Shared governance: getting it started.

Abstract

A shared governance model has been developed and implemented in an intensive care unit of a 290-bed community hospital. This resulted from a large staff turnover due to job dissatisfaction stemming largely from lack of autonomy. Scheduling and floating were major factors. Under the unit manager's leadership, four committees of staff nurses designed an appropriate model of participative management. Surveys of 18 and 30 months indicate significant support of the shared governance system, with improved collegiality, enhanced professional growth and a turnover rate of less than 1 percent.

A comprehensive quality management approach.

Value analysis and shared governance are two management concepts relatively new to the health care industry. Operating together, they complement each other and can result in a comprehensive "quality management" plan. An example of this model in a purchasing decision illustrates the benefits that can result from use of this collaborative, multidisciplinary approach.
