Shift report: a time for learning.

Abstract

Shift report provides an opportunity for professional communication that supports role socialization and development of a cohesive group process in the health care system. In this learning project, nursing students were able to practice professional behaviors, prioritize information, and reduce material to the boundaries of a work setting without relinquishing the holistic approach to the client. Critical thinking, direct application, self-analysis, peer critiques, and problem-solving processes were used by the student groups which will assist in their role transition to professional nursing practice.

Technological risk, participation and deliberation. Some results from three Italian case studies.

Participation is a major issue in environmental research and policy. Public Risk Perception and European Environmental Policy (PRISP), a cross-national inquiry on people's orientations about chemical risks, offered the opportunity to investigate some aspects. In two Italian case studies, interest was focused on the relation between experts and lay people and the function of participation. A key objective was to explore people's opinions about deliberative democracy. This is a currently much-debated alternative to the prevailing 'strategic' forms of democracy, based on the aggregation of preferences or bargaining among conflicting interests. Is there a role for an open confrontation aimed at reaching the common good? Results show that public interest is highly rated, participation is mainly connected to citizenship and understood as a form of co-operation. In a third case study, interest was focused on the analysis of a specific form of collective action according to the 'Critical Mass' rational choice model. In this case, it is argued that a small group of citizens may reduce the costs of starting-up a collective action aimed at pursuing the preferred social goals, i.e. at governing public goods.

[Patient centered medicine--a review with emphasis on the background and documentation].

BACKGROUND: In this paper we present the patient centred clinical method from a historical viewpoint with emphasis on its philosophical and ethical foundations. The paper reviews the documentation and discusses the relevance of the method. MATERIAL AND METHODS: Literature review. RESULTS: The patient centred method was introduced in western clinical medicine as a result of a general shift in science and the introduction of systems theory and relational understanding. The ethical basis for the method is humanistic values with emphasis on respect and mutuality in human relations. Observational studies suggest that the method improves outcome for patients. Experimental evidence is sparse and conflicting. The concept of patient centredness is vague with regard to the management phase of consultations. The relationship between doctor-patient communication, the concept of patient centredness, patient satisfaction, empowerment and outcome needs further clarification and research. INTERPRETATION: Clinical medicine has to encompass the personal and relational level of human life in order to help and guide patients. The patient centred method demonstrates how this can be achieved during the diagnostic phase of the consultation. We need, however, firmer knowledge and ethical consideration of how management negotiations may empower patients and improve their outcome.
