Morphometry in endocrine pathology.

Abstract

The most important purposes of morphometry in endocrine pathology are described. With the introduction of immunocytochemical techniques by Sternberger in 1969 a new field for detailed morphometric analysis in endocrine pathology was opened. The application of morphometry in combination with the immunocytochemical staining of cells is illustrated by studies of the role of gastrin and gastric secretion in the etiology of duodenal ulcer. For such studies morphologic alterations have to be quantified. Morphometry as an instrument for characterizing diseases has been used in various fields, e.g. to analyse nesidioblastosis, diabetes mellitus or papillary carcinomas of the thyroid gland.

[Strategy for peptic ulcer therapy in the era of H. pylori eradication therapy].

The Japanese guideline for gastric ulcer therapy published 2003 has adopted a policy that eradication therapy for H. pylori as the first line therapy on ulcer patients with positive H. pylori status. For NSAID-induced gastric ulcer patients, cessation should be considered. In patients who cannot stop NSAIDs, proton-pump inhibitors or prostaglandin drugs is recommended. Similar strategy can be applied for duodenal ulcer patients. Implementation of this basic strategy in daily clinical practice, however, require further efforts to wider recognition of the new guideline for gastric ulcer therapy as well as to solve several barriers caused by discrepant health reimbursement policy.

[Immediate and remote results of pylorus-preserving and pylorus-reducing operations in complex surgical treatment of perforated pyloric-duodenal ulcers].

The results of surgical treatment of 224 patients with perforated pyloroduodenal ulcers were analyzed. 110 patients were operated using pyloruspreserving and pylorusreducing methods. The new method of pyloroduodenoplasty is suggested. Duodenogastral reflux after pyloruspreserving operations is arised in 4.5% of observations, after pylorusdestructive--in 46.7%.
