[Vascular recanalization using an electric thermal cauterizing catheter--in vitro experiences with a new catheter system].

Recanalisation of arteriosclerotic occlusions by means of laser angioplasty, Kensey catheter and the "hot tip" catheter is undergoing early, successful clinical trials. The thermo-cautery catheter described in this paper uses the well known technique of cutting with high frequency electric currents in order to achieve vascular recanalisation. Seven attempts were made to recanalise occlusions at post mortem; four were successful, but perforation occurred in three. Histologically we found a narrow rim of carbonisation of tissues surrounded by a zone of homogenisation and a transitional zone.

Patterning during pupal commitment of the epidermis in the butterfly, Precis coenia: the role of intercellular communication.

The commitment of cells to pupal development in the larvae of holometabolous insects can be prevented by treatment with juvenile hormone (JH) or a JH mimic during a critical period early in the last larval instar. By treating larvae of different ages with a JH mimic, pupal commitment of the epidermis of the butterfly, Precis coenia, was found to occur in a strict temporal and spatial progression, which was serially homologous and occurred independently in each segment. The mechanism underlying this sequence of pupal commitment was examined by cauterizing regions of the epidermis to observe the effects of local ablation on the pattern of pupal commitment revealed by treatment with the JH mimic. Cautery of the segmental site of origin of pupal commitment, the dorsal midline, suppressed pupal commitment in the rest of the operated segment, indicating that the midline has a special effect on commitment of the rest of the segment. Cautery off the midline produced asymmetries in the pattern of pupal commitment; when placed close to the midline, such cauteries prevented pupal commitment in the region "downstream" of the cautery, suggesting that a signal (diffusible or transducible) emanates from the midline. Finally, cautery of a circle around the midline inhibited pupal commitment only outside the circle, showing that cautery could act as a barrier to the passage of a signal coming from the midline. These results suggest that inductive as well as hormonal signals are involved in the regulation of pupal commitment in the epidermis of the lepidopteran, P. coenia.

Management pitfalls in the use of embolization for the treatment of severe epistaxis.

Angiographic embolization for the treatment of severe recurrent epistaxis was added to the traditional treatment options--nasal packing, cauterization, and surgical vessel ligation--in 1974. Since then, clinical experience has shown that this procedure is safe and effective. When epistaxis cannot be controlled with cautery, nasal packing is the most common next step. As such, it is often performed by emergency physicians and other clinicians who are not otolaryngologists. We report two cases in which intranasal neoplasms were obscured as a result of a significant distortion of the normal anatomy. This distortion was secondary to emergency-room treatment of severe epistaxis by repeated nasal packing followed by angiographic embolization. Pre-embolization angiographic studies and subsequent postembolization endoscopic evaluations did not reveal the presence of the occult neoplasms because of the presence of inflammation and edema after treatment. Clinicians should be aware that nasal packing and embolization can obscure the underlying source of epistaxis, and follow-up radiologic studies and endoscopic evaluations are essential to avoid delays in diagnosis.

Ontogenic origin of salmon GnRH neurons in the ventral telencephalon and the preoptic area in masu salmon.

During the ontogeny of masu salmon Oncorhynchus masou, neurons producing the salmon type of gonadotropin-releasing hormone (sGnRH) were first detected in the olfactory epithelium of the eyed egg and, subsequently, in the brain, suggesting a migration of these cells. Among sGnRH neurons distributed from the olfactory nerve (ON) through the preoptic area (POA), those in the ventral telencephalon (VT) and the POA are indicated to regulate gonadotropin secretion. Thus, it is of interest to know whether all the sGnRH neurons originate from the olfactory epithelium. In the present study, we examined by in situ hybridization whether sGnRH neurons are present in the VT-POA of fish, whose olfactory epithelia including sGnRH clusters were cauterized just after hatching (44 days after fertilization). Fish were sampled in June (212 days after the operation). Neurons expressing sGnRH mRNA were detected in the VT-POA as well as in the ON, ventral olfactory bulb, and transitional area between the olfactory bulb and telencephalon (which is considered to correspond to the terminal nerve ganglion) in the control group. In contrast, neurons expressing sGnRH mRNA were not detected in the VT-POA in the olfactory epithelium lesioned (OEL) group. Furthermore, pituitary sGnRH content in the OEL group was just above the detectable limit and was significantly lower than that in the corresponding control group in both sexes. These results indicate that sGnRH neurons in the VT-POA are derived from the olfactory epithelium in masu salmon, although the possibility cannot be ruled out that sGnRH neurons in the VT-POA arise from the VT-POA, but were delayed in expressing sGnRH because of the trauma of cauterization.

