Enhancing medical database semantics.

Abstract

Medical Databases deal with dynamic, heterogeneous and fuzzy data. The modeling of such complex domain demands powerful semantic data modeling methodologies. This paper describes GSM-Explorer a Case Tool that allows for the creation of relational databases using semantic data modeling techniques. GSM Explorer fully incorporates the Generic Semantic Data Model-GSM enabling knowledge engineers to model the application domain with the abstraction mechanisms of generalization/specialization, association and aggregation. The tool generates a structure that implements persistent database-objects through the automatic generation of customized SQL ANSI scripts that sustain the semantics defined in the higher lever. This paper emphasizes the system architecture and the mapping of the semantic model into relational tables. The present status of the project and its further developments are discussed in the Conclusions.

The author gives an account of the contents of medical and health informatics. Information systems are the second most important area of health informatics and their contribution to the functions of hospitals, policlinics and individual surgeries is substantial. The position in this country as regards implementation and communication of hospital information systems is not simple and the implementation of completely functional systems will still need considerable efforts. Probably it will be also essential to have a new category of university trained workers which exists in other countries, so-called health care administrators or managers.

[Current activity of the World Health Organization in the area of systematic development of quality in health care].

The European Regional Office of WHO develops in recent years overall activities to promote continuous quality development in member countries. Several innovated targets and indicators of the strategy of the programme Health for All as well as the extensive definition apparatus elaborated by the department for ensuring the quality and evaluation of technologies is also focused on improving the quality of care. In the submitted article the author discusses the basic philosophy of procedures of the systematic development of the quality of care which resulted from the common effort of the European Regional Office of WHO and the Association of National Medical Societies. The European Regional Office in collaboration with some European health institutions and medical societies developed also information systems and models of systematic development of quality of care which are excellent examples of what can be achieved on a horizontal level. This applies in particular to the programmes DIABCARE, WHOCARE, ORATEL and OBSUOUID. In the submitted paper examples are given of the implementation of these programmes as well as the pertinent fundamental references. The author mentions briefly also some forms of implementation in some advanced European countries. In the conclusion the author deals briefly with the essential aspect of quality of health care, i.e. the relationship of effectiveness and economy of health care.
