[Age factors in natural mechanisms of detoxication and curative effect of enterosgel in burns].

In the experiment, the age peculiarities of the detoxication system of the liver in burn disease and the effect of enterosgel on them were studied. Decrease in hydroxylase and demethylase activity of hepatic microsomes in relatively stable activity of oxidoreductase as well as increase in activity of aminotransferases, especially that of alanine aminotransferase, which was more pronounced in aged animals, was noted. Under the influence of enterosorption, the increase in functional activity of monooxygenase system of the liver and stabilization of hepatocytic membranes are more pronounced in young animals. This contributed to reduction in lethality, activation of natural mechanisms of detoxication and reparative processes.

[The role of bile acids in the regulation of human gallbladder filling].

An artificial increase of a pool of bile acids due to administration of dehydrocholic, chenodesoxycholic acid preparations and dry bile in 22 healthy people and 179 patients with chronic cholecystitis has been studied for its effect on regulation of the gallbladder filling on an empty stomach using daily echocholecystometry. Filling of the gallbladder under artificial sequestration of bile acids during duodenal probing, enterosorption and cholatogenic diarrhea was studied in 55 patients with cholecystitis. It has been proved that an increase of the pool of bile acids induces intensification of the gallbladder filling on an empty stomach, while a decrease of the pool, vice versa, causes attenuation of its filling. It is found out a size of a pool of bile acids in the human organism is an important humoral factor in the physiological mechanism of the gallbladder filling regulation.

[Treatment of generalized purulent appendicular peritonitis in children].

Experience in the treatment of 137 children with a most severe and complicated course of generalized purulent peritonitis of appendicular genesis is generalized. It is shown that such patients must be concentrated in centers of pediatric surgery providing the possibility of applying a complex of diagnostic examinations and therapeutic measures, including purposeful antibiotic therapy, differentiated immunocorrection, and effective detoxification. The detoxification and immunocorrection effect of discrete plasmapheresis with two-stage "washing out" of the red cells is illustrated in the discussion of 22 cases. It is pointed out that in operations for generalized purulent peritonitis the abdominal cavity must not only be cleansed but also drained adequately and intubation of the intestine must be carried out with aspiration of its content for 3-5 days and application of enterosorption through an intestinal tube. A complex of organizational and therapeutic measures made it possible to reduce considerably the mortality rate in acute appendicitis and its complications in the Moscow Region.

[Prolonged decompression of the small intestine in the surgical treatment of acute intestinal obstruction and peritonitis].

Prolonged decompression of the small intestine was used in the complex surgical treatment of 85 patients with acute ileus (67) and peritonitis (18). The main stage of the operation included liquidation of ileus and elimination of a source of peritonitis. Antegrade nasogastric intubation of the intestine was performed in 54 patients, via gastrostomy--in 3, retrograde intubation via cecostomy--in 15, via appendicostomy--in 4, via ileostomy--in 9. Performance of active transtubal sanation of the intestine by means of antiseptic solutions (53 cases) in combination with enterosorption (17) contributed to essential increase in the effectiveness of treatment.

[A program of combined intensive therapy in critical ischemia of the lower extremities].

Ultraviolet irradiation (UVI) of the blood, adsorption methods of detoxication, hyperbaric oxygenation (HBO), and antioxidant therapy are pathogenetic and etiotropic methods of treatment in multiple-modality conservative therapy of patients with ischemia of the lower limbs. The EKROMI UVI method is rapid and no less effective then the traditional exposure of the blood after Knott. Enterosorption (ES) may replace hemosorption (HS) in lower limb ischemia with good effect; noteworthy that it is a safer method of detoxication. Addition of antihypoxants and antioxidants (AO) to therapeutic complexes administered before HBO improved the efficacy of this procedure. Use of the UVI-HS(ES)-AO-HBO-HS(ES)-UVI scheme for intensive care for obliterating diseases of the arteries levels the shortcomings of each of the methods of the complex and potentiates the efficacy of each of them.

[The validation and efficacy of correcting gastrointestinal disorders in patients with chronic glomerulonephritis who took part in the cleanup of the aftermath of the accident at the Chernobyl Atomic Electric Power Station].

