Blockage of a ventriculoperitoneal (VP) shunt is very common. Here, we describe an unusual case of shunt blockage in an infant. He was shunted for hydrocephalus associated with a Dandy-Walker malformation diagnosed at birth. At surgery, the entire VP shunt was found to have migrated into the subgaleal space, which clinically presented as shunt obstruction. We suggest techniques to avoid this rare, but preventable, complication of shunt surgery.
Neurocutaneous melanosis is an uncommon congenital disorder consisting of benign or malignant melanocytic tumors of the leptomeninges with large or numerous cutaneous congenital melanocytic nevi. The Dandy-Walker malformation occurs as an enlarged posterior fossa with high insertion of the tentorium, hypoplasia or aplasia of the cerebellar vermis, and cystic dilatation of the fourth ventricle. To our knowledge, the association of these two conditions has been reported only 14 times. In this article, we present a newborn patient with neurocutaneous melanosis associated with Dandy-Walker malformation, which was diagnosed by magnetic resonance imaging.
We describe a rare case of Dandy-Walker syndrome malformation and its ocular alterations. A female child, aged 1 year and 9 month, with central nervous system alterations (Dandy-Walker syndrome), associated with low visual acuity, megalocornea (13 mm diameter in right eye and 13.5 mm in the left), fundoscopy with 0.7 optic papilla excavation in both eyes (BE) and temporal paleness in BE, intraocular pressure of 16 mmHg in the right eye and 14 mmHg in the left and anteroposterior diameter of 20.55 mm in both eyes.
