Temporal bone dissection for display.

Abstract

Increasing concern with medicolegal issues has heightened the need for surgical simulation in training. Familiarity with the surgical anatomy of the temporal bone is essential for effective and safe otological surgery. Refinement of surgical technique and intimate knowledge of temporal bone anatomy can be gained by accurate dissection. The products of such endeavours are both illustrative and instructive. The issues, methods and techniques necessary for display of anatomical dissections are discussed.

Fibrillin-containing microfibrils: structure and function in health and disease.

Fibrillin-containing microfibrils are a unique class of connective tissue macromolecules whose critical contribution to the establishment and maintenance of diverse extracellular matrices was underlined by the recent linkage of their principal structural component fibrillin to Marfan syndrome, a heritable disorder with pleiotrophic connective tissue manifestations. The complexity of the structure: function relationships of these macromolecules was highlighted by the recent elucidation of the primary structure of fibrillin and characterisation of fibrillin mutations in Marfan patients. This review examines current understanding of the expression and assembly of fibrillin and describes new approaches which are now being applied to elucidate the many outstanding structural, organisational and functional aspects of the fibrillin-containing microfibrils.

Restoring gingival harmony around single tooth implants.

One of the biggest challenges to restoring a single tooth implant in the esthetic zone of the mouth is the creation of harmonious gingival contour around the restoration. Soft- or hard-tissue deficiencies of the edentulous space are the most common obstacles to achieving gingival symmetry around the proposed restoration. Numerous gingival and osseous grafting and regeneration techniques are available but may complicate treatment by increasing the number of surgical procedures and sites necessary. This article describes a technique for treating mild-to-moderate ridge defects without the required additional surgical procedures or a surgical donor site. Soft-tissue overcontouring is provided around the healing abutment by modification of the surgical flap at second-stage implant surgery. Subsequent gingivoplasty allows establishment of anatomic gingival architecture that surrounds the single implant prosthesis.