The use is substantiated and efficiency is studied of novel treatment methods to deal with gastroenterologic disorders in 102 patients with chronic glomerulonephritis who took part in the elimination of the aftermath of the Chernobyl accident. Positive clinical and endoscopic effects appeared to be higher in those groups who underwent the informative-wave therapy (IWT) in their multiple-modality treatment versus the group on drug therapy only. IWT in combination with enterosorption were found out to be associated with marked detoxicating and sanogenous effects as well as with improvement of the functional state of the alimentary canal and kidneys.

[Clinical picture, course and therapeutic efficiency of pulmonary tuberculosis in patients with organochlorine pesticides and hepatitis B markers].

A total of 356 patients with active forms of pulmonary tuberculosis were examined. Toxicological and radioimmunological (RIA) studies revealed serum organochlorine pesticides (OCP) and virus hepatitis B markers (VBHM) in 225 (63.2%) and 219 (61.6%), respectively. Pulmonary tuberculosis was demonstrated to run worse in patients with OCP and VBHM who tolerated antibiotics worse, showed a high incidence of side effects, lower efficiency of treatment. Pathogenetic benzonal therapy with enterosorption in tuberculosis enhances therapeutical efficiency by 20-25% and reduces the incidence of side effects by 1.5-2 times.

Synthesis of a polymer skeleton at the inner leaflet of liposomal membranes: polymerization of membrane-adsorbed pH-sensitive monomers.

We describe the synthesis of liposomes with an artificial membrane skeleton as a model of the native cellular cytoskeleton. Similar to natural conditions, a flat polymer network is coupled to the inner membrane leaflet like a suspended ceiling via membrane-inserted anchor monomers with a spacer. The polymer is composed of DMAPMA (N-(3-N,N-dimethylaminopropyl) methacrylamide) and TEGDM (tetraethylene glycol dimethacrylate) as a linker and is coupled to the membrane anchor DOGM (1,2-distearyl-3-octaethylene glycol glycerol ether methacrylate). In the first step of the synthesis, DMAPMA and TEGDM are encapsulated into liposomes composed of egg phosphatidylcholine (EPC), and free monomers are removed by gel chromatography. At pH 10, DMAPMA adsorbs to the inner membrane surface, as demonstrated in parallel studies with lipid monolayers using a Langmuir film balance. The polymerization by UV irradiation was initiated with DEAP (2,2-diethoxyacetophenone) as the initiator and was shown to be complete after 15 min. At pH 6, polymer was desorbed from the inner membrane surface to form a lamellar structure similar to that of the cellular cytoskeleton, as shown by electron microscopy. In comparison to NIPAM (N-isopropylacrylamide), which was used as a monomer in a recent study (Stauch, O.; Uhlmann, T.; Frohlich, M.; Thomann, R.; El-Badry, M.; Kim, Y.-K.; Schubert, R. Biomacromolecules 2002, 3, 324-32), DMAPMA shows much slower membrane permeation leading to an essential restriction of the formed polymer to the liposomal interior. The DMAPMA-based composite structure stabilizes the lipid membrane against sodium cholate by a factor of 2.5 as compared to plain EPC liposomes. This is discussed in the context of the situation in the liver, where the cytoskeleton probably plays a crucial role in the stabilization of the membrane against high bile salt concentration.

Gastrointestinal decontamination for enteric-coated aspirin overdose: what to do depends on who you ask.

CONTEXT: Overdoses with enteric-coated preparation are common. The optimal means by which to limit drug absorption in such cases is controversial. OBJECTIVE: To describe the recommendations for gastrointestinal decontamination issued by North American poison control centers for a hypothetical patient, (an adult male with normal vital signs), presenting 1 hour after ingesting 500 mg/kg of enteric-coated aspirin. DESIGN: Telephone survey of 76 poison control centers in North America. Seven toxicologists who contributed to the American Academy of Clinical Toxicology/European Association of Poison Centres and Clinical Toxicologists position statements on gastrointestinal decontamination were also surveyed for informal comparison. RESULTS: Most poison control centers (99%) and all of the toxicologists (100%) participated in the survey. Four centers (5 %) recommended syrup of ipecac and 38 (51%) recommended gastric lavage, compared with 0% and 0% of toxicologists, respectively. Seventy-three centers (97%) recommended at least one dose of activated charcoal, compared with 6 toxicologists (86%). Twenty-one poison centers (28%) recommended whole-bowel irrigation, compared with 3 toxicologists (43%). A total of 36 different courses of action were suggested by respondents at the poison centers. Some of these recommendations were potentially harmful. CONCLUSIONS: Considerable variability exists in the recommendations of North American poison control centers for the gastrointestinal decontamination of patients with large, acute overdoses of enteric-coated aspirin.

