Gallbladder visualization on routine lung perfusion imaging.

Abstract

OBJECTIVE: Two routine lung SPECT studies showed what appeared to be uptake within the gallbladder. This finding was confirmed on coronal views and three-dimensional reconstruction. There was no contamination or problem with the radiopharmaceutical quality control. The patients had not had any prior nuclear procedures. Gallbladder visualization on routine lung imaging has not been previously observed. The purpose of this study was to ascertain the incidence of gallbladder uptake and to determine a possible mechanism. METHODS: Fifty-four patients referred for routine lung perfusion imaging were examined. At the end of the lung imaging, an anterior and/or right lateral 3-5 minute view of the abdomen was obtained. Patients too ill for additional imaging or those with prior cholecystectomies were excluded. RESULTS: Nineteen of the 54 patients showed activity within the gallbladder. Activity within the gut, kidneys, and urinary bladder also were observed. CONCLUSION: Gallbladder activity after Tc-99m macroaggregated albumin injection for lung perfusion studies, is a fairly common finding.

Primary rhabdoid tumor of the gallbladder: report of a case.

A 46-year-old man was operated on for cholelithiasis with chronic cholecystitis. On gross inspection of the resected gallbladder, a slight thickening in the body wall, in an area measuring about 1 x 0.5 cm, was noted. On light microscopic and immunohistochemical examinations, the lesion was diagnosed to be a rhabdoid tumor. After a thorough review of the literature we failed to find any reference to such a lesion in the gallbladder. This is the first known case report of a rhabdoid tumor of the gallbladder.

Factors affecting gallbladder motility: drugs.

Various drugs and medications that inhibit or stimulate gallbladder contraction and basal tone in humans are described. Active gallbladder contraction may be achieved using synthetic hormones such as cholecystokinin, caerulein and motilin, cholinomimetic drugs such as bethanecol, prostigmine, and erythromycin due to its motilin-like effect. Furthermore, cisapride and cholestyramine, may have some excitatory activity on the gallbladder muscle. Intravenous amino acids also induce gallbladder contraction through the release of cholecystokinin. Inhibition of gallbladder contraction induced by a meal, or reduction of the basal fasting tone may be achieved by using atropine and other cholinergics, and by inhibitory hormones such as somatostatin, the nitric acid releaser arginine, the calcium channel antagonist nifedipine, and progesterone. Other drugs such as trimebutine, loperamide and ondansetron may negatively affect gallbladder contraction.
