Teflon injection: pointers and pitfalls.

Abstract

Our objective is to clarify the procedure used in a series of 335 cases of injection of paralyzed vocal cords. Over 29 years, questions have arisen about the procedure, mostly from trainees. They relate to timing, indications, method, and follow-up. Results of the procedure, when properly performed, have been highly successful. Where indications are appropriate, injection is usually uncomplicated, relatively safe, and done with minimum instrumentation. This is our response, in a question-and-answer format, to the most frequent queries.

Protocol-driven care in the intensive care unit: a tool for quality.

Advances in organization and patient management in the intensive care unit (ICU) have led to reductions in the morbidity and mortality suffered by critically ill patients. Two such advances include multidisciplinary teams (MDTs) and the development of clinical protocols. The use of protocols and MDTs does not necessarily guarantee instant improvement in the quality of care, but it does offer useful tools for the pursuit of such objectives. As ICU physicians increasingly assume leadership roles in the pursuit of higher quality ICU care, their knowledge and skills in the discipline of quality improvement will become essential.

Rational use of computerized protocols in the intensive care unit.

Excess information in complex ICU environments exceeds human decision making limits, increasing the likelihood of clinical errors. Explicit decision-support tools have favorable effects on clinician and patient outcomes and can reduce the variation in clinical practice that persists even when guidelines based on reputable evidence are available. Computerized protocols used for complex clinical problems generate, at the point-of-care, patient-specific evidence-based therapy instructions that can be carried out by different clinicians with almost no inter-clinician variability. Individualization of patient therapy is preserved by these explicit protocols since they are driven by patient data. Computerized protocols that aid ICU decision-makers should be more widely distributed.
