NASA/DARPA advanced communications technology satellite project for evaluation of telemedicine outreach using next-generation communications satellite technology: Mayo Foundation participation.

OBJECTIVE: To describe the development of telemedicine capabilities-application of remote consultation and diagnostic techniques-and to evaluate the feasibility and practicality of such clinical outreach to rural and underserved communities with limited telecommunications infrastructures. MATERIAL AND METHODS: In 1992, Mayo Foundation (Rochester, Minn, Jacksonville, Fla, and Scottsdale, Ariz), the National Aeronautics and Space Administration, and the Defense Advanced Research Projects Agency collaborated to create a complex network of fiberoptic landlines, video recording systems, satellite terminals, and specially developed data translators linking Mayo sites with other locations in the continental United States on an on-demand basis. The purpose was to transmit data via the asynchronous transfer mode (ATM) digital communications protocol over the Advanced Communications Technology Satellite. The links were intended to provide a conduit for transmission of data for patient-specific consultations between physicians, evaluation of medical imagery, and medical education for clinical staffs at remote sites. RESULTS: Low-data-rate (LDR) experiments went live late in 1993. Mayo Clinic Rochester successfully provided medical consultation and services to 2 small regional medical facilities. High-data-rate (HDR) experiments included studies of remote digital echocardiography, store-and-forward telemedicine, cardiac catheterization, and teleconsultation for congenital heart disease. These studies combined landline data transmission with use of the satellite. The complexity of the routing paths and network components, immaturity of available software, and inexperience with existing telecommunications caused significant study delays. CONCLUSIONS: These experiments demonstrated that next-generation satellite technology can provide batch and real-time imagery for telemedicine. The first-generation of the ATM and satellite network technology used in these experiments created several technical problems and inconveniences that should be overcome as the network infrastructure matures.

Improved communications and hearing protection in helmet systems: the communications earplug.

Despite significant advances in hearing protection and compliance with protective standards, military personnel are still subject to noise-induced hearing loss in many combat and combat support operations. Although the Army has experienced a decrease of some 15% in primary hearing loss disability cases since 1986, a fiscal year 2000 report documents a 27.5% increase in audiograms, which demonstrated significant threshold shifts in assessed personnel (N = 841/1,077). Compensation for noise-induced hearing loss disability for the Army alone exceeded $180 million in 1998. Thus, communications and hearing protection remain critical issues for personnel involved in Army operations. Aircraft, ground vehicles, and weapons produce noise levels in excess of the limits defined in current hearing conservation standards. Performance of most helmets, improved over the years, remains marginal with regard to speech intelligibility. Furthermore, these helmets do not provide adequate hearing protection. The communications earplug, which consists of a high-quality earphone coupled with an earplug protector, provides the needed extra protection. It weighs less than 15 g and is comfortable when worn over extended periods. It is considered highly acceptable by seasoned Army aviators and crewmembers.

[Communication for patients with severe motor handicaps--clinical experience with a new electronic communication aid].

The use of "Komhilf", a newly developed communication aid for persons with severe physical handicap, is described on the example of two patients with amyotrophic lateral sclerosis. Aside individual letter input, it provides input of complete phrases signifying basic needs or common request, without a need for letter-by-letter spelling. After a short familiarization period, both patients were able to communicate up to two to three hours daily via "Komhilf". Psychic stabilization and increased levels of activity were observed in the patients, which subsequently also entailed increased compliance with other therapeutic measures. Additional features include good transportability, use in lying and sitting (wheelchair) patients alike, easy-to-learn operation, and, not least, a competitive price.

Communications: Should mechanisms be established for sharing among clinical trial investigators experiences in handling problems in design, execution, and analysis? Methodology: the case for improved communications.

Despite their long history, the number that are in operation now, and the current level of expenditures on them, clinical trials have not been the focus of sufficient methodologic research. The paucity of metholologic explorations is further aggravated by the constraints on communications regarding methodology. Those individuals working in the area of clinical trials need to establish some professional identity with the field. Methodology in the areas of design and management of clinical trials needs to be stimulated, perhaps through special requests for applications (RFAs) and requests for proposals (RFPs). Communications could be improved by the establishments of a professional society, the development of an appropriate journal, and establishment of national training and information centers for clinical trials.

[Communication in "functional" and "dysfunctional" families with adolescent children. I. Verbal communication].

This research studies family communications during the critical period of change provoked by the puberty of one of the children. Two groups of families are compared: one group of 15 "functional" families (no psychological consultation has been requested) and one group of 15 "dysfunctional" families (a psychological consultation has been requested for one of the adolescents). The micro-analysis of the verbal communications reveals a few significant differences between the two groups of families. The results point to structural differences between the family systems.

Communication of patients' worries in a psychiatric unit in Nigeria: a comparative study of patient-patient and patient-nurse communication.

