Traumatic hyphema.

Abstract

This report contains a study in retrospect of 50 patients treated for traumatic hyphema between 1964 and 1974 at the Kern Medical Center, Bakersfield, California. The prognosis for visual acuity is summarized in 3 distinct groups. The 50 patients studied were all treated in a similar manner by conservative method of bed rest with elevation of the head from 20 to 30 degrees and binocular patching. A review of our statistics, we believe, warrants this conservative method of treatment.

Factors related to the final visual outcome of 425 patients with traumatic hyphema.

A retrospective study of the visual outcome of 425 in-patients with traumatic hyphaema has been conducted. A multivariate analysis demonstrated that after adjusting for age, sex and pre-existing poor vision, the size of hyphaema on presentation and the presence of retinal damage were significant predictors of a worse final visual outcome (p = 0.00003 and 0.00001 respectively). Topical steroid and/or cycloplegic medication, and the occurrence of secondary haemorrhage did not influence the final visual outcome after adjustment for the other variables. These data illustrate, in an unselected sequential population of patients, the role of these factors in terms of final visual outcome following hyphaema from blunt ocular trauma.

Treatment of posterior eye segment complications after perforating trauma.

168 perforating trauma cases that underwent vitreoretinal surgery in our hospital over a two year period were reviewed. A uniform surgical concept was applied in those severely injured eyes. Showing posterior eye segment complications that required vitreoretinal surgery and having at least light perception were the only two criteria to be eligible for this study. The anatomic and functional results in different trauma groups are discussed as well as indications and duration of silicone oil tamponade.