Phenol cauterization for ingrown toenails.

A new clinic was set up to deal with all toenail problems referred to a large general hospital. Ingrowing toenails were the commonest problem dealt with and phenol cauterization, either partial or complete, was the standard form of treatment. Over a 2-year period 1013 phenol cauterizations were carried out on 631 patients. After a minimum of 12 months follow-up, the recurrence rate was 2.96%. All recurrences were successfully treated by repeat phenol cauterization. We conclude that phenol cauterization should replace surgical ablation in the treatment of ingrown toenails.

Fitz-Hugh-Curtis syndrome. A laparoscopic approach.

Four of our patients had classical Fitz-Hugh-Curtis syndrome. Persistent right upper-gradrant pain prompted extensive medical evaluation; anibiotic therapy fialed to relieve the symptoms. In each case, laparoscopy proved to be an ideal method for obtaining definitive diagnosis and therapy. Typical perihepatic violin-string adhesions were cauterized and divided under direct vision with dramatic resolution of symptoms.

Vasectomy: an update.

Vasectomy remains an important option for contraception. Research findings have clarified many questions regarding patient selection, optimal technique, postsurgical follow-up, and risk of long-term complications. Men who receive vasectomies tend to be non-Hispanic whites, well educated, married or cohabitating, relatively affluent, and have private health insurance. The strongest predictor for wanting a vasectomy reversal is age younger than 30 years at the time of the procedure. Evidence supports the use of the no-scalpel technique to access the vasa, because it is associated with the fewest complications. The technique with the lowest failure rate is cauterization of the vasa with or without fascial interposition. The ligation techniques should be used cautiously, if at all, and only in combination with fascial interposition or cautery. A single postvasectomy semen sample at 12 weeks that shows rare, nonmotile sperm or azoospermia is acceptable to confirm sterility. No data show that vasectomy increases the risk of prostate or testicular cancer.

Tubal sterilization by bipolar laparoscopy: report of 232 cases.

Two hundred thirty-two interval bipolar laparoscopic tubal sterilizations were performed in a university-affiliated community teaching hospital over a 3-year period. Advanced laparoscopy equipment, properly trained laparoscopists, and a system of delineated operating privileges were used; the complication rate was 0.8%. There was 1 intraoperative complication, and 1 postoperative complication that manifested itself on postoperative day 3. Neither complication was related to the electrocauterization. The cauterized segments of fallopian tube were intentionally transected in 195 (84%) of the patients. The equipment and methodology employed are discussed with comparison of bipolar electrocautery to mechanical nonelectric methods of laparoscopic sterilization, ie, bands and clips. The authors' preference for bipolar electrocautery tubal sterilization, using single-segment cauterization with tubal transection, is discussed.

A comparison of the incidence of post operative bleeding between electric cauterization and suture ligation tonsillectomies.

Two-thousand one-hundred tonsillectomies performed from January 1980 to December 1995 by otolaryngologists at Pramongkutklao General Hospital were reviewed. The Dissection-Tyding snare technique was used and hemostasis completed by electric cautery in 690 cases, by suture ligature in 720 cases, by gauze packing alone as a control in 600 cases respectively. It can be concluded that the use of suture ligature offers an efficient and practical method to reduce the incidence of primary tonsillectomies bleeding.

