Follicular dendritic cell tumor of the liver: a clinicopathologic and Epstein-Barr virus study of two cases.

Two cases of hepatic follicular dendritic cell (FDC) tumor are described. Both patients were female, aged 57 and 51 years. They presented with epigastralgia or abdominal fullness and weight loss. The first patient refused surgical resection. She developed progressive polyclonal gammopathy and then bilateral purpura over the legs. Skin biopsy revealed leukocytoclastic vasculitis with granular vascular deposits of IgA and C3. The second patient had marked peripheral blood and tissue eosinophilia. The histological diagnosis was confirmed by positive staining for CD21 and CD23. The stromal lymphocytes were predominantly composed of CD3(+)and CD8(+) cells. In situ hybridization for EBER showed a positive nuclear signal in tumor cells but not in inflammatory cells. Polymerase chain reaction amplification for Exon 3 of the latent membrane protein-1 (LMP-1) gene showed a characteristic 30-bp deletion between nucleotides 168282 and 168253, corresponding to the B95-8 sequence. The unique clinicopathological features of our cases have not been reported for FDC tumors before. The clinical significance of the 30-bp deletion in Exon 3 of the LMP-1 gene in FDC tumor of the liver warrants further investigation.

Receptors involved in the phagocytosis of senescent and diamide-oxidized human RBCs.

BACKGROUND: Senescent RBCs bear IgG and C3 opsonins that are three to four times less than required for similar phagocytosis of experimentally opsonized RBCs. STUDY DESIGN AND METHODS: Studies were performed to determine the phagocyte receptors involved in phagocytosis in vitro. The effect of clustering of opsonins and oxidative damage in the sequestration of RBCs was studied by exposing RBCs to BS3 (bis[sulfosuccinimidyl]-suberate) and diamide (azodicarboxylic acid bis[dimethyl-amide]). RESULTS: Sequestration of senescent RBCs was inhibited by the treatment of lymphokine-activated monocytes with N-acetyl-D-galactoseamine (GalNAc), arginine-glycine-aspartic acid (RGD), or antibodies to CR3, FcgammaRI, FcgammaRII, leukocyte response integrin (LRI), and integrin-associated protein (IAP). Exposure to BS3 alone did not enhance phagocytosis. The addition of serum resulted in opsonin binding. The level of opsonization required for sequestration was higher than on senescent RBCs and was only marginally inhibited by blocking CR3, FcgammaRI and FcgammaRII. Diamide treatment alone did not lead to sequestration. Diamide-treated RBCs exposed to serum bound opsonin much as did senescent RBCs, and sequestration was inhibited by GalNAc, RGD, and antibodies to CR3, FcgammaRI, FcgammaRII, LRI, and IAP. CONCLUSION: Membrane alterations resulting in the binding of opsonins and the sequestration of senescent RBCs may be similar to those that occur on diamide-oxidized RBCs. They suggest the need for cooperative events among oxidation, clustering and cross-linking, and serum opsonization.

Sequential C3 and C4 levels in human milk in relation to prematurity and parity.

Similarly to many immune molecules of human milk, C3 and C4 levels decrease during lactation. We investigated the influence, over the first three weeks of lactation, of both prematurity and parity on the sequential evolution of these levels. Milk C3 and C4 concentrations were measured by immunonephelometry in 494 individual samples collected from 76 lactating mothers. C3 and C4 concentrations were higher in milk from preterm or primiparous mothers. The major differences were observed in milk from days 5-8 and 9-20, likely due to pronounced interindividual variations in levels of days 1-4 milk. Milk from mothers of precocious (33 weeks' gestation or less) preterm newborns presented higher concentrations and a slower decrease of C3 and C4 levels than that from mothers of late (33-37 weeks' gestation) preterm newborns, when compared to term mothers. Finally, the inversion of the C3/C4 ratio occurring over time, previously reported, appeared later in milk from mothers of preterm newborns. The influence of prematurity was even greater in primiparous than in multiparous mothers. Both C3 and C4 levels therefore appear to be influenced in human milk by the parity and prematurity of the delivery. Mothers from preterm newborns seem to provide higher levels of C3 for a longer period post delivery.