Reducing the 137Cs-load in the organism of "Chernobyl" children with apple-pectin.

As a complement of standard radioprotective measures, apple-pectin preparations are given, especially in the Ukraine, to reduce the 137Cs uptake in the organism of children. The question has been raised: is oral pectin also useful when children receive radiologically clean food, or does this polysaccharide only act in binding 137Cs in the gut, blocking its intestinal absorption? In this case, pectin would be useless if radiologically clean food could be given. The study was a randomised, double blind placebo-controlled trial comparing the efficacy of a dry and milled apple-extract containing 15-16% pectin with a similar placebo-powder, in 64 children originating from the same group of contaminated villages of the Gomel oblast. The average 137Cs load was of about 30 Bq/kg bodyweight (BW). The trial was conducted during the simultaneous one-month stay in the sanatorium Silver Spring. In this clean radiological environment only radiologically "clean" food is given to the children. The average reduction of the 137Cs levels in children receiving oral pectin powder was 62.6%, the reduction with "clean" food and placebo was 13.9%, the difference being statistically significant (p <0.01). The reduction of the 137Cs load is medically relevant, as no child in the placebo group reached values below 20 Bq/kg BW (which is considered by Bandazhevsky as potentially associated with specific pathological tissue damages), with an average value of 25.8 +/- 0.8 Bq/kg. The highest value in the apple-pectin group was 15.4 Bq/kg, the average value being 11.3 +/- 0.6 Bq/kg BW.

[The possibilities for preventive diagnosis and nondrug treatment in drug allergy].

The paper is dedicated to the matter of topical interest--that of diagnosing allergic reactions during the preclinical stage and of potentialities of clearing away adverse reactions to drugs without employment of medicinal agents. A characteristic feature of the paper is an original approach based on the authors' method. Reflected in the table are observations of the authors in respect of six entities versus analogous diagnosis without allergy and essentially healthy subjects--blood donors. The development of allergy is accompanied by a striking augmentation of CIC, which fact is to be considered in routine medical practice as a diagnostic criterion to be used together with the tests suggested by the authors.

[The efferent substitution correction of blood trace element disorder in patients with chronic kidney failure].

Reasons are given for expediency of correction of dystraceelementemia in chronic renal insufficiency. Following exposure to the treatment with the multivitaminotraceelement drug three-we PLUS and alpha-erythropoietin eprex patients with terminal renal failure prolonged by chronic hemodialysis (n = 16) demonstrated a decrease in blood concentration of toxic trace elements and in dysbalance of essential trace elements, alleviation of clinical symptoms, increased tolerance to physical loads, improvement of hematological indices and quality of life.

[The use of enterosorbents for preventing and stopping chronic toxic stress in submariners].

The efficiency of prophylactic intestinal sorbents use (algisorb and activated carbon) to prevent chronic toxic stress was investigated in 20 healthy volunteers (atomic submarine personnel) basing on evaluation of central nervous system functional state, hemodynamics, hepatic detoxicative ability and dynamics of trace elements in urine. It was revealed that intestinal sorbents administration in daily dose of 5 g during 20 days promotes gastrointestinal elimination of toxic compounds (cadmium, bismuth, chrome, molybdenum, lithium). It is accompanied with decrease in organism intoxication evidences: reduction in central nervous system excessive excitation, decrease in functional tension of cardiovascular system, recovery of hepatic detoxicative ability. Algisorb has an advantage over activated carbon that allows to recommend this drug in prophylaxis and arresting of chronic toxic stress in submariners both during interfield period and autonomic sailing.