Mechanisms of healing in the injured lung treated with the Nd-YAG laser.

An experimental study was done to evaluate the pattern of healing after sealing an incision in the pulmonary parenchyma with the Nd-YAG laser. Gross and histologic examinations of healing were compared with those of lung cauterized or sutured with chronic catgut. The results indicate that the use of the Nd-YAG laser to seal small vessels and air leaks has distinct advantages over cautery or suture closure of incisions in pulmonary tissue.

Effect of low molecular weight heparan sulphate on angiogenesis in the rat cornea after chemical cauterization.

Vascularization of the cornea occurs in many pathological conditions and can result in loss of visual acuity. It is also thought that vascularization predisposes the cornea to reject grafts by facilitating the detection of foreign antigens in donor material. A rat corneal assay for angiogenesis was adopted in the present study to evaluate the possible angiostatic activity of a low molecular weight heparan sulphate (LMW-HS). Corneal lesions were induced by chemical cauterization at 2 mm from the corneoscleral limbus. Rats were randomized to receive two drops/eye four times daily, for 6 days, of a solution of LMW-HS in vehicle (2.5% carboxymethylcellulose), heparin, heparin plus hydrocortisone, or vehicle alone. After a 6 day-treatment period, the eyes were perfused with india ink and the degree of neovascularization was evaluated. In rats treated with vehicle alone a dense vascular network extending from the corneoscleral limbus to the cauterized site was observed; on the contrary, a markedly reduced vascular network was evidenced in animals treated with LMW-HS. The distribution of basic fibroblast growth factor (bFGF) in the cauterized cornea was also evaluated by using an immunohistochemical method. A marked bFGF immunoreactivity was demonstrated in corneal epithelium and stroma of control rats 12-48 hours after the cautery. These results lead to the assumption that LMW-HS could be used in ophthalmology to inhibit corneal neovascularization.

Ovarian size and response to laparoscopic ovarian electro-cauterization in polycystic ovarian disease.

OBJECTIVES: To evaluate endocrine and ovulatory changes in polycystic ovarian disease (PCOD) in relation to patients' ovarian size. METHODS: Three hundred and seventy-one women with clomiphene citrate-resistant PCOD underwent laparoscopic ovarian cauterization [type I or typical with ovarian volume >8 cm(3) or cross-sectional area >10 cm(2) (n=211), type II with normal size ovary (n=160)]. Serum levels of LH, FSH, DHEAS, PRL, and T before and 10 days after ovarian cautery, spontaneous and induced ovulation and pregnancy rates were compared. RESULTS: Both groups responded to therapy in a similar manner, with a marked decrease in LH, FSH, DHEAS and T levels, with ovulation rates in type I 90.99%, type II 88.75% and pregnancy rates, 73.45% and 71.25%, respectively, with no statistical differences. CONCLUSIONS: Hormonal changes, ovulation and pregnancy rates were similar in the two types of PCOD, therefore it can be concluded that ovarian size is not a prognostic factor for response of PCOD patients to laparoscopic ovarian electro-cauterization.

Treatment of superior limbic keratoconjunctivitis by thermocauterization of the superior bulbar conjunctiva.

Superior limbic keratoconjunctivitis (SLK) is a chronic and recurrent inflammatory disease of the superior tarsal, bulbar and limbal conjunctiva that often responds to topical treatment with silver nitrate. As an alternative treatment in 11 patients (13 eyes), we applied thermal cautery to the inflamed superior bulbar conjunctiva following subconjunctival injection of 2% xylocaine. The overall positive response rate to thermocautery was 73% (8 patients). Of the positive responders, 63% (5 patients) had been considered silver nitrate treatment failures. Keratitis sicca was additionally noted in 55% of the patients studied. Impression cytology of involved superior bulbar conjunctiva was nearly devoid of goblet cells during the acute stage of the disorder. Following successful cauterization, goblet cells returned. Thus, thermocauterization of the superior bulbar conjunctiva appears to be a safe and effective mode of therapy for SLK.