The main question examined by this study is: do psychiatric patients communicate their worries more accurately to nurses than to other patients? The study also looked into the relationship between sex and diagnosis of patients' illness on the one hand, and the number of worries reported on the other. Subjects interviewed were in-patients who had stabilised in the ward. The patients, as well as nurses and colleague patients they nominated, used the "Hospital Patients Worry Inventory" to describe the things patients worry about while in hospital. The results showed that in terms of overall accuracy, other patients were more accurate than nurses in reporting a patient's worries. Males and patients diagnosed as schizophrenic and manic reported a higher number of worries than females and patients diagnosed as depressed. Nurses in this study under-estimated the number of worries experienced by patients. The findings were discussed in the light of the importance of informal medium of communication in the assessment and treatment of psychiatric patients.

Assessing communication in children with psychiatric and communication disorders.

This article presents issues related to the communication assessment of children with psychiatric and communication disorders. Challenges inherent in assessing this population are discussed. Frequently used assessment instruments and procedures are described. Consideration is given to the assessment of very young or low-functioning children and older or high-functioning children.

Communicative intervention for children with psychiatric and communication disorders.

Many children and adolescents with psychiatric disorders exhibit severely compromised language and social communication systems. The nature and scope of their deficits make a multidisciplinary team approach essential to management in which improved functional and socially appropriate communication is a primary goal. For this reason SLPs generally play a key role in the intervention process and share responsibilities for the planning, coordination, and implementation of instructional-educational goals.

Parlaying digital imaging and communications in medicine and open architecture to our advantage: the new Department of Defense picture archiving and communications system.

The Department of Defense (DoD) undertook a major systems specification, acquisition, and implementation project of multivendor picture archiving and communications system (PACS) and teleradiology systems during 1997 with deployment of the first systems in 1998. These systems differ from their DoD predecessor system in being multivendor in origin, specifying adherence to the developing Digital Imaging and Communications in Medicine (DICOM) 3.0 standard and all of its service classes, emphasizing open architecture, using personal computer (PC) and web-based image viewing access, having radiologic telepresence over large geographic areas as a primary focus of implementation, and requiring bidirectional interfacing with the DoD hospital information system (HIS). The benefits and advantages to the military health-care system accrue through the enabling of a seamless implementation of a virtual radiology operational environment throughout this vast healthcare organization providing efficient general and subspecialty radiologic interpretive and consultative services for our medical beneficiaries to any healthcare provider, anywhere and at any time of the night or day.

Internal carotid-posterior communicating artery aneurysm associated with unusually short posterior communicating artery.

We report two cases of a ruptured internal carotid-posterior communicating artery aneurysm in which the ipsilateral posterior communicating artery is short and the P1 and P2 segments of the posterior cerebral artery are long. Unique angiographic and operative findings are described. Vertebral angiography with carotid compression is recommended to diagnose this rare variation. Recognition of this variation is important to avoid an inadvertent occlusion of arteries behind the aneurysm at the time of clipping. Copyright 1999 Harcourt Publishers Ltd.

High scores on the Western Aphasia Battery correlate with good functional communication skills (as measured with the Communicative Effectiveness Index) in aphasic stroke patients.

OBJECTIVE: To examine the correlation between an impairment-level and a functional-level assessment scale of aphasia. DESIGN: Prospective, longitudinal study. SETTING: A stroke rehabilitation unit. SUBJECTS: Sixty-seven aphasic acute stroke patients who were undergoing a multi-disciplinary rehabilitation programme, including conventional speech and language therapy (SLT). INTERVENTION: Patients were assessed on study entry and 4, 8, 12 and 24 weeks after the start of SLT. The language impairment was assessed with the Western Aphasia Battery (WAB) and the communicative functional limitation associated with aphasia was measured with the Communicative Effectiveness Index (CETI).Results. There was a statistically significant correlation between the two scales for all assessment periods (Pearson's r=0.71; P<0.01). CONCLUSION: The study suggests that in the acute and subacute stages of stroke the scores of WAB and CETI can be surmised from one another.

Sequential changes of gap-junctional intercellular communications during multistage rat liver carcinogenesis: direct measurement of communication in vivo.

We have developed a simple method to measure gap-junctional intercellular communication (GJIC), by means of microinjection/dye transfer assay, in liver slices freshly removed from the rat. Using this method and immunostaining of connexin 32 (cx32), the major liver gap junction protein, we studied sequential changes of GJIC during chemical hepatocarcinogenesis in male Fischer-344 rats under a modified Solt-Farber protocol (3 weeks 4 day exposure regimen). Four weeks after commencement of the protocol, there was a substantial decrease in GJIC in the liver parenchyma, which was free from focal lesions. The decrease in GJIC persisted up to at least the 15th week of treatment, while a decrease in the number of immunoreactive cx32 spots was evident only at 4 weeks of post-protocol commencement. Most enzyme-altered (GST-P-positive) focal lesions showed markedly lower GJIC and a significantly lower number of cx32-positive spots than surrounding hepatocytes. Most GST-P-positive foci showed a selective lack of GJIC with surrounding heptocytes. Hepatocellular carcinomas arising 1 year after the carcinogenic regimen had significantly reduced communicational capacity accompanied by a large decrease in cx32 expression. These results suggest that a progressive decrease in homologous as well as heterologous GJIC in preneoplastic lesions occurs during rat hepatocarcinogenesis, and that preneoplastic lesions with the most prominent disorders in GJIC may be more likely to develop into carcinomas.

