Unexplained infertility.

Abstract

Unexplained infertility is a nebulous diagnosis that is justified only after a thorough and meticulous investigation of both partners. Even if the five basic tests constituting the infertility investigation have not revealed an abnormality, there may be a specific cause. Diagnosing unexplained infertility in this setting is somewhat arbitrary. In such couples, spontaneous pregnancies have been reported, with an average cumulative pregnancy rate of 60% after three years. Some additional testing, however, is appropriate. Treatment of unexplained infertility, however, tends to be empiric if no cause is found. Our current therapeutic plan entails superovulation with intrauterine insemination for at least four cycles. A cycle fecundity in the range of 0.23 may be expected but is highly influenced by the woman's age, with cycle fecundity dropping to as low as 0.05 after age 40. Alternatives to superovulation and intrauterine insemination are direct intraperitoneal injection of sperm with or without oocytes and intrafollicular injection of sperm. Gamete intrafallopian transfer and in vitro fertilization (IVF) are appropriate after a trial of empiric therapy. Our preference is to carry out IVF because the knowledge that fertilization has occurred is important in that fertilization failure is a known cause of unexplained infertility.

Nightmares and disorders of dreaming.

Dreams occur during all stages of sleep. Nightmares are common. They can be associated with poor sleep and diminished daytime performance. Frequent nightmares are not related to underlying psychopathology in most children and in some "creative" adults. However, recurrent nightmares are the most defining symptom of post-traumatic stress disorder and may be associated with other psychiatric illnesses. Night terrors are arousal disorders that occur most often in children and usually occur early in the sleep period. Patients with rapid-eye-movement behavior disorder often present with nocturnal injury resulting from the acting out of dreams. Dream disorders may respond to medication, but behavioral treatment approaches have shown excellent results, particularly in patients with post-traumatic stress disorder and recurrent nightmares.

Since it was shown that numerous neurological, psychiatric and internal illnesses have characteristics manifested during, or influenced by sleep, somnology has been playing a clinically more and more important role. Among the 88 diagnoses listed by ICD, not only insomnia, but also sleep-related respiratory disorders, in particular the obstructive sleep apnea syndrome, are of special importance. Sleep apnea is associated with coronary heart disease, myocardial insufficiency and other pathological conditions. Already in the doctor's office, a carefully taken history (nocturnal apnea alternating with irregular snoring, and diurnal sleepiness) can arouse an appropriate suspicion. This can be confirmed by an ambulatory polygraphic exploration. The definitive diagnosis is then established with the aid of polysomnography in the sleep lab where specific treatment is also initiated.
