Rare prostatic carcinomas: histogenesis and morphologic pattern.

Abstract

Most prostate cancers (90%) are acinar adenocarcinomas, originating in the peripheral or other prostatic regions. In a series of 650 cases of prostate carcinoma we found 7 cases of prostate carcinoma with unusual features, including 3 pure duct papillary carcinomas, 1 pure transitional cell carcinoma, 1 mucinous adenocarcinoma and 2 pure small cell carcinomas. Ductal papillary carcinoma consists of papillary fronds and branching, fibro-connective tissue, covered by a single layered to multilayered lining of columnar cells. Also the tumours located in central portions of the prostate derive from periurethral prostatic ducts. Primary transitional cell carcinoma of the prostate implies no pre-existing bladder cancer and arises from indifferent or reserve cells, lying between the luminal epithelium and the basement membrane of the periurethral ducts. The diagnosis of mucinous carcinoma should be reserved for these cases in which a sufficient quantity of extracellular mucin is seen to form pools and lakes. True mucinous carcinoma is likely to be a variant of prostate carcinoma. Classic oat cell carcinomas are composed of small, fairly uniform tumour cells only slightly larger than lymphocytes, arranged in solid nests wherein central necrosis is commonly observed. The pathological finding, clinical course and immunohistochemical studies, indicate that the small cell carcinoma of the prostate is most likely to be a neuroendocrine neoplasm.

[Mucinous sweat gland carcinoma of a toe with metastasis to the maxilla. A case report].

Malignant tumors of the maxilla are rare. In addition to primary tumors metastatic disease has to be considered. Renal cell carcinomas, carcinomas of the breast, lung and adenocarcinomas are constituting the most frequent primary lesions for maxillary metastases. To the best of our knowledge, we present the first case of a mucinous sweat gland carcinoma metastasizing to the maxilla four years after primary diagnosis. Thirty-two months after resecting the hemimaxilla, there are no signs of local recurrence. Although skin tumors only rarely develop distant metastases, sweat gland carcinomas should be included in the differential diagnosis of tumors metastasizing to the maxilla.

Utility of CEA and CA 19-9 tumor markers in diagnosis and prognostic assessment of mucinous epithelial cancers of the appendix.

BACKGROUND AND OBJECTIVES: Tumor markers are a clinical tool frequently used in oncology in association with other clinical and radiologic information. For gastrointestinal cancer, carcinoembryonic antigen (CEA) and carbohydrate antigen 19-9 (CA 19-9) tumor markers have found selected clinical application. The use of these tumor markers in mucinous epithelial tumors of the appendix has not been previously determined. METHODS: In patients with peritoneal dissemination of a mucinous epithelial malignancy of the appendix, tumor markers CEA and CA 19-9 were prospectively recorded preoperatively within 1 week prior to definitive treatment. Also, if the appendiceal tumor recurred, the tumor marker was determined. The accuracy of these two tumor markers in the management of this disease was determined for these two specific clinical situations. RESULTS: CEA was elevated in 56% of 532 patients and CA 19-9 was elevated in 67.1% of these patients. Although the absolute level of tumor marker did not correlate with prognosis, a normal value indicated an improved survival. CEA was elevated in 35.2% of 110 patients determined to have recurrent disease; CA 19-9 was elevated in 62.9% and at least one of the tumor markers was elevated in 68.2% of patients. An elevated CEA tumor marker at the time of recurrence indicated a reduced prognosis. CONCLUSIONS: Both CEA and CA 19-9 tumor markers were elevated in a majority of these patients and should be a valuable diagnostic tool previously underutilized in this group of patients. These tumor markers were also of benefit in the assessment of prognosis in that a normal level indicated an improved prognosis. At the time of a reoperative procedure, CEA and CA 19-9 tumor markers gave information regarding the progression of disease. These tumor markers have practical value in the management of epithelial appendiceal malignancy with peritoneal dissemination.

Pathobiological significance of vascular endothelial growth factor and Maspin expressions in human gastric carcinoma.

AIM: To investigate the correlation between expression of vascular endothelial growth factor (VEGF) and cell differentiation, invasion, metastasis and Maspin expression in gastric carcinoma. METHODS: Formalin-fixed paraffin-embedded tissue specimens from 73 cases of gastric carcinoma were studied with SP immunohistochemistry, using anti-VEGF monoclonal antibody, and thirty-nine of them were studied using anti-Maspin monoclonal antibody. VEGF expression was compared with the clinical stage, lymph node metastasis, and Borrmann's and WHO's classification of gastric carcinoma. RESULTS: The positive rate of VEGF expression was significantly higher in adjacent non-carcinoma epithelia (ANCE) than in non-metaplastic, non-carcinoma gastric epithelia (NMNCE), which were at least 4 cm distant from the primary tumor (P = 0.000, chi2 = 73.03). The positive rate of VEGF expression was significantly higher in advanced gastric carcinoma (AGC) than in early gastric carcinoma (EGC) (P = 0.032, chi2 = 4.62). The positive rate of VEGF expression in gastric carcinomas with lymph node metastases was significantly higher than that in those without metastasis (P = 0.006, chi2 = 7.47). Maspin was weakly expressed in 16 out of 39 cases of NMNCE, and the positive immunoreaction was limited to gland cells of the stomach body. There was no significant correlation between the expression of VEGF and histological or gross classifications, and correlation between the expressions of VEGF and Maspin in gastric carcinoma (P = 0.648, chi2 = 0.21). CONCLUSION: Expression of VEGF is significantly correlated to the malignant biological behaviors of gastric carcinoma, but there is no significant correlation between the expression of VEGF and Maspin.