Picture archiving and communication system--part 2 cost-benefit considerations for picture archiving and communication system.

Picture archiving and communication system (PACS) has become an important component of many radiology departments. PACS is expected to make the departments more efficient, reduce operating costs, and improve the communication between the radiologist and the referring physician. Even if the cost-savings are not substantial because of the capital expense of PACS, cost justification is very important in demonstrating the substantial advantages of PACS at either little or no additional expense. Some cost-benefit considerations for PACS are discussed in relation to a hospital-wide implementation.

Effects of information-giving and communication during orthodontic consultation and treatment. Part 3: Optimized orthodontist-patient communication.

Effective, orthodontist-patient communication must meet certain minimum requirements whose importance might vary, depending on the individual case and stage of treatment. In the course of treatment the following tasks are of primary importance: 1. Before the start of treatment, during preliminary consultation, information tailored in extent and quality to the individual patient has to be provided, a professional assessment of treatment needs made, and a relationship of mutual trust established. 2. In the initial phase of treatment, the individual shaping and optimisation of treatment needs is a matter of priority if initial acceptance of the appliance is to be assured. The patient should be advised how to meet the treatment needs in his individual situation. 3. In the further course of treatment, the fulfillment of the demands has to be repeatedly clarified. Feedback with regard to patient cooperation should be handled with caution. A tight recall schedule, a time-keeping record and involvement of the parents are obvious interventions in cases of inadequate cooperation. The treatment may also have to be reevaluated.

[Communication and iatrogenesis through communication].

Human intercommunication means transfer of information through many channels, with codes that have biological and socio-cultural determinism. The biological determinism is identifiable through the development of the phonetics and ergomotricity functions, through the possibility of perception of the sensorial organs. The channels of human intercommunication can be verbal, linguistic (the most specialised channel), and nonverbal, extra linguistic channels (mimics, written model, drawing, walking), that are effectively implicated in defining a direct dialogue between the doctor and the patient, completing and emphasising the communication.

Current status of communicable and non-communicable diseases in India.

India is going through a period of transition, both epidemiological and demographic transition. Infectious diseases are still persisting as major health problems in spite of having national programmes for the control of most of these diseases for almost half a century now. This paper focuses on two national programmes: the success story of the National Leprosy Eradication Programme; and the National Anti-Malaria Programme that has failed to achieve its objectives. There are re-emerging infectious diseases which are adding to the burden of diseases. In addition, there is an increasing prevalence of non-communicable diseases as a result of lifestyle changes and urbanization. These are the challenges that are to be tackled in the new millennium.

[Surveillance of communicable diseases in Europe; more than just a means of communication among national institutes].

The re-emergence of infectious diseases as a threat to the health of the public has led to the initiation of several surveillance programmes in the European community. The impact of these Europe-wide surveillance programmes, including the surveillance of antibiotic resistance, will depend on the ability of the epidemiologists involved to gather validated data that accurately reflect the trends in the occurrence of infectious diseases and of antibiotic resistances. The results of surveillance should be communicated not only between the national institutes but should also reach the medical communities and the public at large in the participating countries.

Stimulation of intercellular communication of poor-communicating cells by gap-junction-competent cells enhances the HSV-TK/GCV bystander effect in vitro.

We have previously shown that gap-junctional intercellular communication (GJIC) appears to play a role in the bystander effect that is observed in anticancer suicide gene therapy mediated by herpes simplex virus (HSV) thymidine kinase (tk) and ganciclovir (GCV). We now report that when connexin-expressing (Cx+) cells are present within a noncommunicating population of cells (Cx-), there is GJIC between the Cx+ and Cx- cells and that due to this stimulation of GJIC, the bystander effect also occurs when the 2 cell types are mixed. We transfected HeLa cells, which do not express any detectable level of connexin, with Cx43. The Cx+ and Cx- HeLa cells were further transfected with the tk gene, giving 4 phenotypes: Cx+tk-, Cx+tk+, Cx-tk+ and Cx-tk-. We observed GJIC between Cx+ and Cx- cells, but not between Cx- and Cx- cells, regardless of the tk genotype. Similarly, we observed the HSV-tk/GCV bystander effect in Cx+tk-/Cx-tk+ and Cx+tk+/Cx-tk- cocultures. The extent of the bystander effect in cocultures of Cx+tk- and Cx-tk+ cells was stronger than in cocultures of Cx+tk+ and Cx-tk- cells when each mixture had the same ratio of Cx+ and tk+ cells. These results suggest that Cx-expressing HeLa cells stimulate GJIC capacity between them and non-Cx-expressing HeLa cells, which mediates the bystander effect in mixtures of Cx+ cells and Cx- cells in vitro. Thus, Cx expression even in only a limited fraction of tumor cells may enhance the efficacy of the HSV-tk/GCV strategy by inducing a bystander effect.
