field workers weighed a sample of 212 children 6 to 42 months old from the ViSION project, adjusted the weights using a job aid describing the weights of common clothing by season and age, and then weighed the clothing to calculate the actual clothing and child weights. Fieldworker estimates of the weight of the clothing that children wore during weighing were remarkably good. In nearly all cases (207 of 212; 97.7%), the difference between the estimated and actual clothing weight was less than the precision of the child scales (+/- 50 g), and most (181 of 212; 84.5%) were within 25 g. Thus, the calculated child weights were, in fact, equivalent to the actual child weights. Using simulations, we found that improperly accounting for clothing weight can overestimate weight-for-age by 0.1 to 0.4 Z score. Accurate weights are possible, even under adverse conditions. Our training methods, clothing album, and job aid might benefit nutrition research and programming in Viet Nam as well as settings with colder climates.

Radiological assessment of petroleum pipe scale from pipe-rattling operations.

Petroleum pipe scale, consisting of concentrated inorganic solids such as barium sulfate, can deposit on the inside of down-hole pipes during the normal course of oil field pumping operations. A portion of this scale has been shown to contain naturally occurring radioactive materials (NORM), predominantly compounds of radium. When these pipes are removed from the well, there is a potential for radiation doses to the oil field workers handling the pipes, especially as the pipes are cleaned for reuse. A thorough sampling and measurement protocol was applied under a variety of weather conditions in an outdoor laboratory to obtain an accurate indication of the radiological and aerodynamic characteristics of scale release and dust dispersion during petroleum pipe scale removal from out-of-service pipes with a restored, historically relevant outdoor pipe-cleaning machine. Exposure rate data were also obtained for both the pre-cleaned pipes, and the general area inhabited by workers during the descaling operation. Four radiation exposure pathways were investigated: inhalation of pipe scale dust generated during pipe rattling, incidental ingestion of the pipe scale dust, external exposure from uncleaned pipes, and external exposure from pipe scale dispersed on the ground. Pipes from three oil fields were rattled to collect as much industry-representative data as possible. The Ra specific activity of the pipe scale ranged from 33.6 +/- 0.4 to 65.5 +/- 0.7 Bq g, depending on the formation. A median atmospheric dust loading of 0.13 mg m was measured in the operator breathing zone. The respirable fraction was observed to be about 42% to 46%. Based on cleaning 20 pipes per day,250 d per year on average, annual committed effective doses for the operator and helper ranged from 0.11 mSv (11 mrem) to 0.45 mSv(45 mrem) for inhalation and from 19 microSv (1.9 mrem) to 97 microSv (9.7 mrem) for incidental ingestion. Worker annual external dose from the pipe racks ranged from 0 to 0.28 mSv (28 mrem). In the deposition experiment, more than 99% by weight of the deposited scale fell within 2 m of the machine centerline, the vast majority of which was in the downwind direction. The dose from this deposited material dominated the worker dose estimates. The annual external dose from dispersed material was estimated to be 2.8 mSv (280 mrem) for the operator and 4.1 mSv (410 mrem) for the helper.

Acute respiratory infections: a longitudinal study of 151 children in Burkina-Faso.

A longitudinal study of 151 children under five years of age was performed in a rural village of Burkina-Faso (West Africa). During systematic examinations by a physician during the rainy season, 44% of the children were found to be ill and 59% of these had an acute respiratory infection (ARI). During the dry season, the rates were 48% and 73% respectively. Weekly interviews by a field worker showed 6.2 attacks of ARI during the six months of the rainy season and 7.0 during the six months of the dry season. Risk factors for ARI were malnutrition assessed by arm circumference, and a high birth rank. At the 'field-dispensary', ARI accounted for 41% of the visits; lower respiratory infections (LRI) accounted for 24%, similar to the proportion of LRI among illnesses found during the systematic examination.

[The use of data from the literature on nutritional education projects: a qualitative study].

The study is based on the observation that data from the existing literature is underutilised during the project development stage. The objective is to carry out a study to track the current status quo on how the knowledge from the literature is used and incorporated. Semi-structured interviews of professionals working in nutritional education of youth were conducted to assess this aspect in the French-speaking community in Belgium. The study permitted the identification of which types of data are being used by the respondents, as well as which elements can have a positive or negative impact their incorporation. Moreover, it was observed that most of the field workers who participated in the study would prefer to have access to more data relevant to their local context than data on a specific topic. The study highlighted the need for more training to be provided to the nutritional educators working in the field, in particular in the areas of information needs and their interpretation of these needs in light of the data available.

[Evaluation of a questionnaire on the home environment of asthmatic patients].

Indoor environment outpatient asthmatics plays a major role in modulating asthma severity. Therefore many surveys devoted to studying risk factors for asthma include a questionnaire about housing characteristics. However no such questionnaire has been to date validated. In this study, we tried to validate such a questionnaire by comparing answers of a group of asthmatics outpatient to a questionnaire to de visu finding performed by a field worker during a home visit. Thirty asthmatic patients were included in the study. Housing characteristics were generally correctly acknowledged. Questions on the heating system should be clearly and simply formulated. Energetic supply of stoves and ventilation system is not always known. In the field of house equipment, especially bedding, photographs or drawings can be useful.
